A programme management approach for ensuring coherence in IS (higher) education
This paper examines IS higher education, concentrating on issues of ‘coherence’ in IS curricula. While curriculum coherence can be jeopardized by poor curriculum design, misalignment between module content and/or misalignment between module or course aims can cause serious coherence issues over time. Misalignment of this type is exacerbated by the traditional processes of curriculum (re)design, which rely heavily on the (singular) interpretation of highly abstract documents, such as module syllabi and course specifications – often produced by curriculum designers in isolation. To improve curriculum coherence, this paper examines the use of a programme management framework as a means of (a) ‘humanizing’ the abstract aims and goals of curricula schemes and (b) managing the delivery and evolution of curricula in relation to the stakeholders in the process of delivery. The practical use of the framework is examined in the context of a Masters-level course in ‘Information Systems Management’. An action research approach is used to demonstrate the practical utility of the framework in terms of (a) improving communication of curricula, (b) improving the coherence between modules and between modules and course and (c) removing content redundancy. Guidelines are presented that generalize the findings in order that key practices may be adopted by others
Business process simulation: An alternative modelling technique for the information system development process
This paper discusses the idea that even though information systems development
(ISD) approaches have long advocated the use of integrated organisational views, the
modelling techniques used have not been adapted accordingly and remain focused on
the automated information system (IS) solution. Existing research provides evidence
that business process simulation (BPS) can be used at different points in the ISD
process to provide better integrated organisational views that aid the design of
appropriate IS solutions. Despite this fact, research in this area is not extensive;
suggesting that the potential of using BPS for the ISD process is not yet well
understood. The paper uses the findings from three different case studies to illustrate
the ways BPS has been used at different points in the ISD process. It compares the
results against IS modelling techniques, highlighting the advantages and
disadvantages that BPS has over the latter. The research necessary to develop
appropriate BPS tools and give guidance on their use in the ISD process is discussed
Analytical characteristics and comparative evaluation of Aptima HCV quant Dx assay with the Abbott RealTime HCV assay and Roche COBAS AmpliPrep/COBAS TaqMan HCV quantitative test v2.0
Abstract Background The Aptima HCV Quant Dx assay (Aptima assay) is a fully automated quantitative assay on the Panther® system. This assay is intended for confirmation of diagnosis and monitoring of HCV RNA in plasma and serum specimens. The purpose of the testing described in this paper was to evaluate the performance of the Aptima assay. Methods The analytical sensitivity, analytical specificity, precision, and linearity of the Aptima assay were assessed. The performance of the Aptima assay was compared to two commercially available HCV assays; the Abbott RealTime HCV assay (Abbott assay, Abbott Labs Illinois, USA) and the Roche COBAS Ampliprep/COBAS Taqman HCV Quantitative Test v2.0 (Roche Assay, Roche Molecular Systems, Pleasanton CA, USA). The 95% Lower Limit of Detection (LoD) of the assay was determined from dilutions of the 2nd HCV WHO International Standard (NIBSC 96/798 genotype 1) and HCV positive clinical specimens in HCV negative human plasma and serum. Probit analysis was performed to generate the 95% predicted detection limits. The Lower Limit of Quantitation (LLoQ) was established for each genotype by diluting clinical specimens and the 2nd HCV WHO International Standard (NIBSC 96/798 genotype 1) in HCV negative human plasma and serum. Specificity was determined using 200 fresh and 536 frozen HCV RNA negative clinical specimens including 370 plasma specimens and 366 serum specimens. Linearity for genotypes 1 to 6 was established by diluting armored RNA or HCV positive clinical specimens in HCV negative serum or plasma from 8.08 log IU/mL to below 1 log IU/mL. Precision was tested using a 10 member panel made by diluting HCV positive clinical specimens or spiking armored RNA into HCV negative plasma and serum. A method comparison was conducted against the Abbott assay using 1058 clinical specimens and against the Roche assay using 608 clinical specimens from HCV infected patients. In addition, agreement between the Roche assay and the Aptima assay using specimens with low HCV concentrations (</= 25 IU/mL by Roche) was tested using 107 clinical specimens. Results The 95% LoD was 5.1 IU/mL or lower for serum and 4.8 IU/mL or lower for plasma depending on the HCV genotype. The LLoQ for the assay was 10 IU/mL. Specificity was 100% with 95% confidence intervals of 99.6 to 100% for serum and plasma data combined. The assay demonstrated good linearity across the range for all genotypes. The Precision as estimated by the standard deviation (sd) was 0.17 log or lower across the range of the assay for both serum and plasma. HCV viral load results were compared using the Aptima assay and the Abbott assay giving a slope of 1.06, an intercept of 0.08 and an R2 of 0.98. HCV viral load results were compared for the Aptima and Roche assays giving a slope of 1.05, an intercept of −0.12 and an R2 of 0.96. Positive and negative agreement for the Aptima assay vs the Roche assay was 89% for low level specimens. Conclusion The Aptima assay is a highly sensitive and specific assay. The assay gave comparable HCV viral load results when compared to the Abbott and Roche assays. The performance of the Aptima assay makes it an excellent candidate for the detection and monitoring of HCV
