    Integration Of Hec-Ras And Hec-Hms With Gis In Flood Modeling And Flood Hazard Mapping

    Floods are among the most devastating disasters in terms of socio-economics and casualties. However, these natural disasters can be managed and their effects can be minimized by flood modeling performed before the occurrence of a flood. In this study, flood modeling was developed for the Goksu River Basin, Mersin, Turkiye. Flood hazard and risk maps were prepared by using GIS, HEC-RAS, and HEC-HMS. In hydraulic modeling, Manning's n values were obtained from 2018 CORINE data, return period flow rates (Q25, Q50, Q100, Q500) were obtained from HEC-HMS, and the application was carried out on a 5 m resolution digital surface model. In the study area, the water depths could reach up to 10 m, and water speeds were approximately 0.7 m/s. Considering these values and the fact that the study area is an urban area, hazard maps were obtained according to the UK Department for Environment, Food and Rural Affairs (DEFRA) method. The results indicated that possible flood flow rates from Q25 to Q500, from 1191.7 m3/s to 1888.3 m3/s, were detected in the study area with HEC-HMS. Flooding also occurred under conditions of the Q25 flow rate (from 4288 km2 to 5767 km2), and the impacted areas were classified as extremely risky by the DEFRA method.Sustainabilit

    Evaluation of Relationship Between Mandibular Third Molars and Mandibular Canal: Comparison of Findings Obtained by Panoramic Radiography and Cone-Beam Computed Tomography

    Statement of the problem: It is important for oral surgeons to determine the precise location of mandibular third molar before extraction. The close rela-tionship of mandibular canal and mandibular third molar is a risk factor for alveolar nerve damage. Objective: The purpose of this study is to assess the relationship between mandibular canal and the mandibular third molar which has already been seen in close relationship on panoramic radiograph (PR) by the compari-son of findings of the cone-beam computed tomography (CBCT) and the PR. Materials & Methods: Close relationship between the mandibular third molar and mandibular canal was scanned in 180 patients by the PR. Images were also obtained by CBCT from patients among which a close-relationship was detected between mandibular canal and mandibular third molar. Results: Close relationship was detected between mandibular third mo-lar and mandibular canal on 26 of 180 PR and evaluated 46 mandibular third molars. In 41 cases, both the findings in PR and in CBCT images showed comp-liance with each other. As a result it was clearly seen that the mandibular third molars which were detected to be in close relationship on PR were mostly also in close relationship with mandibular canal on CBCT-images. Conclusions: If the close relationship is not seen on the panoramic ima-ges then there is no need to get CBCT for extracting the mandibular third mo-lars. By this way complication risks are eliminated with less radiation. However CBCT should be definitely used in cases which complete assessment cannot be made in no way due to artifacts such as superposition in PR

    Some drugs have been known to damage to esophagus for a long time. Half of the cases reported are of tetracycline and its derivatives. The damage caused by these drugs is depends on the drug itself and the patient. In this paper we present 5 patients having diagnosed esophageal damage endoscopically after due to doxycyclin use. The mean age of the patients was 26 years. Three of them for acne and 2 for heir complaints gynecological infection were taking these drugs. Lesions were located at the middle in 4 cases and lover part in 1 patient. The common complaint was retrosternal pain and heartburn after taking the drug with insufficient water or without water. All the patients were relieved by symtomatic teratment. Esophageal damage is to be remembered in patients complaning sudden pain and difficult swallowing on doxycyclin treatment and endoscopic procedure should be employed for definition of diagnosis and evaluation of the severity of the damage. After treatment, endoscopic control is not necessary. Physicians must not forget to advice the patients to take these drugs with splendid amount of water.Bir çok ilacın özefagus hasarlarına neden olduğu uzun zamandır bilinmektedir. Şimdiye kadar bildirilen vakaların yaklaşık yarısı tetrasiklin grubu ilaçlara bağlıdır. İlaçların özefagusta oluşturduğu hasarlar, ilacın kendisine ve ilacı alan hastaya bağlı nedenlerle oluşmaktadır. Yazımızda doksisiklin kullanımına bağlı endoskopik olarak tespit edilen özefagus hasarları gelişen 5 hasta sunulmuştur. Hastaların yaş ortalaması 26 yıl idi. Dört hasta kadın, 1 hasta erkekti. Hastaların 3’ü akne nedeniyle, 2’si jinekolojik enfeksiyonlar nedeniyle bu ilacı kullanmaktaydı. Lezyonların 4’ü özefagus orta kesimde 1’i distale yakın yerleşimliydi. Ortak yakınmaları ilacı susuz veya az bir suyla alımından hemen sonra başlayan retrosternal ağrı ve yutma güçlüğüydü. Hastaların tamamı semptomatik tedaviyle düzeldi. Sonuç olarak doksisiklin kullanan, ani başlayan retrosternal ağrı ve yutma güçlüğü tarifleyen hastalarda özefagus hasarı akla gelmeli ve tanıyı kesinleştirmek ve hasarın şiddetini değerlendirmek için mutlaka endoskopi yapılmalıdır. Tedaviden sonra endoskopik kontrol her zaman gerekli değildir. Doksisiklin kullanan hastalara ilacı susuz kesinlikle içmemeleri önerilmelidi

