Optimizing fire station locations for the Istanbul metropolitan municipality
Copyright @ 2013 INFORMSThe Istanbul Metropolitan Municipality (IMM) seeks to determine locations for additional fire stations to build in Istanbul; its objective is to make residences and historic sites reachable by emergency vehicles within five minutes of a fire station’s receipt of a service request. In this paper, we discuss our development of a mathematical model to aid IMM in determining these locations by using data retrieved from its fire incident records. We use a geographic information system to implement the model on Istanbul’s road network, and solve two location models—set-covering and maximal-covering—as what-if scenarios. We discuss 10 scenarios, including the situation that existed when we initiated the project and the scenario that IMM implemented. The scenario implemented increases the city’s fire station coverage from 58.6 percent to 85.9 percent, based on a five-minute response time, with an implementation plan that spans three years

Sexual and urinary functioning after rectal surgery: a prospective comparative study with a median follow-up of 8.5 years
The purpose of this study was to prospectively compare rectal resection (RR) with colonic resection on sexual, urinary and bowel function and quality of life in both short-term and long-term. Eighty-three patients who underwent RR were compared to 53 patients who underwent a colonic resection leaving the rectum in situ (RIS). A questionnaire assessing sexual, urinary and bowel functioning with a quality of life questionnaire (SF-36) was sent to all participants preoperatively, 3 and 12 months postoperatively and approximately 8 years after the onset of the study. Short-term dysfunction included diminished sexual activity in female RR patients at 3 months and significantly more erectile dysfunction in RR patients 1 year postoperatively. Long-term dysfunction included more frequent and more severe erectile dysfunction in RR patients compared to RIS patients. These short-term and long-term outcomes did not influence overall quality of life. The incidence of urinary dysfunction was comparable between both groups. Bowel functioning was significantly better in the RIS group compared to the RR group 3 months and 1 year postoperatively. Patients who underwent RR experienced up to 1 year postoperatively more sexual and bowel function problems than RIS patients. However, short-term and long-term dysfunction did not influence overall quality of life. Erectile dysfunction in male RR patients persisted in time, whereas other aspects of sexual, urinary and bowel function after RR and colonic resection are similar after a median follow-up of 8.5 year
Quality of life of women with a screen-detected versus clinically detected breast cancer in the Netherlands:a prospective cohort study
Purpose: Breast cancer (BC) screening enables early detection of BC, which may lead to improved quality of life (QoL). We aim to compare QoL between women with a screen-detected and clinically detected BC in the Netherlands. Methods: We used data from the ‘Utrecht cohort for Multiple BREast cancer intervention studies and Long-term evaluation’ (UMBRELLA) between October 2013 and March 2022. Patients were categorized as screen-detected or clinically detected. We analysed three questionnaires, namely EORTC QLQ C-30, BR23, and HADS (Hospital Anxiety and Depression Scale) completed by BC patients shortly after diagnosis (T1) and one-year after treatment (T2). Independent t-tests were performed to compare QoL average differences between the two groups. Bonferroni-corrected p-value significance threshold of 0.00057 was used. The magnitude of differences was calculated using Cohen’s d. The clinical relevance of QLQ-C30 differences was assessed based on interpretation guideline of EORTC-QLQ-C30 results.Results: After applying inclusion and exclusion criteria, there were 691 women with screen-detected BC and 480 with clinically detected BC. Generally, screen-detected BC patients reported a better QoL. At T1, their average QLQ-C30 summary score was higher (86.1) than clinically detected BC patients (83.0) (p < 0.0001). Cohen’s d for all items ranged between 0.00 and 0.39. A few QLQ-C30 score differences were clinically relevant, indicating better outcomes in emotional functioning, general health, constipation, and fatigue for women with screen-detected BC. Conclusions: In the Netherlands, women with screen-detected BC reported statistically significant and better QoL than women with clinically detected BC. However, clinical relevance of the differences is limited.</p