Investigating knowledge management factors affecting Chinese ICT firms performance: An integrated KM framework

Semiautomatic Assessment of the Terminal Ileum and Colon in Patients with Crohn Disease Using MRI (the VIGOR++ Project)
Rationale and Objectives: The objective of this study was to develop and validate a predictive magnetic resonance imaging (MRI) activity score for ileocolonic Crohn disease activity based on both subjective and semiautomatic MRI features. Materials and Methods: An MRI activity score (the “virtual gastrointestinal tract [VIGOR]” score) was developed from 27 validated magnetic resonance enterography datasets, including subjective radiologist observation of mural T2 signal and semiautomatic measurements of bowel wall thickness, excess volume, and dynamic contrast enhancement (initial slope of increase). A second subjective score was developed based on only radiologist observations. For validation, two observers applied both scores and three existing scores to a prospective dataset of 106 patients (59 women, median age 33) with known Crohn disease, using the endoscopic Crohn's Disease Endoscopic Index of Severity (CDEIS) as a reference standard. Results: The VIGOR score (17.1 × initial slope of increase + 0.2 × excess volume + 2.3 × mural T2) and other activity scores all had comparable correlation to the CDEIS scores (observer 1: r = 0.58 and 0.59, and observer 2: r = 0.34–0.40 and 0.43–0.51, respectively). The VIGOR score, however, improved interobserver agreement compared to the other activity scores (intraclass correlation coefficient = 0.81 vs 0.44–0.59). A diagnostic accuracy of 80%–81% was seen for the VIGOR score, similar to the other scores. Conclusions: The VIGOR score achieves comparable accuracy to conventional MRI activity scores, but with significantly improved reproducibility, favoring its use for disease monitoring and therapy evaluation
Critical knowledge management factors and organizational performance : an investigation of Chinese hi-tech enterprises
This study sets out to explore the critical factors of knowledge management (KM) that impact on the organizational performance of Chinese enterprises. It attempts to investigate the relationships among KM factors including KM strategy, KM enablers (Organizational culture, Organization structure, People, and Technology), KM processes, and organizational performance. The conceptual framework of knowledge management guiding this research is developed from prior research (Alavi, 1997; Davenport, 1999; Lam, 2000; Leonard-Barton, 1995; Nonaka and Takeuchi, 1995; Ramireza nd Dickenson, 2006; Zack, 1999). Previous KM research in the Chinese context has focused on the use of object-perspective measures such as number of created ideas or patents. There appears to be a relative neglect of the relationships between those KM factors. This research focused on exploring the relationships between those KM factors and investigating how they impact on organizational performance in the context of an emerging economy - China. This research adopted the mixed-methods (Creswell, 2003) methodological approach, which involved the use of qualitative and quantitative methods in addressing the research questions raised in this study. The results of this research suggest that organizational culture variables are found to be essential for knowledge creation. In particular, trust is a significant predictor of all knowledge creation processes. The findings of this study confirm that enterprises will be able to obtain strategic benefits of KM through effective knowledge creation processes. The research findings also imply that companies should align their knowledge strategies along with knowledge creation processes. The key to understanding KM in the Chinese context is recognizing the networking nature of the Chinese society which operates on the basis of "Guanxi". In Chinese enterprises, the socialization and externalization elements are remarkably similar to the Japanese situation that Nonaka and Takeuchi (1995) described, as the Chinese are highly networked, hold tacit knowledge within these networks and are prepared to make this knowledge explicit only within the context of these pre-existing relationships. The departmental focus of Chinese enterprises mean that the combination of tacit knowledge is not straightforward, while learning-by-doing is important to sustain their development. The research also indicates that internalization is also problematic in Chinese enterprises because of individuals' fear of admitting mistakes. Prevalence of Drug-Resistant HIV-1 Variants in Untreated Individuals in Europe: Implications for Clinical Management
BackgroundInfection with drug-resistant human immunodeficiency virus type 1 (HIV-1) can impair the response to combination therapy. Widespread transmission of drug-resistant variants has the disturbing potential of limiting future therapy options and affecting the efficacy of postexposure prophylaxis penta increase-spacing 1>MethodsWe determined the baseline rate of drug resistance in 2208 therapy-naive patients recently and chronically infected with HIV-1 from 19 European countries during 1996-2002 ResultsIn Europe, 1 of 10 antiretroviral-naive patients carried viruses with ⩾1 drug-resistance mutation. Recently infected patients harbored resistant variants more often than did chronically infected patients (13.5% vs. 8.7%; P=.006). Non-B viruses (30%) less frequently carried resistance mutations than did subtype B viruses (4.8% vs. 12.9%; P<.01). Baseline resistance increased over time in newly diagnosed cases of non-B infection: from 2.0% (1/49) in 1996-1998 to 8.2% (16/194) in 2000-2001 ConclusionsDrug-resistant variants are frequently present in both recently and chronically infected therapy-naive patients. Drug-resistant variants are most commonly seen in patients infected with subtype B virus, probably because of longer exposure of these viruses to drugs. However, an increase in baseline resistance in non-B viruses is observed. These data argue for testing all drug-naive patients and are of relevance when guidelines for management of postexposure prophylaxis and first-line therapy are update
