    Use of chromium picolinate and biotin in the management of type 2 diabetes: an economic analysis

    This paper addresses the potential economic benefits of chromium picolinate plus biotin (Diachrome ®) use in people with Type 2 diabetes (T2DM). The economic model was developed to estimate the impact on health care systems’ costs by improved HbA1C levels with chromium picolinate plus biotin (Diachrome). Lifetimes cost savings were estimated by adjusting a benchmark from the literature, using a price index to adjust for inflation. The cost of diabetes is highly dependent on the HbA1C level with higher initial levels and higher annual increments increasing the cost. Improvement in glycemic control has proven to be cost-effective in delaying the onset and progression of T2DM, reducing the risk for diabetes-associated complications and lowering utilization and cost of care. Chromium picolinate plus biotin (Diachrome) showed greater improvement of glycemic control in poorly controlled T2DM patients (HbA1C \u3e=10%) compared to their better controlled counterparts (HbA1C \u3c 10%). This improvement was additive to that achieved by oral hypoglycemic medications and correlates to calculated levels of cost savings. Average 3-year cost savings for chromium picolinate plus biotin (Diachrome) use could range from 1,636forapoorlycontrolledpatientwithdiabeteswithoutheartdiseasesorhypertension,to1,636 for a poorly controlled patient with diabetes without heart diseases or hypertension, to 5,435 for a poorly controlled patient with diabetes, heart disease, and hypertension. Average 3-year cost savings was estimated to be between 3.9billionand3.9 billion and 52.9 billion for the 16.3 million existing patients with diabetes. Chromium picolinate plus biotin (Diachrome) use among the 1.17 million newly diagnosed patients with T2DM each year could deliver lifetime cost savings of 42billion,or42 billion, or 36,000 per T2DM patient. Affordable, safe, and convenient, chromium picolinate plus biotin (Diachrome) could prove to be a cost-effective complement to existing pharmacological therapies for controlling T2DM.     Journal ArticleThe role of quasicylindrical waves and surface plasmon polaritons in beam shaping with resonant nanogratings is investigated. It is shown that the field on the grating surface can be strongly influenced by plasmons and quasicylindrical waves in the infrared. A method that combines far-field measurements with the fast Fourier transform to map the field amplitude at the grating surface is demonstrated. For samples with a small degree of geometric asymmetry, it is shown that the imaginary part of the transform (with null zeroth-order component) can better map the amplitude of the resonant surface waves than the full complex-valued transform.     Behavioral implications of shortlisting procedures

    We consider two-stage “shortlisting procedures” in which the menu of alternatives is first pruned by some process or criterion and then a binary relation is maximized. Given a particular first-stage process, our main result supplies a necessary and sufficient condition for choice data to be consistent with a procedure in the designated class. This result applies to any class of procedures with a certain lattice structure, including the cases of “consideration filters,” “satisficing with salience effects,” and “rational shortlist methods.” The theory avoids background assumptions made for mathematical convenience; in this and other respects following Richter’s classical analysis of preference-maximizing choice in the absence of shortlisting

    We analyse the computational complexity of finding Nash equilibria in simple stochastic multiplayer games. We show that restricting the search space to equilibria whose payoffs fall into a certain interval may lead to undecidability. In particular, we prove that the following problem is undecidable: Given a game G, does there exist a pure-strategy Nash equilibrium of G where player 0 wins with probability 1. Moreover, this problem remains undecidable if it is restricted to strategies with (unbounded) finite memory. However, if mixed strategies are allowed, decidability remains an open problem. One way to obtain a provably decidable variant of the problem is restricting the strategies to be positional or stationary.     Spatio-temporal correlations can drastically change the response of a MAPK pathway

    Multisite covalent modification of proteins is omnipresent in eukaryotic cells. A well-known example is the mitogen-activated protein kinase (MAPK) cascade, where in each layer of the cascade a protein is phosphorylated at two sites. It has long been known that the response of a MAPK pathway strongly depends on whether the enzymes that modify the protein act processively or distributively: distributive mechanism, in which the enzyme molecules have to release the substrate molecules in between the modification of the two sites, can generate an ultrasensitive response and lead to hysteresis and bistability. We study by Green's Function Reaction Dynamics, a stochastic scheme that makes it possible to simulate biochemical networks at the particle level and in time and space, a dual phosphorylation cycle in which the enzymes act according to a distributive mechanism. We find that the response of this network can differ dramatically from that predicted by a mean-field analysis based on the chemical rate equations. In particular, rapid rebindings of the enzyme molecules to the substrate molecules after modification of the first site can markedly speed up the response, and lead to loss of ultrasensitivity and bistability. In essence, rapid enzyme-substrate rebindings can turn a distributive mechanism into a processive mechanism. We argue that slow ADP release by the enzymes can protect the system against these rapid rebindings, thus enabling ultrasensitivity and bistability

    Solution of the X-ray edge problem for 2D electrons in a magnetic field

    The absorption and emission spectra of transitions between a localized level and a two-dimensional electron gas, subjected to a weak magnetic field, are calculated analytically. Adopting the Landau level bosonization technique developed in previous papers, we find an exact expression for the relative intensities of spectral lines. Their envelope function, governed by the interaction between the electron gas and the core hole, is reminescent of the famous Fermi edge singularity, which is recovered in the limit of a vanishing magnetic field.Comment: 4 pages, 1 figur

    h-Principles for the Incompressible Euler Equations

    Recently, De Lellis and Sz\'ekelyhidi constructed H\"older continuous, dissipative (weak) solutions to the incompressible Euler equations in the torus T3\mathbb T^3. The construction consists in adding fast oscillations to the trivial solution. We extend this result by establishing optimal h-principles in two and three space dimensions. Specifically, we identify all subsolutions (defined in a suitable sense) which can be approximated in the H1H^{-1}-norm by exact solutions. Furthermore, we prove that the flows thus constructed on T3\mathbb T^3 are genuinely three-dimensional and are not trivially obtained from solutions on T2\mathbb T^2.Comment: 29 pages, no figure

    Identification of A-colored Stars and Structure in the Halo of the Milky Way from SDSS Commissioning Data

    A sample of 4208 objects with magnitude 15 < g* < 22 and colors of main sequence A stars has been selected from 370 square degrees of Sloan Digital Sky Survey (SDSS) commissioning observations. The data is from two long, narrow stripes, each with an opening angle of greater than 60 deg, at Galactic latitudes 36 < abs(b) < 63 on the celestial equator. An examination of the sample's distribution shows that these stars trace considerable substructure in the halo. Large overdensities of A-colored stars in the North at (l,b,R) = (350, 50, 46 kpc) and in the South at (157, -58, 33 kpc) and extending over tens of degrees are present in the halo of the Milky Way. Using photometry to separate the stars by surface gravity, both structures are shown to contain a sequence of low surface gravity stars consistent with identification as a blue horizontal branch (BHB). Both structures also contain a population of high surface gravity stars two magnitudes fainter than the BHB stars, consistent with their identification as blue stragglers (BSs). From the numbers of detected BHB stars, lower limits to the implied mass of the structures are 6x10^6 M_sun and 2x10^6 M_sun. The fact that two such large clumps have been detected in a survey of only 1% of the sky indicates that such structures are not uncommon in the halo. Simple spheroidal parameters are fit to a complete sample of the remaining unclumped BHB stars and yield (at r < 40 kpc) a fit to a halo distribution with flattening (c/a = 0.65+/-0.2) and a density falloff exponent of alpha = -3.2+/-0.3.Comment: AASTeX v5_0, 26 pages, 1 table, 20 figures, ApJ accepte
