Oxygen Therapy during Exercise in Patients with Interstitial Lung Diseases
Introduction: ILDs are a varied group of diffuse parenchymal lung diseases associated with high morbidity and mortality. Current treatments can only slow their progression but not cure the disease. Other treatments such as oxygen therapy can also be used as support. We know very little about the effects of oxygen therapy on patients with ILDs. The aim of this study was to collect data from the literature in order to determine whether oxygen therapy can actually decrease the mortality rate or whether it is only suitable for supportive therapy for patients with ILDs. Methods: We reviewed the literature since 2010 on oxygen therapy during exercise in patients with ILDs. Studies that used cardio-pulmonary tests were excluded. We only reviewed those that used the 6 min walking test (6MWT) or the free walking test. We located 11 relevant articles. Results: All the articles except a Japanese study showed an augmentation in oxyhaemoglobin saturation during exercise when oxygen was supplied. A 2018 study called AmbOx provided important data on the effects of oxygen therapy during daily activities, showing significant improvements in quality of life. Conclusions: This review showed that the literature on the benefits of oxygen therapy in patients with ILDs does not provide sufficient evidence to draft specific guidelines. It was not possible to conclude whether oxygen therapy has an effect on mortality or can only play a supportive role. © 2022 by the authors. Licensee MDPI, Basel, Switzerland
Evaluación de sustentabilidad comparativa de dos sistemas agrícolas de San Carlos, Mendoza, Argentina
Siendo la Agroecología una realidad creciente a nivel local, y siendo importante su promoción, pretendemos contribuir a la construcción de herramientas para evaluar sustentabilidad. Se evaluó de forma comparativa dos agroecosistemas, poniendo en evidencia aquellos aspectos que le otorgan mayor sustentabilidad relativa a uno respecto de otro. La metodología nos permitió la cuantificación y el análisis de los agroecosistemas desde un enfoque holístico y sistémico simplificando el concepto de sustentabilidad en valores simples y objetivos, llamados indicadores, detectando la tendencia del sistema hacia una condición más o menos sustentable. Se identificaron los puntos críticos, que en el caso del sistema convencional son la escasa biodiversidad y la alta dependencia de insumos externos y en el caso del sistema en transición son el manejo ineficiente del agua y la dependencia de ingresos extraprediales; en ambos el manejo del suelo es una fortaleza. Las diferencias entre ellos se relacionan con la lógica de quienes los conducen, más reduccionista en el primer caso y más integral en el segundo. Combined Inhibition of Hedgehog and HDAC6: In Vitro and In Vivo Studies Reveal a New Role for Lysosomal Stress in Reducing Glioblastoma Cell Viability
Glioblastoma multiforme (GBM) is the most common and malignant brain tumor in
adults. The invasiveness and the rapid progression that characterize GBM negatively impact patients’
survival. Temozolomide (TMZ) is currently considered the first-choice chemotherapeutic agent.
Unfortunately, over 50% of patients with GBM do not respond to TMZ treatment, and the mutation-
prone nature of GBM enables the development of resistance mechanisms. Therefore, efforts have been
devoted to the dissection of aberrant pathways involved in GBM insurgence and resistance in order to
identify new therapeutic targets. Among them, sphingolipid signaling, Hedgehog (Hh) pathway, and
the histone deacetylase 6 (HDAC6) activity are frequently dysregulated and may represent key targets
to counteract GBM progression. Given the positive correlation between Hh/HDAC6/sphingolipid
metabolism in GBM, we decided to perform a dual pharmacological inhibition of Hh and HDAC6
through cyclopamine and tubastatin A, respectively, in a human GMB cell line and zebrafish embryos.
