Five-year follow-up of bilateral stimulation of the subthalamic nucleus in advanced Parkinson's disease
Background: Although the short-term benefits of bilateral stimulation of the subthalamic nucleus in patients with advanced Parkinson's disease have been well documented, the long-term outcomes of the procedure are unknown.
Methods: We conducted a five-year prospective study of the first 49 consecutive patients whom we treated with bilateral stimulation of the subthalamic nucleus. Patients were assessed at one, three, and five years with levodopa (on medication) and without levodopa (off medication), with use of the Unified Parkinson's Disease Rating Scale. Seven patients did not complete the study: three died, and four were lost to follow-up.
Results: As compared with base line, the patients' scores at five years for motor function while off medication improved by 54 percent (P<0.001) and those for activities of daily living improved by 49 percent (P<0.001). Speech was the only motor function for which off-medication scores did not improve. The scores for motor function on medication did not improve one year after surgery, except for the dyskinesia scores. On-medication akinesia, speech, postural stability, and freezing of gait worsened between year 1 and year 5 (P<0.001 for all comparisons). At five years, the dose of dopaminergic treatment and the duration and severity of levodopa-induced dyskinesia were reduced, as compared with base line (P<0.001 for each comparison). The average scores for cognitive performance remained unchanged, but dementia developed in three patients after three years. Mean depression scores remained unchanged. Severe adverse events included a large intracerebral hemorrhage in one patient. One patient committed suicide.
Conclusions: Patients with advanced Parkinson's disease who were treated with bilateral stimulation of the subthalamic nucleus had marked improvements over five years in motor function while off medication and in dyskinesia while on medication. There was no control group, but worsening of akinesia, speech, postural stability, freezing of gait, and cognitive function between the first and the fifth year is consistent with the natural history of Parkinson's disease
Pneumopathie médicamenteuse sous sirolimus : interaction avec l’atorvastatine ?
Introduction
Sirolimus is an immunosupressant used in renal transplantation because of its lack of nephrotoxicity. We report four cases of pneumonitis due to sirolimus, possibly revealing an interaction with atorvastatin.
Case report
Four patients (previously on long-term treatment with atorvastatin) presented with respiratory symptoms between 3 and 56 months after starting treatment with sirolimus following renal transplantation. Thoracic CT scans showed bilateral areas of peripheral alveolar consolidation. Bronchial lavage showed a lymphocytic alveolitis. Open-lung biopsy showed organizing pneumonia associated with diffuse alveolar damage and caseating granulomata. We attributed the pneumonitis to sirolimus on account of clinical and radiological resolution within 1 to 6 months of stopping treatment. We raise the possibility of an association between sirolimus and atorvastatin by competition for their hepatic degradation pathway via cytochrome P450 3A4.
Conclusion
Sirolimus causes drug-induced pneumonitis that is predominantly an organizing pneumonia. Atorvastatin may encourage its development by competition with sirolimus in the liver

Modeling and numerical study of primary breakup under diesel conditions
A recently introduced stochastic model for reduced numerical simulation of primary jet breakup is evaluated by comparing model predictions to DNS results for primary jet breakup under diesel conditions. The model uses one-dimensional turbulence (ODT) to simulate liquid and gas time advancement along a lateral line of sight. This one-dimensional domain is interpreted as a Lagrangian object that is advected downstream at the jet bulk velocity, thus producing a flow state expressed as a function of streamwise and lateral location. Multiple realizations are run to gather ensemble statistics that are compared to DNS results. The model incorporates several empirical extensions of the original ODT model that represent the phenomenology governing the Weber number dependence of global jet structure. The model as previously formulated, including the assigned values of tunable parameters, is used here without modification in order to test its capability to predict various statistics of droplets generated by primary breakup. This test is enabled by the availability of DNS results that are suitable for model validation. Properties that are examined are the rate of bulk liquid mass conversion into droplets, the droplet size distribution, and the dependence of droplet velocities on droplet diameter. Quantities of greatest importance for engine modeling are found to be predicted with useful accuracy, thereby demonstrating a more detailed predictive capability by a highly reduced numerical model of primary jet breakup than has previously been achieved
