    Implementation of A New Single-Stage Bridgeless Boost Half-Bridge AC/DC Converter with Bidirectional Switch

    This paper discussed on the practical implementation of a newly proposed isolated bridgeless single stage AC-DC converter. The step for the practical implementation are discussed in details. These included the design of the printed circuit board, the values of the electronic and magnetic components.  In addition, the experiment setup and procedures is also presented. This new circuit topology is implemented and tested at 115 Vac, 50 Hz of input supply and 20 Vdc output voltage with maximum output power of 100 W. It is also tested at input voltage with higher frequency of 500Hz to confirm the elimination of zero crossing of the input current

    Development of Nation State of Malaysia Based on Mahathir Mohamad’s Leadership Model Framework

    Each Prime Minister has his or her own leadership and achievements while leading the development of the nation-state. Mahathir Mohamad's leadership and achievements in driving Malaysia's modernization are well known because the effort has succeeded in benefiting Malaysians so that he can be appointed as the Father of Malaysian Modernization. The effectiveness of his leadership can be assessed based on the achievements that have been successfully achieved by the country through the implementation of actions and policies that he led throughout leading the country. Therefore, this article aims to discuss the elements of Mahathir's leadership by tracing his leadership when implementing actions and policies to modernize Malaysia so that it can achieve a proud achievement in the context of the development of the Malaysian nation. This study uses the method of document analysis by analyzing the text of Mahathir's speech as the main source and applying thematic analysis as an analysis approach. The text of the speech was analyzed thematically to identify the themes that exist and refer to the elements of leadership applied by him throughout the implementation of actions and policies to modernize the country. The results of the analysis show that there are 11 elements of leadership practiced by Mahathir such as injecting intellectual, injecting motivation, practicing good values in governing, valuing togetherness, exhibiting ideal influence, organized in leadership, assertive in leading, far-sighted, considerate of each individual, appreciating the contribution of the people, and confident in the ability to lead. Indirectly, the results of the analysis reflect that a country can achieve a proud achievement in the process of modernization if the country's leaders practice certain elements of leadership. The implications of this study, first, reflect the importance of a leader's leadership evaluation based on the achievements that have been achieved because it can identify a set of leadership elements practiced by the leader. Second, it reflects the importance of the role of leaders in determining and shaping the definition of nation-state development that should be highlighted in a country. The contribution of this study is to introduce a leadership model framework based on Mahathir's achievements throughout his administration of the Malaysian nation. In addition, it gives an overview of the elements of his leadership as a successful national leader. This leadership model framework is also suitable to be applied by leaders or related parties involved or interested in leadership-related discussions

    An overview of excavatability classification for bedded and non bedded rock in a tropical region

    Surface excavation works in tropically weathered rock are often reported to be challenging due to too many uncertainties such as rock mass and rock properties, environment, selection of the best excavation method, machine characteristics, cost and production rate. The purpose of this study is to present an overview of previous assessments of excavatability and demonstrate the reliability of these methods for assessing the weathered state of rocks in tropical regions. In surface excavation operations, the heterogeneity of the rock mass presents significant issues, particularly in sedimentary locations where the same rock mass contains multiple rock layers with varying weathering grades and strengths. Furthermore, the non-bedded rock mass displays a distinct profile, with boulders occurring throughout the entire zone of highly weathered rock. In tropical weathered rocks, the weathering profile of the rock mass can be very erratic, changeable, and dominant in dictating the behavior of the rock. Disputes in rock engineering properties for practitioners involved in construction work, due to weathering and other issues such as the complex behavior of soil-like and rock-like material arise at the site. These disputes affect practitioners working in the field. A critical review of the current excavatability assessment was conducted. The parameters are then contrasted with the current excavatability assessment procedure, and the outcomes are also specified. At the end of the study, the significance parameter on geological, geotechnical, and geophysics could be determined for various types of rock; thus a reliable classification system could be proposed for efficient assessment. Additionally, a classification system that takes into account the geological factors that are important for surface excavation in tropical areas is greatly needed to assess excavatability

    A series of regression models to predict the weathering index of tropical granite rock mass

    In the recent past, several weathering indicators have been developed to describe its state of weathering. The state of rock weathering is a useful indicator to estimate the integrity of tropically weathered rock material and mass which weatherability plays an important role in a tropical region. Through a ground assessment tool, the strength and durability of the rock mass could be estimated and complex or adopted to simplify the early prediction of the complex engineering parameter. This paper presents several models of the Weathering Index (WI) using selected significant parameters using statistical analysis. For this purpose, several sites have been chosen to represent granitic rock mass. Forty (40) numbers of samples were collected and tested comprising from four (4) sites in Malaysia. Several laboratory tests have been conducted such as Point Load Index (Is(50)), dry density, Slake Durability 1 (SD1), Slake Durability 2 (SD2) and moisture content. The field and laboratory data sets are used to determine the WI by using simple regression and MLR analysis Significant parameters found to be useful in determining the WI are selected namely SD1, dry density, Is(50), and block volume. These parameters were selected based on stepwise analysis using Statistical Package for the Social Sciences (SPSS). Following the models’ implementation, the models were evaluated and the best prediction model was selected after considering statistical coefficients, such as coefficient of determination (R2), variance account for (VAF), and root mean squared error (RMSE), as well as utilizing a straightforward ranking approach. The findings of this study could contribute to the more accurate prediction of WI using a more simplistic field and laboratory parameters. Therefore, the WI is useful during the initial stages and planning of rock excavation work and provides a good description of weathering grade and rock mass properties, which will affect excavatability in granitic areas

