    Accuracy of user-adjusted axial length measurements by optical biometry in eyes having combined phacovitrectomy for macular-off rhegmatogenous retinal detachment

    PURPOSE: To evaluate the accuracy of user-adjusted axial length measured by optical biometry (OB) for intraocular lens (IOL) power calculations in eyes having combined phacovitrectomy for macula-off rhegmatogenous retinal detachment (RRD). SETTING: Department of Ophthalmology, Calderdale Royal Hospital, Halifax, UK. DESIGN: Prospective Retrospective case series review of 22 consecutive eyes that underwent phacovitrectomy for macula-off RRD. METHODS: Axial lengths (ALs) were measured using OB with user adjustment to identify a posterior peak corresponding to the eye’s AL and ultrasound (US). These measurements were compared and analysed for accuracy to each other and the accurate indication of the eye’s AL. RESULTS: User-adjusted OB was similar to US and post-operative OB measurements. There was no statistically significant difference between the means of the AL measurements derived from user-adjusted OB and ultrasound AL (p=0.964). User-adjusted OB was not statistically significantly different to post-operative OB (p=0.242). Compared to postoperative OB, IOL power was within 0.5 Dioptre in 12 out of 13 cases (92%; 95% confidence interval (77.8%, 100.0%) for user-adjusted OB, and in only 10 out of 13 cases (77%; 95% confidence interval (54.0%, 99.8%) of US measurements. CONCLUSIONS: User-adjusted OB may be used as an alternative method for the measurement of AL in macula-off RRD for primary repair by combined phacovitrectomy. OB will, however, require assessment of agreement with US AL in cases where a posterior peak is not easily identifiable.     Refractive change following pseudophakic vitrectomy: a retrospective review

    Background To assess the occurrence and magnitude of refractive change in pseudophakic eyes undergoing 20 gauge pars plana vitrectomy without scleral buckling and to investigate possible aetiological factors. Methods Retrospective case note review of 87 pseudophakic eyes undergoing 20 gauge pars plana vitrectomy for a variety of vitreo-retinal conditions over a three-year period. Anterior chamber depth (ACD) was measured before and after vitrectomy surgery in 32 eyes. Forty-three pseudophakic fellow eyes were used as controls. Results Eighty-seven eyes (84 patients) were included in the study. Mean spherical equivalent refraction prior to vitrectomy was -0.20 dioptres, which changed to a mean of -0.65 dioptres postoperatively (standard deviation of refractive change 0.59, range-2.13 to 0.75 dioptres) (p < 0.001). Sixty-one of the 87(70%) eyes experienced a myopic shift and 45(52%) eyes had a myopic shift of -0.5 dioptres or more. Mean fellow eye refraction was -0.19 dioptres preoperatively and -0.17 dioptres postoperatively (p = 0.14)(n = 37) Mean ACD preoperatively was 3.29 mm and postoperatively 3.27 mm (p = 0.53) (n = 32) and there was no significant change in ACD with tamponade use. Regression analysis revealed no statistically significant association between changes in anterior chamber depth, as well as a wide variety of other pre-, intra and postoperative factors examined, and the refractive change observed. Conclusion Significant refractive changes occur in some pseudophakic patients undergoing 20 g pars plana vitrectomy. The mean change observed was a small myopic shift but the range was large. The aetiology of the refractive change is uncertain

    Responsive Hydrogels Based Lens Structure with Configurable Focal Length for Intraocular Lens (IOLs) Application

    Stimuli-responsive hydrogel has attracted wide interests in bio-applications, especially in bio-optical systems that need tuning and adjustment of their optical performances. In this paper, we have investigated the materials and structural designs for the potential intraocular Lens (IOLs) design by studying swelling induced morphology changes and subsequently optical properties of ionic responsive Poly (Acrylamide-co-sodium acrylate) hydrogel. The equilibrium swelling ratio and swelling kinetics of gel were measured under both free standing and confined conditions. The Poly (Acrylamide-co-sodium acrylate) hydrogel has shown reversible swelling response to the ion concentration. Autonomous focusing of the gel lens was demonstrated under the certain ionic stimulus.