Till death (or an intruder) do us part: intrasexual-competition in a monogamous Primate
Polygynous animals are often highly dimorphic, and show large sex-differences in the degree of intra-sexual competition and aggression, which is associated with biased operational sex ratios (OSR). For socially monogamous, sexually monomorphic species, this relationship is less clear. Among mammals, pair-living has sometimes been assumed to imply equal OSR and low frequency, low intensity intra-sexual competition; even when high rates of intra-sexual competition and selection, in both sexes, have been theoretically predicted and described for various taxa. Owl monkeys are one of a few socially monogamous primates. Using long-term demographic and morphological data from 18 groups, we show that male and female owl monkeys experience intense intra-sexual competition and aggression from solitary floaters. Pair-mates are regularly replaced by intruding floaters (27 female and 23 male replacements in 149 group-years), with negative effects on the reproductive success of both partners. Individuals with only one partner during their life produced 25% more offspring per decade of tenure than those with two or more partners. The termination of the pair-bond is initiated by the floater, and sometimes has fatal consequences for the expelled adult. The existence of floaters and the sporadic, but intense aggression between them and residents suggest that it can be misleading to assume an equal OSR in socially monogamous species based solely on group composition. Does electoral violence affect vote choice and willingness to vote? Conjoint analysis of a vignette experiment
Across many new democracies, voters routinely elect candidates associated with violence. Though electoral violence is common, there is little understanding of how it affects voting behaviour. This article examines how electoral violence affects turnout and vote choice. To this end, a vignette experiment is set in a nationally representative survey in Kenya, where electoral violence has been present since the 1990s. In the experiment, voters choose between two rival politicians. The experiment randomises candidates’ attributes, their rumoured use of electoral violence, and their record of reducing poverty. Conjoint analysis is used to isolate the effects of the candidates’ randomised attributes on turnout and vote choice. In contrast to the assumptions made in the literature on electoral violence, voters are less likely to vote for candidates rumoured to have used electoral violence, even when the candidate is a coethnic or a copartisan. This sanctioning effect, however, is not consistent across all voters. Victims of electoral violence and the poorest respondents are less likely to sanction candidates rumoured to have used violence, especially when these candidates have a good record of reducing poverty. The results show that voting turnout decreases when participants are asked to choose between candidates who are rumoured to have used electoral violence. These results are robust to including respondent and interviewer characteristics that might have affected participation in the experiment and how respondents voted. These findings explain why candidates using violence can win elections and why electoral violence has been difficult to eradicate in settings characterised by clientelism and instances of political discourse justifying the use of violence
Effect of different weaning age (21, 28 or 35 days) on production, growth and certain parameters of the digestive tract in rabbits
The effect of different weaning ages, that is, 21 (G21), 28 (G28) or 35 (G35) days, on growth and certain parameters of the
digestive tract was examined in rabbits to assess the risk of early weaning attributable to the less-developed digestive system.
On days 35 and 42, G35 rabbits had 10% to 14% and 10% higher BW, respectively ( P,0.05), than those weaned at days 21 and
28. In the 4th week of life, early weaned animals had 75% higher feed intake than G28 and G35 rabbits ( P,0.05). The relative
weight of the liver increased by 62% between 21 and 28 days of age, and thereafter it decreased by 76% between 35 and
42 days of age ( P,0.05), with G21 rabbits having 29% higher weight compared with G35 animals on day 35 ( P,0.05).
