The impact of co-located NHS walk-in centres on emergency departments
Objectives:
To determine the impact of establishing walk-in centres alongside emergency departments
on attendance rates, visit duration, process, costs and outcome of care.
Methods:
Eight hospitals with co-located emergency departments and walk-in centres were compared
with eight matched emergency departments without walk-in centres. Site visits were
conducted. Routine data about attendance numbers and use of resources were analysed. A
random sample of records of patients attending before and after walk-in centres opened
were also assessed. Patients who had not been admitted to hospital were sent a postal
questionnaire.
Results:
In most sites, the walk-in centres did not have a distinct identity and there were few
differences in the way services were provided compared with control sites. Overall, there
was no evidence of an increase in attendance at sites with walk-in centres, but considerable
variability across sites. The proportion of patients managed within the four-hour NHS target
improved at sites both with and without walk-in centres. There was no evidence of any
difference in re-consultation rates, costs of care or patient outcomes at sites with or without
walk-in centres.
Conclusions:
Most hospitals in this study implemented the walk-in centre concept to a very limited extent.
Consequently there was no evidence of any impact on attendance rates, process, costs or
outcome of care
Rapid fabrication of annuloplasty rings by electron beam melting
Electron Beam Melting (EBM) is an Additive Manufacturing (AM) technology capable of producing intricate parts by melting powder metal with the aid of an electron beam gun. EBM has facilitated the production of standard and customisable implants. Customizable implants such as orthopaedic implants, cranial implants and dental implants have already been developed and implanted successfully after being fabricated by AM technology. Other medical devices can also benefit from the possibilities offered by AM. An example of such a medical device would be the annuloplasty ring. Standard annuloplasty rings are implanted whenever a patient is diagnosed with mitral valve regurgitation. This problem arises when the mitral valve does not close properly, causing back leakage through the closed valve resulting in blood flowing to the atrium instead of the aorta during systole. The latest designs of annuloplasty rings allow restoration of the mitral annulus configuration to a saddle-shaped shape.peer-reviewe
Comparing care at walk-in centres and at accident and emergency departments: an exploration of patient choice, preference and satisfaction
Objectives:
To explore the impact of establishing walk-in centres alongside emergency departments on
patient choice, preference and satisfaction.
Methods:
A controlled, mixed-method study comparing eight emergency departments with co-located
walk-in centres with the same number of ‘traditional’ emergency departments. This paper
focuses on the results of a cross-sectional questionnaire survey of users.
Results:
Survey data demonstrated that patients were frequently unable to distinguish between being
treated at a walk-in centre or an A&E department, and even where this was the case,
opportunities to exercise choice about their preferred care provider were often limited. Few
made an active choice to attend a co-located walk-in centre. Patients attending walk-in
centres were just as likely to be satisfied overall with the care they received as their
counterparts who were treated in the co-located A&E facility, although a small proportion of
walk-in centre users did report greater satisfaction with some specific aspects of their care
and consultation.
