    Linear, Deterministic, and Order-Invariant Initialization Methods for the K-Means Clustering Algorithm

    Over the past five decades, k-means has become the clustering algorithm of choice in many application domains primarily due to its simplicity, time/space efficiency, and invariance to the ordering of the data points. Unfortunately, the algorithm's sensitivity to the initial selection of the cluster centers remains to be its most serious drawback. Numerous initialization methods have been proposed to address this drawback. Many of these methods, however, have time complexity superlinear in the number of data points, which makes them impractical for large data sets. On the other hand, linear methods are often random and/or sensitive to the ordering of the data points. These methods are generally unreliable in that the quality of their results is unpredictable. Therefore, it is common practice to perform multiple runs of such methods and take the output of the run that produces the best results. Such a practice, however, greatly increases the computational requirements of the otherwise highly efficient k-means algorithm. In this chapter, we investigate the empirical performance of six linear, deterministic (non-random), and order-invariant k-means initialization methods on a large and diverse collection of data sets from the UCI Machine Learning Repository. The results demonstrate that two relatively unknown hierarchical initialization methods due to Su and Dy outperform the remaining four methods with respect to two objective effectiveness criteria.     Laparoscopy in management of appendicitis in high-, middle-, and low-income countries: a multicenter, prospective, cohort study.

    BACKGROUND: Appendicitis is the most common abdominal surgical emergency worldwide. Differences between high- and low-income settings in the availability of laparoscopic appendectomy, alternative management choices, and outcomes are poorly described. The aim was to identify variation in surgical management and outcomes of appendicitis within low-, middle-, and high-Human Development Index (HDI) countries worldwide. METHODS: This is a multicenter, international prospective cohort study. Consecutive sampling of patients undergoing emergency appendectomy over 6 months was conducted. Follow-up lasted 30 days. RESULTS: 4546 patients from 52 countries underwent appendectomy (2499 high-, 1540 middle-, and 507 low-HDI groups). Surgical site infection (SSI) rates were higher in low-HDI (OR 2.57, 95% CI 1.33-4.99, p = 0.005) but not middle-HDI countries (OR 1.38, 95% CI 0.76-2.52, p = 0.291), compared with high-HDI countries after adjustment. A laparoscopic approach was common in high-HDI countries (1693/2499, 67.7%), but infrequent in low-HDI (41/507, 8.1%) and middle-HDI (132/1540, 8.6%) groups. After accounting for case-mix, laparoscopy was still associated with fewer overall complications (OR 0.55, 95% CI 0.42-0.71, p < 0.001) and SSIs (OR 0.22, 95% CI 0.14-0.33, p < 0.001). In propensity-score matched groups within low-/middle-HDI countries, laparoscopy was still associated with fewer overall complications (OR 0.23 95% CI 0.11-0.44) and SSI (OR 0.21 95% CI 0.09-0.45). CONCLUSION: A laparoscopic approach is associated with better outcomes and availability appears to differ by country HDI. Despite the profound clinical, operational, and financial barriers to its widespread introduction, laparoscopy could significantly improve outcomes for patients in low-resource environments.     Fuzzy Inference Systems for Invariant Pattern Recognition in MFL NDE

    Defect related information present in NDE signals is frequently obscured by the presence of operational variables inherent in the system. A typical NDE system comprises of an energy source, a test specimen and a sensor array. Operational variables include uncontrollable changes in source signal strength and/or frequency, variations in the sensitivity of the sensor and alterations in the material properties of the test specimen. These operational variables can confuse subsequent signal interpretation schemes, such as those relying on artificial neural networks. Invariant pattern recognition methods are required to ensure accurate signal characterization in terms of the underlying defect geometry. This paper describes a generalized invariance transformation technique to compensate for operational variables in NDE systems. An application to magnetic flux leakage (MFL) inspection of gas transmission pipelines is presented.     A novel underuse model shows that inactivity but not ovariectomy determines the deteriorated material properties and geometry of cortical bone in the tibia of adult rats