    Thermal analysis of oriental beech wood treated with some borates as fire retardants

    Wood is widely used as a construction material for many applications. To have knowledge about the thermal degradation characteristic of wood could be crucial for indoor and outdoor applications. Chemical treatments could improve the resistance of wood against fire and heat. In this study, to investigate the thermal properties of Oriental beech (Fagus orientalis) wood treated with 0,25%; 1% and 4,70% aqueous solutions of boric acid, borax, disodium octaborate tetrahydrate and mixtures of and in different proportions, thermogravimetric analysis, differential-thermogravimetry, and differentialthermal analysis were performed. Our results showed that borate treatment decreased the Tmax (maximum degradation temperature) and increased residual char amount. Higher concentration levels of borates resulted in higher char content of Oriental beech wood specimens. Residual char content of borate treated wood samples at fixed concentration of 4,70% ranked as in the following order: disodium octaborate tetrahydrate > boric acid + borax (7:3) > boric acid + borax (1:1) > boric acid > borax >Control. The highest residual char content was achieved for the sample impregnated with 4,70% disodium octaborate tetrahydrate among the all treated samples

    Radiographic and histopathologic evaluation of radiolucent lesions involving impacted teeth: A multicenter study

    Objective: The purpose of this retrospective study was to evaluate the radiographic and histopathologic features of the pathologic lesions associated with an impacted tooth in the maxilla and mandible of patients who were admitted to three different university hospitals located in different cities. Materials and Methods: One hundred one patients (36 females and 65 males) aged between 8 and 67 and who have radiolucent lesions associated with the impacted teeth were included in this study. Data related to the age and gender of the patients, and the findings of cone-beam computed tomography, and histopathologic diagnosis of the lesions were recorded and analyzed. Results: Majority of the lesions were in the posterior region of the mandible (62.4%), related to the mandibular third molars (59.4%), and were diagnosed as a dentigerous cyst. The most common features of the lesions were unilocular radiolucency (91.1%), well-circumscribed (90.1%), and expansive (85.1%). A statistically significant relationship was found between the migration of the impacted tooth/teeth related to the lesion (p<0.05) and the expansion of the lesion (p<0.01) according to gender. A statistically significant relationship was found between the migration of the impacted tooth/teeth related to the lesion (p<0.05) and the histopathological diagnosis of the lesion (p<0.01) according to age groups. Conclusion: Knowing all of the clinical, radiological and histopathological features of the lesions provide the surgeon to reach the correct diagnosis. Thus, the doctors achieve high success in treatment with the right treatment plan.Amaç: Bu retrospektif çalışmanın amacı, farklı şehirlerde bulunan üç farklı üniversite hastanesine başvuran hastaların üst ve alt çenesindeki gömülü diş ile ilişkili patolojik lezyonların radyografik ve histopatolojik özelliklerini değerlendirmektir. Gereç ve Yöntemler: Bu çalışmaya gömülü diş ile ilişkili radyolüsent lezyonu olan 8-67 yaş aralığındaki 101 hasta (36 kadın ve 65 erkek) dahil edildi. Hastaların yaşı ve cinsiyeti, lezyonların konik-ışınlı bilgisayarlı tomografi bulguları ve histopatolojik tanısı ile ilgili veriler kaydedildi ve analiz edildi. Bulgular: Lezyonların çoğu mandibula posterior bölgede (%62,4), mandibular üçüncü molar dişlerle (%59,4) ilişkiliydi ve dentigeröz kist tanısı aldı. Lezyonlarda en fazla görülen özellikler uniloküler radyolüsensi (%91,1), iyi sınırlı (%90,1) ve ekspansif (%85,1) olması idi. Cinsiyete göre lezyon ile ilişkili gömülü diş/dişlerin migrasyonu (p<0,05) ve lezyonun ekspansiyonu (p<0,01) arasında istatiksel olarak anlamlı bir ilişki bulundu. Yaş gruplarına göre lezyon ile ilişkili diş/dişlerin migrasyonu (p<0,05) ve lezyonun histopatolojik tanısı (p<0,01) arasında istatiksel olarak anlamlı bir ilişki bulundu. Sonuç: Lezyonların tüm klinik, radyolojik ve histopatolojik özelliklerinin bilinmesi cerrahın doğru tanıya ulaşmasını sağlar. Böylece hekimler doğru tedavi planı ile tedavide yüksek başarı elde ederler

