Prospecting for new group A streptococcal vaccine candidates
Background & objectives: Most group A streptococcal (GAS) vaccine strategies focused on the surface M protein of the GAS. However, vaccine based on M protein have some drawbacks. In the present study, we used two approaches to identify new proteins and peptides that may have utility as vaccine candidates. Methods: A whole gel elution procedure was used to separate GAS surface antigens into 9 size fractionated pools. Mice were vaccinated with each pool and antibody titre, opsonic ability and protective capacity measured. In an alternative approach BioInformatics was used to identify putative GAS surface proteins. Peptides from within these proteins were then selected on the basis of predicted antigenicity or location. These peptides were conjugated to keyhole lymphocyanin (KLH) and immunogenicity measured in a mouse model. Results: One pool of GAS surface proteins (approximately 29kDa) induced antibodies that were both opsonic and potentially protective. Immunoflourescent microscopy demonstrated that these antibodies bound to the surface of M1 GAS. Amino acid sequencing subsequently identified superoxide dismutase as the major antigen in this pool. A BioInformatic search of the M1 GAS genome and subsequent analysis identified several peptides that fulfilled criteria as potential vaccine candidates. Each peptide when conjugated to KLH was able to induce a strong antibody response. Interpretation & conclusion: Several new antigens were identified that may have potential as vaccine targets. A future GAS vaccine may have multiple peptide epitopes, providing protection against multiple GAS strains
Ethical Gaps in Ophthalmology in the United States

Professional ethical guidelines, when they exist, are stricter than what the law requires, but are silent on several contemporary controversies. Conscientious guidelines can vary from loosest to strictest as they apply to individuals with wide variability. The relationship of ophthalmology to society changes, and ethical guidelines lag for some of the interactions.Conclusion: The rules of ethics for ophthalmology need to be updated and evidence of activity and oversight made public. Who should be prioritized for renal transplantation?: Analysis of key stakeholder preferences using discrete choice experiments
Background
Policies for allocating deceased donor kidneys have recently shifted from allocation based on Human Leucocyte Antigen (HLA) tissue matching in the UK and USA. Newer allocation algorithms incorporate waiting time as a primary factor, and in the UK, young adults are also favoured. However, there is little contemporary UK research on the views of stakeholders in the transplant process to inform future allocation policy. This research project aimed to address this issue.
Methods
Discrete Choice Experiment (DCE) questionnaires were used to establish priorities for kidney transplantation among different stakeholder groups in the UK. Questionnaires were targeted at patients, carers, donors / relatives of deceased donors, and healthcare professionals. Attributes considered included: waiting time; donor-recipient HLA match; whether a recipient had dependents; diseases affecting life expectancy; and diseases affecting quality of life.
Results
Responses were obtained from 908 patients (including 98 ethnic minorities); 41 carers; 48 donors / relatives of deceased donors; and 113 healthcare professionals. The patient group demonstrated statistically different preferences for every attribute (i.e. significantly different from zero) so implying that changes in given attributes affected preferences, except when prioritizing those with no rather than moderate diseases affecting quality of life. The attributes valued highly related to waiting time, tissue match, prioritizing those with dependents, and prioritizing those with moderate rather than severe diseases affecting life expectancy. Some preferences differed between healthcare professionals and patients, and ethnic minority and non-ethnic minority patients. Only non-ethnic minority patients and healthcare professionals clearly prioritized those with better tissue matches.
Conclusions
Our econometric results are broadly supportive of the 2006 shift in UK transplant policy which emphasized prioritizing the young and long waiters. However, our findings suggest the need for a further review in the light of observed differences in preferences amongst ethnic minorities, and also because those with dependents may be a further priority.</p
Solar-type dynamo behaviour in fully convective stars without a tachocline
In solar-type stars (with radiative cores and convective envelopes), the
magnetic field powers star spots, flares and other solar phenomena, as well as
chromospheric and coronal emission at ultraviolet to X-ray wavelengths. The
dynamo responsible for generating the field depends on the shearing of internal
magnetic fields by differential rotation. The shearing has long been thought to
take place in a boundary layer known as the tachocline between the radiative
core and the convective envelope. Fully convective stars do not have a
tachocline and their dynamo mechanism is expected to be very different,
although its exact form and physical dependencies are not known. Here we report
observations of four fully convective stars whose X-ray emission correlates
with their rotation periods in the same way as in Sun-like stars. As the X-ray
activity - rotation relationship is a well-established proxy for the behaviour
of the magnetic dynamo, these results imply that fully convective stars also
operate a solar-type dynamo. The lack of a tachocline in fully convective stars
therefore suggests that this is not a critical ingredient in the solar dynamo
and supports models in which the dynamo originates throughout the convection

Comparison of Large Language Models in Diagnosis and Management of Challenging Clinical Cases

