    The stigmatisation of people with chronic back pain

    This study responded to the need for better theoretical understanding of experiences that shape the beliefs, attitudes and needs of chronic back patients attending pain clinics. The aim was explore and conceptualise the experiences of people of working age who seek help from pain clinics for chronic back pain. Methods. This was a qualitative study, based on an interpretative phenomenological approach (IPA). During in-depth interviews in their homes, participants were invited to 'tell their story' from the time their pain began. Participants were twelve male and six female patients, aged between 28 and 62 years, diagnosed as having chronic benign back pain. All had recently attended one of two pain clinics as new referrals. The interview transcripts were analysed thematically. Findings. Stigmatisation emerged as a key theme from the narrative accounts of participants. The findings expose subtle as well as overt stigmatising responses by family, friends, health professionals and the general public which appeared to have a profound effect on the perceptions, self esteem and behaviours of those interviewed. Conclusions. The findings suggest that patients with chronic back pain feel stigmatised by the time they attend pain clinics and this may affect their attitudes and behaviours towards those offering professional help. Theories of chronic pain need to accommodate these responses, while pain management programmes need to address the realities and practicalities of dealing with stigma in everyday life

    Skyrmion fluctuations at a first-order phase transition boundary

    Magnetic skyrmions are topologically protected spin textures with promising prospects for applications in data storage. They can form a lattice state due to competing magnetic interactions and are commonly found in a small region of the temperature - magnetic field phase diagram. Recent work has demonstrated that these magnetic quasi-particles fluctuate at the μeV energy scale. Here, we use a coherent x-ray correlation method at an x-ray free-electron laser to investigate these fluctuations in a magnetic phase coexistence region near a first-order transition boundary where fluctuations are not expected to play a major role. Surprisingly, we find that the relaxation of the intermediate scattering function at this transition differs significantly compared to that deep in the skyrmion lattice phase. The observation of a compressed exponential behavior suggests solid-like dynamics, often associated with jamming. We assign this behavior to disorder and the phase coexistence observed in a narrow field-window near the transition, which can cause fluctuations that lead to glassy behavior

    Boundary-layer effects on acoustic transmission through narrow slit cavities

    PublishedJournal ArticleWe explore the slit-width dependence of the resonant transmission of sound in air through both a slit array formed of aluminum slats and a single open-ended slit cavity in an aluminum plate. Our experimental results accord well with Lord Rayleigh's theory concerning how thin viscous and thermal boundary layers at a slit's walls affect the acoustic wave across the whole slit cavity. By measuring accurately the frequencies of the Fabry-Perot-like cavity resonances, we find a significant 5% reduction in the effective speed of sound through the slits when an individual viscous boundary layer occupies only 5% of the total slit width. Importantly, this effect is true for any airborne slit cavity, with the reduction being achieved despite the slit width being on a far larger scale than an individual boundary layer's thickness. This work demonstrates that the recent prevalent loss-free treatment of narrow slit cavities within acoustic metamaterials is unrealistic.The authors would like to thank DSTL for their financial support

    Can shoulder range of movement be measured accurately using the Microsoft Kinect sensor plus Medical Interactive Recovery Assistant (MIRA) software?

    BackgroundThis study compared the accuracy of measuring shoulder range of movement (ROM) with a simple laptop-sensor combination vs. trained observers (shoulder physiotherapists and shoulder surgeons) using motion capture (MoCap) laboratory equipment as the gold standard. MethodsThe Microsoft Kinect sensor (Microsoft Corp., Redmond, WA, USA) tracks 3-dimensional human motion. Ordinarily used with an Xbox (Microsoft Corp.) video game console, Medical Interactive Recovery Assistant (MIRA) software (MIRA Rehab Ltd., London, UK) allows this small sensor to measure shoulder movement with a standard computer. Shoulder movements of 49 healthy volunteers were simultaneously measured by trained observers, MoCap, and the MIRA device. Internal rotation was assessed with the shoulder abducted 90° and external rotation with the shoulder adducted. Visual estimation and MIRA measurements were compared with gold standard MoCap measurements for agreement using Bland-Altman methods. Results There were 1670 measurements analyzed. The MIRA evaluations of all 4 cardinal shoulder movements were significantly more precise, with narrower limits of agreement, than the measurements of trained observers. MIRA achieved ±11° (95% confidence interval [CI], 8.7°-12.6°) for forward flexion vs. ±16° (95% CI, 14.6°-17.6°) by trained observers. For abduction, MIRA showed ±11° (95% CI, 8.7°-12.8°) against ±15° (95% CI, 13.4°-16.2°) for trained observers. MIRA attained ±10° (95% CI, 8.1°-11.9°) during external rotation measurement, whereas trained observers only reached ±21° (95% CI, 18.7°-22.6°). For internal rotation, MIRA achieved ±9° (95% CI, 7.2°-10.4°), which was again better than TOs at ±18° (95% CI, 16.0°-19.3°). ConclusionsA laptop combined with a Microsoft Kinect sensor and the MIRA software can measure shoulder movements with acceptable levels of accuracy. This technology, which can be easily set up, may also allow precise shoulder ROM measurement outside the clinic setting

