    The health and wellbeing of adults working in early childhood education

    This article reports the results of a survey of 168 New Zealand early childhood workers and describes their health status, behaviours and concerns. The respondents included 73 childcare teachers, 58 kindergarten teachers and 37 home-based educators. Although 92 per cent of respondents reported that they had good or excellent health, statistically significant differences were found between the groups for days absent because of illness, accidental injuries, job-related stress and ergonomic aspects of their work. All groups reported an increase in various physical symptoms since working with children, in particular backaches, muscle strain and fatigue. One-quarter of respondents experienced an illness related to their work with children during the past year, most commonly respiratory and gastrointestinal illnesses. The study alerts early childhood education employers to the importance of managing health issues such as workload and stress, occupational injuries, the provision of an ergonomically healthy work environment and adequate conditions of employment

    Rare coding variants in PLCG2, ABI3, and TREM2 implicate microglial-mediated innate immunity in Alzheimer's disease

    We identified rare coding variants associated with Alzheimer’s disease (AD) in a 3-stage case-control study of 85,133 subjects. In stage 1, 34,174 samples were genotyped using a whole-exome microarray. In stage 2, we tested associated variants (P<1×10-4) in 35,962 independent samples using de novo genotyping and imputed genotypes. In stage 3, an additional 14,997 samples were used to test the most significant stage 2 associations (P<5×10-8) using imputed genotypes. We observed 3 novel genome-wide significant (GWS) AD associated non-synonymous variants; a protective variant in PLCG2 (rs72824905/p.P522R, P=5.38×10-10, OR=0.68, MAFcases=0.0059, MAFcontrols=0.0093), a risk variant in ABI3 (rs616338/p.S209F, P=4.56×10-10, OR=1.43, MAFcases=0.011, MAFcontrols=0.008), and a novel GWS variant in TREM2 (rs143332484/p.R62H, P=1.55×10-14, OR=1.67, MAFcases=0.0143, MAFcontrols=0.0089), a known AD susceptibility gene. These protein-coding changes are in genes highly expressed in microglia and highlight an immune-related protein-protein interaction network enriched for previously identified AD risk genes. These genetic findings provide additional evidence that the microglia-mediated innate immune response contributes directly to AD development

    The personal safety of district nurses: A critical analysis

    A workplace safety study of district nurses in New Zealand was conducted to explore personal safety experiences. A qualitative methodology informed by Critical Social Theory was employed. This paper details the findings and implications derived from data collected from six district nurses in two urban New Zealand Health Services who recalled incidents in which they felt their personal safety was compromised. Data were collected through individual interviews and a focus group discussion with the participants. Data analysis revealed two-fold risks to nurse safety; these were associated with client behaviour as well as risks embedded in the organisational structure. Findings suggest a number of practical issues involving basic security measures require urgent attention and would mitigate the risk arising institutional policy that prioritises nurse safety, and has in place post-incident plans and support structures. The complex power relationships that shape the experience of nursing in a community impinged on the ability of the nurses in this study to confidently and safely fulfil their role. An organisational commitment to a culture of safety would help address the powerlessness experienced by district nurses

    Obesity in pre-school children: Issues and challenges for community based child health nurses

    Childhood obesity is becoming a topical issue in both the health literature and the popular media and increasingly child health nurses are observing preschool children who appear to be disproportionately heavy for their height when plotted on standardised growth charts. In this paper literature related to childhood obesity in New Zealand and internationally is explored to identify current issues, and the implications of these issues for nurses in community based child health practice are discussed. Themes that emerged from the literature relate to the measurement of obesity, links between childhood and adult obesity and issues for families. A theme in the literature around maternal perception was of particular interest. Studies that investigated maternal perceptions of childhood obesity found that mothers identified their child as being overweight or obese only when it imposed limitations on physical activity or when the children were teased rather than by referring to individual growth graphs. The implications for nursing in the area of child health practice is discussed as nurses working in this area need an understanding of the complex and often emotive issues surrounding childhood obesity and an awareness of the reality of people's lives when devising health promotion strategies

    A survey of role stress, coping and health in Australian and New Zealand hospital nurses

    Background: Previous research has identified international and cultural differences in nurses' workplace stress and coping responses. We hypothesised an association between problem-focused coping and improved health, emotion-focused coping with reduced health, and more frequent workplace stress with reduced health. Objectives: Test the above hypotheses with Australian and New Zealand nurses, and compare Australian and New Zealand nurses' experience of workplace stress, coping and health status. Participants and settings: Three hundred and twenty-eight New South Wales (NSW) and 190 New Zealand (NZ) volunteer acute care hospital nurses (response rate 41%) from randomly sampled nurses. Design and method: Postal survey consisting of a demographic questionnaire, the Nursing Stress Scale, the WAYS of Coping Questionnaire and the SF-36 Health Survey Version 2. Results: Consistent with hypotheses, more frequent workplace stress predicted lower physical and mental health. Problem-focused coping was associated with better mental health. Emotion-focused coping was associated with reduced mental health. Contrary to hypotheses, coping styles did not predict physical health. NSW and NZ scored effectively the same on sources of workplace stress, stress coping methods, and physical and mental health when controlling for relevant variables. Conclusions: Results suggest mental health benefits for nurses who use problem-solving to cope with stress by addressing the external source of the stress, rather than emotion-focused coping in which nurses try to control or manage their internal response to stress. Cultural similarities and similar hospital environments could account for equivalent findings for NSW and NZ. © 2006 Elsevier Ltd. All rights reserved

    The relationship between workplace stress, coping strategies and health status in New Zealand nurses

    This study was part of an international project examining workplace stress among nurses and their coping strategies, and the relationship between stress, coping and health in the Asia-Pacific region. The aim of the present study was to identify dominant workplace stressors for New Zealand nurses, their most used coping strategies, and variables that best predict their mental and physical health. Postal surveys were sent to 190 randomly selected New Zealand nurses employed in clinical areas. Workload was the most common stressor, while "planful" problem-solving, seeking social support, and self-controlling were the most frequently used ways of coping. The link between stressors such as workload and reduced mental health is concerning, especially as effective coping strategies such as problem-solving are already predominantly used by nurses. The findings suggest that nurses' mental health could benefit from a workload that minimises stress, and from increased support in the workplace and encouragement of planned problem-solving
