Role of ABCB1 C3435T variant in response to antiepileptic drugs in epilepsy: a review
Over-expression of P-glycoprotein (P-gp), the encoded product of the ATP-binding cassette (ABC), sub-family B, member 1 (ABCB1/MDR1) gene, plays an important role in mediating multidrug resistance to antiepileptic drugs (AEDs) in about 30% of patients with epilepsy. Genetic variation may in part explain inter-individual differences in phenotype-genotype relationships in the pharmacological response of epilepsy patients to AEDs. The synonymous C3435T polymorphism is one of the most common allelic variants in the ABCB1/MDR1 gene, proposed in the causation of refractory epilepsy. Many studies have shown the relationship between C3435T polymorphism and refractoriness to AEDs in epilepsy. However, there is controversy between the findings of various studies, that is, whether ABCB1/MDR1 C3435T gene polymorphism is associated with response to AEDs in epilepsy patients. This review provides a background and discusses the results of investigations on possible confounding factors affecting the interpretation and implementation of association studies in this area
Rethinking the Impact of Activity Design on a Mobile Learning Trail: The Missing Dimension of the Physical Affordances
This paper investigates the relationship between activity design and discourse on a mobile learning trail, considering the physical affordances of the real world platform in designing contextual learning experiences. We adopted a context-oriented and process-oriented pedagogical approach in designing the mobile learning trail conducted at Singapore Sentosa Island. Activities were categorized into performative and knowledge-generative on a continuum from well-structured to ill-structured activities. To examine the impact of activity design on discourse types, all audio-recorded verbal data of the three groups of secondary students was analysed with respect to two key dimensions in the knowledge construction process, namely, the epistemic and the social. Analysis showed that activity types and physical affordances of the learning environment have a definitive bearing on group discursive moves. Importantly, the presence of a real world context could generate critical thinking and collaborative knowledge building even for mundane performative activities when they are embedded with unforeseen contextual variables. We argue that the design of activity and the degree of its structuredness, and the assumed desired learning outcomes, are very much subjected to the affordances of physical and social resources in the mobile learning environment-a missing dimension that could possibly be overlooked and understudied.X1144Ysciessciscopu

Epilepsy Is a Risk Factor for Sudden Cardiac Arrest in the General Population
Background
People with epilepsy are at increased risk for sudden death. The most prevalent cause of sudden death in the general population is sudden cardiac arrest (SCA) due to ventricular fibrillation (VF). SCA may contribute to the increased incidence of sudden death in people with epilepsy. We assessed whether the risk for SCA is increased in epilepsy by determining the risk for SCA among people with active epilepsy in a community-based study.
Methods and Results
This investigation was part of the Amsterdam Resuscitation Studies (ARREST) in the Netherlands. It was designed to assess SCA risk in the general population. All SCA cases in the study area were identified and matched to controls (by age, sex, and SCA date). A diagnosis of active epilepsy was ascertained in all cases and controls. Relative risk for SCA was estimated by calculating the adjusted odds ratios using conditional logistic regression (adjustment was made for known risk factors for SCA). We identified 1019 cases of SCA with ECG-documented VF, and matched them to 2834 controls. There were 12 people with active epilepsy among cases and 12 among controls. Epilepsy was associated with a three-fold increased risk for SCA (adjusted OR 2.9 [95%CI 1.1–8.0.], p = 0.034). The risk for SCA in epilepsy was particularly increased in young and females.
Conclusion
Epilepsy in the general population seems to be associated with an increased risk for SCA

