    Rapport d'évaluation de l'Atelier de formation sur la valorisation agricole des ressources organiques locales : 3 au 9 décembre 2009, Guadeloupe

    L'Atelier de formation sur la valorisation agricole des ressources organiques est né d'un besoin en renforcement des capacités des agents du développement agricoles de Guadeloupe. L'atelier, d'une durée de 35 heures, s'est déroulé du 3 au 9 décembre 2009. Il a réuni 30 participants, ingénieurs et techniciens appartenant pour la majorité aux SICA cannières, bananières et "cultures maraichères", en charge des secteurs "approvisionnement en intrants" et "conseillers agricoles", provenant aussi de la Chambre d'agriculture et d'usine sucrière. L'atelier a abordé 7 thèmes : la situation des déchets organiques en Guadeloupe, les techniques de caractérisation de la matière organique (MO), la fabrication de compost et d'engrais organique, les indicateurs de prévision de la transformation des (MO), la valeur fertilisante de la matière organique, le cas particulier des boues de STEP et les contraintes et règlementaires. Cours magistraux, débats ouverts à la suite de chaque exposé, travaux dirigés, et visites de terrain se sont alternés tout au long des 5 jours. A l'issue de l'atelier, les participants ont acquis les principes fondamentaux pour apporter des solutions agronomiques par la valorisation agricole de tout type de MO et par la gestion " organique " de la fertilité des sols cultivés ; ils seront capables d'élaborer une stratégie de gestion des MO adaptées à la région qui prenne en compte les avantages et inconvénients écologiques des différentes MO et de la réglementation en vigueur qui s'appuie sur un éventail de techniques de valorisation (compostage, agro-épuration¿). Les participants ont affiché la volonté de constituer un réseau local pour réfléchir aux voies et moyens d'élaboration d'une véritable filière "organique" en Guadeloupe.     Challenges and aspirations: lived experiences of teacher-education student-mothers of a state university

    The COVID-19 pandemic has caused significant disruptions in education globally leading to the implementation of different learning modalities. And it highly affects the most vulnerable groups such as women and children. This study explored the lived experiences of teacher-education student-mothers of a state university in the Philippines towards the end of the pandemic using a phenomenological approach. Twenty-one student-mothers voluntarily participated in the study who were i) bona fide students of the campus, ii) biological mothers, iii) married or living together with a partner, and iv) living with their child. From the data analyses, four major themes emerged: i) unexpected pregnancy, ii) heightened stress during the pandemic, iii) maintaining a positive outlook in life, and iv) managing dual roles. These results implied that the teacher-education student-mothers were highly stressed during the pandemic but they had extrinsic motivators to finish studies such as their children and family. The lived experiences of teacher-education student-mothers can serve as an inspiration to other students to pursue their studies even amid adversities. It is recommended that there be an enhancement of student services tailored to support the mental health of marginalized students

    Utilidad de la determinación de niveles sanguíneos de agentes biológicos y de la respuesta inmune en el manejo de los pacientes con Enfermedad Inflamatoria Intestinal.

