Population-level risks of alcohol consumption by amount, geography, age, sex, and year: a systematic analysis for the Global Burden of Disease Study 2020
Background: The health risks associated with moderate alcohol consumption continue to be debated. Small amounts of alcohol might lower the risk of some health outcomes but increase the risk of others, suggesting that the overall risk depends, in part, on background disease rates, which vary by region, age, sex, and year. Methods: For this analysis, we constructed burden-weighted dose–response relative risk curves across 22 health outcomes to estimate the theoretical minimum risk exposure level (TMREL) and non-drinker equivalence (NDE), the consumption level at which the health risk is equivalent to that of a non-drinker, using disease rates from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2020 for 21 regions, including 204 countries and territories, by 5-year age group, sex, and year for individuals aged 15–95 years and older from 1990 to 2020. Based on the NDE, we quantified the population consuming harmful amounts of alcohol. Findings: The burden-weighted relative risk curves for alcohol use varied by region and age. Among individuals aged 15–39 years in 2020, the TMREL varied between 0 (95% uncertainty interval 0–0) and 0·603 (0·400–1·00) standard drinks per day, and the NDE varied between 0·002 (0–0) and 1·75 (0·698–4·30) standard drinks per day. Among individuals aged 40 years and older, the burden-weighted relative risk curve was J-shaped for all regions, with a 2020 TMREL that ranged from 0·114 (0–0·403) to 1·87 (0·500–3·30) standard drinks per day and an NDE that ranged between 0·193 (0–0·900) and 6·94 (3·40–8·30) standard drinks per day. Among individuals consuming harmful amounts of alcohol in 2020, 59·1% (54·3–65·4) were aged 15–39 years and 76·9% (73·0–81·3) were male. Interpretation: There is strong evidence to support recommendations on alcohol consumption varying by age and location. Stronger interventions, particularly those tailored towards younger individuals, are needed to reduce the substantial global health loss attributable to alcohol. Global disparities in surgeons’ workloads, academic engagement and rest periods: the on-calL shIft fOr geNEral SurgeonS (LIONESS) study
: The workload of general surgeons is multifaceted, encompassing not only surgical procedures but also a myriad of other responsibilities. From April to May 2023, we conducted a CHERRIES-compliant internet-based survey analyzing clinical practice, academic engagement, and post-on-call rest. The questionnaire featured six sections with 35 questions. Statistical analysis used Chi-square tests, ANOVA, and logistic regression (SPSS® v. 28). The survey received a total of 1.046 responses (65.4%). Over 78.0% of responders came from Europe, 65.1% came from a general surgery unit; 92.8% of European and 87.5% of North American respondents were involved in research, compared to 71.7% in Africa. Europe led in publishing research studies (6.6 ± 8.6 yearly). Teaching involvement was high in North America (100%) and Africa (91.7%). Surgeons reported an average of 6.7 ± 4.9 on-call shifts per month, with European and North American surgeons experiencing 6.5 ± 4.9 and 7.8 ± 4.1 on-calls monthly, respectively. African surgeons had the highest on-call frequency (8.7 ± 6.1). Post-on-call, only 35.1% of respondents received a day off. Europeans were most likely (40%) to have a day off, while African surgeons were least likely (6.7%). On the adjusted multivariable analysis HDI (Human Development Index) (aOR 1.993) hospital capacity > 400 beds (aOR 2.423), working in a specialty surgery unit (aOR 2.087), and making the on-call in-house (aOR 5.446), significantly predicted the likelihood of having a day off after an on-call shift. Use of feed additives in animal agriculture: gut response in broilers
Feed additives such as enzymes, probiotics, prebiotics and acidifiers are few examples of suitable alternatives to in – feed antibiotics in animal agriculture due to the ban of antibiotic use. However, the Nigerian poultry industry still experiences cases of antibiotic use. The gut benefits of alternatives to antibiotics often culminate in improved growth performance but there are or may be certain gut responses that attribute to the observed performance of poultry birds. In order to ascertain this, an experiment was conducted to evaluate the effect of antibiotic administration and enzyme supplementation on gut pH and bacteria counts. One hundred and fifty day-old broiler chicks were randomly distributed to three dietary treatments having five replicates and 10 birds per replicate. The experiment lasted forty – two (42) days and was arranged as a completely randomized design. A maize- soybean meal diet not supplemented with antibiotic or enzyme served as the control. Birds fed diet II had antibiotic (Dicoxin plus ®) administered to them. Birds fed diet III had their diets supplemented with enzyme (Roxazyme G2G®). Results indicated pH of the crop to be significantly (P<0.01) reduced by feed additive. The pH of the Ileum was least (5.65) under antibiotic administration but was not significantly different (p>0.05) different that of birds fed control and enzyme supplemented diet. Gizzard pH was acidic but not significantly (P>0.05) across all the treatments. Lactobacillus and Coliform counts were significant (P<0.05) affected by feed additive addition. It can be concluded that feed additive impact on gut pH can influence the type and population of bacteria present in the gastrointestinal tract
