    Electrocardiographic (ECG) criteria for determining left ventricular mass in young healthy men; data from the LARGE Heart study

    Background: Doubts remain over the use of the ECG in identifying those with increased left ventricular (LV) mass. This is especially so in young individuals, despite their high prevalence of ECG criteria for LV hypertrophy. We performed a study using cardiovascular magnetic resonance (CMR), which provides an in vivo non-invasive gold standard method of measuring LV mass, allowing accurate assessment of electrocardiography as a tool for defining LV hypertrophy in the young.Methods and results: Standard 12-lead ECGs were obtained from 101 Caucasian male army recruits aged (mean +/- SEM) 19.7 +/- 0.2 years. LV mass was measured using CMR. LV mass indexed to body surface area demonstrated no significant correlation with the Cornell Amplitude criteria or Cornell Product for LV hypertrophy. Moderate correlations were seen with the Sokolow-Lyon Amplitude (0.28) and Sokolow-Lyon Product (0.284). Defining LV hypertrophy as a body surface area indexed left ventricular mass of 93 g/m(2), calculated sensitivities [and specificities] were as follows; 38.7% [74.3%] for the Sokolow-Lyon criteria, 43.4% [61.4%] for the Sokolow-Lyon Product, 19.4% [91.4%] for Cornell Amplitude, and 22.6% [85.7%] for Cornell Product. These values are substantially less than those reported for older age groups.Conclusion: ECG criteria for LV hypertrophy may have little value in determining LV mass or the presence of LV hypertrophy in young fit males

    Association of p60c-src with endosomal membranes in mammalian fibroblasts.

    We have examined the subcellular localization of p60c-src in mammalian fibroblasts. Analysis of indirect immunofluorescence by three-dimensional optical sectioning microscopy revealed a granular cytoplasmic staining that co-localized with the microtubule organizing center. Immunofluorescence experiments with antibodies against a number of membrane markers demonstrated a striking co-localization between p60c-src and the cation-dependent mannose-6-phosphate receptor (CI-MPR), a marker that identifies endosomes. Both p60c-src and the CI-MPR were found to cluster at the spindle poles throughout mitosis. In addition, treatment of interphase and mitotic cells with brefeldin A resulted in a clustering of p60c-src and CI-MPR at a peri-centriolar position. Biochemical fractionation of cellular membranes showed that a major proportion of p60c-src co-enriched with endocytic membranes. Treatment of membranes containing HRP to alter their apparent density also altered the density of p60c-src-containing membranes. Similar density shift experiments with total cellular membranes revealed that the majority of membrane-associated p60c-src in the cell is associated with endosomes, while very little is associated with plasma membranes. These results support a role for p60c-src in the regulation of endosomal membranes and protein trafficking

    The practice of flexible practice: discussion paper

    The term flexible practice describes a way of addressing the needs of GPs to spend time with family or on activities outside their time at work. It has been reported that GPs spend an average of 51.4 hours working per week (CDHFS, 1996) and other data suggest that rural general practitioners work in excess of this. However, it is not just an issue of total working hours. Flexible practice also relates to issues such as quality of life, ability to take leave at short notice, the option to work parttime, ways of dispersing on call duties between different practitioners and health services, easy entry and graceful exit from practices. Other terms used to describe flexible practice arrangements are sustainable practice or sustainable model of practice. Women rural GPs, in particular, have noted that their greatest stress was the conflict between their career and their personal life (Tolhurst et al., 1998, Kilmartin et al., 2002) as they most often carry the main responsibility for the care and rearing of children (Levitt and McEwin, 2001). The three issues contributing to this stress were described as total hours worked, time on call and not finding enough time to keep up their professional knowledge (Tolhurst et al., 1998). Women also commented that the least satisfying part of medical practice was lack of time for family and personal life. Childcare options are often very limited in rural areas, which further contributes to the problem (Tolhurst et al., 1998). In response to these issues, âflexibility was identified as the key to the development and construction of policies and programs to support female GPs in rural and remote practiceâ (Levitt and McEwin, 2001). Lippert (2002) reiterates this from her own research, noting the need for greater flexibility in practice and training arrangements and valuing varied working styles and practice arrangements

