Non-secretory multiple myeloma with extramedulary plasmocytoma
Non-secretory multiple myeloma (NSMM) is an uncommon variation of the classic form of multiple myeloma (MM) that has au analogous clinical and radiologic presentation except for the absence of the M-protein in sernm and/or urine. NSMM may have or not detectable monoclonal free light chain in urine/serum. We describe here a case of a 46-year-old woman who presented with back pain and lower limb weakness. A tumour lesion at DI level and multiple osteolytic bone lesions were found. This woman had no other symptoms and her basic blood biochemistry were nonnal. She had normal serum and urine protein electrophoresis. Biopsy from the D1 vertebra showed plasmocytoma. In the bone marrow, 20% atypical plasma cell fow1d, all were being CD38+ and CD 138+ in flowcytometry, confirming the diagnosis of myeloma presenting as NSMM. A high index of suspicion is needed to diagnose cases like this, as the biology ofNSMM is not completely understood
Chronic Disseminated Intravascular Coagulation: A Case Report
In health there is a balance between the coagulation and anti-coagulation systems, but in disseminated intravascular coagulation (DIC) the coagulation mechanism is activated inappropriately and in a diffuse way. This may lead to thrombosis, but more often haemorrhage occurs when the clotting factors are exhausted. DIC may present as acute, subacute, and rarely chronic form. Here we present a case of chronic DIC following pelvic inflammatory disease (PID) as a consequence of repeated menstruation regulation (MR). Multifocal Extensive Spinal Tuberculosis with Retropharyngeal Abscess
An unusual case of a young boy presenting with spinal tuberculosis involving cervical & thoracic vertebrae, along with retropharyngeal abscess is reported. The patient presented with progressive quadriparesis, fever, night sweat and cervical lymphadenopathy. The lab studies confirmed tuberculosis and patient received anti-tubercular chemotherapy. After development of quadriparesis, spinal surgery was done. The post operative course was uneventful and the patient is on gradual neurological recovery. Leishmanial Hepatitis with Chronic Hepatitis B Infection Treated Successfully with Liquid Form of Liposomal Amphotericin B - A Case Report
Hepatic involvement either due to leishmaniasis or due to coexisting viral infection sometime poses a problem for the clinicians. Atypical presentation is also challenging for them. Christmas Disease (Hemophilia B) A Case Report
We report a 25 years old man developed Haemarthrosis of left hip joint with a history of recurrent swelling and pain in multiple joints and prolonged bleeding following minor trauma since childhood. Subsequent investigations revealed Christmas disease (Haemophilia B). Hemophilia B is an X-linked bleeding disorder. This case emphasises the importance of considering a diagnosis of haemophilia in a man with unexplained bleeding, even in the absence of a positive family history.