    Contexto: Los fármacos anti-TNF son tratamientos efectivos en la Enfermedad Inflamatoria Intestinal (EII), sin embargo, hasta un 40% de los pacientes no responden a la terapia, y entre 10-50% dejan de responder al mismo. La monitorización de los niveles del fármaco y sus anticuerpos puede resultar útil a la hora de optimizar el manejo de estos pacientes. Recientemente, se ha relacionado el gen HLA-DQA1*05 como posible marcador de inmunogenicidad y pérdida de respuesta a anti-TNF. Objetivo: Evaluar la utilidad de la determinación de niveles de Infliximab/Ac antiInfliximab y Adalimumab/Ac anti-Adalimumab en los pacientes con EII que van a iniciar tratamiento con agentes biológicos. Evaluar la influencia del alelo HLA-DQA1*05 en la respuesta y mantenimiento del tratamiento anti-TNF en dicha población. Diseño: Estudio observacional y prospectivo. Población a estudio: Se consideraron para la inclusión todos los pacientes que iniciaron tratamiento con Infliximab (IFX) o Adalimumab (ADA) por EII durante el periodo de septiembre 2020 hasta abril 2022 en el Hospital Universitario de Canarias. Material y métodos: Se incluyeron 112 pacientes en tratamiento con IFX (25) y ADA (87). Se recogieron variables demográficas, clínicas, analíticas, índice de actividad, valoración global subjetiva y pruebas complementarias de forma prospectiva. Los niveles de IFX se registraron en la semana 6, 14, 30, y a los 12 y 24 meses; y los de ADA se registraron en la semana 4, 10, 30, y a los 12 y 24 meses. Se determinó el valor del HLADQA1*05 y su relación con la pérdida de respuesta/formación de anticuerpos anti-TNF. Tamaño muestral: Se estimó la conveniencia de disponer de unos 450 niveles plasmáticos, y asumiendo un 20% de pérdidas, se requieren de unos 80 pacientes. Resultados: De los 87 pacientes analizados con ADA, 67 (77%) tenían Enfermedad Crohn (EC) y 20 (23%) Colitis Ulcerosa (CU). En la semana 0, el 86% presentaban enfermedad activa, 54,5% PCR >5 mg/L y 74,4% calprotectina fecal >150 μg/kg. De los 43 pacientes que en el momento del análisis llevaban 30 semanas de tratamiento, un 12,5% presentaban enfermedad activa, estando 32,5% de ellos intensificados. De los 13 pacientes que llevaban 12 meses, el 100% se mantuvieron estables y 7,7% intensificados. Se observó que niveles de ADA >7 μg/mL en post-inducción se correlacionaron con una tendencia a enfermedad estable tanto en semana 10 (p=0.19) como en semana 30 (p=0.51), si bien no se alcanzó significación estadística por bajo tamaño muestral. En el análisis de regresión simple, los pacientes que recibieron tratamiento previo con antiTNF, tienen más riesgo de desarrollar anticuerpos anti-fármaco de manera precoz (p=0.05). En el análisis del HLA-DQ1*05, el 43,6% de los pacientes eran portadores de al menos un alelo HLA-DQA1*05, de los cuales un 3% desarrolló anticuerpos en la semana 4 (p=0.24), y un 8% en la semana 10 (p=0.32). Conclusión: La monitorización de niveles de fármaco y sus anticuerpos parece útil en el seguimiento de pacientes con EII para optimizar el tratamiento. Los pacientes que recibieron tratamiento previo con anti-TNF años antes, parecen tener más riesgo de desarrollar anticuerpos anti-fármaco, así como podría también estar relacionado con ser portador del alelo HLA-DQA1*05. Es necesario prolongar el seguimiento y aumentar el número de pacientes para restablecer conclusiones definitivas.Background: Anti-TNF drugs are effective treatments in inflammatory bowel disease (IBD), however, up to 40% of patients do not respond to therapy, and between 10-50% stop responding to it. To evaluate the influence of the HLA-DQA1*05 allele on the response and maintenance of anti-TNF treatment in this population. Design: Observational and prospective study. Study population: All patients who started treatment with infliximab (IFX) or adalimumab (ADA) for IBD during the period from September 2020 to April 2022 in real clinical practice of the University Hospital of the Canary Islands were considered for inclusion. Material and methods: 112 patients treated with IFX (25), and ADA (87) were included. Demographic, clinical, analytical variables, activity index, subjective global assessment (SGA) and complementary tests were prospectively collected. IFX levels were recorded at weeks 6, 14, 30, and at 12 and 24 months; and those of ADA were recorded at weeks 4, 10, 30, and at 12 and 24 months. The value of HLA-DQA1*05 and its relationship with the loss of response/formation of anti-TNF antibodies was determined. Sample size: The convenience of having about 450 plasma levels was estimated, and assuming a 20% loss, about 80 patients are required. Results: Of the 87 patients analyzed with ADA, 67 (77%) had Crohn's Disease (CD) and 20 (23%) Ulcerative Colitis (UC). At week 0, 86% had active disease, 54.5% CRP >5 mg/L and 74.4% fecal calprotectin >150 μg/kg. Of the 43 patients who had 30 weeks of treatment at the time of the analysis, 12,5% had active disease, 32.5% of them being intensified. Of the 13 patients who reached 12 months, 100% remained stable and 7.7% intensified. It was observed that ADA levels >7 μg/mL post-induction correlated with a tendency to stable disease both at week 10 (p=0.19) and at week 30 (p=0.51), although statistical significance was not reached, due to low sample size. In the simple regression analysis, patients who received previous treatment with anti-TNF have a higher risk of developing anti-drug antibodies early (p=0.05). In the HLA-DQ1*05 analysis, 43.6% of the patients were carriers of at least one HLA-DQA1*05 allele, of which 3% developed antibodies at week 4 (p=0.24), and 8% at week 10 (p=0.32). Conclusion: The monitoring of drug levels and their antibodies seems useful in the follow-up of patients with IBD to optimize treatment. Patients who received previous treatment with anti-TNF years before appear to be at higher risk of developing anti-drug antibodies, as well as it could also be related to being a carrier of the HLA-DQA1*05 allele. It is necessary to prolong follow-up and increase the number of patients to establish definitive conclusions

    An Exploration of the Occupational Needs of College Students

    This paper analyzes the barriers and supports for Dominican University of California students in meeting their occupational needs and what services might be beneficial to them. To be able to answer this question, we conducted a survey using convenience sampling with twenty-seven Dominican undergraduate students, interviewed staff, and conducted a focus group with the students who participated in the survey. Our results indicated four main themes which include: occupational needs, barriers to meeting occupational needs, supports to meeting occupational needs, and services recommended. For the implications for occupational therapy, we found that OTs are beneficial in several aspects of students’ lives and can be implemented on and around the campus

    Antecedents and Consequents of Multiculturalism and Social Diversity among Law Enforcement Workplace in Metro Manila

    This study describes the antecedents and consequents of multiculturalism and social diversity, and the perceptions of law enforcement personnel towards the consequents of multiculturalism and social diversity in the law enforcement workplace. This quantitative descriptive research determined the perceptions of 100 law enforcement personnel from the purposively selected law enforcement workplaces in Metro Manila. Data were analyzed using frequency and percentage distribution, mean and rank. The majority of the respondents recognized the utmost importance of multiculturalism and social diversity in a Law Enforcement workplace. Managing diversity in the law enforcement workplace means creating conditions that maximize the potential of its personnel to enhance organizational performance; and Multiculturalism promotes the integration of different cultures and the sharing of one's ideas and perspectives that lead to innovations have the highest mean scores of 3.75, respectively. On the other hand, Inclusive organizations can screen out highly prejudiced individuals and will find it easy to manage diverse employees with a mean score of 2.69.  It can be gleaned that the majority of the respondents perceived that employing a culturally diverse law enforcement workforce has positive consequences to its people, workplace culture, and organizational climate. Therefore, law enforcement officers should support heterogeneous workplace advocacies, and that policymakers should reinvigorate the rigor of existing policies to sustain multicultural and social diversity in the entire organization
