Vehical Reversing: Pojecting Scarcity is Facing an “Identity Crisis”: To Cluster A Monotheist Perception on Contemporary Sacred Architecture
The sacred manifests with multi-valence; different aspects define how sacred a space can be. Worshipping spaces are one of the renowned types of Architecture which gets adapted to various scenarios and locations. Over 60% of Muslims are living in countries under civil law where mosque figure plays a significant role in their daily life. There are predefined images that an Interfaith person (Individuals of different religions) resonates; minarets, domes, and ornaments. The UK was introduced to Islam in the 18th Century yet its Holy spaces are still seen as an imposed urban layer. This research addresses the Islamic Sacred Architecture (ISA) with a special focus on spatial observance and spiritual connection. It evokes whether ISA is adapting to the Western design trends and forgetting its traditional roots. Taking a questionnaire-based approach (n=40), the study provides insights on how the Muslim population of the UK perceives the modernism of ISA. Such movement may result in traditional elements with religious belongings facing an “identity crisis”. Clustering analysis was conducted creating personas of the Public perception: The Devoted (20%), The Considerate (35%), and The Apathetic (45%). Building on the existing literature, the contemporary qualities that eliminate the main features of ISA test practitioners. Initial findings reported that the modern ISA is debatable where views of the Public are between acceptance and negligence. The outcomes highlight the importance of understanding the design requirements associated with Holy spaces, and the role of the architect in designing upon. Modern Architecture is not a failure, but claiming itself as a new identity of the traditional ISA is

Unusual sites of hepatocellular carcinoma metastasis: Case report.
Hepatocellular carcinoma is an increasingly frequent cause of cancer-related death. The majority of patients with hepatocellular carcinoma are asymptomatic. In rare cases, patients may present with symptoms of extrahepatic metastases. Early identification can lead to timely treatment and prevent poor outcomes. We report three cases of patients with hepatocellular carcinoma with unusual sites of metastasis, including clival, mandible, and cardiac involvement

Fungal infections in liver cirrhosis
Liver cirrhosis is a chronic condition that is associated with a variety of complications across organ systems. Patients with cirrhosis also suffer from immune dysfunction, which may predispose them to catastrophic bacterial and fungal infections. Bacterial infections in liver cirrhosis have been well-documented, however, data remains scarce regarding fungal infections. Candida and Aspergillus have been reported as the most common pathogens among patients with cirrhosis, causing both invasive and non-invasive infections. However, other pathogens such as Coccidioides, Pneumocystis, Cryptococcus, and Rhizopus have been increasing in incidence. Diagnosis of fungal infection is often difficult, particularly in regards to distinguishing colonization from invasive infection. Serum markers such as beta-D-glucan (BDG) and galactomannan are beneficial diagnostic tools in conjunction with fungal cultures and imaging modalities. Bronchoscopy with bronchoalveolar lavage (BAL) or lung biopsy can be useful adjuncts as well. Liver transplantation is another important consideration as invasive fungal infection (IFI) is a contraindication to transplant surgery. Additionally, patients are at increased risk for infection due to immunosuppression in the post-transplant period. We aim to discuss the mechanisms responsible for immune dysfunction in advanced liver disease, the epidemiology of fungal infections in this population, as well as presentations and management considerations pertaining to specific pathogens and antifungal regimens
Geospatial Analysis of COVID-19 and Otolaryngologists Above Age 60
Objective: The 2019 novel coronavirus (COVID-19) is disproportionately impacting older individuals and healthcare workers. Otolaryngologists are especially susceptible with the elevated risk of aerosolization and corresponding high viral loads. This study utilizes a geospatial analysis to illustrate the comparative risks of older otolaryngologists across the United States during the COVID-19 pandemic. Methods and materials: Demographic and state population data were extracted from the State Physician Workforce Reports published by the AAMC for the year 2018. A geospatial heat map of the United States was then constructed to illustrate the location of COVID-19 confirmed case counts and the distributions of ENTs over 60 years for each state. Results: In 2018, out of a total of 9578 practicing U.S. ENT surgeons, 3081 were older than 60 years (32.2%). The states with the highest proportion of ENTs over 60 were Maine, Delaware, Hawaii, and Louisiana. The states with the highest ratios of confirmed COVID-19 cases to the number of total ENTs over 60 were New York, New Jersey, Massachusetts, and Michigan. Results: In 2018, out of a total of 9578 practicing U.S. ENT surgeons, 3081 were older than 60 years (32.2%). Human aquaporins: regulators of transcellular water flow
Background: Emerging evidence supports the view that (AQP) aquaporin water channels are regulators of transcellular
water flow. Consistentwith their expression in most tissues, AQPs are associatedwith diverse physiological
and pathophysiological processes.