    International consensus statement on allergy and rhinology: Allergic rhinitis – 2023

    Background: In the 5 years that have passed since the publication of the 2018 International Consensus Statement on Allergy and Rhinology: Allergic Rhinitis (ICAR-Allergic Rhinitis 2018), the literature has expanded substantially. The ICAR-Allergic Rhinitis 2023 update presents 144 individual topics on allergic rhinitis (AR), expanded by over 40 topics from the 2018 document. Originally presented topics from 2018 have also been reviewed and updated. The executive summary highlights key evidence-based findings and recommendation from the full document. Methods: ICAR-Allergic Rhinitis 2023 employed established evidence-based review with recommendation (EBRR) methodology to individually evaluate each topic. Stepwise iterative peer review and consensus was performed for each topic. The final document was then collated and includes the results of this work. Results: ICAR-Allergic Rhinitis 2023 includes 10 major content areas and 144 individual topics related to AR. For a substantial proportion of topics included, an aggregate grade of evidence is presented, which is determined by collating the levels of evidence for each available study identified in the literature. For topics in which a diagnostic or therapeutic intervention is considered, a recommendation summary is presented, which considers the aggregate grade of evidence, benefit, harm, and cost. Conclusion: The ICAR-Allergic Rhinitis 2023 update provides a comprehensive evaluation of AR and the currently available evidence. It is this evidence that contributes to our current knowledge base and recommendations for patient evaluation and treatment

    Gender Gap in Parental Leave Intentions: Evidence from 37 Countries

    Despite global commitments and efforts, a gender‐based division of paid and unpaid work persists. To identify how psychological factors, national policies, and the broader sociocultural context contribute to this inequality, we assessed parental‐leave intentions in young adults (18–30 years old) planning to have children (N = 13,942; 8,880 identified as women; 5,062 identified as men) across 37 countries that varied in parental‐leave policies and societal gender equality. In all countries, women intended to take longer leave than men. National parental‐leave policies and women's political representation partially explained cross‐national variations in the gender gap. Gender gaps in leave intentions were paradoxically larger in countries with more gender‐egalitarian parental‐leave policies (i.e., longer leave available to both fathers and mothers). Interestingly, this cross‐national variation in the gender gap was driven by cross‐national variations in women's (rather than men's) leave intentions. Financially generous leave and gender‐egalitarian policies (linked to men's higher uptake in prior research) were not associated with leave intentions in men. Rather, men's leave intentions were related to their individual gender attitudes. Leave intentions were inversely related to career ambitions. The potential for existing policies to foster gender equality in paid and unpaid work is discussed

    The global burden of adolescent and young adult cancer in 2019:a systematic analysis for the Global Burden of Disease Study 2019

    Background In estimating the global burden of cancer, adolescents and young adults with cancer are often overlooked, despite being a distinct subgroup with unique epidemiology, clinical care needs, and societal impact. Comprehensive estimates of the global cancer burden in adolescents and young adults (aged 15-39 years) are lacking. To address this gap, we analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, with a focus on the outcome of disability-adjusted life-years (DALYs), to inform global cancer control measures in adolescents and young adults. Methods Using the GBD 2019 methodology, international mortality data were collected from vital registration systems, verbal autopsies, and population-based cancer registry inputs modelled with mortality-to-incidence ratios (MIRs). Incidence was computed with mortality estimates and corresponding MIRs. Prevalence estimates were calculated using modelled survival and multiplied by disability weights to obtain years lived with disability (YLDs). Years of life lost (YLLs) were calculated as age-specific cancer deaths multiplied by the standard life expectancy at the age of death. The main outcome was DALYs (the sum of YLLs and YLDs). Estimates were presented globally and by Socio-demographic Index (SDI) quintiles (countries ranked and divided into five equal SDI groups), and all estimates were presented with corresponding 95% uncertainty intervals (UIs). For this analysis, we used the age range of 15-39 years to define adolescents and young adults. Findings There were 1.19 million (95% UI 1.11-1.28) incident cancer cases and 396 000 (370 000-425 000) deaths due to cancer among people aged 15-39 years worldwide in 2019. The highest age-standardised incidence rates occurred in high SDI (59.6 [54.5-65.7] per 100 000 person-years) and high-middle SDI countries (53.2 [48.8-57.9] per 100 000 person-years), while the highest age-standardised mortality rates were in low-middle SDI (14.2 [12.9-15.6] per 100 000 person-years) and middle SDI (13.6 [12.6-14.8] per 100 000 person-years) countries. In 2019, adolescent and young adult cancers contributed 23.5 million (21.9-25.2) DALYs to the global burden of disease, of which 2.7% (1.9-3.6) came from YLDs and 97.3% (96.4-98.1) from YLLs. Cancer was the fourth leading cause of death and tenth leading cause of DALYs in adolescents and young adults globally. Interpretation Adolescent and young adult cancers contributed substantially to the overall adolescent and young adult disease burden globally in 2019. These results provide new insights into the distribution and magnitude of the adolescent and young adult cancer burden around the world. With notable differences observed across SDI settings, these estimates can inform global and country-level cancer control efforts. Copyright (C) 2021 The Author(s). Published by Elsevier Ltd