Each case was presented in identical, open-ended text fashion to each LLM and open-ended responses regarding differential diagnosis, next diagnostic tests and recommended treatments were requested. Responses were recorded and assessed for accuracy against published correct answers. The main outcome was accuracy of LLMs against the correct answers. Secondary outcomes included comparative performance on the differential diagnosis, ancillary testing, and treatment subtests; and readability of responses.Results: Scores were normally distributed and ranged from 0– 35 (with a maximum score of 60) with a mean ± standard deviation of 19 ± 9. Scores for three of the LLMs (ChatGPT 3.5, Claude Pro, and Copilot Pro) were statistically significantly higher than the mean. Two of the high-performing LLMs were paid subscription (Claude Pro and Copilot Pro) and one was free (ChatGPT 3.5). While there were no clinical or statistical differences between ChatGPT 3.5 and Claude Pro, a separation of +5 points, or 0.56 standard deviations, between Copilot Pro and the other highly ranked LLMs was present. Readability of all tested programs were above the AMA (American Medical Association) reading level recommendations to public consumers of eight grade.Conclusion: Subscription LLMs were more prevalent among highly ranked LLMs suggesting that these perform better as ophthalmic assistants. While readability was poor for the average person, the content was understood by a board-certified ophthalmologist. The accuracy of LLMs is not high enough to recommend patient care in standalone mode, but aiding clinicians in patient care and prevent oversights is promising.Keywords: large language model, LLM, chatbot, artificial intelligence, A
The geography of recent genetic ancestry across Europe
The recent genealogical history of human populations is a complex mosaic
formed by individual migration, large-scale population movements, and other
demographic events. Population genomics datasets can provide a window into this
recent history, as rare traces of recent shared genetic ancestry are detectable
due to long segments of shared genomic material. We make use of genomic data
for 2,257 Europeans (the POPRES dataset) to conduct one of the first surveys of
recent genealogical ancestry over the past three thousand years at a
continental scale. We detected 1.9 million shared genomic segments, and used
the lengths of these to infer the distribution of shared ancestors across time
and geography. We find that a pair of modern Europeans living in neighboring
populations share around 10-50 genetic common ancestors from the last 1500
years, and upwards of 500 genetic ancestors from the previous 1000 years. These
numbers drop off exponentially with geographic distance, but since genetic
ancestry is rare, individuals from opposite ends of Europe are still expected
to share millions of common genealogical ancestors over the last 1000 years.
There is substantial regional variation in the number of shared genetic
ancestors: especially high numbers of common ancestors between many eastern
populations likely date to the Slavic and/or Hunnic expansions, while much
lower levels of common ancestry in the Italian and Iberian peninsulas may
indicate weaker demographic effects of Germanic expansions into these areas
and/or more stably structured populations. Recent shared ancestry in modern
Europeans is ubiquitous, and clearly shows the impact of both small-scale
migration and large historical events. Population genomic datasets have
considerable power to uncover recent demographic history, and will allow a much
fuller picture of the close genealogical kinship of individuals across the

The convective storm initiation project
Copyright @ 2007 AMSThe Convective Storm Initiation Project (CSIP) is an international project to understand precisely where, when, and how convective clouds form and develop into showers in the mainly maritime environment of southern England. A major aim of CSIP is to compare the results of the very high resolution Met Office weather forecasting model with detailed observations of the early stages of convective clouds and to use the newly gained understanding to improve the predictions of the model. A large array of ground-based instruments plus two instrumented aircraft, from the U.K. National Centre for Atmospheric Science (NCAS) and the German Institute for Meteorology and Climate Research (IMK), Karlsruhe, were deployed in southern England, over an area centered on the meteorological radars at Chilbolton, during the summers of 2004 and 2005. In addition to a variety of ground-based remote-sensing instruments, numerous rawin-sondes were released at one- to two-hourly intervals from six closely spaced sites. The Met Office weather radar network and Meteosat satellite imagery were used to provide context for the observations made by the instruments deployed during CSIP. This article presents an overview of the CSIP field campaign and examples from CSIP of the types of convective initiation phenomena that are typical in the United Kingdom. It shows the way in which certain kinds of observational data are able to reveal these phenomena and gives an explanation of how the analyses of data from the field campaign will be used in the development of an improved very high resolution NWP model for operational use.This work is funded by the National Environment Research Council following an initial award from the HEFCE Joint Infrastructure Fund
Effective Rheology of Bubbles Moving in a Capillary Tube
We calculate the average volumetric flux versus pressure drop of bubbles
moving in a single capillary tube with varying diameter, finding a square-root
relation from mapping the flow equations onto that of a driven overdamped
pendulum. The calculation is based on a derivation of the equation of motion of
a bubble train from considering the capillary forces and the entropy production
associated with the viscous flow. We also calculate the configurational