Piloting Community Rangeland Health Workers (CRHWs) in Ethiopia: A ground-breaking innovation for One Health application in pastoral areas
Relationships between Plant Biomass and Species Richness under Different Farming Systems and Grazing Land Management in Montane Grasslands of Kokosa District, Southern Ethiopia
The study was conducted in a montane grassland of Kokosa District, West Arsi Zone of Oromia Region, southern Ethiopia. The objective of the study was to investigate the relationships between aboveground plant biomass and species richness in three farming systems and four grazing management systems. A total of 180 quadrats (each 1 m x 1 m) were sampled in the three farming systems (dominant livestock-enset, enset-livestock and enset-livestock-cereal) and four grazing management systems (communal, enclosure, stream bank and benchmark). All the farming system and grazing management have different management practices. Plant species composition and aboveground plant biomass at different sites were quantified. Altogether 50 plant species (34 grasses, 4 legumes, 3 sedges and 9 forbs) were recorded in the montane grassland of Kokosa District. Even though the majority of the plant species share the different farming systems and grazing management practices, the highest number of species (39) was recorded in the enset-livestock farming system, whereas the lowest (33) species were recorded in the enset- livestock-cereal farming system when all the grazing management and farming systems were combined. Significantly, the highest species richness (4.9 species m-2) was recorded in the enclosure grazing management site whereas the lowest (3.4 species m-2) was recorded in the benchmark grazing site when all grazing sites and farming systems were combined. The relationship between species richness and biomass was detected in the montane grassland. There was significant difference (P ≤ 0.05) in species richness for a combination analysis of farming system by grazing management system of the montane grassland. The highest biomass was recorded in the benchmark grazing management sites while the lowest was recorded in the communal grazing sites. On the contrary, maximum species richness was found in the enclosure grazing management sites which had intermediate biomass yield and the lowest species richness was recorded in the benchmark grazing areas with the maximum biomass records. Thus, species richness was observed first to ascend along with biomass increment up to 1932 kg ha-1 and then declined at constant increase of biomass. An increase in biomass in the benchmark grazing sites was not accompanied by an increase in species richness suggesting the dominance of few species in these sites. Livestock-rangeland management practices and community perceptions towards rangeland degradation in South Omo zone of Southern Ethiopia
A survey was conducted in Hamer and Benna-Tsemay districts of the South Omo zone of Ethiopia, with the objectives of assessing the range-livestock management practices and perceptions of the different pastoral groups (Hamer, Benna, and Tsemay) towards rangeland degradation. This information is considered to be vital to future pastoral development planning and interventions. The information was gathered through group discussions, personal observations, and using a structured questionnaire where each household was taken as a unit of analysis. The average family size per household was for Hamer = 7.05 for Benna = 7.93 and for Tsemay = 7 with nearly 98.1% of the respondents without any kind of education. All pastoral groups derived their main income from the sale of animals, which was followed by the sale of honey as in the case of Hamer and Tsemay pastoralists. The average livestock per household was 25.7, 10 and 2.8 tropical livestock unit (TLU) cattle, goat and sheep, respectively. The major livestock production constraints were drought, feed and water shortage and animal health problems. The different pastoral groups have the opinion that the condition of their rangeland is poor, mainly due to overgrazing, drought and increase in human population. Furthermore, there was also a problem of bush encroachment which is an indicator of rangeland degradation. There are no range improvement practices undertaken to improve the condition of the rangelands. Mobility is the first measure taken to solve shortage of livestock feed and water but many of the pastoralists replied that they face many problems during migrations. Because of the unfavorable climatic condition for cultivation, most of the respondents of Hamer and Tsemay pastoralists and about 35% from Benna still prefer communal land tenure, where resources are shared. In conclusion, the indigenous knowledge of the pastoralists about range-livestock management and their environment should be incorporated while planning range-livestock development projects for the study districts
