    A pilot study of subjective well-being in colorectal cancer patients and their caregivers

    Background: Traditional endpoints in oncology are based on measuring the tumor size and combining this with a time factor. Current studies with immunotherapy show that even when median survival is unaltered, a significant proportion of patients can achieve prolonged survival. Objective tumor response does not always mean “overall” improvement, especially if toxicity is harsh. Novel agents are significantly expensive, and it is therefore crucial to measure the impact on “quality” of life, in addition to “quantity”. Materials and methods: We studied the preferences and experiences of cancer patients and their caregivers, measuring subjective well-being (SWB) ratings, EQ5D descriptions and time trade-off preferences. Results: We studied 99 patients and 88 caregivers. Life satisfaction ratings were similar between the two groups, but daily mood was significantly lower in caregivers (P<0.1). Anxiety/depression affected SWB, while pain and mobility did not. Positive thoughts about health were associated with better daily moods in both groups, and stage IV cancer was associated with lower life satisfaction. Cancer in remission was associated with better daily moods, but, interestingly, not with patient life satisfaction. Patients with better daily mood and positive thoughts about family were less willing to “trade-off” life years. Conclusion: Caregivers are as anxious or depressed as patients, and report similar levels of life satisfaction but lower daily mood. A focus on SWB could provide a valid assessment of treatment benefit. Given the interesting results of this pilot study, we suggest a larger study should be conducted, measuring SWB over time

    Flexible Access Control, Federated Identity and Heterogeneous Metadata Supports for Repositories

    In this paper, we present a new framework complete with implementation, for a digital repository that will address some of the most difficult issues facing repository managers today: how to enable federated identity access, rapidly changing access control requirements, and the management of multiple metadata standards for different types of digital objects. Our work draws together leading industry standards in the area of authentication, authorization, and metadata management, and apply them in a new and innovative way to the repository landscape. As a demonstration, we apply our work to a speech annotation research project which makes use of a repository to manage its culturally sensitive data

    A Comparison of Three Computer-based Methods Used to Determine EMG Signal Amplitude

    Electromyography is a commonly used method to determine relative effort and neuromuscular drive to skeletal muscle. A limitation of the interpretation of EMG within the literature is the many methods used to determine the intensity of muscle activation. In the current study, ten healthy young adults performed a level walking task while EMG was recorded from the tibialis anterior, medial gastrocnemius and fibularis longus. The EMG data were rectified and smoothed using the root mean squared (RMS). Peak RMS (pRMS), mean RMS (mRMS) and integrated EMG (iEMG) were normalized to the peak value within the subject and were used to determine EMG amplitude. A 3x3 repeated measures analysis of variance was used to determine significant differences between the methods of determining EMG amplitude. The findings of the current study show that pRMS produced significantly lower EMG amplitudes than mRMS or iEMG values. Furthermore, mRMS and iEMG produced nearly identical normalized EMG amplitudes. Based on the findings of this study and the components of each measurement of EMG amplitude, it is suggested to use mRMS to determine EMG amplitude

    Prognostic Indicators and Outcome Measures for Surgical Removal of Symptomatic Nonadvanced Cataract

    This article is made available in accordance with the publisher's public repositories policy.Objectives To report changes in perceived visual functioning after surgery for symptomatic cataract with preoperative corrected distance visual acuity [CDVA] of 0.4 logMAR or better (Snellen equivalent, 20/50) and to investigate the relationship between any observed changes and preoperative physical characteristics and psychophysical consequences of the lens opacity and any changes in psychophysical findings after the procedure. Methods Eighty-five patients with cataract completed a validated questionnaire concerning functional vision satisfaction and a series of visual performance assessments before and 2 months after cataract surgery. The lens optical density and Lens Opacities Classification System III score of the cataract were recorded. Correlations between changes in the Rasch-analyzed questionnaire score and changes in visual performance after cataract surgery, as well as preoperative psychophysical measures, lens optical density, and Lens Opacities Classification System III score, were determined. Results The mean (SD) questionnaire score improved from 2.15 (0.36) to 1.54 (0.41) (P > .05 for both). Conclusion Psychophysical tests alternative to CDVA better represent improvements in self-reported visual functioning following removal of symptomatic nonadvanced cataract
