    Importance of the dietary fat on the prevention and control of metabolic disturbances and cardiovascular disease

    A Organização Mundial da Saúde (OMS) reiterou recentemente que o consumo de dietas inadequadas e a inatividade física estão entre os dez principais fatores de mortalidade. Diversos ensaios aleatorizados demonstram que intervenções alimentares adequadas podem diminuir ou prevenir significativamente o aparecimento de várias doenças crônicas não transmissíveis. Neste contexto, o papel da dieta vem sendo exaustivamente avaliado em estudos clínicos e epidemiológicos. Assim, já foi bem estabelecido na literatura que a quantidade e o tipo de gordura alimentar exercem influência direta sobre fatores de risco cardiovascular, tais como a concentração de lípides e de lipoproteínas plasmáticas, bem como sua associação a processos inflamatórios. Os ácidos graxos participam de complexos sistemas de sinalização intracelular, função que vem sendo bastante explorada. Os ácidos graxos poli-insaturados não somente influenciam a composição das membranas, metabolismo celular e sinais de tradução, mas também modulam a expressão de genes, regulando a atividade e a produção de diversos fatores de transcrição. A proposta deste artigo é rever tópicos relevantes referentes ao metabolismo de lípides e os relacionar a terapias nutricionais que possam contribuir para a prevenção e o tratamento de doenças associadas.The World Health Organization (WHO) has recently reinforced the fact that inadequate diets, along with physical inactivity, are among the ten main determinant factors of mortality. Several randomized trials demonstrated that dietary interventions may lower or even prevent the occurrence of several non-communicable diseases. In this context, the role of diet has been exhaustively evaluated in several clinical and epidemiological studies. Thus, it is well established in literature that the amount and type of dietary fat have a direct influence on cardiovascular risk factors, such as lipids and plasma lipoprotein concentration, as well as their association with inflammatory processes. Fatty acids also participate in complex intracellular signaling systems, a function which has been currently investigated. Dietary polyunsaturated fatty acids (PUFA) act not only by altering membrane lipid composition, cellular metabolism and signal transduction, but also modulating gene expression by regulating the activity and/or production of different nuclear transcription factors. The aim of this article is to review important topics regarding the lipids metabolism and correlate them with nutritional therapies that may contribute to the prevention and treatment of related diseases

    Extra-adrenal regeneration of glucocorticoids by 11beta-hydroxysteroid dehydrogenase type 1:physiological regulator and pharmacological target for energy partitioning

    The major glucocorticoid in man, cortisol, plays important roles in regulating fuel metabolism, energy partitioning and body fat distribution. In addition to the control of cortisol levels in blood by the hypothalamic-pituitary-adrenal axis, intracellular cortisol levels within target tissues can be controlled by local enzymes. 11Beta-hydroxysteroid dehydrogenase type 1 (11beta-HSD1) catalyses the regeneration of active cortisol from inert cortisone, thereby amplifying cortisol levels and glucocorticoid receptor activation in adipose tissue, liver and other tissues. 11Beta-HSD1 is under complex tissue-specific regulation and there is evidence that it adjusts local cortisol concentrations independently of the plasma cortisol concentrations, e.g. in response to changes in diet. In obesity 11beta-HSD1 mRNA and activity in adipose tissue are increased. The mechanism of this up-regulation remains uncertain; polymorphisms in the HSD11B1 gene have been associated with metabolic complications of obesity, including hypertension and type 2 diabetes, but not with obesity per se. Extensive data have been obtained in mice with transgenic over-expression of 11beta-HSD1 in liver and adipocytes, targeted deletion of 11beta-HSD1, and using novel selective 11beta-HSD1 inhibitors; these data support the use of 11beta-HSD1 inhibitors to lower intracellular glucocorticoid levels and treat both obesity and its metabolic complications. Moreover, in human subjects the non-selective 'prototype' inhibitor carbenoxolone enhances insulin sensitivity. Results of clinical studies with novel potent selective 11beta-HSD1 inhibitors are therefore eagerly awaited. The present article focuses on the physiological role of glucocorticoids in regulating energy partitioning, and the evidence that this process is modulated by 11beta-HSD1 in human subjects

