    DCE-MRI and parametric imaging in monitoring response to neoadjuvant chemotherapy in breast carcinoma : a preliminary report

    Purpose: Neoadjuvant chemotherapy is recommended in patients with locally advanced breast cancer. Dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) enables evaluation of the tumour neovasculature that occurs prior to any volume change, which helps identify early treatment failures and allows prompt implementation of second-line therapy. Material and methods: We conducted a prospective study in 14 patients with histopathologically proven breast cancer. DCE-MRI data were acquired using multisection, T1-weighted, 3D vibe sequences with fat suppression before, during, and after IV bolus injection (0.1 mmol/kg body weight, Gadoversetamide, Optimark). Post-processing of dynamic contrast perfusion data was done with the vendor's Tissue 4D software to generate various dynamic contrast parameters, i.e. Ktrans, Kep, Ve, initial area under the time signal curve (IAUC), apparent diffusion coefficient (ADC), and enhancement curve. Patients underwent MRI examinations at baseline, and then after two cycles, and finally at completion of chemotherapy. Results: Based on Sataloff criteria for pathological responses, four patients out of 14 were responders, and 10 were non-responders. At the 2nd MRI examination, IAUC was significantly smaller in responders than in non-responders (p = 0.023). When the results of the first and second MRI examinations were compared, Kep decreased from baseline to the second MRI (p = 0.03) in non-responders and in responders (p = 0.04). This change was statistically significant in both groups. The ADC values increased significantly in responders from baseline to the third MRI (p = 0.012). Conclusions: In our study, IAUC and ADC were the only parameters that reliably differentiated responders from non-responders after two and three cycles of chemotherapy

    Distinct p21 requirements for regulating normal and self-reactive T cells through IFN-γ production.

    Self/non-self discrimination characterizes immunity and allows responses against pathogens but not self-antigens. Understanding the principles that govern this process is essential for designing autoimmunity treatments. p21 is thought to attenuate autoreactivity by limiting T cell expansion. Here, we provide direct evidence for a p21 role in controlling autoimmune T cell autoreactivity without affecting normal T cellresponses. We studied C57BL/6, C57BL/6/lpr and MRL/lpr mice overexpressing p21 in T cells, and showed reduced autoreactivity and lymphadenopathy in C57BL/6/lpr, and reduced mortality in MRL/lpr mice. p21 inhibited effector/memory CD4(+) CD8(+) and CD4(-)CD8(-) lpr T cell accumulation without altering defective lpr apoptosis. This was mediated by a previously non-described p21 function in limiting T cell overactivation and overproduction of IFN-γ, a key lupus cytokine. p21 did not affect normal T cell responses, revealing differential p21 requirements for autoreactive and normal T cell activity regulation.     Mathematical Optimization of the Tactical Allocation of Machining Resources in Aerospace Industry

    In the aerospace industry, efficient management of machining capacity is crucial to meet the required service levels to customers (which includes, measures of quality and production lead-times) and to maintain control of the tied-up working capital. We introduce a new multi-item, multi-level capacitated planning model with a medium-to-long term planning horizon. The model can be used by most companies having functional workshops where costly and/or time- and resource demanding preparations (or qualifications) are required each time a product needs to be (re)allocated to a machining resource. Our goal is to identify possible product routings through the factory which minimizes the maximum excess resource loading above a given loading threshold, while incurring as low qualification costs as possible. In Paper I (Bi-objective optimization of the tactical allocation of jobtypes to machines), we propose a new bi-objective mathematical optimization model for the Tactical Resource Allocation Problem (TRAP). We highlight some of the mathematical properties of the TRAP which are utilized to enhance the solution process. Another contribution is a modified version of the bi-directional ϵ\epsilon -constraint method especially tailored for our problem. We perform numerical tests on industrial test cases generated for our class of problem which indicates computational superiority of our method over conventional solution approaches. In Paper II (Robust optimization of a bi-objective tactical resource allocation problem with uncertain qualification costs), we address the uncertainty in the coefficients of one of the objective functions considered in the bi-objective TRAP. We propose a new bi-objective robust efficiency concept and highlight its benefits over existing robust efficiency concepts. We also suggest a solution approach for identifying all the relevant robust efficient (RE) solutions. Our proposed approach is significantly faster than an existing approach for robust bi-objective optimization problems

In Paper III, we extend the TRAP with an inventory of semi-finished as well as finished parts, resulting in a tri-objective mixed-integer (linear) programming model. We create a criterion space partitioning approach that enables solving sub-problems simultaneously. In Paper IV, using our knowledge from our previous work we embarked upon a task to generalize our findings to develop an approach for any discrete tri-objective optimization problem.     Awareness about cervical cancers among health workers in Shimla district, Himachal Pradesh, India

    Background: Cervical cancer, although largely preventable, remains the most common cause of cancer mortality among women in low-resource countries. Aim of this study was to assess knowledge and awareness of cervical cancer prevention among health workers in Shimla district, Himachal Pradesh.Methods: A cross sectional study was conducted among multipurpose health care workers (both males and females) in Shimla district, Himachal Pradesh. A self-administered close ended questionnaire was used. The questionnaire, consisted of two parts. The first part comprised of questions relating to demographic data and the second part had questions on knowledge about cervical cancers. The data was analyzed using the statistical analysis program SPSS version 16.0.  Tests used were Chi square, t test and ANOVA. A p-values <0.05 were considered  statistically significant.Results: Out of 122 subjects, there were 45 (36.9%) males and 77 (63.1%) females. The mean knowledge percent of the population was 55.9+16.4. Among the age groups it was highest for the age group of 21-30 years (62.5+23.6) and lowest for the age group of 51-60 years (53.6+16.4). Knowledge about risk factors was correctly reported by 44.3% of population. Who should be screened for cervical cancers was reported correctly by 29.5% and at what interval screening should be done was reported by 14.8%.Conclusions: Half of the population had moderate overall knowledge about cervical cancers, but the knowledge about risk factors and screening eligibility and screening interval was inadequate. So, education programs should be arranged for the health workers in order to prevent cervical cancers