    Solar cycle signals in sea level pressure and sea surface temperature

    We identify solar cycle signals in 155 years of global sea level pressure (SLP) and sea surface temperature (SST) data using a multiple linear regression approach. In SLP we find in the North Pacific a statistically significant weakening of the Aleutian Low and a northward shift of the Hawaiian High in response to higher solar activity, confirming the results of previous authors. We also find a weak but broad reduction in pressure across the equatorial Pacific. In SST we identify a weak El Niño-like pattern in the tropics, unlike the strong La Niña-like signal found recently by some other authors. We show that the latter have been influenced by the technique of compositing data from peak years of the sunspot cycle as these years have often coincided with the negative phase of the ENSO cycle. Furthermore, the date of peak annual sunspot number generally falls a year or more in advance of the broader maximum of the 11-year solar cycle so that analyses which incorporate data from all years represent more coherently the difference between periods of high and low solar activity on these timescales

    The solar cycle signal in sea level pressure during 1856–2007 is analyzed. Using composites of data from January–February in solar cycle peak years the strong positive signal in the region of the Aleutian low, found by previous authors, is confirmed. It is found, however, that signals in other regions of the globe, particularly in the South Pacific, are very sensitive to the choice of reference climatology. Also investigated is the relationship between solar activity and sea surface temperatures in the tropical eastern Pacific. A marked overall association of higher solar activity with colder temperatures in the tropical Pacific that is not restricted to years of peak sunspot number is noted. The ENSO-like variation following peak years that has been suggested by other authors is not found as a consistent signal. Both the SLP and SST signals vary coherently with the solar cycle and neither evolves on an ENSO-like time scale. The solar signals are weaker during the period spanning approximately 1956–97, which may be due to masking by a stronger innate ENSO variability at that time

    The influence of solar variability and the quasi-biennial oscillation on lower atmospheric temperatures and sea level pressure

    Our fundamental aim is to investigate solar cycle signals in sea level pressure. In order to see if these may relate, especially at high latitudes, to the solar influence on the stratosphere we start by investigating the temperature of the winter polar stratosphere and its dependence on the state of the Sun and the phase of the Quasi-Biennial Oscillation (QBO). We find that the choice of pressure level used to define the phase of the QBO is important in determining how the solar and QBO influences appear to act in combination. Informed by this we carry out a multiple linear regression analysis of zonal mean temperatures throughout the lower stratosphere and troposphere. A combined solar*QBO temporal index exhibits strongly in the lower stratosphere, but in much of the troposphere any influence of the QBO, either on its own or coupled to solar effects is much smaller than the pure solar signal. We use a similar approach to analyse sea level pressure (SLP) data, first using a standard QBO time series dating back to 1953. We find at high latitudes that individually the solar and QBO signals are weak but that the compound solar*QBO temporal index shows a significant signal. This is such that combinations of low solar activity with westerly QBO and high solar activity with easterly QBO are both associated with a strengthening in the polar modes; while the opposite combinations coincide with a weakening. By employing a QBO dataset reconstructed back to 1900, we extend the SLP analysis back to that date and also find a robust signal in the surface SAM; though weaker for surface NAM. Our results suggest that solar variability, modulated by the phase of QBO, influences zonal mean temperatures at high latitudes in the lower stratosphere, in the mid-latitude troposphere and sea level pressure near the poles. Thus a knowledge of the state of the Sun, and the phase of the QBO might be useful in surface climate prediction

    Every year, suicide is among the top 20 leading causes of death globally for all ages. Unfortunately, suicide is difficult to prevent, in large part because the prevalence of risk factors is high among the general population. In this review, clinical and psychological risk factors are examined and methods for suicide prevention are discussed. Prevention strategies found to be effective in suicide prevention include means restriction, responsible media coverage, and general public education, as well identification methods such as screening, gatekeeper training, and primary care physician education. Although the treatment for preventing suicide is difficult, follow-up that includes pharmacotherapy, psychotherapy, or both may be useful. However, prevention methods cannot be restricted to the individual. Community, social, and policy interventions will also be essentia

    Scans for signatures of selection in Russian cattle breed genomes reveal new candidate genes for environmental adaptation and acclimation

    Domestication and selective breeding has resulted in over 1000 extant cattle breeds. Many of these breeds do not excel in important traits but are adapted to local environments. These adaptations are a valuable source of genetic material for efforts to improve commercial breeds. As a step toward this goal we identified candidate regions to be under selection in genomes of nine Russian native cattle breeds adapted to survive in harsh climates. After comparing our data to other breeds of European and Asian origins we found known and novel candidate genes that could potentially be related to domestication, economically important traits and environmental adaptations in cattle. The Russian cattle breed genomes contained regions under putative selection with genes that may be related to adaptations to harsh environments (e.g., AQP5, RAD50, and RETREG1). We found genomic signatures of selective sweeps near key genes related to economically important traits, such as the milk production (e.g., DGAT1, ABCG2), growth (e.g., XKR4), and reproduction (e.g., CSF2). Our data point to candidate genes which should be included in future studies attempting to identify genes to improve the extant breeds and facilitate generation of commercial breeds that fit better into the environments of Russia and other countries with similar climates