Risk of recurrent arterial ischemic stroke in childhood: A prospective international study
Background and Purpose - Published cohorts of children with arterial ischemic stroke (AIS) in the 1990s to early 2000s reported 5-year cumulative recurrence rates approaching 20%. Since then, utilization of antithrombotic agents for secondary stroke prevention in children has increased. We sought to determine rates and predictors of recurrent stroke in the current era. Methods - The Vascular Effects of Infection in Pediatric Stroke (VIPS) study enrolled 355 children with AIS at 37 international centers from 2009 to 2014 and followed them prospectively for recurrent stroke. Index and recurrent strokes underwent central review and confirmation, as well as central classification of causes of stroke, including arteriopathies. Other predictors were measured via parental interview or chart review. Results - Of the 355 children, 354 survived their acute index stroke, and 308 (87%) were treated with an antithrombotic medication. During a median follow-up of 2.0 years (interquartile range, 1.0-3.0), 40 children had a recurrent AIS, and none had a hemorrhagic stroke. The cumulative stroke recurrence rate was 6.8% (95% confidence interval, 4.6%-10%) at 1 month and 12% (8.5%-15%) at 1 year. The sole predictor of recurrence was the presence of an arteriopathy, which increased the risk of recurrence 5-fold when compared with an idiopathic AIS (hazard ratio, 5.0; 95% confidence interval, 1.8-14). The 1-year recurrence rate was 32% (95% confidence interval, 18%-51%) for moyamoya, 25% (12%-48%) for transient cerebral arteriopathy, and 19% (8.5%-40%) for arterial dissection. Conclusions - Children with AIS, particularly those with arteriopathy, remain at high risk for recurrent AIS despite increased utilization of antithrombotic agents. Intrinsic and Extrinsic Performance Limits of Graphene Devices on SiO2
The linear dispersion relation in graphene[1,2] gives rise to a surprising
prediction: the resistivity due to isotropic scatterers (e.g. white-noise
disorder[3] or phonons[4-8]) is independent of carrier density n. Here we show
that acoustic phonon scattering[4-6] is indeed independent of n, and places an
intrinsic limit on the resistivity in graphene of only 30 Ohm at room
temperature (RT). At a technologically-relevant carrier density of 10^12 cm^-2,
the mean free path for electron-acoustic phonon scattering is >2 microns, and
the intrinsic mobility limit is 2x10^5 cm^2/Vs, exceeding the highest known
inorganic semiconductor (InSb, ~7.7x10^4 cm^2/Vs[9]) and semiconducting carbon
nanotubes (~1x10^5 cm^2/Vs[10]). We also show that extrinsic scattering by
surface phonons of the SiO2 substrate[11,12] adds a strong temperature
dependent resistivity above ~200 K[8], limiting the RT mobility to ~4x10^4
cm^2/Vs, pointing out the importance of substrate choice for graphene

Bridging the data gaps in the epidemiology of hepatitis C virus infection in Malaysia using multi-parameter evidence synthesis
BACKGROUND: Collecting adequate information on key epidemiological indicators is a prerequisite to informing a public health response to reduce the impact of hepatitis C virus (HCV) infection in Malaysia. Our goal was to overcome the acute data shortage typical of low/middle income countries using statistical modelling to estimate the national HCV prevalence and the distribution over transmission pathways as of the end of 2009. METHODS: Multi-parameter evidence synthesis methods were applied to combine all available relevant data sources - both direct and indirect - that inform the epidemiological parameters of interest. RESULTS: An estimated 454,000 (95% credible interval [CrI]: 392,000 to 535,000) HCV antibody-positive individuals were living in Malaysia in 2009; this represents 2.5% (95% CrI: 2.2-3.0%) of the population aged 15-64 years. Among males of Malay ethnicity, for 77% (95% CrI: 69-85%) the route of probable transmission was active or a previous history of injecting drugs. The corresponding proportions were smaller for male Chinese and Indian/other ethnic groups (40% and 71%, respectively). The estimated prevalence in females of all ethnicities was 1% (95% CrI: 0.6 to 1.4%); 92% (95% CrI: 88 to 95%) of infections were attributable to non-drug injecting routes of transmission. CONCLUSIONS: The prevalent number of persons living with HCV infection in Malaysia is estimated to be very high. Low/middle income countries often lack a comprehensive evidence base; however, evidence synthesis methods can assist in filling the data gaps required for the development of effective policy to address the future public health and economic burden due to HCV. Photonic quantum state transfer between a cold atomic gas and a crystal
Interfacing fundamentally different quantum systems is key to build future
hybrid quantum networks. Such heterogeneous networks offer superior
capabilities compared to their homogeneous counterparts as they merge
individual advantages of disparate quantum nodes in a single network
architecture. However, only very few investigations on optical
hybrid-interconnections have been carried out due to the high fundamental and
technological challenges, which involve e.g. wavelength and bandwidth matching
of the interfacing photons. Here we report the first optical quantum
interconnection between two disparate matter quantum systems with photon
storage capabilities. We show that a quantum state can be faithfully
transferred between a cold atomic ensemble and a rare-earth doped crystal via a
single photon at telecommunication wavelength, using cascaded quantum frequency
conversion. We first demonstrate that quantum correlations between a photon and
a single collective spin excitation in the cold atomic ensemble can be
transferred onto the solid-state system. We also show that single-photon
time-bin qubits generated in the cold atomic ensemble can be converted, stored
and retrieved from the crystal with a conditional qubit fidelity of more than
. Our results open prospects to optically connect quantum nodes with
different capabilities and represent an important step towards the realization
of large-scale hybrid quantum networks
