Does IT standardization help to boost cost and profit efficiency? Empirical evidence from German savings banks
This paper investigates the impact of IT standardization on bank performance based on a panel of 457 German savings banks over the period from 1996 to 2006. We measure IT standardization as the fraction of IT expenses for centralized services over banks' total IT expenses. Bank efficiency, in turn, is measured by traditional accounting performance indicators as well as by cost and profit efficiencies that are estimated by a stochastic frontier approach. Our results suggest that IT standardization is conducive to cost efficiency. The relation is positive and robust for small and medium-sized banks but vanishes for very large banks. Furthermore, our study confirms the often cited computer paradox by showing that total IT expenditures negatively impact cost efficiency and have no influence on bank profits. To the best of our knowledge, this paper is first to empirically explore whether IT standardization enhances efficiency by employing genuine data of banks' IT expenditures.
Algoritmos para la detección de aceite de avellana en aceite de oliva
7 pages, 5 figures, 5 tables.[EN] The fraudulent addition of hazelnut oil to olive oil can be only
detected in high proportions (20-25%) using the A-stigmastenol
and the difference between triacylglycerols of equivalent carbon
number 42, determined experimentally by HPLC and calculated
theoretically from the fatty acid composition (AECN42). A new
method lies on a sequential comparison of the values of several
algorithms with a database built with data obtained from genuine
virgin olive oils. The algorithms are: LLLexp en función de %L; (LLL/OLLn)exp - (LLL/OLLn)teor, vs AECN44 and (ECN441LLL), vs %L; being:
LLLnexpO LLnexpa nd ECN44ew the percentage of triacylglycerols
determined by HPLC, LLLth, OLLnthWr and EcN44th,, the
percentage of those calculated theoretically from the fatty acid
wmposition; AECN44 the difference between the experimental and
theoretica~v alue of ECN44; and finally %L the percentage of linoleic
acid. The database has been built considering the values obtained
from olive oils of diierent fatty acid composition and from admixtures
between them. The method allows the detection of low percentages
We thank to Ministry of Agriculture for the

Primary stabilization of humeral shaft fractures: an experimental study of different osteosynthesis methods
The purpose of this study was to assess primary stabilization of humeral shaft fractures using three different methods of fixation, represented by a DCP type plate, applied as a bridge plate, an uncommon synthesis material named SPS®, not previously described in literature and also used as a bridge plate, and a third type of material constituted by an intramedullary nail, with an uncommon locking provided by a distal cortical screw and a proximal Ender-type wire. MATERIAL AND METHOD: Twenty-one pairs of human humeri were divided into three groups, each using one type of material for fixation, the bones of which were osteotomized, stabilized and submitted to nondestructive flexion-compression and torsion assays up to 200 N and 100 N respectively, and, in a crossing mechanism, the groups were again submitted to other torsion and flexion-compression assays, supported by statistical analysis. RESULTS: The bridge-DCP group showed good resistance to the applied forces, similarly to the SPS® group, which, although presenting greater deflection, showed great elastic capacity. The intramedullary nail group showed good results in the flexion-compression assay due to the tutor mechanism of the intramedullary nails, but did not show resistance to the torsion forces
Influence of chronotype on the incidence and severity of perinatal depression in the “Life-ON” study
Background: Perinatal depression (PND) is a severe complication of pregnancy, but there are no established risk factors predicting the disease. Evening chronotype has been associated with unhealthy lifestyle habits and adverse outcomes during pregnancy. In this study, we aimed to clarify whether chronotype can predict symptoms and/or occurrence of PND. Methods: Two hundred ninety-nine women were followed-up from the first trimester of pregnancy until 6 months postpartum. Chronotype was assessed at baseline using the MEQ, while mood was repeatedly assessed by depression rating scales (EPDS, HDRS, MADRS). The influence of time and chronotype on EPDS, HDRS and MADRS, was estimated by constructing multilevel linear mixed regression models. A Cox proportional-hazard regression model was built to evaluate the association between chronotype and incidence of depression. Results: Chronotype modulated PND symptom severity depending on time of assessment, with evening chronotypes having a higher risk for developing PND symptoms, as assessed by EPDS, at postpartum visits V4 (5–12 days) and V5 (19–26 days). These also had less healthy lifestyle habits and were more likely to suffer from gestational diabetes mellitus and undergo cesarean delivery as compared to other chronotypes. Limitations: Only a minority of women were classified as evening chronotypes. The long follow-up phase of the study led to missing data. Conclusions: Pregnant evening chronotypes show unhealthy lifestyle habits and sociodemographic characteristics commonly associated with a higher risk for PND. They also have a higher risk of developing PND symptoms in the first month after delivery. Chronotype should therefore be routinely assessed during pregnancy to identify women potentially at risk for developing PND
Knowing is trusting? An experimental test of the role of information in advisory
The recent economic crisis still lingering in Europe has deeply affected the way individuals look at the investment market. Understanding the trust processes underlying the decision to invest with financial intermediaries is of particular importance both at managerial (product development and advertisement) and at normative level (how intermediaries are regulated). Using an online experiment, this paper investigates whether discrepancies in the financial literacy of investors and brokers can be used to explain the decision to trust – thus, to invest in the financial market. The results show that trust is affected by the information disclosure in somewhat unexpected ways
Laparoscopic Fertility-Sparing Surgery for Early Ovarian Malignancies
The demand for fertility-sparing surgery (FSS) has increased in the last decade due to
increased maternal age, increased incidence of ovarian malignancies in younger patients, and technical advances in surgery. Data on oncological safety and fertility outcomes of patients with ovarian
cancer after laparoscopic FSS are sparse, but some retrospective studies have shown that open FSS
may be offered to selected patients. We assessed the role of minimally invasive FSS in comparison
with radical surgery (RS) in terms of oncological safety and reproductive outcomes after FSS in this
multicenter study. Eighty patients with FIGO stage I/II ovarian cancer treated with laparoscopic
FSS or RS between 01/2000 and 10/2018 at the participating centers (comprehensive gynecological
cancer centers with minimally invasive surgical expertise) were included in this retrospective analysis
of prospectively kept data. Case–control (n = 40 each) matching according to the FIGO stage was
performed. Progression-free survival [150 (3–150) and 150 (5–150) months; p = 0.61] and overall
survival [36 (3–150) and 50 (1–275) months; p = 0.65] did not differ between the FSS and RS groups.
Eight (25.8%) women became pregnant after FSS, resulting in seven (22.5%) deliveries; three (37.5%)
patients conceived after in vitro fertilization, and five (62.5%) conceived spontaneously. Laparoscopic
FSS seems to be applicable and oncologically safe for patients with early-stage ovarian cancer, with
adequate fertility outcomes
Evaluation of compliance and outcomes of a management protocol for massive postpartum hemorrhage at a tertiary care hospital in Pakistan

