    Active removal of inorganic phosphate from cerebrospinal fluid by the choroid plexus

    The P-i concentration of mammalian cerebrospinal fluid (CSF) is about one-half that of plasma, a phenomenon also shown here in the spiny dogfish, Squalus acanthias. The objective of the present study was to characterize the possible role of the choroid plexus (CP) in determining CSF P-i concentration. The large sheet-like fourth CP of the shark was mounted in Ussing chambers where unidirectional P-33(i) fluxes revealed potent active transport from CSF to the blood side under short-circuited conditions. The flux ratio was 8: 1 with an average transepithelial resistance of 87 +/- 17.9 Omega . cm(2) and electrical potential difference of + 0.9 +/- 0.17 mV (CSF side positive). Active P-i absorption from CSF was inhibited by 10 mM arsenate, 0.2 mM ouabain, Na+ -free medium, and increasing the K+ concentration from 5 to 100 mM. Li+ stimulated transport twofold compared with Na+-free medium. Phosphonoformic acid (1 mM) had no effect on active Pi transport. RT-PCR revealed both P-i transporter (PiT) 1 and PiT2 (SLC20 family) gene expression, but no Na+ -P-i cotransporter II (SLC34 family) expression, in the shark CP. PiT2 immunoreactivity was shown by immunoblot analysis and localized by immunohistochemistry in (or near) the CP apical microvillar membranes of both the shark and rat. PiT1 appeared to be localized primarily to vascular endothelial cells. Taken together, these data indicate that the CP actively removes P-i from CSF.     A description of the needs of family members of patients hospitalized in a coronary care unit

    The purpose of this descriptive, qualitative study was to identify the needs of family members of patients hospitalized in a coronary care unit for 24-48 hours. The participants were 8 spouses of patients hospitalized in a coronary care unit for less than 48 hours. The instrument used was developed by the investigator and included four open-ended questions with probes. Eight qualitative content analyses at the latent level were performed on the data obtained from structured interviews. A five-step process adapted from Colazzi (1978) was used to organized the data inductively into categories. The following nine categories of needs were identified: information, support, proximity, assurance, comfort, helpful, express feelings, express thoughts, and reflect. The findings indicated that all of the participants had all nine categories of needs. Implications for nursing practice and research were presented

    Differences in Body Satisfaction Between Black and White Heterosexual College-Aged Men

    Although there is an increasing amount of research concerning body satisfaction among heterosexual White men, few of these studies have adequately represented heterosexual Black men. This was a quantitative survey study aimed to illuminate gaps in the literature regarding Black men\u27s body satisfaction experiences. The study used surveys and tested research questions to determine whether college-aged heterosexual Black (n = 220, 55%) and White (n = 180, 45%) men differed in their body satisfaction experiences and whether race significantly moderated the relation between sociocultural influences and body satisfaction in the two groups. This study was based on the social comparison theory, and examined sociocultural influences (i.e., media images, parents, peers, internalization of cultural appearance standards, and drives for muscularity) known to be associated with body satisfaction. Results from t test analyses indicated that Black men were significantly more satisfied with their appearance and weight, were significantly more confident that other people liked their appearance, and reported less social pressures to have an attractive body in comparison to their White peers. A series of moderated regression analyses failed to indicate that race moderated the relation between the sociocultural influences and body satisfaction. Seventy-five percent of both groups were dissatisfied with their bodies and desired to be more muscular. Social change implications include alerting clinicians that Black men, like White men, should be screened for problems with body satisfaction: Results may stimulate research to determine why Black men have greater overall body satisfaction than White men, and lead to culturally-specific guidelines for identifying and treating body dissatisfaction

    Thyroid and pituitary gland development from hatching through metamorphosis of a teleost flatfish, the Atlantic halibut

