Identifying as a Teacher of Reading: A Case Study of Pre-Service Teachers' Beliefs About Reading and the Teaching of Reading Over the Duration of a Required ELA Course
Teacher candidates (TCs) prior understandings and knowledge around reading instruction and reading acquisition emerge through various experiences and have the potential to contradict notions presented in the curriculum and pedagogy of English Language Arts courses. As TCs engage in coursework related to reading pedagogy, tensions and cognitive dissonance may be negotiated. This qualitative study, approached through a social constructivist framework, explored beliefs about the teaching and learning of reading held by elementary TCs. Using case study methodology, this inquiry surfaced initial and negotiated beliefs by reflecting on the following research questions: What patterns and themes in TC backgrounds appear alongside particular beliefs, held early in a required curriculum course in English Language Arts, about the nature of reading and the teaching of reading? What patterns and themes emerge in the post-course reflections of TCs regarding their ideas about reading development and reading instruction. Methods for data collection and analysis included content analysis, attribute and descriptive coding, and thematic analysis of a pre and post-survey and individual interviews. Results of this study demonstrated that early understandings held by TCs were narrow and often misaligned with current understandings of reading development and pedagogy. Post-course reflections, however, indicated negotiated beliefs, adding to previous studies that have suggested methods courses offer opportunities for revised understandings. Implications of this study relate to the importance of required reading pedagogy courses in teacher education programs as one avenue for TCs to surface, examine, and refine their understandings related to reading instruction and development
Eco-anxiety and environmental history: a forum
Environmental historians, like others who study and write about the environment, have long worked with the emotional and psychological impacts of environmental change, including grief, anxiety, rage, and despair. But the increasing prevalence of ecological anxiety in recent years, prompted by new indicators of planetary distress, suggests the need for new histories which address humans as subject together with other species to these disruptions in earth systems. We suggest that disturbed earth systems demand histories that are more fluid and more expansive, and more aware of human vulnerabilities. We present several possible modes for these histories, approaching human vulnerability with the languages of emotion and mental illness and through acute affective responses to the production of historical narratives. What, asks each contribution, do we do with these anxieties and emotions? How do we write the psychological and affective dimensions of extreme climates and weather events in contemporary histories? Implicitly, and sometimes explicitly, temporal and spatial scale are modulated through these case studies of emotional entanglements and vulnerability
Geochemistry of vent fluid particles formed during initial hydrothermal fluid–seawater mixing along the Mid-Atlantic Ridge
We present geochemical data of black smoker particulates filtered from hydrothermal fluids with seawater-dilutions ranging from 0–99%. Results indicate the dominance of sulphide minerals (Fe, Cu, and Zn sulphides) in all samples taken at different hydrothermal sites on the Mid-Atlantic Ridge. Pronounced differences in the geochemistry of the particles between Logatchev I and 5°S hydrothermal fields could be attributed to differences in fluid chemistry. Lower metal/sulphur ratios (Me/H2S < 1) compared to Logatchev I result in a larger amount of particles precipitated per liter fluid and the occurrence of elemental sulphur at 5°S, while at Logatchev I Fe oxides occur in larger amounts. Systematic trends with dilution degree of the fluid include the precipitation of large amounts of Cu sulphides at a low dilution and a pronounced drop with increasing dilution. Moreover, Fe (sulphides or oxides) precipitation increases with dilution of the vent fluid by seawater. Geochemical reaction path modeling of hydrothermal fluid–seawater mixing and conductive cooling indicates that Cu sulphide formation at Logatchev I and 5°S mainly occurs at high temperatures and low dilution of the hydrothermal fluid by seawater. Iron precipitation is enhanced at higher fluid dilution, and the different amounts of minerals forming at 5°S and Logatchev I are thermodynamically controlled. Larger total amounts of minerals and larger amounts of sulphide precipitate during the mixing path when compared to the cooling path. Differences between model and field observations do occur and are attributable to closed system modeling, to kinetic influences and possibly to organic constituents of the hydrothermal fluids not accounted for by the model

Immunoglobulin, glucocorticoid, or combination therapy for multisystem inflammatory syndrome in children: a propensity-weighted cohort study