The relative weight of the whole gastrointestinal (GI) tract increased by 49% and 22% after weaning in G21 and G28 rabbits,
respectively ( P,0.05). On day 28, the relative weight of the GI tract was 19% higher in G21 than in G28 rabbits, whereas on day
35 G21 and G28 animals had a 12% heavier GI tract compared with G35 rabbits ( P,0.05). Age influenced the ratio of stomach,
small intestine and caecum within the GI tract; however, no effect of different weaning age was demonstrated. The pH value of
the stomach and caecum decreased from 5.7 to 1.6 and from 7.1 to 6.3, respectively, whereas that of the small intestine increased
from 6.8 to 8.4 ( P,0.05); the differences between groups were not statistically significant. Strictly anaerobic culturable bacteria
were present in the caecum in high amounts (108), already at 14 days of age; no significant difference attributable to weaning age
was demonstrable. The concentration of total volatile fatty acids (tVFA) was higher in G21 than in G28 and G35 throughout the
experimental period ( P,0.05). The proportion of acetic and butyric acid within tVFA increased, whereas that of propionic acid
decreased, resulting in a C3 : C4 ratio decreasing with age. Early weaning (G21) resulted in higher butyric acid and lower propionic
acid proportions on day 28 ( P,0.05). No interaction between age and treatment was found, except in relative weight of the GI
tract and caecal content. In conclusion, early weaning did not cause considerable changes in the digestive physiological
parameters measured, but it resulted in 10% lower growth in rabbits
Submarine record of volcanic island construction and collapse in the Lesser Antilles arc: First scientific drilling of submarine volcanic island landslides by IODP Expedition 340
IODP Expedition 340 successfully drilled a series of sites offshore Montserrat, Martinique and Dominica in the Lesser Antilles from March to April 2012. These are among the few drill sites gathered around volcanic islands, and the first scientific drilling of large and likely tsunamigenic volcanic island-arc landslide deposits. These cores provide evidence and tests of previous hypotheses for the composition and origin of those deposits. Sites U1394, U1399, and U1400 that penetrated landslide deposits recovered exclusively seafloor-sediment, comprising mainly turbidites and hemipelagic deposits, and lacked debris avalanche deposits. This supports the concepts that i/ volcanic debris avalanches tend to stop at the slope break, and ii/ widespread and voluminous failures of pre-existing low-gradient seafloor sediment can be triggered by initial emplacement of material from the volcano. Offshore Martinique (U1399 and 1400), the landslide deposits comprised blocks of parallel strata that were tilted or micro-faulted, sometimes separated by intervals of homogenized sediment (intense shearing), while Site U1394 offshore Montserrat penetrated a flat-lying block of intact strata. The most likely mechanism for generating these large-scale seafloor-sediment failures appears to be propagation of a decollement from proximal areas loaded and incised by a volcanic debris avalanche. These results have implications for the magnitude of tsunami generation. Under some conditions, volcanic island landslide deposits comprised of mainly seafloor sediment will tend to form smaller magnitude tsunamis than equivalent volumes of subaerial block-rich mass flows rapidly entering water. Expedition 340 also successfully drilled sites to access the undisturbed record of eruption fallout layers intercalated with marine sediment which provide an outstanding high-resolution dataset to analyze eruption and landslides cycles, improve understanding of magmatic evolution as well as offshore sedimentation processes. The relationship between eruptive activity, flank collapse, and sea level at volcanic islands: A long-term (>1 Ma) record offshore Montserrat, Lesser Antilles
Hole U1395B, drilled southeast of Montserrat during Integrated Ocean Drilling Program Expedition 340, provides a long (>1 Ma) and detailed record of eruptive and mass-wasting events (>130 discrete events). This record can be used to explore the temporal evolution in volcanic activity and landslides at an arc volcano. Analysis of tephra fall and volcaniclastic turbidite deposits in the drill cores reveals three heightened periods of volcanic activity on the island of Montserrat (?930 ka to ?900 ka, ?810 ka to ?760 ka, and ?190 ka to ?120 ka) that coincide with periods of increased volcano instability and mass-wasting. The youngest of these periods marks the peak in activity at the Soufrière Hills volcano. The largest flank collapse of this volcano (?130 ka) occurred towards the end of this period, and two younger landslides also occurred during a period of relatively elevated volcanism. These three landslides represent the only large (>0.3 km3) flank collapses of the Soufrière Hills edifice, and their timing also coincides with periods of rapid sea-level rise (>5 m/ka). Available age data from other island arc volcanoes suggests a general correlation between the timing of large landslides and periods of rapid sea-level rise, but this is not observed for volcanoes in intra-plate ocean settings. We thus infer that rapid sea-level rise may modulate the timing of collapse at island arc volcanoes, but not in larger ocean-island settings