    Subnational mapping of HIV incidence and mortality among individuals aged 15–49 years in sub-Saharan Africa, 2000–18 : a modelling study

    Background: High-resolution estimates of HIV burden across space and time provide an important tool for tracking and monitoring the progress of prevention and control efforts and assist with improving the precision and efficiency of targeting efforts. We aimed to assess HIV incidence and HIV mortality for all second-level administrative units across sub-Saharan Africa. Methods: In this modelling study, we developed a framework that used the geographically specific HIV prevalence data collected in seroprevalence surveys and antenatal care clinics to train a model that estimates HIV incidence and mortality among individuals aged 15–49 years. We used a model-based geostatistical framework to estimate HIV prevalence at the second administrative level in 44 countries in sub-Saharan Africa for 2000–18 and sought data on the number of individuals on antiretroviral therapy (ART) by second-level administrative unit. We then modified the Estimation and Projection Package (EPP) to use these HIV prevalence and treatment estimates to estimate HIV incidence and mortality by second-level administrative unit. Findings: The estimates suggest substantial variation in HIV incidence and mortality rates both between and within countries in sub-Saharan Africa, with 15 countries having a ten-times or greater difference in estimated HIV incidence between the second-level administrative units with the lowest and highest estimated incidence levels. Across all 44 countries in 2018, HIV incidence ranged from 2 ·8 (95% uncertainty interval 2·1–3·8) in Mauritania to 1585·9 (1369·4–1824·8) cases per 100 000 people in Lesotho and HIV mortality ranged from 0·8 (0·7–0·9) in Mauritania to 676· 5 (513· 6–888·0) deaths per 100 000 people in Lesotho. Variation in both incidence and mortality was substantially greater at the subnational level than at the national level and the highest estimated rates were accordingly higher. Among second-level administrative units, Guijá District, Gaza Province, Mozambique, had the highest estimated HIV incidence (4661·7 [2544·8–8120·3]) cases per 100000 people in 2018 and Inhassunge District, Zambezia Province, Mozambique, had the highest estimated HIV mortality rate (1163·0 [679·0–1866·8]) deaths per 100 000 people. Further, the rate of reduction in HIV incidence and mortality from 2000 to 2018, as well as the ratio of new infections to the number of people living with HIV was highly variable. Although most second-level administrative units had declines in the number of new cases (3316 [81· 1%] of 4087 units) and number of deaths (3325 [81·4%]), nearly all appeared well short of the targeted 75% reduction in new cases and deaths between 2010 and 2020. Interpretation: Our estimates suggest that most second-level administrative units in sub-Saharan Africa are falling short of the targeted 75% reduction in new cases and deaths by 2020, which is further compounded by substantial within-country variability. These estimates will help decision makers and programme implementers expand access to ART and better target health resources to higher burden subnational areas

    International expert consensus on the current status and future prospects of artificial intelligence in metabolic and bariatric surgery

    Artificial intelligence (AI) is transforming the landscape of medicine, including surgical science and practice. The evolution of AI from rule-based systems to advanced machine learning and deep learning algorithms has opened new avenues for its application in metabolic and bariatric surgery (MBS). AI has the potential to enhance various aspects of MBS, including education and training, decision-making, procedure planning, cost and time efficiency, optimization of surgical techniques, outcome and complication prediction, patient education, and access to care. However, concerns persist regarding the reliability of AI-generated decisions and associated ethical considerations. This study aims to establish a consensus on the role of AI in MBS using a modified Delphi method. A panel of 68 leading metabolic and bariatric surgeons from 35 countries participated in this consensus-building process, providing expert insights into the integration of AI in MBS. Of the 28 statements evaluated, a consensus of at least 70% was achieved for all, with 25 statements reaching consensus in the first round and the remaining three in the second round. Experts agreed that AI has the potential to enhance the evaluation of surgical skills in MBS by providing objective, detailed assessments, enabling personalized feedback, and accelerating the learning curve. Most experts also recognized AI’s role in identifying qualified candidates for MBS referrals, helping patient and procedure selection, and addressing specific clinical questions. However, concerns were raised about the potential overreliance on AI-generated recommendations. The consensus emphasized the need for ethical guidelines governing AI use and the inclusion of AI’s role in decision-making within the patient consent process. Furthermore, the results suggest that AI education should become an essential component of future surgical training. Advancements in AI-driven robotics and AI-integrated genomic applications were also identified as promising developments that could significantly shape the future of MBS
