SARS-CoV-2: The Monster Causes COVID-19
Coronaviruses are viruses whose particles look like crowns. SARS-CoV-2 is the seventh member of the human coronavirus family to cause COVID-19 which is regarded as a once-in-a-century pandemic worldwide. It holds has the characteristics of a pandemic, which has broy -55ught many serious negative impacts to human beings. It may take time for humans to fight the pandemic. In addition to humans, SARS-CoV-2 also infects animals such as cats. This review introduces the origins, structures, pathogenic mechanisms, characteristics of transmission, detection and diagnosis, evolution and variation of SARS-CoV-2. We summarized the clinical characteristics, the strategies for treatment and prevention of COVID-19, and analyzed the problems and challenges we face
Case Report: Recurrent Autoimmune Hypoglycemia Induced by Non-Hypoglycemic Medications
We present a case of recurrent autoimmune hypoglycemia induced by non-hypoglycemic agents. We review reported cases of autoimmune hypoglycemia related to non-hypoglycemic agents, and discuss the effects of different detection methods for insulin autoantibodies on the results obtained. We aim to provide information for clinicians and a warning for medication usage. Considering the increasing number of clopidogrel-induced AIH cases and the hypoglycemia-induced increase in the risk of cardiovascular events, we recommend that cardiovascular disease patients being treated with clopidogrel be informed of this rare side effect and that clinicians be vigilant for the possibility of autoimmune hypoglycemia in this patient population
Impact of rigid container material type and loading volume on the sterilization of thoracoscopic instruments
ObjectiveTo explore the impacts of material type and loading volume of rigid containers on the hydrogen peroxide low temperature plasma sterilization of thoracoscopic instruments, to identify the best rigid containers and loading volume of thoracoscopic instruments.MethodsThoracoscopic instruments sterilized by STERRAD® 100NX hydrogen peroxide low temperature plasma in Shanghai Pulmonary Hospital affiliated to Tongji University from August to September 2024 were selected as the research items. According to the material of rigid containers, the instruments were divided into polyethylene case group (A), stainless steel case group (B) and silicone resin case group (C). In terms of the loading volume, the rigid containers were divided into (loading capacity 0.05).ConclusionWhen using hydrogen peroxide low temperature plasma to sterilize thoracoscopic instruments, it is recommended to use stainless steel or silicone resin rigid containers with a controlled loading capacity (≤12) to ensure optimal sterilization quality
A real-world clinicopathological model for predicting pathological complete response to neoadjuvant chemotherapy in breast cancer
PurposeThis study aimed to develop and validate a clinicopathological model to predict pathological complete response (pCR) to neoadjuvant chemotherapy (NAC) in breast cancer patients and identify key prognostic factors.MethodsThis retrospective study analyzed data from 279 breast cancer patients who received NAC at Zhejiang Provincial People’s Hospital from 2011 to 2021. Additionally, an external validation dataset, comprising 50 patients from Lanxi People’s Hospital and Second Affiliated Hospital, Zhejiang University School of Medicine from 2022 to 2023 was utilized for model verification. A multivariate logistic regression model was established incorporating clinical, ultrasound features, circulating tumor cells (CTCs), and pathology variables at baseline and post-NAC. Model performance for predicting pCR was evaluated. Prognostic factors were identified using survival analysis.ResultsIn the 279 patients enrolled, a pathologic complete response (pCR) rate of 27.96% (78 out of 279) was achieved. The predictive model incorporated independent predictors such as stromal tumor-infiltrating lymphocyte (sTIL) levels, Ki-67 expression, molecular subtype, and ultrasound echo features. The model demonstrated strong predictive accuracy for pCR (C-statistics/AUC 0.874), especially in human epidermal growth factor receptor 2 (HER2)-enriched (C-statistics/AUC 0.878) and triple-negative (C-statistics/AUC 0.870) subtypes, and the model performed well in external validation data set (C-statistics/AUC 0.836). Incorporating circulating tumor cell (CTC) changes post-NAC and tumor size changes further improved predictive performance (C-statistics/AUC 0.945) in the CTC detection subgroup. Key prognostic factors included tumor size >5cm, lymph node metastasis, sTIL levels, estrogen receptor (ER) status and pCR. Despite varied pCR rates, overall prognosis after standard systemic therapy was consistent across molecular subtypes.ConclusionThe developed predictive model showcases robust performance in forecasting pCR in NAC-treated breast cancer patients, marking a step toward more personalized therapeutic strategies in breast cancer
Deep auto-encoder network for mechanical fault diagnosis of high-voltage circuit breaker operating mechanism
To improve the accuracy of the mechanical fault diagnosis of the operating mechanism and fully exploit the characteristic information in the vibration signal of the high-voltage circuit breaker, a mechanical fault diagnosis method of the operating mechanism of the high-voltage circuit breaker based on the deep self-encoding network is proposed. First, the vibration signal of the switch operating mechanism is extracted, the wavelet packet conversion is performed, and the vibration signal of each frequency band is divided into equal times. The energy of the time–frequency subplane of the vibration signal is then calculated, and the time–frequency energy distribution is used as a switch. Finally, a breaker failure diagnostic model based on the deep self-coding network is established. Pretraining and tuning and a 126 kV high-voltage switch are used to simulate different types of faults and validate the method. Experimental results show that this method can acquire sample failure data and perform failure diagnosis, and the diagnosis accuracy rate reaches 97.5%. The deep self-coding network can fully pierce deep information on the switch vibration signal
A Novel Remote Sensing Index for Extracting Impervious Surface Distribution from Landsat 8 OLI Imagery
The area of urban impervious surfaces is one of the most important indicators for determining the level of urbanisation and the quality of the environment and is rapidly increasing with the acceleration of urbanisation in developing countries. This paper proposes a novel remote sensing index based on the coastal band and normalised difference vegetation index for extracting impervious surface distribution from Landsat 8 multispectral remote sensing imagery. The index was validated using three images covering urban areas of China and was compared with five other typical index methods for the extraction of impervious surface distribution, namely, the normalised difference built-up index, index-based built-up index, normalised difference impervious surface index, normalised difference impervious index, and combinational built-up index. The results showed that the novel index provided higher accuracy and effectively distinguished impervious surfaces from bare soil, and the average values of the recall, precision, and F1 score for the three images were 95%, 91%, and 93%, respectively. The novel index provides better applicability in the extraction of urban impervious surface distribution from Landsat 8 multispectral remote sensing imagery.</jats:p
Primary Supratentorial Atypical Teratoid/Rhabdoid Tumor in Children
Atypical teratoid/rhabdoid tumor is a highly malignant tumor of the central nervous system, usually occurring in the posterior fossa in infants and young children. Supratentorial example is relatively rare, especially with involvement of the cerebral ventricle system. Herein, we reported 2 cases of atypical teratoid/rhabdoid tumor located in the septum pellucidum within the lateral and third ventricles and right parietooccipital region, respectively. Histopathologically, both of the tumors were composed of rhabdoid tumor cells and mesenchymal components, without primitive neuroectodermal tumor or epithelial differentiation. Immunohistochemical staining showed that these tumor cells reacted positively for vimentin, S-100 protein, synaptophysin, and neuron-specific enolase. Only 1 case was found to be epithelial membrane antigen reactive. The tumor cells lacked nuclear expression of INI1. These cases emphasize that atypical teratoid/rhabdoid tumor should be also considered in the list of differential diagnosis, even when these rhabdoid tumor cells do not arise in the predilection sites. </jats:p