    The prospects for detecting a candidate supersymmetric dark matter particle at the LHC are reviewed, and compared with the prospects for direct and indirect searches for astrophysical dark matter. The discussion is based on a frequentist analysis of the preferred regions of the Minimal supersymmetric extension of the Standard Model with universal soft supersymmetry breaking (the CMSSM).     Use of chromium picolinate and biotin in the management of type 2 diabetes: an economic analysis

    This paper addresses the potential economic benefits of chromium picolinate plus biotin (Diachrome ®) use in people with Type 2 diabetes (T2DM). The economic model was developed to estimate the impact on health care systems’ costs by improved HbA1C levels with chromium picolinate plus biotin (Diachrome). Lifetimes cost savings were estimated by adjusting a benchmark from the literature, using a price index to adjust for inflation. The cost of diabetes is highly dependent on the HbA1C level with higher initial levels and higher annual increments increasing the cost. Improvement in glycemic control has proven to be cost-effective in delaying the onset and progression of T2DM, reducing the risk for diabetes-associated complications and lowering utilization and cost of care. Chromium picolinate plus biotin (Diachrome) showed greater improvement of glycemic control in poorly controlled T2DM patients (HbA1C \u3e=10%) compared to their better controlled counterparts (HbA1C \u3c 10%). This improvement was additive to that achieved by oral hypoglycemic medications and correlates to calculated levels of cost savings. Average 3-year cost savings for chromium picolinate plus biotin (Diachrome) use could range from 1,636forapoorlycontrolledpatientwithdiabeteswithoutheartdiseasesorhypertension,to1,636 for a poorly controlled patient with diabetes without heart diseases or hypertension, to 5,435 for a poorly controlled patient with diabetes, heart disease, and hypertension. Average 3-year cost savings was estimated to be between 3.9billionand3.9 billion and 52.9 billion for the 16.3 million existing patients with diabetes. Chromium picolinate plus biotin (Diachrome) use among the 1.17 million newly diagnosed patients with T2DM each year could deliver lifetime cost savings of 42billion,or42 billion, or 36,000 per T2DM patient. Affordable, safe, and convenient, chromium picolinate plus biotin (Diachrome) could prove to be a cost-effective complement to existing pharmacological therapies for controlling T2DM.     We calculate the structural and electronic properties of OsB2 using density functional theory with or without taking into account spin-orbit (SO) interaction. Our results show that the bulk modulus with and without SO interaction are 364 and 365 Gpa respectively, both are in good agreement with experiment (365-395 Gpa). The evidence of covalent bonding of Os-B, which plays an important role to form a hard material, is indicated both in charge density, atoms in molecules analysis, and density of states analysis. The good metallicity and hardness of OsB2 might suggest its potential application as hard conductors.Comment: Figures improve

    Nanoscale β\beta-Nuclear Magnetic Resonance Depth Imaging of Topological Insulators

    Considerable evidence suggests that variations in the properties of topological insulators (TIs) at the nanoscale and at interfaces can strongly affect the physics of topological materials. Therefore, a detailed understanding of surface states and interface coupling is crucial to the search for and applications of new topological phases of matter. Currently, no methods can provide depth profiling near surfaces or at interfaces of topologically inequivalent materials. Such a method could advance the study of interactions. Herein we present a non-invasive depth-profiling technique based on β\beta-NMR spectroscopy of radioactive 8^8Li+^+ ions that can provide "one-dimensional imaging" in films of fixed thickness and generates nanoscale views of the electronic wavefunctions and magnetic order at topological surfaces and interfaces. By mapping the 8^8Li nuclear resonance near the surface and 10 nm deep into the bulk of pure and Cr-doped bismuth antimony telluride films, we provide signatures related to the TI properties and their topological non-trivial characteristics that affect the electron-nuclear hyperfine field, the metallic shift and magnetic order. These nanoscale variations in β\beta-NMR parameters reflect the unconventional properties of the topological materials under study, and understanding the role of heterogeneities is expected to lead to the discovery of novel phenomena involving quantum materials.Comment: 46 pages, 12 figures in Proc.