The combined administration of these compounds elicited a more significant reduction of GMB cell
viability than did single treatments in vitro and in cells orthotopically transplanted in the zebrafish
hindbrain ventricle. We demonstrated, for the first time, that the inhibition of these pathways induces
lysosomal stress which results in an impaired fusion of lysosomes with autophagosomes and a
block of sphingolipid degradation in GBM cell lines. This condition, which we also recapitulated in
zebrafish embryos, suggests an impairment of lysosome-dependent processes involving autophagy
and sphingolipid homeostasis and might be instrumental in the reduction of GBM progression

Social protection as a strategy for HIV prevention, education promotion and child marriage reduction among adolescents: a cross-sectional population-based study in Lesotho
Background: Lesotho’s government has shown consistent efforts to implement social protection programmes. However, while recent evidence established a positive causal relationship between some of these programmes and food security there is little evidence on the extent to which these initiatives are associated with better educational and sexual and reproductive health outcomes among vulnerable adolescents in Lesotho. Methods and Findings: The study uses cross-sectional, nationally representative data from the 2018 Lesotho Violence Against Children and Youth Survey. Our research examined the association between social protection receipt and educational and sexual and reproductive health outcomes among adolescents and young people (13–24 years) living in poverty. We employed multivariate logistic regression controlling for age, orphanhood, HIV status and sex. Social protection receipt was defined as household receipt of financial support from a governmental, non-governmental, or community-based program that provides income. Additionally, we fitted a marginal effects model by sex. Among the 3,506 adolescent females and males living in the two lowest poverty quintiles, receipt of social protection was associated with improvements in multiple adolescent outcomes: higher odds of consistent condom use (aOR 1.64, 95% CI 1.17–2.29), educational attainment (aOR 1.79, 95% CI 1.36–2.36), and school enrolment (aOR 2.19, 95% CI 1.44–3.34). Stratified analyses by sex showed that social protection receipt was also associated with reduced likelihood of child marriage among females (aOR 0.59, 95% CI 0.42–0.83) and higher odds of educational attainment and school enrolment among males (aOR 2.53, 95% CI 1.59–4.03 and aOR 3.11, 95% CI 1.56–6.19, respectively). Conclusions: Our study provides evidence that social protection programs are associated with improved educational, sexual and reproductive health and child marriage prevention outcomes among adolescents living in poverty. Implementing and expanding such social protection initiatives could prove instrumental in improving the well-being of vulnerable adolescents. Contributions: Social protection programs have been increasing in sub-Saharan African countries, playing a pivotal role in poverty reduction, with Lesotho being no exception. Despite the optimistic outlook brought about by the implementation of the National Social Protection Strategy Lesotho I (2014-19) and II (2021–2031), the impact of these programs on some specific outcomes that concern the lives of the most vulnerable adolescents in Lesotho remains to some extent unexplored. Additionally, Lesotho grapples with high rates of HIV, adolescent pregnancy, child marriage and early school dropout, which can further contribute to poor long-term health and social outcomes among adolescents. In this study, we used data from the 2018 Lesotho Violence Against Children and Youth Survey (VACS) to examine the association between receiving social protection and multiple adolescent outcomes: educational, sexual and reproductive. The findings revealed that social protection programs, particularly the existing government-provided cash transfers, are significantly associated with multiple better outcomes among adolescents living in the poorest households in Lesotho. Such cash transfer schemes in Lesotho are associated with improved sexual and reproductive health outcomes for adolescent females, including reduced child marriage rates, and improved educational outcomes for males. These findings indicate that government-led social protection programmes are positively associated with favourable outcomes that can improve the quality of life for adolescents in resource-limited settings

Over the last decade endoscopic sphenopalatine artery (SPA) ligation has become a popular treatment option for posterior epistaxis and has been shown to be the most effective and cost-efficient definitive treatment for posterior epistaxis. SPA ligation is usually performed under general anesthesia. The majority of epistaxis patients are elderly, frail and have multiple medical conditions. These patients are therefore not always amenable to general anesthesia. In this article we describe two cases where posterior epistaxis was successfully treated with sphenopalatine artery ligation under local anesthesia and should be considered suitable for patients with severe posterior epistaxis who are not fit for a general anesthetic.Keywords: epistaxis, sphenopalatine artery ligation, local anesthesi