    Monitorização domiciliar da glicemia em pacientes com diabetes mellitus do tipo 1

    OBJECTIVE: To determine which of two simplified blood glucose monitoring schemes promotes better metabolic control in type1 diabetic patients during 12 months of participation in educational groups. METHODS: A crossover clinical trial involving 21 patients divided into two groups was conducted. They were submitted to a two monitoring schemes:2 alternate daily preprandial measurements and 2 alternate daily pre-and postprandial measurements. The effectiveness of the schemes was evaluated based on HbA1c. Variations in mean HbA1c were analyzed by Friedman test. RESULTS: The groups were homogenous in terms of sociodemographic and clinical variables (p>0.05). Mean HbA1c levels ranged from 8.48 (±1.00) to 7.37 (±0.99) over time in Group A and from 9.89 (±0.86) to 8.34 (±1.06) in Group B. The analysis of the HbA1c showed a significant reduction in the first and last 6 months and over the 12 months of the study in two groups (p<0.05). The preprandial scheme demonstrated the largest number and highest percentage of significant drops in HbA1c. CONCLUSIONS: The two monitoring improved the metabolic control and the preprandial scheme was more effective.OBJETIVO: Identificar cual de los dos esquemas de monitorización propuestos posibilita realizar un mejor control metabólico, en diabéticos del tipo1, durante los 12 meses de participación en grupos educativos. MÉTODO: Ensayo clínico cruzado con 21 pacientes divididos en dos grupos y sometidos a dos diferentes esquemas de monitorización. La efectividad de los esquemas fue evaluada por medio de la HbA1c. La variación de los promedios de HbA1c fue analizada con la prueba de Friedman. RESULTADOS: Durante todo el estudio la variación de los promedios de HbA1c, para el grupo A, fue de 8,48(±1,00) la 7,37(±0,99) y de 9,89(±0,86) la 8,34(±1,06) para el grupo B. Los análisis de la variación de la HbA1c colocaron en evidencia una reducción significativa (p<0,05) en los dos grupos, en los 3 periodos evaluados: primeros y últimos 6 meses y durante los 12 meses de estudio. CONCLUSIONES: Los dos esquemas mejoraron el control metabólico y el esquema antes de las comidas fue más efectivo.OBJETIVO: Identificar qual de dois esquemas simplificados de monitorização da glicemia viabiliza melhor controle metabólico, em pacientes com diabetes mellitus tipo 1, ao longo de 12 meses de participação em grupos educativos. MÉTODO: Ensaio clínico cruzado, com 21 pacientes divididos em dois grupos. Eles foram submetidos a dois esquemas de monitorização: duas medidas diárias pré-prandiais alternadas e duas medidas diárias pré e pós-prandiais alternadas. A efetividade dos esquemas foi avaliada pelos níveis de HbA1c. Para estudar a variação das médias das HbA1c aplicou-se o teste não paramétrico de Friedman. RESULTADOS: Os grupos eram homogêneos ao início do estudo com relação às variáveis sócio-demográficas e clinicas(p>0,05). A variação das médias de HbA1c, ao longo do tempo para o grupo A foi de 8,48(±1,00) a 7,37(±0,99) e para o grupo B de 9,89(±0,86) a 8,34(±1,06). O resultado da análise da variação da HbA1c mostrou redução significativa nos dois grupos, nos primeiros e últimos 6 meses e ao longo dos 12 meses nos dois grupos (p<0,05). O Esquema de monitorizações pré-prandiais possibilitou o maior número e os maiores percentuais de quedas estatisticamente significativas nos níveis de hemoglobina glicada.     Rorschach Comprehensive System on the evaluation of psychodynamic psychotherapy

