    Delayed Repair of Myelomeningoceles

    Objective: Myelomeningocele is a defect that is typically surgically repaired within the first few days of life in developed countries to minimize the risk of meningitis. If left unrepaired, these children may survive to have their meningocele sac epithelialize. The surgical reduction and closure of an epithelialized myelomeningocele represents a unique challenge for the neurosurgeon, as it requires a modification of the typical closure technique. Methods: 10 years experience in 97 patients with the delayed (\u3e6 months) repair of myelomeningoceles was the basis of this report. Results: Repair technique in a child with a myelomeningocele that was not repaired at birth presented a surgical challenge whose solutions are presented herein. Conclusion: Delayed closure of myelomeningoceles is facilitated by adherence to lessons learned form surgical experience on medical missions to Guatemala

The locally strengthened areas within the reinforcement region have load distribution devices to redistribute load in order to (i) locally strengthen an area around damage induced by an initial momentary and direct transmitted load, and (ii) limit growth and propagation of damage induced by an initial momentary and direct transmitted load during a subsequent unbalance load.     The Department of Defense (DoD) defined System Operational Effectiveness (SOE) model provides an exceptional framework for an affordable approach to the development and operation of space launch vehicles and their supporting infrastructure. The SOE model provides a focal point from which to direct and measure technical effectiveness and process efficiencies of space launch vehicles. The application of the SOE model to a space launch vehicle's development and operation effort leads to very specific approaches and measures that require consideration during the design phase. This paper provides a mapping of the SOE model to the development of space launch vehicles for human exploration by addressing the SOE model key points of measurement including System Performance, System Availability, Technical Effectiveness, Process Efficiency, System Effectiveness, Life Cycle Cost, and Affordable Operational Effectiveness. In addition, the application of the SOE model to the launch vehicle development process is defined providing the unique aspects of space launch vehicle production and operations in lieu of the traditional broader SOE context that examines large quantities of fielded systems. The tailoring and application of the SOE model to space launch vehicles provides some key insights into the operational design drivers, capability phasing, and operational support systems

    Improving and disaggregating N2O emission factors for ruminant excreta on temperate pasture soils

    pre-printCattle excreta deposited on grazed grasslands are a major source of the greenhouse gas (GHG) nitrous oxide (N2O). Currently, many countries use the IPCC default emission factor (EF) of 2% to estimate excreta-derived N2O emissions. However, emissions can vary greatly depending on the type of excreta (dung or urine), soil type and timing of application. Therefore three experiments were conducted to quantify excreta-derived N2O emissions and their associated EFs, and to assess the effect of soil type, season of application and type of excreta on the magnitude of losses. Cattle dung, urine and artificial urine treatments were applied in spring, summer and autumn to three temperate grassland sites with varying soil and weather conditions. Nitrous oxide emissions were measured from the three experiments over 12 months to generate annual N2O emission factors. The EFs from urine treated soil was greater (0.30–4.81% for real urine and 0.13–3.82% for synthetic urine) when compared with dung (− 0.02–1.48%) treatments. Nitrous oxide emissions were driven by environmental conditions and could be predicted by rainfall and temperature before, and soil moisture deficit after application; highlighting the potential for a decision support tool to reduce N2O emissions by modifying grazing management based on these parameters. Emission factors varied seasonally with the highest EFs in autumn and were also dependent on soil type, with the lowest EFs observed from well-drained and the highest from imperfectly drained soil. The EFs averaged 0.31 and 1.18% for cattle dung and urine, respectively, both of which were considerably lower than the IPCC default value of 2%.     Nonlinear stability analysis of plane Poiseuille flow by normal forms

    In the subcritical interval of the Reynolds number 4320\leq R\leq R_c\equiv 5772, the Navier--Stokes equations of the two--dimensional plane Poiseuille flow are approximated by a 22--dimensional Galerkin representation formed from eigenfunctions of the Orr--Sommerfeld equation. The resulting dynamical system is brought into a generalized normal form which is characterized by a disposable parameter controlling the magnitude of denominators of the normal form transformation. As rigorously proved, the generalized normal form decouples into a low--dimensional dominant and a slaved subsystem. {}From the dominant system the critical amplitude is calculated as a function of the Reynolds number. As compared with the Landau method, which works down to R=5300, the phase velocity of the critical mode agrees within 1 per cent; the critical amplitude is reproduced similarly well except close to the critical point, where the maximal error is about 16 per cent.     Outbreak of Aeromonas hydrophila wound infections association with mud football

    On 16 February 2002, a total of 26 people presented to the emergency department of the local hospital in the rural town of Collie in southwest Western Australia with many infected scratches and pustules distributed over their bodies. All of the patients had participated in a “mud football” competition the previous day, in which there had been 100 participants. One patient required removal of an infected thumbnail, and another required surgical debridement of an infected toe. Aeromonas hydrophila was isolated from all 3 patients from whom swab specimens were obtained. To prepare the mud football fields, a paddock was irrigated with water that was pumped from an adjacent river during the 1-month period before the competition. A. hydrophila was subsequently isolated from a water sample obtained from the river.
