Comparison between dopaminergic agents and physical exercise as treatment for periodic limb movements in patients with spinal cord injury
Study design: Randomized controlled trial of physical exercise and dopaminergic agonist in persons with spinal cord injury and periodic leg movement (PLM).Objective: the objective of the present study was to compare the effectiveness of physical exercise and of a dopaminergic agonist in reducing the frequency of PLM.Setting: Centro de Estudos em Psicobiologia e Exercicio. Universidade Federal de São Paulo, Brazil.Methods: A total of 13 volunteers (mean age: 31.6 +/- 8.3 years) received L-DOPA ( 200 mg) and benserazide ( 50 mg) 1 h before sleeping time for 30 days and were then submitted to a physical exercise program on a manual bicycle ergometer for 45 days ( 3 times a week).Results: Both L-DOPA administration (35.11 - 19.87 PLM/h, P<0.03) and physical exercise (35.11 - 18.53 PLM/h, P<0.012) significantly reduced PLM; however, no significant difference was observed between the two types of treatment.Conclusions: the two types of treatment were found to be effective in the reduction of PLM; however, physical exercise is indicated as the first treatment approach, while dopaminergic agonists or other drugs should only be recommended for patients who do not respond to this type of treatment.UNIFESP, Dept Psychobiol, BR-04020060 São Paulo, BrazilUNIFESP, Dept Psychobiol, BR-04020060 São Paulo, BrazilWeb of Scienc
Quality of Life as an outcome in Alzheimer's disease and other dementias- obstacles and goals
<p>Abstract</p> <p>Background</p> <p>The number of individuals at risk for dementia will probably increase in ageing societies as will the array of preventive and therapeutic options, both however within limited economic resources. For economic and medical purposes valid instruments are required to assess disease processes and the efficacy of therapeutic interventions for different forms and stages of illness. In principal, the impact of illness and success of an intervention can be assessed with biomedical variables, e.g. severity of symptoms or frequency of complications of a disease. However, this does not allow clear judgement on clinical relevance or comparison across different diseases.</p> <p>Discussion</p> <p>Outcome model variables such as quality of life (QoL) or health care resource utilization require the patient to appraise their own well-being or third parties to set preferences. In Alzheimer's disease and other dementias the evaluation process performed by the patient is subject to the disease process itself because over progress of the disease neuroanatomical structures are affected that mediate evaluation processes.</p> <p>Summary</p> <p>Published research and methodological considerations thus lead to the conclusion that current QoL-instruments, which have been useful in other contexts, are ill-suited and insufficiently validated to play a major role in dementia research, decision making and resource allocation. New models integrating biomedical and outcome variables need to be developed in order to meet the upcoming medical and economic challenges.</p
In Silico Identification of Specialized Secretory-Organelle Proteins in Apicomplexan Parasites and In Vivo Validation in Toxoplasma gondii
Apicomplexan parasites, including the human pathogens Toxoplasma gondii and Plasmodium falciparum, employ specialized secretory organelles (micronemes, rhoptries, dense granules) to invade and survive within host cells. Because molecules secreted from these organelles function at the host/parasite interface, their identification is important for understanding invasion mechanisms, and central to the development of therapeutic strategies. Using a computational approach based on predicted functional domains, we have identified more than 600 candidate secretory organelle proteins in twelve apicomplexan parasites. Expression in transgenic T. gondii of eight proteins identified in silico confirms that all enter into the secretory pathway, and seven target to apical organelles associated with invasion. An in silico approach intended to identify possible host interacting proteins yields a dataset enriched in secretory/transmembrane proteins, including most of the antigens known to be engaged by apicomplexan parasites during infection. These domain pattern and projected interactome approaches significantly expand the repertoire of proteins that may be involved in host parasite interactions

Associations between maternal metabolic parameters during pregnancy and fetal and child growth trajectories from 20 weeks' gestation to 5 years of age: Secondary analysis from the ROLO longitudinal birth cohort study