Scope of review: AQP knockout studies suggest that the regulatory role of AQPs, rather than their action as passive
channels, is their critical function. Transport through all AQPs occurs by a common passive mechanism, but their
regulation and cellular distribution varies significantly depending on cell and tissue type; the role of AQPs in cell
volumeregulation (CVR) is particularly notable. This reviewexamines the regulatory role of AQPs in transcellular
water flow, especially in CVR.We focus on key systems of the human body, encompassing processes as diverse as
urine concentration in the kidney to clearance of brain oedema.
Major conclusions: AQPs are crucial for the regulation of water homeostasis, providing selective pores for the
rapidmovement ofwater across diverse cellmembranes and playing regulatory roles in CVR. Gatingmechanisms
have been proposed for human AQPs, but have only been reported for plant andmicrobial AQPs. Consequently, it
is likely that the distribution and abundance of AQPs in a particular membrane is the determinant of membrane
water permeability and a regulator of transcellular water flow.
General significance: Elucidating the mechanisms that regulate transcellular water flow will improve our understanding
of the human body in health and disease. The central role of specific AQPs in regulating water homeostasis
will provide routes to a range of novel therapies. This article is part of a Special Issue entitled Aquaporins
Fundus autofluorescence imaging: Fundamentals and clinical relevance.
Fundus autofluorescence (FAF), a relatively new imaging modality, focuses on the fluorescent properties of pigments in the retina to generate images that help us view various disease processes from a different perspective. It aids us in the understanding of the pathophysiology of different retinal disorders. Recently, FAF imaging is being used commonly to help us in the diagnosis, prognosis as well as in determining the treatment response of various retinal disorders. It generates an image based on the distribution pattern of a fluorescent pigment called lipofuscin. Knowing the distribution pattern of lipofuscin in the normal retina is key to understanding an FAF image representing a retinal pathology. Like most other imaging modalities, FAF comes with its own limitations, taking steps to overcome these limitations will be of utmost importance in using this imaging modality to its fullest potential
Variation of choroidal thickness and vessel diameter in patients with posterior non-infectious uveitis
Abstract Background
Choroidal thickness (CTh) and choroidal vessel diameter (VD) in the Haler’s layer were evaluated as markers of inflammatory insult in non-infectious uveitis (NIU). Spectral-domain optical coherence tomography (Spectralis®, Heidelberg Engineering Inc.) scans were acquired from 23 normal subjects (39 eyes – group 1), 7 subjects with high myopia (14 eyes – group 2), and 19 patients with NIU (23 eyes – group 3). In groups 1 and 2, CTh and VD were measured at 3 different points of the same horizontal OCT scan passing through the fovea and a mean calculated. Mean CTh and VD were calculated in 2 other locations, 2 mm superior and inferior from the chosen foveal horizontal scan. In group 3, three measurements of CTh and VD were obtained within 1 mm of a horizontal scan passing through a retinal lesion; mean CTh and VD were then computed. A ratio (R) between the VD and the corresponding CTh was calculated. Results
Group 1, 2 and 3 mean age was 29.6, 29.1 and 45.9 years, respectively. Sixteen normal subjects, three myopic subjects and six NIU patients were male.. Group 1 mean CTh did not differ from group 2 (261.6±45.6 vs. 260.2±50.6 µm µm; p\u3e0.05); mean VD was marginally higher in Group 2 (159.8±32.2 vs. 163.2±33.2 µm; p\u3e0.05). Group 3 demonstrated thinner CTh (193.6±54.6 µm) than Groups 1 and 2 (p = 0.02 and \u3c0.001). Group 3 mean VD (123.6±37.4 µm) was also less than that in Groups 1 and 2; the difference was statistically significant only when compared to group 2, p = 0.01. R did not differ across groups (p-values \u3e0.05), indicating that variations in CTh and VD followed the same trend. Conclusions
The study reports potential quantitative OCT-derived parameters that may be explored in future trials of non-infectious uveitis. Thinning of choroid and decrease of vessel diameter are observed in patients with chronic NIU compared to controls
