L’intégration des services sociaux et des services de santé au Québec : du modèle à la réalité
À la fin des années soixante au Québec, à la faveur d’une enquête sur le domaine de la santé et du bien-être social, un modèle social est développé pour définir un nouveau « régime de la santé ». L’enquête n’a pas porté sur la santé publique, mais le ministère des Affaires sociales nouvellement créé regroupe mission sociale et mission de santé sous une même autorité centrale; des structures périphériques sont mises en place pour concrétiser ces missions, mais sans véritables liens entre elles. Au fil des années, plusieurs réformes du système de santé et de services sociaux vont se succéder, modifiant les structures sans bouleverser vraiment la nature de ces missions et leurs relations. En 1985, le ministère des Affaires sociales devient le ministère de la Santé et des Services sociaux; ce changement traduit un certain recul de la prise en charge de la dimension sociale de la santé. Par contre, la prise en compte de celle-ci dans la nouvelle politique de santé publique paraît encourageante.Following an inquiry into health and social well-being, beginning at the end of the 1960s a social model was developed that defined a new “health regime.” The inquiry did not treat public health directly, but the newly created Ministry of Social Affairs combined the social and health mandates in a single administration. Supporting structures were created to act on these mandates, but without real links among them. Over time several waves of reforms of the health system and social services altered the structures but without really overturning their mandates and relationship. In 1985 the Ministry of Social Affairs became the Ministry of Health and Social Services, a name-change that potentially signalled a move away from attention to the social dimensions of health. Aluminum effect on photoluminescence properties of sol–gel-derived Eu3+-activated silicate glasses
Clustering of rare earth in glasses, aluminum effect: experiments and modeling
Luminescent spectra of Eu3+-doped sol-gel glasses have been analyzed during the densification process and compared according to the presence or not of aluminum as a codoping ion. A transition temperature from hydrated to dehydroxyled environments has been found different for doped and codoped samples. However, only slight modifications have been displayed from luminescence measurements beyond this transition. To support the experimental analysis, molecular dynamics simulations have been performed to model the doped and codoped glass structures. Despite no evidence of rare earth clustering reduction due to aluminum has been found, the modeled structures have shown that the luminescent ions are mainly located in aluminum-rich domains. The synthesis of both experimental and numerical analyses has lead us to interpret the aluminum effect as responsible for differences in structure of the luminescent sites rather than for an effective dispersion of the rare earth ions.