OBJECTIVE: To examine the association between maternal metabolic parameters in pregnancy and growth trajectories up to 5 years of age. METHODS: Data from mother-child pairs who participated in the ROLO study, a randomized trial examining the impact of a low glycaemic index diet on the recurrence of macrosomia, were analysed. Fetal and child growth trajectories were developed from longitudinal measurements from 20 weeks gestation up to 5 years of age. We examined associations between maternal fasting glucose, insulin, HOMA-IR and leptin, taken in early pregnancy (14-16 weeks) and late pregnancy (28 weeks), and weight (kg) and abdominal circumference (cm) trajectories using linear spline multilevel models. RESULTS: We found no strong evidence of associations between any maternal metabolic parameters and fetal to childhood weight and abdominal circumference trajectories from 20 weeks gestation to 5 years. CONCLUSION: In a cohort of women with obesity with infants at risk of macrosomia, maternal metabolic markers were not strongly associated with trajectories of weight or abdominal circumference from 20 weeks gestation to 5 years of age

The main post-polio syndrome (PPS) symptoms are new-onset weakness, new-onset atrophy, fatigue, cold intolerance, and pain associated with sleep disturbances. The polysomnographic study is the gold pattern to analyze sleep disorders. OBJECTIVE: To assess pain, intolerance to cold and periodic limb movements (PLM) index before and after the use of MIG3 bioceramic fabrics over 4 weeks. METHOD: 12 patients with PPS from UNIFESP/EPM. All patients were submitted to polysomnography and infra-red examinations with answered scales of pain and intolerance to cold before and after the use of MIG3 bioceramics fabrics. RESULTS: There were significant decreases in pain and PLM index. CONCLUSION: MIG3 bioceramic fabrics can help in the treatment of pain and PLM in PPS patients.<br>Dentre as manifestações clínicas da SPP destacam-se nova fraqueza, fadiga, dor, intolerância ao frio, nova atrofia e transtornos do sono. A polissonografia de noite inteira permanece sendo padrão ouro para análise do sono e diagnóstico de transtornos do sono. OBJETIVO: Avaliar o comportamento da dor, intolerância ao frio (IF) e índice de movimentos periódicos de membros (PLMs) após uso de colchonete e roupas com biocerâmica MIG3 por 4 semanas. MÉTODO: 12 pacientes com SPP, da UNIFESP/EPM. Todos realizaram exames de polissonografia e infravermelho e responderam a questionários de dor e IF antes e após o uso dos materiais. RESULTADOS: Houve diminuição significativa da dor e dos PLMs. CONCLUSÃO: Os tecidos com biocerâmica MIG3 podem ser um coadjuvante ao tratamento da dor e dos PLMs nos pacientes com SPP
Lipoma do ângulo pontocerebelar: relato de caso Lipoma of the cerebellopontine angle: case report
Os lipomas do ângulo pontocerebelar são tumores muito raros. Em geral são assintomáticos, achados fortuitos em autópsia, tomografia computadorizada (TC) de crânio ou ressonância nuclear magnética do encéfalo. Os autores apresentam o caso de paciente, feminina, branca, 14 anos, há três anos com diminuição da audição à esquerda e há um ano com cefaléia, tipo hemicrânia à esquerda. A TC de crânio evidenciou lesão hipodensa, sem captação de contraste, no ângulo pontocerebelar esquerdo. A paciente foi submetida a cirurgia por via retrossigmóidea para abordagem da lesão, que se apresentou de coloração amarelada, característica de tecido adiposo. Tendo em vista o envolvimento de estruturas nervosas (VII e VIII nervos cranianos), optou-se pela exérese parcial da lesão. A paciente evoluiu com melhora da cefaléia, sem piora do déficit auditivo. Após três anos de seguimento, mantém o quadro estável. Conclui-se que os lipomas do ângulo pontocerebelar, quando assintomáticos, podem ser tratados conservadoramente. Entretanto aqueles associados a sintomatologia persistente e progressiva devem ser operados. A exérese total ou parcial vai depender do envolvimento ou não das estruturas neurovasculares adjacentes.<br>Lipoma of the cerebellopontine angle is a very rare tumor. We report the case of a 14-years-old female, with left side deafness during three years, associated with headache. CT scan showed an hypodense mass, without enhancement at the cerebellopontine angle. The patient was treated surgically by left retrosigmoid approach. The lesion involved the eighth and seventh cranial nerves and only a partial removal was performed. The postoperative course was uneventful. She had no more headache; the deafness of the left side remained unchanged. Asymptomatic lipoma of the cerebellopontine angle can be treated conservatively, although those with progressive symptoms should be treated surgically, with total or partial remove based on their neurovascular involvement
