    Introduction: Pitutary adenomas account for 10-15% of all brain tumours, it is the third most frequently diagnosed brain tumour .The growing of pituitary tumors may compress the surrounding structures especially optic nerve, cause visual field defects including bitemporal hemianopia , visual disturbance. Aims and objective: To estimate the effect of factors affecting visual recovery in pituitary adenoma .Materials and method: this is a prospective, observational study will be conducted on patients of pituitary tumour admitted between January 2020 and January 2022 in the department of Neurosurgery at the G. R. Medical College &J.A. Group of Hospitals, Gwalior(M.P.) Result & Conclusion: In our study 46.7% patients have duration of symptoms < 6 months and 6/6 - 6/24 visual acuity was present in 73.3% patients & 13.3% had optic atrophy(primary/secondary), 43.3%patients had bi-temporal hemianopia. In our study Suprasellar extension was present in 90% of patients & Vascular invasion was seen in 26.7% of patients. Good results are seen in patients who have a lesser duration of symptoms, and good pre-operative visual acuity has improved the final visual outcom

    Background:Despite extraordinary advances in modern radiology and laboratory investigations an accurate diagnosis of acute appendicitis cannot be made in atypical cases. No single diagnostic aid can dramatically reduce the rate of negative appendicectomy.Methods: To reduce the rate of negative appendicectomies by the combined use of diagnostic modalities in a prospective study from July 2011 to December 2012 was done. 100 patients with right lower quadrant abdominal pain fulfilling the inclusion and exclusion criterion underwent appendicectomy in Mahatma Gandhi Medical College and Hospital, Jaipur, India.Results:The results of the diagnostic aids were reported in combination using the ‟or” rule and were correlated with intraoperative and histopathological findings. Chi-square test was applied to calculate the p value for the association between the variables of studied. The mean age was 26±11.25 years (10-59 years) and there were 70 males and 30 females in the study. Histopathological examination confirmed appendicitis in 90 patients with 10 negative appendicectomies. A negative appendicectomy rate of 5.71% in men and 20% in women was observed.Conclusions:Alvarado score in combination with ultrasonography is a valuable tool for diagnosing acute appendicitis inspite of sophisticated investigations like CT, thus reducing the cost of treatment and preventing negative appendicectomy rate.</jats:p

    EVALUATION OF HYPERBILIRUBINAEMIA AS A NEW DIAGNOSTIC MARKER FOR ACUTE APPENDICITIS AND ITS ROLE IN THE PREDICTION OF APPENDICULAR PERFORATION

    BACKGROUND There are various investigations recommended to diagnose acute appendicitis; however, till date there is no confirmatory laboratory marker to diagnose preoperatively acute appendicitis & appendicular perforation. The purpose of study is to evaluate hyperbilirubinaemia as a new diagnostic marker for acute appendicitis and its role in the prediction of appendicular perforation. Preoperative assessment of serum bilirubin appears to be a promising new laboratory marker for diagnosing acute appendicitis & have a predictive potential for the diagnosis of appendicular perforation. METHODS A prospective analytical study of 100 cases comprising of a non-randomised cohort. RESULTS Hyperbilirubinaemia was found in most of the patients diagnosed with acute appendicitis (68.23%) or appendicular perforation (73.33%). The mean total bilirubin level in patients diagnosed with acute appendicitis was 1.34 mg% while in patients diagnosed with appendicular perforation was 2.12 mg%. CONCLUSIONS Preoperative assessment of serum bilirubin should be routinely performed in cases of acute appendicitis as it can help in diagnosis of acute appendicitis as well as also serve as an important maker of acute gangrenous appendicitis

    Finish hard turning: A review of minimum quantity lubrication using paraffin-based nanofluids

    A wide range of cooling techniques for hard turning machining continues to be proposed and assessed. In this review, the overall characteristics of cutting tools and stainless steel materials were reviewed in terms of vibration, surface roughness, cutting force, and tool life while using minimum quantity lubrication (MQL) with paraffin-based nanofluids. Nanoparticles are particularly appealing in MQL due to its remarkable improvement in the cutting conditions. Under aggressive machining conditions, the lubricant media tends to evaporate or disintegrate when in contact with the cutting tool. With the addition of high thermal conductivity nanoparticle additives as cutting fluid, the performance of the MQL technique has improved remarkably. This review exposed that a few work has used MQL with nanofluid when machining martensitic stainless steel AISI 420 using TiAlN-coated carbide cutting tool. Furthermore, the application of MQL via paraffin oil, ?-Fe2O3, and xGnP nanofluid when machining hardened stainless steel using coated carbide cutting tools has not yet been examined
