International audienceLes travaux de l’Observatoire sociétal des cancers, mesure 30 du Plan cancer 2009/2013, ont été confiés à la Ligue nationale contre le cancer qui, par son maillage territorial et sa proximité avec la population, est en capacité d’établir des diagnostics sur les impacts sociaux du cancer, tant au niveau de l’individu qu’au niveau de la société dans son ensemble. C’est grâce à ces réalités du vécu, analysées et commentées, accompagnées de témoignages et de propositions, que les travaux rapportés dans ce troisième rapport de l’Observatoire sociétal des cancers doivent nourrir les choix stratégiques en matière de politique de lutte contre le cancer. L’Observatoire a ainsi inspiré plusieurs orientations du Plan cancer 2014/2019 concernant notamment la «continuité de vie» pendant et après la maladie. Tout au long de ce rapport, le lecteur constatera que la vie des malades est impactée par la crise économique et par des cumuls d’obstacles générés par certaines règles des politiques sanitaires et sociales. Il pourra être surpris, par exemple, de constater que les contours de la prise en charge, dite à 100 %, des malades atteints de cancer sont mal connus et laissent subsister des «restes à charge» coûteux pour les familles, en particulier pour celles qui sont dans des situations fragiles au plan économique et social. De même, le lecteur pourra s’étonner que le monde du travail et le cancer soient deux réalités qui ne se comprennent pas toujours. Ce rapport, en dépit du sentiment de désespérance qu’il pourrait générer, est au contraire un révélateur de la «force de vie» qui anime les malades et leurs proches et qui doit mobiliser tous les acteurs sensibles à une lutte inconditionnelle et efficace contre le cancer.Professeur Jacqueline Godet présidente de la Ligue contre le cance

Internal and external cooling methods and their effect on body temperature, thermal perception and dexterity

Objective The present study aimed to compare a range of cooling methods possibly utilised by occupational workers, focusing on their effect on body temperature, perception and manual dexterity. Methods Ten male participants completed eight trials involving 30 min of seated rest followed by 30 min of cooling or control of no cooling (CON) (34C, 58% relative humidity). The cooling methods utilised were: ice cooling vest (CV0), phase change cooling vest melting at 14C (CV14), evaporative cooling vest (CVEV), arm immersion in 10C water (AI), portable water-perfused suit (WPS), heliox inhalation (HE) and ice slushy ingestion (SL). Immediately before and after cooling, participants were assessed for fine (Purdue pegboard task) and gross (grip and pinch strength) manual dexterity. Rectal and skin temperature, as well as thermal sensation and comfort, were monitored throughout. Results Compared with CON, SL was the only method to reduce rectal temperature (P = 0.012). All externally applied cooling methods reduced skin temperature (P0.05). Conclusion The present study observed that ice ingestion or ice applied to the skin produced the greatest effect on rectal and skin temperature, respectively. AI should not be utilised if workers require subsequent fine manual dexterity. Pleosporales

One hundred and five generic types of Pleosporales are described and illustrated. A brief introduction and detailed history with short notes on morphology, molecular phylogeny as well as a general conclusion of each genus are provided. For those genera where the type or a representative specimen is unavailable, a brief note is given. Altogether 174 genera of Pleosporales are treated. Phaeotrichaceae as well as Kriegeriella, Zeuctomorpha and Muroia are excluded from Pleosporales. eIF4A2 drives repression of translation at initiation by Ccr4-Not through purine-rich motifs in the 5'UTR

Background: Regulation of the mRNA life cycle is central to gene expression control and determination of cell fate. miRNAs represent a critical mRNA regulatory mechanism, but despite decades of research, their mode of action is still not fully understood.Results: Here, we show that eIF4A2 is a major effector of the repressive miRNA pathway functioning via the Ccr4-Not complex. We demonstrate that while DDX6 interacts with Ccr4-Not, its effects in the mechanism are not as pronounced. Through its interaction with the Ccr4-Not complex, eIF4A2 represses mRNAs at translation initiation. We show evidence that native eIF4A2 has similar RNA selectivity to chemically inhibited eIF4A1. eIF4A2 exerts its repressive effect by binding purine-rich motifs which are enriched in the 5'UTR of target mRNAs directly upstream of the AUG start codon.Conclusions: Our data support a model whereby purine motifs towards the 3' end of the 5'UTR are associated with increased ribosome occupancy and possible uORF activation upon eIF4A2 binding.</p

Rapid Diagnostic Algorithms as a Screening Tool for Tuberculosis: An Assessor Blinded Cross-Sectional Study

Background: A major obstacle to effectively treat and control tuberculosis is the absence of an accurate, rapid, and low-cost diagnostic tool. A new approach for the screening of patients for tuberculosis is the use of rapid diagnostic classification algorithms. Methods: We tested a previously published diagnostic algorithm based on four biomarkers as a screening tool for tuberculosis in a Central European patient population using an assessor-blinded cross-sectional study design. In addition, we developed an improved diagnostic classification algorithm based on a study population at a tertiary hospital in Vienna, Austria, by supervised computational statistics. Results: The diagnostic accuracy of the previously published diagnostic algorithm for our patient population consisting of 206 patients was 54% (CI: 47%–61%). An improved model was constructed using inflammation parameters and clinical information. A diagnostic accuracy of 86% (CI: 80%–90%) was demonstrated by 10-fold cross validation. An alternative model relying solely on clinical parameters exhibited a diagnostic accuracy of 85% (CI: 79%–89%). Conclusion: Here we show that a rapid diagnostic algorithm based on clinical parameters is only slightly improved by inclusion of inflammation markers in our cohort. Our results also emphasize the need for validation of new diagnostic algorithms in different settings and patient populations