The interferon receptor-1 promoter polymorphisms affect the outcome of Caucasians with HBeAg-negative chronic HBV infection
The outcome of HBeAg-negative chronic hepatitis B virus (HBV) patients who may remain in the inactive carrier state (IC) or progress to HBeAg-negative chronic hepatitis B may be affected by the host genetic profile. Genetic polymorphisms within not only the promoter but also the coding sequence of the interferon receptor 1 (INFAR1) gene have been associated with susceptibility to chronic HBV infection, but their role on the outcomes of HBeAg-negative patients has not been evaluated. We examined the association of INFAR1 promoter polymorphisms with the phase of chronic HBV infection in a demographically characterized Caucasian cohort of 183 consecutive HBeAg-negative chronic HBV patients.Using a combination of conventional and allele-specific polymerase chain reactions, bidirectional sequencing and DNA-fragment analysis, we performed typing of three Single Nucleotide Polymorphisms (SNPs -568G/C, -408C/T, -3C/T) and one Variable Number Tandem Repeat [VNTR -77(GT)n] within the INFR1 promoter sequence.The genetic polymorphisms examined were found to be associated with the phase of HBeAg-negative chronic HBV patients. Using a multiple logistic regression model adjusting for age, gender and origin of the individuals, we found that patients with linked genotypes -408CT_-3CT were more likely to be ICs (OR = 2.42 vs. CC, P = 0.036). Also, given the partial linkage between SNP -568G/C and VNTR -77(GT)n, we found that linked genotypes -77(GT)n ≤ 8/≤8_-568GC and -77(GT)n ≤ 8/≤8_-568CC were detected more frequently among ICs (OR = 11.69, P = 0.005 and OR = 7.56, P = 0.001 vs. -77(GT)n >8/>8_-568GG respectively).These findings suggest that these genetic variations represent important factors associated with the clinical phase of HBeAg-negative chronic HBV infection
Reducing HIV infection in people who inject drugs is impossible without targeting recently-infected subjects.
Objective Although our understanding of viral transmission among people who inject drugs (PWID) has improved, we still know little about when and how many times each injector transmits HIV throughout the duration of infection. We describe HIV dynamics in PWID to evaluate which preventive strategies can be efficient. Design Due to the notably scarce interventions, HIV-1 spread explosively in Russia and Ukraine in 1990s. By studying this epidemic between 1995 and 2005, we characterized naturally occurring transmission dynamics of HIV among PWID. Method We combined publicly available HIV pol and env sequences with prevalence estimates from Russia and Ukraine under an evolutionary epidemiology framework to characterize HIV transmissibility between PWID. We then constructed compartmental models to simulate HIV spread among PWID. Results In the absence of interventions, each injector transmits on average to 10 others. Half of the transmissions take place within 1 month after primary infection, suggesting that the epidemic will expand even after blocking all the post–first month transmissions. Primary prevention can realistically target the first month of infection, and we show that it is very efficient to control the spread of HIV-1 in PWID. Treating acutely infected on top of primary prevention is notably effective. Conclusion As a large proportion of transmissions among PWID occur within 1 month after infection, reducing and delaying transmissions through scale-up of harm reduction programmes should always form the backbone of HIV control strategies in PWID.Growing PWID populations in the developing world,where primary prevention is scarce, constitutes a public health time bomb
Human endogenous retrovirus-K HML-2 integration within RASGRF2 is associated with intravenous drug abuse and modulates transcription in a cell-line model
HERV-K HML-2 (HK2) has been proliferating in the germ line of humans at least as recently as 250,000 years ago, with some integrations that remain polymorphic in the modern human population. One of the solitary HK2 LTR polymorphic integrations lies between exons 17 and 18 of RASGRF2, a gene that affects dopaminergic activity and is thus related to addiction. Here we show that this antisense HK2 integration (namely RASGRF2-int) is found more frequently in persons who inject drugs compared with the general population. In a Greek HIV-1–positive population (n = 202), we found RASGRF2-int 2.5 times (14 versus 6%) more frequently in patients infected through i.v. drug use compared with other transmission route controls (P = 0.03). Independently, in a United Kingdom-based hepatitis C virus-positive population (n = 184), we found RASGRF2-int 3.6 times (34 versus 9.5%) more frequently in patients infected during chronic drug abuse compared with controls (P ≺ 0.001). We then tested whether RASGRF2-int could be mechanistically responsible for this association by modulating transcription of RASGRF2. We show that the CRISPR/Cas9-mediated insertion of HK2 in HEK293 cells in the exact RASGRF2 intronic position found in the population resulted in significant transcriptional and phenotypic changes. We also explored mechanistic features of other intronic HK2 integrations and show that HK2 LTRs can be responsible for generation of cis-natural antisense transcripts, which could interfere with the transcription of nearby genes. Our findings suggest that RASGRF2-int is a strong candidate for dopaminergic manipulation, and emphasize the importance of accurate mapping of neglected HERV polymorphisms in human genomic studies