Background: Multisystem inflammatory syndrome in children (MIS-C), a hyperinflammatory condition associated with SARS-CoV-2 infection, has emerged as a serious illness in children worldwide. Immunoglobulin or glucocorticoids, or both, are currently recommended treatments. Methods: The Best Available Treatment Study evaluated immunomodulatory treatments for MIS-C in an international observational cohort. Analysis of the first 614 patients was previously reported. In this propensity-weighted cohort study, clinical and outcome data from children with suspected or proven MIS-C were collected onto a web-based Research Electronic Data Capture database. After excluding neonates and incomplete or duplicate records, inverse probability weighting was used to compare primary treatments with intravenous immunoglobulin, intravenous immunoglobulin plus glucocorticoids, or glucocorticoids alone, using intravenous immunoglobulin as the reference treatment. Primary outcomes were a composite of inotropic or ventilator support from the second day after treatment initiation, or death, and time to improvement on an ordinal clinical severity scale. Secondary outcomes included treatment escalation, clinical deterioration, fever, and coronary artery aneurysm occurrence and resolution. This study is registered with the ISRCTN registry, ISRCTN69546370. Findings: We enrolled 2101 children (aged 0 months to 19 years) with clinically diagnosed MIS-C from 39 countries between June 14, 2020, and April 25, 2022, and, following exclusions, 2009 patients were included for analysis (median age 8·0 years [IQR 4·2–11·4], 1191 [59·3%] male and 818 [40·7%] female, and 825 [41·1%] White). 680 (33·8%) patients received primary treatment with intravenous immunoglobulin, 698 (34·7%) with intravenous immunoglobulin plus glucocorticoids, 487 (24·2%) with glucocorticoids alone; 59 (2·9%) patients received other combinations, including biologicals, and 85 (4·2%) patients received no immunomodulators. There were no significant differences between treatments for primary outcomes for the 1586 patients with complete baseline and outcome data that were considered for primary analysis. Adjusted odds ratios for ventilation, inotropic support, or death were 1·09 (95% CI 0·75–1·58; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids and 0·93 (0·58–1·47; corrected p value=1·00) for glucocorticoids alone, versus intravenous immunoglobulin alone. Adjusted average hazard ratios for time to improvement were 1·04 (95% CI 0·91–1·20; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids, and 0·84 (0·70–1·00; corrected p value=0·22) for glucocorticoids alone, versus intravenous immunoglobulin alone. Treatment escalation was less frequent for intravenous immunoglobulin plus glucocorticoids (OR 0·15 [95% CI 0·11–0·20]; p<0·0001) and glucocorticoids alone (0·68 [0·50–0·93]; p=0·014) versus intravenous immunoglobulin alone. Persistent fever (from day 2 onward) was less common with intravenous immunoglobulin plus glucocorticoids compared with either intravenous immunoglobulin alone (OR 0·50 [95% CI 0·38–0·67]; p<0·0001) or glucocorticoids alone (0·63 [0·45–0·88]; p=0·0058). Coronary artery aneurysm occurrence and resolution did not differ significantly between treatment groups. Interpretation: Recovery rates, including occurrence and resolution of coronary artery aneurysms, were similar for primary treatment with intravenous immunoglobulin when compared to glucocorticoids or intravenous immunoglobulin plus glucocorticoids. Initial treatment with glucocorticoids appears to be a safe alternative to immunoglobulin or combined therapy, and might be advantageous in view of the cost and limited availability of intravenous immunoglobulin in many countries. Preservice Teachers Defining and Redefining Reading and the Teaching of Reading
This qualitative study was developed to explore changing beliefs about literacy teaching and learning in the context of one required introductory English Language Arts course. Research questions targeted shifts in individual as well as group responses from a current class of teacher candidates interested in PreK-Grade 8 classroom teaching. Research methods included pre- and post-course surveys, and reflective narrative statements on mid-term “exit cards”; these tools were designed to support course improvement as well as gather data that might facilitate the development of other courses and programs. Important implications emerged for teacher preparation in the area of literacy education. Many of the teacher candidates reported transformation of ideas in relation to the conceptualization of reading and the teaching of reading, shifting from word-oriented approaches to meaning-oriented approaches. Offering opportunities to recall, unpack, and share experiences with other teacher candidates appeared to provide a useful context for interrogating and reinforcing perspectives, or adopting new beliefs, about the teaching of reading.</jats:p
Chorioamnionitis Induces a Specific Signature of Placental ABC Transporters Associated with an Increase of miR-331-5p in the Human Preterm Placenta