    Our goal in this study was to determine to what extent the physiologic consequences of ovariectomy (OVX) in bones are exacerbated by a lack of daily activity such as walking. We forced 14-week-old female rats to be inactive for 15 weeks with a unique experimental system that prevents standing and walking while allowing other movements. Tibiae, femora, and 4th lumbar vertebrae were analyzed by peripheral quantitative computed tomography (pQCT), microfocused X-ray computed tomography (micro-CT), histology, histomorphometry, Raman spectroscopy, and the three-point bending test. Contrary to our expectation, the exacerbation was very much limited to the cancellous bone parameters. Parameters of femur and tibia cortical bone were affected by the forced inactivity but not by OVX: (1) cross-sectional moment of inertia was significantly smaller in Sham-Inactive rat bones than that of their walking counterparts; (2) the number of sclerostin-positive osteocytes per unit cross-sectional area was larger in Sham-Inactive rat bones than in Sham-Walking rat bones; and (3) material properties such as ultimate stress of inactive rat tibia was lower than that of their walking counterparts. Of note, the additive effect of inactivity and OVX was seen only in a few parameters, such as the cancellous bone mineral density of the lumbar vertebrae and the structural parameters of cancellous bone in the lumbar vertebrae/tibiae. It is concluded that the lack of daily activity is detrimental to the strength and quality of cortical bone in the femur and tibia of rats, while lack of estrogen is not. Our inactive rat model, with the older rats, will aid the study of postmenopausal osteoporosis, the etiology of which may be both hormonal and mechanical

    In Silico Investigation of Potential Src Kinase Ligands from Traditional Chinese Medicine

    Src kinase is an attractive target for drug development based on its established relationship with cancer and possible link to hypertension. The suitability of traditional Chinese medicine (TCM) compounds as potential drug ligands for further biological evaluation was investigated using structure-based, ligand-based, and molecular dynamics (MD) analysis. Isopraeroside IV, 9alpha-hydroxyfraxinellone-9-O-beta-D-glucoside (9HFG) and aurantiamide were the top three TCM candidates identified from docking. Hydrogen bonds and hydrophobic interactions were the primary forces governing docking stability. Their stability with Src kinase under a dynamic state was further validated through MD and torsion angle analysis. Complexes formed by TCM candidates have lower total energy estimates than the control Sacaratinib. Four quantitative-structural activity relationship (QSAR) in silico verifications consistently suggested that the TCM candidates have bioactive properties. Docking conformations of 9HFG and aurantiamide in the Src kinase ATP binding site suggest potential inhibitor-like characteristics, including competitive binding at the ATP binding site (Lys295) and stabilization of the catalytic cleft integrity. The TCM candidates have significantly lower ligand internal energies and are estimated to form more stable complexes with Src kinase than Saracatinib. Structure-based and ligand-based analysis support the drug-like potential of 9HFG and aurantiamide and binding mechanisms reveal the tendency of these two candidates to compete for the ATP binding site

    Measurement of the Λb0Λ(1520)μ+μ\Lambda_{b}^{0}\to \Lambda(1520) \mu^{+}\mu^{-} differential branching fraction

    The branching fraction of the rare decay Λb0Λ(1520)μ+μ\Lambda_{b}^{0}\to \Lambda(1520) \mu^{+}\mu^{-} is measured for the first time, in the squared dimuon mass intervals, q2q^2, excluding the J/ψJ/\psi and ψ(2S)\psi(2S) regions. The data sample analyzed was collected by the LHCb experiment at center-of-mass energies of 7, 8, and 13 TeV, corresponding to a total integrated luminosity of $9\ \mathrm{fb}^{-1}.Theresultinthehighest.     Pooled analysis of who surgical safety checklist use and mortality after emergency laparotomy

    Background: The World Health Organization (WHO) Surgical Safety Checklist has fostered safe practice for 10 years, yet its place in emergency surgery has not been assessed on a global scale. The aim of this study was to evaluate reported checklist use in emergency settings and examine the relationship with perioperative mortality in patients who had emergency laparotomy. Methods: In two multinational cohort studies, adults undergoing emergency laparotomy were compared with those having elective gastrointestinal surgery. Relationships between reported checklist use and mortality were determined using multivariable logistic regression and bootstrapped simulation. Results: Of 12 296 patients included from 76 countries, 4843 underwent emergency laparotomy. After adjusting for patient and disease factors, checklist use before emergency laparotomy was more common in countries with a high Human Development Index (HDI) (2455 of 2741, 89⋅6 per cent) compared with that in countries with a middle (753 of 1242, 60⋅6 per cent; odds ratio (OR) 0⋅17, 95 per cent c.i. 0⋅14 to 0⋅21, P &lt; 0⋅001) or low (363 of 860, 42⋅2 percent; OR 0⋅08, 0⋅07 to 0⋅10, P &lt; 0⋅001) HDI. Checklist use was less common in elective surgery than for emergency laparotomy in high-HDI countries (risk difference −9⋅4 (95 per cent c.i. −11⋅9 to −6⋅9) per cent; P &lt; 0⋅001), but the relationship was reversed in low-HDI countries (+12⋅1 (+7⋅0 to +17⋅3) per cent; P &lt; 0⋅001). In multivariable models, checklist use was associated with a lower 30-day perioperative mortality (OR 0⋅60, 0⋅50 to 0⋅73; P &lt; 0⋅001). The greatest absolute benefit was seen for emergency surgery in low-and middle-HDI countries. Conclusion: Checklist use in emergency laparotomy was associated with a significantly lower perioperative mortality rate. Checklist use in low-HDI countries was half that in high-HDI countries

