Suche nach der Ursache des chronischen Erschöpfungssyndroms: Eine Geschichte mit Fortsetzung
Zusammenfassung: Die Ursache des chronischen Erschöpfungssyndroms ist Gegenstand einer durchaus engagierten Debatte zwischen Wissenschaft, Behandlern, betroffenen Patienten und Selbsthilfegruppen, deren Form und Intensität nicht nur durch die Inhalte der bisherigen Forschungsergebnisse, sondern auch durch die dadurch erhoffte Legitimisierung bzw. befürchtete Delegitimisierung dieses Syndroms und der damit verbundenen Leiden sowie Beeinträchtigungen zu erklären ist. Anhand historischer und aktueller Beispiele somatischer Erklärungsmodelle von chronischen Erschöpfungszuständen sollen die Mechanismen dieser Abläufe beschrieben und erläutert werde

Perceiving Pitch Absolutely: Comparing absolute and relative pitch possessors in a pitch memory task
Background: The perceptual-cognitive mechanisms and neural correlates of Absolute Pitch (AP) are not fully understood. The aim of this fMRI study was to examine the neural network underlying AP using a pitch memory experiment and contrasting two groups of musicians with each other, those that have AP and those that do not. Results: We found a common activation pattern for both groups that included the superior temporal gyrus (STG) extending into the adjacent superior temporal sulcus (STS), the inferior parietal lobule (IPL) extending into the adjacent intraparietal sulcus (IPS), the posterior part of the inferior frontal gyrus (IFG), the pre-supplementary motor area (pre-SMA), and superior lateral cerebellar regions. Significant between-group differences were seen in the left STS during the early encoding phase of the pitch memory task (more activation in AP musicians) and in the right superior parietal lobule (SPL)/intraparietal sulcus (IPS) during the early perceptual phase (ITP 0–3) and later working memory/multimodal encoding phase of the pitch memory task (more activation in non-AP musicians). Non-significant between-group trends were seen in the posterior IFG (more in AP musicians) and the IPL (more anterior activations in the non-AP group and more posterior activations in the AP group). Conclusion: Since the increased activation of the left STS in AP musicians was observed during the early perceptual encoding phase and since the STS has been shown to be involved in categorization tasks, its activation might suggest that AP musicians involve categorization regions in tonal tasks. The increased activation of the right SPL/IPS in non-AP musicians indicates either an increased use of regions that are part of a tonal working memory (WM) network, or the use of a multimodal encoding strategy such as the utilization of a visual-spatial mapping scheme (i.e., imagining notes on a staff or using a spatial coding for their relative pitch height) for pitch information
Chronisches Erschöpfungssyndrom: Evidenzbasierte Psychotherapie bei chronischer organisch unklarer Erschöpfung
Zusammenfassung: Chronische Erschöpfungszustände ohne bzw. ohne adäquate somatische Ursache sind für Betroffene sowie für Behandler ein Problem, da mit dem Fehlen einer eindeutigen nosologischen Zuordnung und eines eindeutigen Ätiologiemodells auch die Behandlungsoptionen nicht immer direkt auf der Hand liegen bzw. Gegenstand von Kontroversen sind (s.Gaab, in diesem Heft). Zur Behandlung des chronischen Erschöpfungssyndroms ("chronic fatigue syndrome", CFS) werden verschiedene psychotherapeutische Ansätze neben graduierter Aktivierung vorgeschlagen. Ziel des vorliegenden Beitrags ist es, den aktuellen Stand der Evidenzbasierung von Psychotherapie bei CFS auf Basis vorliegender Metaanalysen und neuerer randomisierter kontrollierter Studien darzustellen. Robuste Evidenz zeigt sich für die kognitive Verhaltenstherapie (KVT) mit im Durchschnitt moderater Wirksamkeit im Hinblick auf die Schwere der Erschöpfungssymptomatik und die körperliche Funktionseinschränkung nach Therapieabschluss sowie in kurz- bis mittelfristigen Katamnesen. Klinisch relevante Verbesserungen sind bei 40-50% der so behandelten Personen zu beobachten. Andere Psychotherapieverfahren wurden bislang selten evaluiert - vielversprechende Ergebnisse weisen jedoch Einzelstudien zu Gesprächspsychotherapie und Psychoedukation auf. Insgesamt besteht die Notwendigkeit, in der Therapie Konzepte zur Rückfallprophylaxe stärker zu berücksichtigen und die Nachhaltigkeit der therapeutisch erzielten Verbesserungen empirisch abzusicher
Understanding Public Views about Air Quality and Air Pollution Sources in the San Joaquin Valley, California.
