Background Independence and Asymptotic Safety in Conformally Reduced Gravity
We analyze the conceptual role of background independence in the application
of the effective average action to quantum gravity. Insisting on a background
independent renormalization group (RG) flow the coarse graining operation must
be defined in terms of an unspecified variable metric since no rigid metric of
a fixed background spacetime is available. This leads to an extra field
dependence in the functional RG equation and a significantly different RG flow
in comparison to the standard flow equation with a rigid metric in the mode
cutoff. The background independent RG flow can possess a non-Gaussian fixed
point, for instance, even though the corresponding standard one does not. We
demonstrate the importance of this universal, essentially kinematical effect by
computing the RG flow of Quantum Einstein Gravity in the ``conformally
reduced'' Einstein--Hilbert approximation which discards all degrees of freedom
contained in the metric except the conformal one. Without the extra field
dependence the resulting RG flow is that of a simple -theory. Including
it one obtains a flow with exactly the same qualitative properties as in the
full Einstein--Hilbert truncation. In particular it possesses the non-Gaussian
fixed point which is necessary for asymptotic safety.Comment: 4 figures

Hints of (trans-Planckian) asymptotic freedom in semiclassical cosmology
We employ the semiclassical approximation to the Wheeler-DeWitt equation in
the spatially flat de Sitter Universe to investigate the dynamics of a
minimally coupled scalar field near the Planck scale. We find that, contrary to
naive intuition, the effects of quantum gravitational fluctuations become
negligible and the scalar field states asymptotically approach plane-waves at
very early times. These states can then be used as initial conditions for the
quantum states of matter to show that each mode essentially originated in the
minimum energy vacuum. Although the full quantum dynamics cannot be solved
exactly for the case at hand, our results can be considered as supporting the

On the Possibility of Quantum Gravity Effects at Astrophysical Scales
The nonperturbative renormalization group flow of Quantum Einstein Gravity
(QEG) is reviewed. It is argued that at large distances there could be strong
renormalization effects, including a scale dependence of Newton's constant,

Safety and Availability of Clofazimine in the Treatment of Multidrug and Extensively Drug-Resistant Tuberculosis: Analysis of Published Guidance and Meta-Analysis of Cohort Studies
Objectives: Given the spread of multidrug-resistant tuberculosis (MDR-TB), new therapies are urgently needed, including the repurposing of existing drugs. We aimed to assess key considerations for the clinical and programmatic use of clofazimine (Cfz), a riminophenazine with anti-mycobacterial activity currently used to treat leprosy. Design: Fixed and random effects meta-analysis of cohort studies and systematic review Setting: Electronic and manual searches were combined. Inclusion criteria: Observational studies on treatment of multidrug- and extremely drug- resistant tuberculosis with clofazimine or a clofazimine-containing regimen, and published guidance and documents relating to cost and availability were eligible. Results: Five observational studies enrolled 861 patients, of which 602 received Cfz. The pooled proportion of adverse drug reactions requiring discontinuation of Cfz treatment was 0.1% (95% CI: [0.0, 0.6%]), and the median frequency of all adverse events was 5.1%. Cfz showed in vitro efficacy against Mycobacterium tuberculosis, and Cfz-containing regimens may have had a useful role in the treatment of patients with drug-resistant strains and who had limited alternative treatment options. However, Cfz uptake remains insufficient to meet global needs; there is only one internationally quality-assured manufacturer, which produces a limited quantity of the drug prioritised for treatment of leprosy, the only indication for which the drug is registered. Conclusions: While the data were limited, Cfz was associated with a risk for adverse drug reactions comparable to that of first-line TB treatment, which could be reasonably managed under programmatic conditions. Study of Two-Step Mechanisms in Pion Absorption on 6Li, 12C via Deuteron Emission
The (pi+,pd), and (pi+,dd) reactions were investigated with pions of 100 and
165 MeV kinetic energy on 6Li and 12C targets. In comparison with previously
published (pi+,pp) data on the same targets and at the same beam energies,
kinematic regions were identified in which the neutron pickup process n+p->d
dominated the observed deuteron yield. The importance of this mechanism
increases with energy, contributing half of the observed cross section at 165
MeV. The contribution of direct quasi-triton absorption is significant only at

Absence of an acute insulin response predicts onset of type 2 diabetes in a Caucasian population with impaired glucose tolerance
Context: In persons with impaired glucose tolerance (IGT), both impaired insulin secretion and insulin resistance contribute to the conversion to type 2 diabetes mellitus (T2DM). However, few studies have used criterion standard measures to asses the predictive value of impaired insulin secretion and insulin resistance for the conversion to T2DM in a Caucasian IGT population. Objectives: The objective of the study was to determine the predictive value of measures of insulin secretion and insulin resistance derived from a hyperglycemic clamp, including the disposition index, for the development of T2DM in a Caucasian IGT population. Design, Setting, and Participants: The population-based Hoorn IGT study consisted of 101 Dutch IGT subjects (aged < 75 yr), with mean 2-h plasma glucose values, of two separate oral glucose tolerance tests, between 8.6 and 11.1 mmol/liter. A hyperglycemic clamp at baseline was performed to assess first-phase and second-phase insulin secretion and insulin sensitivity. During follow-up, conversion to T2DM was assessed by means of 6-monthly fasting glucose levels and yearly oral glucose tolerance tests. Results: The cumulative incidence of T2DM was 34.7%. Hazard ratio for T2DM development adjusted for age, sex, and body mass index was 5.74 [95% confidence interval (CI) 2.60-12.67] for absence of first insulin peak, 1.58 (95% CI 0.60-4.17) for lowest vs. highest tertile of insulin sensitivity, and 1.78 (95% CI 0.65-4.88) for lowest vs. highest tertile of the disposition index. Conclusions: In these Caucasian persons with IGT, the absence of the first insulin peak was the strongest predictor of T2DM. Copyright © 2008 by The Endocrine Society