Abstract

Background

Massive postpartum hemorrhage is a life threatening obstetric emergency. In order to prevent the complications associated with this condition, an organized and step-wise management protocol should be immediately initiated.

Methods

An evidence based management protocol for massive postpartum hemorrhage was implemented at Aga Khan University Hospital, Karachi, Pakistan after an audit in 2005. We sought to evaluate the compliance and outcomes associated with this management protocol 3 years after its implementation. A review of all deliveries with massive primary postpartum hemorrhage (blood loss ≥ 1500 ml) between January, 2008 to December, 2008 was carried out. Information regarding mortality, mode of delivery, possible cause of postpartum hemorrhage and medical or surgical intervention was collected. The estimation of blood loss was made via subjective and objective assessment.

Results

During 2008, massive postpartum hemorrhage occurred in 0.64% cases (26/4,052). No deaths were reported. The mean blood loss was 2431 ± 1817 ml (range: 1500 - 9000 ml). Emergency cesarean section was the most common mode of delivery (13/26; 50%) while uterine atony was the most common cause of massive postpartum hemorrhage (14/26; 54%). B-lynch suture (24%) and balloon tamponade (60%) were used more commonly as compared to our previously reported experience. Cesarean hysterectomy was performed in 3 cases (12%) for control of massive postpartum hemorrhage. More than 80% compliance was observed in 8 out of 10 steps of the management protocol. Initiation of blood transfusion at 1500 ml blood loss (89%) and overall documentation of management (92%) were favorably observed in most cases.

Conclusion

This report details our experience with the practical implementation of a management protocol for massive postpartum hemorrhage at a tertiary care hospital in a developing country. With the exception of arterial embolization, relatively newer, simpler and potentially safer techniques are now being employed for the management of massive postpartum hemorrhage at our institution. Particular attention should be paid to the documentation of the management steps while ensuring a stricter adherence to the formulated protocols and guidelines in order to further ameliorate patient outcomes in emergency obstetrical practice. More audits like the one we performed are important to recognize and rectify any deficiencies in obstetrical practice in developing countries. Dissemination of the same is pivotal to enable an open discourse on the improvement of existing obstetrical strategies.</p
The German Banking System and the Global Financial Crisis: Causes, Developments and Policy Responses
Germany's banking sector has been severely hit by the global financial crisis. In a German context as of February, 2009, this paper reviews briefly the structure of the banking industry, quantifies effects of the crisis on banks and surveys responses of economic policy. It is argued that policy design needs to enhance transparency and enforce the liability principle. In addition, economic policy should not eclipse principles of competition policy.