    Fish larval development, not least the spectacular process of flatfish metamorphosis, appears to be under complex endocrine control, many aspects of which are still not fully elucidated. In order to obtain data on the functional development of two major endocrine glands, the pituitary and the thyroid, during flatfish metamorphosis, histology, immunohistochemistry and in situ hybridization techniques were applied on larvae of the Atlantic halibut (Hippoglossus hippoglossus), a large, marine flatfish species, from hatching through metamorphosis. The material was obtained from a commercial hatchery. Larval age is defined as day-degrees (D =accumulated daily temperature from hatching). Sporadic thyroid follicles are first detected in larvae at 142 D (27 days post-hatch), prior to the completion of yolk sack absorption. Both the number and activity of the follicles increase markedly after yolk sack absorption and continue to do so during subsequent development. The larval triiodothyronine (T3) and thyroxine (T4) content increases, subsequent to yolk absorption, and coincides with the proliferation of thyroid follicles. A second increase of both T3 and T4 occurs around the start of metamorphosis and the T3 content further increases at the metamorphic climax. Overall, the T3 content is lower than T4. The pituitary gland can first be distinguished as a separate organ at the yolk sack stage. During subsequent development, the gland becomes more elongated and differentiates into neurohypophysis (NH), pars distalis (PD) and pars intermedia (PI). The first sporadic endocrine pituitary cells are observed at the yolk sack stage, somatotrophs (growth hormone producing cells) and somatolactotrophs (somatolactin producing cells) are first observed at 121 D (23 days post-hatch), and lactotrophs (prolactin producing cells) at 134 D (25 days post-hatch). Scarce thyrotrophs are evident after detection of the first thyroid follicles (142 D ), but coincident with a phase in which follicle number and activity increase (260 D ). The somatotrophs are clustered in the medium ventral region of the PD, lactotrophs in the anterior part of the PD and somatolactotrophs are scattered in the mid and posterior region of the pituitary. At around 600 D , coinciding with the start of metamorphosis, somatolactotrophs are restricted to the interdigitating tissue of the NH. During larval development, the pituitary endocrine cells become more numerous. The present data on thyroid development support the notion that thyroid hormones may play a significant role in Atlantic halibut metamorphosis. The time of appearance and the subsequent proliferation of pituitary somatotrophs, lactotrophs, somatolactotrophs and thyrotrophs indicate at which stages of larval development and metamorphosis these endocrine cells may start to play active regulatory roles.This work has been carried out within the projects ‘‘Endocrine Control as a Determinant of Larval Quality in Fish Aquaculture’’ (CT-96-1422) and ‘‘Arrested development: The Molecular and Endocrine Basis of Flatfish Metamorphosis’’ (Q5RS-2002-01192), with financial support from the Commission of the European Communities.     Youth Empowered Advocating for Health (YEAH): Facilitating Partnerships Between Prevention Scientists and Black Youth to Promote Health Equity

    Structural racism inflicts a disproportionate burden of stress and trauma within Black communities, resulting in physical and mental health inequities that impact Black youth. Yet few multilevel interventions exist to address these deeply rooted inequities from a preventive standpoint, and even fewer are informed by the participatory input of the impacted communities. To bridge these gaps, we developed a community-based prevention strategy that promotes agency and active resistance to structural racism, Youth Empowered Advocating for Health (YEAH), and implemented it across various settings. We outline the development, implementation, and expansion of YEAH as a tool for promoting optimal health among Black communities. Lastly, we discuss lessons learned and offer a framework outlining key principles for prevention scientists to partner with Black youth and engage them in translational science to address structural racism. This framework is aimed at driving policies, practices, and procedures that promote equitable and sustainable change for and with Black communities

    Clinical Skills Development in the Virtual Learning Environment: Adapting to a New World

    The rapid transition to distance learning in response to the unexpected SARS-CoV-2/COVID-19 pandemic led to disruption of clinical skills development, which are typically conducted face-to-face. Consequently, faculty adapted their courses, using a multitude of active learning modalities, to meet student learning objectives in the didactic and experiential settings. Strategies and considerations to implement innovative delivery methods and address potential challenges are elucidated.     Survival following early-stage colon cancer: An ACCENT-based comparison of patients versus a matched international general population

    Background: Post-treatment survival experience of early colon cancer (CC) patients is well described in the literature, which states that cure is probable for some patients. However, comparisons of treated patients' survival versus that expected from a matched general population (MGP) are limited. Patients and methods: A total of 32 745 patients from 25 randomized adjuvant trials conducted from 1977 to 2012 in 41 countries were pooled. Observed long-term survival of these patients was compared with expected survival matched on sex, age, country, and year, both overall and by stage (II and III), sex, treatment [surgery, 5-fluorouracil (5-FU), 5-FU + oxaliplatin], age (<70 and 70+), enrollment year (pre/post 2000), and recurrence (yes/no). Comparisons were made at randomization and repeated conditional on survival to 1, 2, 3, and 5 years. CC and MGP equivalence was tested, and observed Kaplan-Meier survival rates compared with expected MGP rates 3 years out from each landmark. Analyses were also repeated in patients without recurrence. Results: Within most cohorts, long-term survival of CC patients remained statistically worse than the MGP, though conditional survival generally improved over time. Among those surviving 5 years, stage II, oxaliplatin-treated, elderly, and recurrence-free patients achieved subsequent 3-year survival rates within 5% of the MGP, with recurrence-free patients achieving equivalence. Conclusions: Conditional on survival to 5 years, long-term survival of most CC patients on clinical trials remains modestly poorer than an MGP, but achieves MGP levels in some subgroups. These findings emphasize the need for access to quality care and improved treatment and follow-up strategies