    A retrospective evaluation of condylar morphology using panoramic radiography in a sample of Turkish population

    Background: The temporomandibular joint is one of the most complex anatomic structures. It takes a great role in masticatory system and helps to make possible some functions such as speaking, chewing and swallowing. Clinicians should have sufficient anatomical knowledge to assess relationships of the hard and soft tissues, including the mandibular condyle, glenoid fossa, articular eminence of the temporal bone, the articular disc and its attachments. The aim of this study was to evaluate the frequency of different types of mandibular condyles and its distribution according to the age and gender. It was also evaluated whether the condyle types were bilaterally symmetrical or not. Materials and methods: A total of 1315 digital panoramic images which obtained from the patients suffering from the dental problems were assessed. Demographic data, condylar morphology were noted. All obtained data were analyzed by using descriptive statistics. Morphology of mandibular condyles were classified into four shapes as identified in other studies, namely: Type I - Oval shape, Type II - Diamond shape, Type III - Bird beak shape, Type IV - Crooked finger shape. Two independent examiners, who have 19 and 7 years of experience in oral and dentomaxillofacial radiology, made a consensus and evaluated all images. Results: A total of 1315 digital panoramic images were assessed. 767[58.3%] of the patients were female and 548[41.6%] were male. The age range of patients was from 18 to 84 years. Right-left condyle types were found to be symmetrical in the range of 67% of the subpopulation examined in the study. For the consensus, ‘oval’ condyle was common on both the right and left, while ‘crooked finger’ condyle was the rarest. Conclusions: The TMJ is the most important structure for all jaw functions such as speech, swallowing. In order for all these functions to continue in a healthy way, the anatomical structure should be known very well down to the finest detail. Identification of anatomical structures and their variations can play an important role in implant dentistry. Clinicians commonly prefer conventional radiologic methods to evaluate dentomaxillofacial region. Although the most of the variations are asymptomatic and require no treatment, correct identification of these findings will reduce unnecessary further diagnostic assessments and will provide more appropriate treatment plans

    Characterization of greater middle eastern genetic variation for enhanced disease gene discovery

    The Greater Middle East (GME) has been a central hub of human migration and population admixture. The tradition of consanguinity, variably practiced in the Persian Gulf region, North Africa, and Central Asia1-3, has resulted in an elevated burden of recessive disease4. Here we generated a whole-exome GME variome from 1,111 unrelated subjects. We detected substantial diversity and admixture in continental and subregional populations, corresponding to several ancient founder populations with little evidence of bottlenecks. Measured consanguinity rates were an order of magnitude above those in other sampled populations, and the GME population exhibited an increased burden of runs of homozygosity (ROHs) but showed no evidence for reduced burden of deleterious variation due to classically theorized ‘genetic purging’. Applying this database to unsolved recessive conditions in the GME population reduced the number of potential disease-causing variants by four- to sevenfold. These results show variegated genetic architecture in GME populations and support future human genetic discoveries in Mendelian and population genetics

    Protective psychosocial factors and dental caries in children and adolescents: a systematic review and meta-analysis

    BACKGROUND: Psychosocial protective factors include dispositional and family attributes that may reduce the occurrence of dental caries. AIM: This review analysed the evidence on the relationship between protective psychosocial factors and dental caries in children and adolescents. DESIGN: Primary studies involving children and adolescents were searched in the following electronic databases: Medline, SCOPUS, LILACS, SciELO, and Web of Science. The reference lists were also screened. Protective psychosocial factor descriptors were in accordance with the salutogenic theory. The outcome was clinical measure of dental caries. Quality assessments were performed using the Newcastle-Ottawa scale. RESULTS: The final search resulted in 35 studies, including 7 cohort, one case-control, and 27 cross-sectional studies. Most studies were of moderate quality. Meta-analyses revealed that low parental internal locus of control (cohort studies: OR = 1.42, 95% CI: 1.20-1.64; cross-sectional studies: OR = 1.30, 95% CI: 1.19-1.41), high parental external chance (OR = 1.20, 95% CI: 1.10-1.29), and high maternal sense of coherence (OR = 0.77, 95% CI: 0.62-0.93) were associated with dental caries in children. High social support (OR = 0.81, 95% CI: 0.68-0.93) and greater self-efficacy (OR = 1.50, 95% CI: 1.12-1.22) were also associated with dental caries in adolescents. CONCLUSIONS: The current evidence suggests that some salutogenic factors are important protective factors of dental caries during childhood and adolescence

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

    Abstract: Purpose: Intensive care unit (ICU) patients are particularly susceptible to developing pressure injuries. Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P &lt; 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction.