The San Joaquin Valley of California has poor air quality and high rates of asthma. Surveys were collected from 744 residents of the San Joaquin Valley from November 2014 to January 2015 to examine the public's views about air quality. The results of this study suggest that participants exposed to high PM2.5 (particulate matter less than 2.5 microns in size) concentrations perceived air pollution to be of the worst quality. Air quality in the San Joaquin Valley was primarily perceived as either moderate or unhealthy for sensitive groups. Females perceived air pollution to be of worse quality compared to males. Participants perceived unemployment, crime, and obesity to be the top three most serious community problems in the San Joaquin Valley. Participants viewed cars and trucks, windblown dust, and factories as the principle contributors to air pollution in the area. There is a need to continue studying public perceptions of air quality in the San Joaquin Valley with a more robust survey with more participants over several years and seasons
Orientation and temperature dependence of some mechanical properties of the single-crystal nickel-base superalloy Rene N4. 3: Tension-compression anisotropy
Single crystal superalloy specimens with various crystallographic directions along their axes were tested in compression at room temperature, 650, 760, 870, and 980 deg C. These results are compared with the tensile behavior studied previously. The alloy, Rene N4, was developed for gas turbine engine blades and has the nominal composition 3.7 Al, 4.2 Ti, 4 Ta, 0.5 Nb, 6 W, 1.5 Mo 9 Cr. 7.5 Co, balance Ni, in weight percent. Slip trace analysis showed that primary cube slip occurred even at room temperature for the 111 specimens. With increasing test temperature more orientations exhibited primary cube slip, until at 870 deg C only the 100 and 011 specimens exhibited normal octahedral slip. The yield strength for octahedral slip was numerically analysed using a model proposed by Lall, Chin, and Pope to explain deviations from Schmid's Law in the yielding behavior of a single phase Gamma prime alloy, Ni3(Al, Nb). The Schmid's Law deviations in Rene N4 were found to be largely due to a tension-compression anisotropy. A second effect, which increases trength for orientations away from 001, was found to be small in Rene N4. Analysis of recently published data on the single crystal superalloy PWA 1480 yielded the same result
Behavioral and Neural Correlates of Executive Functioning in Musicians and Non-Musicians
Executive functions (EF) are cognitive capacities that allow for planned, controlled behavior and strongly correlate with academic abilities. Several extracurricular activities have been shown to improve EF, however, the relationship between musical training and EF remains unclear due to methodological limitations in previous studies. To explore this further, two experiments were performed; one with 30 adults with and without musical training and one with 27 musically trained and untrained children (matched for general cognitive abilities and socioeconomic variables) with a standardized EF battery. Furthermore, the neural correlates of EF skills in musically trained and untrained children were investigated using fMRI. Adult musicians compared to non-musicians showed enhanced performance on measures of cognitive flexibility, working memory, and verbal fluency. Musically trained children showed enhanced performance on measures of verbal fluency and processing speed, and significantly greater activation in pre-SMA/SMA and right VLPFC during rule representation and task-switching compared to musically untrained children. Overall, musicians show enhanced performance on several constructs of EF, and musically trained children further show heightened brain activation in traditional EF regions during task-switching. These results support the working hypothesis that musical training may promote the development and maintenance of certain EF skills, which could mediate the previously reported links between musical training and enhanced cognitive skills and academic achievement
Placebos in der Psychotherapieforschung – eine systematische Analyse am Beispiel der systematischen Desensibilisierung
Hintergrund: Die Voraussetzungen der randomisierten, Placebo-kontrollierten Evaluation - die Ununterscheidbarkeit der Therapiebedingungen für die Patienten und die Verblindung der Therapeuten - sind in der Psychotherapieforschung nicht uneingeschränkt gegeben. Das Ziel dieser qualitativen systematischen Übersichtsarbeit ist es, am Beispiel der systematischen Desensibilisierung (SD) die Vorgehensweisen zur Bestimmung der Behandlungsspezifität zu beschreiben und deren theoretische und praktische Implikationen zu diskutieren. Methodik: Auf der Basis einer systematischen Literaturrecherche in PsycINFO und PubMed für den Zeitraum von 1976-2015 wurden Psychotherapiestudien gesucht, in denen Patienten mit Angstsymptomen einer SD- oder Placebo-Gruppe zugewiesen und verglichen wurden. Nach Extraktion der gewählten Moderator-Variablen konnten 11 Studien eingeschlossen werden. Ergebnisse: Die Ergebnisse zu den spezifischen Effekten der SD waren nicht einheitlich. Eine durchgeführte Moderator-Analyse zeigte, dass der Nachweis spezifischer Effekte von der Glaubwürdigkeit des Placebos und dem dabei erlebten Ausmaß emotionaler Erfahrungen abhängig ist. Schlussfolgerung: Ob ein bestimmtes psychotherapeutisches Verfahren als spezifisch einzustufen ist, hängt von der Operationalisierung der Placebo-Kontrolle ab. Insbesondere bei SD scheint die Einschränkung des zu bearbeitenden Themas und in der Folge die Unterbindung der emotionalen Erfahrung des Patienten innerhalb der Kontrollbedingung eine Voraussetzung dafür zu sein, spezifische Effekte der SD zu finden
Latino and Non-Latino Perceptions of the Air Quality in California's San Joaquin Valley.
The San Joaquin Valley (SJV) of California has poor air quality, high rates of asthma, and high rates of obesity. Informational campaigns aimed at increasing awareness of the health impacts of poor air quality and promoting behavior change need to be tailored to the specific target audiences. The study examined perceptions of air quality, perceived health impacts, and methods of accessing information about air quality between Latinos and other groups in the SJV. Residents of the SJV (n = 744) where surveyed via one of three methods: community organizations (256), public locations (251), and an internet panel (237). The results suggest that people perceive the air quality in their region to be generally unhealthy, particularly for sensitive groups. The air quality is more likely to be reported as being unhealthy by people with health problems and less unhealthy by Latinos and people who report regularly exercising. Latinos are more likely to report working outdoors regularly, but also more likely to report being able to reduce their exposure if the air quality is unhealthy. The results report differences in informational sources about air quality, suggesting that informational campaigns should target high risk groups using a variety of media
