    Background and purpose - The AO Pediatric Comprehensive Classification of Long Bone Fractures (PCCF) describes the localization and morphology of fractures, and considers severity in 2 categories: (1) simple, and (2) multifragmentary. We evaluated simple and multifragmentary fractures in a large consecutive cohort of children diagnosed with long bone fractures in Switzerland. Patients and methods - Children and adolescents treated for fractures between 2009 and 2011 at 2 tertiary pediatric surgery hospitals were retrospectively included. Fractures were classified according to the AO PCCF. Severity classes were described according to fracture location, patient age and sex, BMI, and cause of trauma. Results - Of all trauma events, 3% (84 of 2,730) were diagnosed with a multifragmentary fracture. This proportion was age-related: 2% of multifragmentary fractures occurred in school-children and 7% occurred in adolescents. In patients diagnosed with a single fracture only, the highest percentage of multifragmentation occurred in the femur (12%, 15 of 123). In fractured paired radius/ulna bones, multifragmentation occurred in 2% (11 of 687); in fractured paired tibia/fibula bones, it occurred in 21% (24 of 115), particularly in schoolchildren (5 of 18) and adolescents (16 of 40). In a multivariable regression model, age, cause of injury, and bone were found to be relevant prognostic factors of multifragmentation (odds ratio (OR) > 2). Interpretation - Overall, multifragmentation in long bone fractures in children was rare and was mostly observed in adolescents. The femur was mostly affected in single fractures and the lower leg was mostly affected in paired-bone fractures. The clinical relevance of multifragmentation regarding growth and long-term functional recovery remains to be determined

    Single-parameter non-adiabatic quantized charge pumping

    Controlled charge pumping in an AlGaAs/GaAs gated nanowire by single-parameter modulation is studied experimentally and theoretically. Transfer of integral multiples of the elementary charge per modulation cycle is clearly demonstrated. A simple theoretical model shows that such a quantized current can be generated via loading and unloading of a dynamic quasi-bound state. It demonstrates that non-adiabatic blockade of unwanted tunnel events can obliterate the requirement of having at least two phase-shifted periodic signals to realize quantized pumping. The simple configuration without multiple pumping signals might find wide application in metrological experiments and quantum electronics.Comment: 4 pages, 4 figure

    Operon conservation and the evolution of trans-splicing in the phylum Nematoda

    The nematode Caenorhabditis elegans is unique among model animals in that many of its genes are cotranscribed as polycistronic pre-mRNAs from operons. The mechanism by which these operonic transcripts are resolved into mature mRNAs includes trans-splicing to a family of SL2-like spliced leader exons. SL2-like spliced leaders are distinct from SL1, the major spliced leader in C. elegans and other nematode species. We surveyed five additional nematode species, representing three of the five major clades of the phylum Nematoda, for the presence of operons and the use of trans-spliced leaders in resolution of polycistronic pre-mRNAs. Conserved operons were found in Pristionchus pacificus, Nippostrongylus brasiliensis, Strongyloides ratti, Brugia malayi, and Ascaris suum. In nematodes closely related to the rhabditine C. elegans, a related family of SL2-like spliced leaders is used for operonic transcript resolution. However, in the tylenchine S. ratti operonic transcripts are resolved using a family of spliced leaders related to SL1. Non-operonic genes in S. ratti may also receive these SL1 variants. In the spirurine nematodes B. malayi and A. suum operonic transcripts are resolved using SL1. Mapping these phenotypes onto the robust molecular phylogeny for the Nematoda suggests that operons evolved before SL2-like spliced leaders, which are an evolutionary invention of the rhabditine lineage

    We prove uniqueness of ground state solutions Q=Q(x)0Q = Q(|x|) \geq 0 for the nonlinear equation (Δ)sQ+QQα+1=0(-\Delta)^s Q + Q - Q^{\alpha+1}= 0 in R\mathbb{R}, where 0<s<10 < s < 1 and 0<α<4s12s0 < \alpha < \frac{4s}{1-2s} for s<1/2s < 1/2 and 0<α<0 < \alpha < \infty for s1/2s \geq 1/2. Here (Δ)s(-\Delta)^s denotes the fractional Laplacian in one dimension. In particular, we generalize (by completely different techniques) the specific uniqueness result obtained by Amick and Toland for s=1/2s=1/2 and α=1\alpha=1 in [Acta Math., \textbf{167} (1991), 107--126]. As a technical key result in this paper, we show that the associated linearized operator L+=(Δ)s+1(α+1)QαL_+ = (-\Delta)^s + 1 - (\alpha+1) Q^\alpha is nondegenerate; i.\,e., its kernel satisfies kerL+=span{Q}\mathrm{ker}\, L_+ = \mathrm{span}\, \{Q'\}. This result about L+L_+ proves a spectral assumption, which plays a central role for the stability of solitary waves and blowup analysis for nonlinear dispersive PDEs with fractional Laplacians, such as the generalized Benjamin-Ono (BO) and Benjamin-Bona-Mahony (BBM) water wave equations.Comment: 45 page