Conclusions:
Whilst one of the key policy goals underpinning the co-location of walk-in centres next to an
A&E department was to provide patients with more options for accessing healthcare and
greater choice, leading in turn to increased satisfaction, this evaluation was able to provide
little evidence to support this. The high percentage of patients expressing a preference for
care in an established emergency department compared to a new walk-in centre facility
raises questions for future policy development. Further consideration should therefore be
given to the role that A&E focused walk-in centres play in the Department of Health’s
current policy agenda, as far as patient choice is concerned
How a Diverse Research Ecosystem Has Generated New Rehabilitation Technologies: Review of NIDILRR’s Rehabilitation Engineering Research Centers
Over 50 million United States citizens (1 in 6 people in the US) have a developmental, acquired, or degenerative disability. The average US citizen can expect to live 20% of his or her life with a disability. Rehabilitation technologies play a major role in improving the quality of life for people with a disability, yet widespread and highly challenging needs remain. Within the US, a major effort aimed at the creation and evaluation of rehabilitation technology has been the Rehabilitation Engineering Research Centers (RERCs) sponsored by the National Institute on Disability, Independent Living, and Rehabilitation Research. As envisioned at their conception by a panel of the National Academy of Science in 1970, these centers were intended to take a “total approach to rehabilitation”, combining medicine, engineering, and related science, to improve the quality of life of individuals with a disability. Here, we review the scope, achievements, and ongoing projects of an unbiased sample of 19 currently active or recently terminated RERCs. Specifically, for each center, we briefly explain the needs it targets, summarize key historical advances, identify emerging innovations, and consider future directions. Our assessment from this review is that the RERC program indeed involves a multidisciplinary approach, with 36 professional fields involved, although 70% of research and development staff are in engineering fields, 23% in clinical fields, and only 7% in basic science fields; significantly, 11% of the professional staff have a disability related to their research. We observe that the RERC program has substantially diversified the scope of its work since the 1970’s, addressing more types of disabilities using more technologies, and, in particular, often now focusing on information technologies. RERC work also now often views users as integrated into an interdependent society through technologies that both people with and without disabilities co-use (such as the internet, wireless communication, and architecture). In addition, RERC research has evolved to view users as able at improving outcomes through learning, exercise, and plasticity (rather than being static), which can be optimally timed. We provide examples of rehabilitation technology innovation produced by the RERCs that illustrate this increasingly diversifying scope and evolving perspective. We conclude by discussing growth opportunities and possible future directions of the RERC program
Differences in Factors Associated With Preterm and Term Stillbirth: A Secondary Cohort Analysis of the DESiGN Trial
OBJECTIVE: To identify whether maternal and pregnancy characteristics associated with stillbirth differ between preterm and term stillbirth. DESIGN: Secondary cohort analysis of the DESiGN RCT. SETTING: Thirteen UK maternity units. POPULATION: Singleton pregnant women and their babies. METHODS: Multiple logistic regression was used to assess whether the 12 factors explored were associated with stillbirth. Interaction tests assessed for a difference in these associations between the preterm and term periods. MAIN OUTCOME MEASURE: Stillbirth stratified by preterm (<37+0 weeks') and term (37+0-42+6 weeks') births. RESULTS: A total of 195 344 pregnancies were included. Six hundred and sixty-seven were stillborn (3.4 per 1000 births), of which 431 (65%) were preterm. Significant interactions were observed for maternal age, ethnicity, IMD, BMI, parity, smoking, PAPP-A, gestational hypertension, pre-eclampsia and gestational diabetes but not for chronic hypertension and pre-existing diabetes. Stronger associations with term stillbirth were observed in women with obesity compared to BMI 18.5-24.9 kg/m2 (BMI 30.0-34.9 kg/m2 term adjusted OR 2.1 [95% CI 1.4-3.0] vs. preterm aOR 1.1 [0.8-1.7]; BMI ≥ 35.0 kg/m2 term aOR 2.2 [1.4-3.4] vs. preterm aOR 1.5 [1.2-1.8]; p-interaction < 0.01), nulliparity compared to parity 1 (term aOR 1.7 [1.1-2.7] vs. preterm aOR 1.2 [0.9-1.6]; p-interaction < 0.01) and Asian ethnicity compared with White (p-interaction < 0.01). A weaker or lack of association with term, compared to preterm, stillbirth was observed for older maternal age, smoking and pre-eclampsia. CONCLUSION: Differences in association exist between mothers experiencing preterm and term stillbirth. These differences could contribute to design of timely surveillance and interventions to further mitigate the risk of stillbirth
Using engineering design tools in multidisciplinary distributed student teams
Collaborative design practice in distributed student teams is becoming more popular as technology
makes it easier to communicate ideas with others that are geographically distant. However, a challenge
for students is to use design tools which they are not familiar with. These design tools usually differ
from each other and engineers may find it much more difficult to share their ideas. This could make
the whole design process longer and less successful. Each year the University of Malta, City
University London and University of Strathclyde organise a joint collaborative design project,
involving engineering students with different disciplines and cultural backgrounds. In this paper, the
patterns of use of design tools by students to collaborate with each other are investigated. Based on
survey results of students, this paper proposes an approach which can be utilised by engineering
students to enhance collaboration in multidisciplinary distributed design teams.peer-reviewe
