    Interactions between obstructive sleep apnea syndrome and insulin resistance

    Previous studies have shown Obstructive Sleep Apnea (OSA) as a risk factor for development of cardiovascular and cerebrovascular disease. However, controversies remain as to whether these changes are consequences of the associated obesity or OSA itself results in endocrine and metabolic changes, including impairment of insulin sensitivity, growth hormone, secretion inflammatory cytokines alterations, activation of peripheral sympathetic activity, and hipothalamic-pituitary-adrenal (HPA) axis, that may predispose to vascular disease. Furthermore many cardiovascular risk factors, such as hypertension, obesity, insulin resistance and type 2 diabetes, are strongly associated with OSA. In this article, we will review the evidence and discuss possible mechanisms underlying these links and the pathophysiology of OSA morbidities.Estudos anteriores mostraram que pacientes com Apnéia Obstrutiva do Sono (AOS) apresentam maior risco para doenças cardiovasculares. Entretanto, permanece controverso se essa associação depende da obesidade ou se ocorre devido a alterações fisiológicas decorrentes da desordem do sono, como ativação do sistema nervoso simpático, da inflamação e desordens do eixo corticotrófico e somatotrófico, que predispõem a danos vasculares. Além disso, muitos fatores de risco para doenças cardiovasculares (DCV) estão fortemente associados ao distúrbio respiratório, entre eles hipertensão, obesidade, resistência à insulina e diabetes tipo 2 (DM2).     Flow-by-flow chemical stratigraphy and evolution of thirteen Serra Geral Group basalt flows from Vista Alegre, southernmost Brazil

    The geochemical characterization of thirteen Serra Geral Group flows in the Vista Alegre region (RS-SC), southern Brazil, displays the homogeneous basaltic composition near 50 wt.% SiO2. Each of the five basal flows (Pitanga-type, high-Ti/Y ~600, TiO2 > 3 wt.%) and eight upper flows (Paranapanema-type, medium Ti/Y ~400, TiO2 > 2 wt.%) can be identified from their chemical composition; sets of flows have parallel variation in chemical composition. The flow-by-flowcorrelation in four sections shows the horizontal position of the flows in three profiles and an approximately 200-m downdrop of the Itapiranga block with respect to the Frederico Westphalen block. The world-class amethyst geode mineralization and the systematic presence of native copper in the basalts make the correlation of great geological and economic significance.<br>A caracterização geoquímica de treze derrames do Grupo Serra Geral na região de Vista Alegre (RS e SC), sul do Brasil, exibe uma composição basáltica homogênea próxima a 50% de SiO2. Os cinco derrames basais são classificados quimicamente como tipo Pitanga (alto Ti/Y ~600 e TiO2 > 3 em peso percentual), os demais oito derrames possuem médio Ti/Y ~400 com TiO2 ~2.5 em peso percentual, sendo classificados como magma tipo Paranapanema. Cada derrame pode ser identificado através de sua composição química e correlacionado, com variação paralela entre os perfis estudados. A correlação derrame a derrame nos quatro perfis demonstra uma posição horizontal em três perfis e um rejeito vertical de aproximadamente 200 m do bloco Itapiranga em relação ao bloco Frederico Westphalen. A presença de jazidas de ametista em geodos e a sistemática ocorrência de cobre nativo nos basaltos da região tornam a correlação de grande significado geológico e econômico

    CT evaluation of apical canal transportation associated with stainless steel hand files, oscillatory technique and ProTaper rotary system

    This study used computed tomography (CT) to compare apical canal transportation in mesiobuccal canals of maxillary molars prepared with different techniques. Sixty teeth were assigned to 3 groups (n=20), according to the technique used for root canal instrumentation: hand instrumentation with K-Flexofiles, K-Flexofiles activated by an oscillatory system and ProTaper NiTi rotary system. Pre and postinstrumentation CT images were obtained 3 mm short of the apical foramen and were superimposed to compare canal transportation. Data were analyzed statistically by ANOVA and Tukey’s test using the SPSS software (α=0.05). In the buccal direction, the manual technique produced significantly less canal transportation than the oscillatory technique (p<0.05) and both were similar to the rotary technique (p>0.05). In the distal and distopalatal directions, the oscillatory technique produced more canal transportation (p<0.05). In the mesiopalatal direction, the oscillatory technique produced more canal transportation than the manual technique (p<0.05), and both were similar to the rotary technique (p>0.05). In conclusion, all techniques produced canal transportation, and the oscillatory technique produced the greatest removal of root dentin toward the innerside of the root curvature.Este estudo utilizou a tomografia computadorizada (TC) para comparar o transporte do canal radicular nos canais mésiovestibulares de molares superiores preparados por diferentes técnicas. Sessenta molares superiores foram divididos em 3 grupos (n=20) de acordo com a técnica utilizada para o preparo do canal radicular: instrumentação manual com limas K-Flexofile, limas K-Flexofile acopladas a um sistema oscilatório e sistema rotatório ProTaper. Imagens de TC pré e pós-instrumentação foram obtidas 3 mm aquém do forame apical e superpostas para comparar o transporte do canal. Os dados foram analisados estatisticamente pela Análise de Variância (ANOVA) e Teste de Tukey utilizando o software SPSS (α=0,05). Na direção vestibular, a técnica manual produziu significantemente menos transporte do canal radicular do que a técnica oscilatória (p<0,05) e ambas foram similares a técnica rotatória (p>0,05). Nas direções distal e disto-palatina, a técnica oscilatória produziu mais transporte do canal radicular (p<0,05). Na direção mésio-palatina, a técnica oscilatória produziu mais transporte do canal radicular do que a técnica manual (p<0,05), sendo que ambas foram similares à técnica rotatória (p>0,05). Em conclusão, todas as técnicas produziram transporte do canal radicular e a técnica oscilatória produziu os maiores desgastes de dentina na direção interna da curvatura

    Improved Health-Related Quality of Life in Older Kidney Recipients 1 Year After Transplantation

    Background. We aimed to evaluate changes in health-related quality of life (HRQoL) in patients 65 years or older from time of kidney transplantation (KTx) until 1 year postengraftment. Methods. A single-center prospective study was conducted. HRQoL was measured pre-KTx and at 2, 6, and 12 months postengraftment using self-reported Kidney Disease and Quality of Life short-form version 1.3. Intraindividual scores before and after KTx were evaluated. Liu Comorbidity Index was registered at enlisting. short-form-36 scores were additionally compared with scores from an age-matched population. Results. From January 1, 2013, until November 30, 2016, a total of 289 waitlisted patients were included. By September 1, 2017, 134 had reached 1 year postengraftment, and valid questionnaires were available in 120 (90%) patients. Mean age at KTx was 71.6 years (±4.3 years), 71% were male. Living donor was used in 21%, and preemptive KTx was performed in 30% of the recipients. Median waiting time for KTx from deceased donor was 16 months (range, 0.6-50.5 months). A total of 79 (66%) recipients had a Liu Comorbidity Index score of 3 or less. All HRQoL scores except the domain social function improved at 2 months postengraftment and remained stable or continued to improve at 1 year. HRQoL scores 12 months postengraftment were similar to those described in an age-matched general population except for the domain social function which remained at a significantly lower level. Time in dialysis was the most important variable associated with impaired HRQoL postengraftment. Conclusions. HRQoL scores showed clinically significant improvement in older KTx recipients 1 year posttransplant