Background/Aims: The ATP-binding cassette (ABC) transporters mediate drug biodisposition and immunological responses in the placental barrier. In vitro infective challenges alter expression of specific placental ABC transporters. We hypothesized that chorioamnionitis induces a distinct pattern of ABC transporter expression. Methods: Gene expression of 50 ABC transporters was assessed using TaqMan® Human ABC Transporter Array, in preterm human placentas without (PTD; n=6) or with histological chorioamnionitis (PTDC; n=6). Validation was performed using qPCR, immunohistochemistry and Western blot. MicroRNAs known to regulate P-glycoprotein (P-gp) were examined by qPCR. Results: Up-regulation of ABCB9, ABCC2 and ABCF2 mRNA was detected in chorioamnionitis (p<0.05), whereas placental ABCB1 (P-gp; p=0.051) and ABCG2 (breast cancer resistance protein-BCRP) mRNA levels (p=0.055) approached near significant up-regulation. In most cases, the magnitude of the effect significantly correlated to the severity of inflammation. Upon validation, increased placental ABCB1 and ABCG2 mRNA levels (p<0.05) were observed. At the level of immunohistochemistry, while BCRP was increased (p<0.05), P-gp staining intensity was significantly decreased (p<0.05) in PTDC. miR-331-5p, involved in P-gp suppression, was upregulated in PTDC (p<0.01) and correlated to the grade of chorioamnionitis (p<0.01). Conclusions: Alterations in the expression of ABC transporters will likely lead to modified transport of clinically relevant compounds at the inflamed placenta. A better understanding of the potential role of these transporters in the events surrounding PTD may also enable new strategies to be developed for prevention and treatment of PTD
Development and psychometric property testing of a skin tear knowledge assessment instrument (OASES) in 37 countries
Aim: To develop and psychometrically evaluate a skin tear knowledge assessment
instrument (OASES).
Design: Prospective psychometric instrument validation study.
Method: The skin tear knowledge assessment instrument was developed based on
a literature review and expert input (N = 19). Face and content validity were assessed in a two-round Delphi procedure by 10 international experts affiliated with
the International Skin Tear Advisory Panel (ISTAP). The instrument was psychometrically tested in a convenience sample of 387 nurses in 37 countries (April–May 2020).
Validity of the multiple-choice test items (item difficulty, discriminating index, quality
of the response alternatives), construct validity, and test–retest reliability (stability)
were analysed and evaluated in light of international reference standards.
Results: A 20-item instrument, covering six knowledge domains most relevant to skin
tears, was designed. Content validity was established (CVI = 0.90–1.00). Item difficulty varied between 0.24 and 0.94 and the quality of the response alternatives between 0.01–0.52. The discriminating index was acceptable (0.19–0.77). Participants
with a theoretically expected higher knowledge level had a significantly higher total
score than participants with theoretically expected lower knowledge (p < .001). The
1-week test-retest intraclass correlation coefficient (ICC) was 0.83 (95% CI = 0.78–
0.86) for the full instrument and varied between 0.72 (95% CI = 0.64–0.79) and 0.85
(95% CI = 0.81–0.89) for the domains. Cohen's Kappa coefficients of the individual
items ranged between 0.21 and 0.74.
Conclusion: The skin tear knowledge assessment instrument is supported by acceptable psychometric properties and can be applied in nursing education, research, and
practice to assess knowledge of healthcare professionals about skin tears.
Impact: Prevention and treatment of skin tears are a challenge for healthcare professionals. The provision of adequate care is based on profound and up-to-date
knowledge. None of the existing instruments to assess skin tear knowledge is psychometrically tested, nor up-to-date. OASES can be used worldwide to identify education, practice, and research needs and priorities related to skin tears in clinical

Intraoperative transfusion practices in Europe
Transfusion of allogeneic blood influences outcome after surgery. Despite widespread availability of transfusion guidelines, transfusion practices might vary among physicians, departments, hospitals and countries. Our aim was to determine the amount of packed red blood cells (pRBC) and blood products transfused intraoperatively, and to describe factors determining transfusion throughout Europe. We did a prospective observational cohort study enrolling 5803 patients in 126 European centres that received at least one pRBC unit intraoperatively, during a continuous three month period in 2013. The overall intraoperative transfusion rate was 1.8%; 59% of transfusions were at least partially initiated as a result of a physiological transfusion trigger- mostly because of hypotension (55.4%) and/or tachycardia (30.7%). Haemoglobin (Hb)- based transfusion trigger alone initiated only 8.5% of transfusions. The Hb concentration [mean (sd)] just before transfusion was 8.1 (1.7) g dl and increased to 9.8 (1.8) g dl after transfusion. The mean number of intraoperatively transfused pRBC units was 2.5 (2.7) units (median 2). Although European Society of Anaesthesiology transfusion guidelines are moderately implemented in Europe with respect to Hb threshold for transfusion (7-9 g dl), there is still an urgent need for further educational efforts that focus on the number of pRBC units to be transfused at this threshold