    Precision measurement of CP\it{CP} violation in the penguin-mediated decay Bs0ϕϕB_s^{0}\rightarrow\phi\phi

    A flavor-tagged time-dependent angular analysis of the decay Bs0ϕϕB_s^{0}\rightarrow\phi\phi is performed using pppp collision data collected by the LHCb experiment at % at s=13\sqrt{s}=13 TeV, the center-of-mass energy of 13 TeV, corresponding to an integrated luminosity of 6 fb^{-1}. The CP\it{CP}-violating phase and direct CP\it{CP}-violation parameter are measured to be ϕssˉs=0.042±0.075±0.009\phi_{s\bar{s}s} = -0.042 \pm 0.075 \pm 0.009 rad and λ=1.004±0.030±0.009|\lambda|=1.004\pm 0.030 \pm 0.009 , respectively, assuming the same values for all polarization states of the ϕϕ\phi\phi system. In these results, the first uncertainties are statistical and the second systematic. These parameters are also determined separately for each polarization state, showing no evidence for polarization dependence. The results are combined with previous LHCb measurements using pppp collisions at center-of-mass energies of 7 and 8 TeV, yielding ϕssˉs=0.074±0.069\phi_{s\bar{s}s} = -0.074 \pm 0.069 rad and lambda=1.009±0.030|lambda|=1.009 \pm 0.030. This is the most precise study of time-dependent CP\it{CP} violation in a penguin-dominated BB meson decay.     Observation of the decay Λb0<i>→</i> χ<sub>c1</sub>pπ<SUP><i>-</i></SUP>

    The Cabibbo-suppressed decay Λb0χc1pπ\Lambda_b^0\rightarrow\chi_{c1}p\pi^- is observed for the first time using data from proton-proton collisions corresponding to an integrated luminosity of 6fb1^{-1}, collected with the LHCb detector at a centre-of-mass energy of 13TeV. Evidence for the Λb0χc2pπ\Lambda_b^0\rightarrow\chi_{c2}p\pi^- decay is also found. Using the Λb0χc1pK\Lambda_b^0\rightarrow\chi_{c1}pK^- decay as normalisation channel, the ratios of branching fractions are measured to be B(Λb0χc1pπ)B(Λb0χc1pK)=(6.59±1.01±0.22)×102,B(Λb0χc2pπ)B(Λb0χc1pπ)=0.95±0.30±0.04±0.04,B(Λb0χc2pK)B(Λb0χc1pK)=1.06±0.05±0.04±0.04,\begin{array}{rcl} \frac{ \mathcal{B} (\Lambda_b^0\rightarrow\chi_{c1}p\pi^-)}{\mathcal{B} (\Lambda_b^0\rightarrow\chi_{c1}pK^-)} & = & (6.59 \pm 1.01 \pm 0.22 ) \times 10^{-2} \,, \frac{\mathcal{B} (\Lambda_b^0\rightarrow\chi_{c2}p\pi^-)}{\mathcal{B} (\Lambda_b^0\rightarrow\chi_{c1}p\pi^-)} & = & 0.95 \pm 0.30 \pm 0.04 \pm 0.04 \,, \frac{\mathcal{B} (\Lambda_b^0\rightarrow\chi_{c2}pK^-)}{\mathcal{B} (\Lambda_b^0\rightarrow\chi_{c1}pK^-)} & = & 1.06 \pm 0.05 \pm 0.04 \pm 0.04 \,,\end{array} where the first uncertainty is statistical, the second is systematic and the third is due to the uncertainties in the branching fractions of χc1,2J/ψγ\chi_{c1,2}\rightarrow J/\psi\gamma decays