    The proposal aimed to evaluate changes due to psychoanalytic psychotherapy on individuals assisted in a university clinic. Thus, 68 adult subjects, both genders, were evaluated before and after one year of psychotherapy. The Rorschach Comprehensive System protocols were analyzed according to their level of complexity, half on low [R↓+F↑] and half on high [R↑+F↓]. Significant statistical differences were found for (a) increase in low complexity group and decrease in 'high' complexity group on: R; EA; es; SumY; SumShd; FC+CF+C; CF+C; M; active; FD; H+(H)+Hd+(Hd); H pure, DEPI, (b) decrease in low and increase in high on Lambda and F pure. A confluence of these results was observed; the high complexity group showing less turbulence of their emotions, and the low complexity group displaying openness to experiences, even if of a more discomforting kind. The therapy proved to be beneficial to the individuals.A proposta foi avaliar mudanças decorrentes de psicoterapia psicanalítica em pessoas assistidas em um serviço universitário. Assim, 68 pessoas adultas, de ambos os sexos, foram avaliadas antes e depois de um ano de psicoterapia. Os protocolos de Rorschach Sistema Compreensivo foram analisados de acordo com seu grau de complexidade, metade em menor [R↓+F↑] e metade em maior [R↑+F↓]. Diferenças estatisticamente significantes foram encontradas com (a) aumento no grupo de menor complexidade e diminuição no grupo de maior complexidade em: R; EA; es; SumY; SumShd; FC+CF+C; CF+C; M; ativo; FD; H+(H)+Hd+(Hd); H puro, DEPI, (b) diminuição no menor e aumento no maior em Lambda e F puro. Observou-se confluência dos resultados, com o grupo de maior complexidade com menos turbulência em suas emoções e o de menor complexidade expressando abertura às experiências, mesmo se desconfortáveis.     Métodos laboratoriais para a detecção da resistência à meticilina nos Staphylococcus coagulase negativos de infecções oculares

    PURPOSE: To evaluate different methods of oxacillin susceptibility testing of ocular isolates, considering polymerase chain reaction (PCR) as the 'gold standard', and to compare the in vitro susceptibility to oxacillin with that of other antimicrobials used in ophthalmologic practice. METHODS: The Vitek gram-positive identification card was used to identify ocular coagulase negative Staphylococcus species. The presence of the mecA gene was determined by the polymerase chain reaction assay with a combination of two primer sets (mecA and 16S rRNA) in a single region. Results were analyzed and compared with other oxacillin susceptibility methods: PBP2a detection by rapid slide latex agglutination test (SLA); oxacillin E-test; the Vitek automated gram-positive susceptibility card (GPS-105); the oxacillin salt agar screening test (OSAS) at a concentration of 6.0, 1.0 and 0.75 µg oxacillin per ml and the cefoxitin disk diffusion test (CDD). Automated susceptibility was also determined to other antimicrobial agents (fluoroquinolones, penicillin G, amoxicillin-ampicillin, cefazolin, ampicillin-sulbactam, erythromycin, clindamycin, gentamicin, tetracycline, trimethoprim-sulfamethoxazole, vancomycin and rifampin. RESULTS: Of the 69 CoNS isolates tested, 71% were mecA-positive and 29% mecA-negative. All methods tested had a statistically significant agreement with polymerase chain reaction. There was a tendency of positive polymerase chain reaction predomination among the S. epidermidis isolates in comparison to non-epidermidis isolates, although this was not statistically significant (78.3% vs. 56.5%; chi2= 2.54; P= 0.11). The oxacillin salt agar screening test (0.75 µg oxacillin/ml) showed the best performance, with 100% sensitivity and negative predictive value; 95% specificity and 98% positive predictive value. Using the E-test, the mecA-positive isolates were statistically significantly more resistant to ciprofloxacin, ofloxacin, gatifloxacin and moxifloxacin (P= 0.002; P= 0.008; P= 0.002 and P= 0.003, respectively). There was a statistically significant higher proportion of resistance of the coagulase negative Staphylococcus mecA-positives for: penicillin G, amoxicillin-ampicillin, cefazolin, ampicillin-sulbactam, erythromycin, clindamycin, gentamicin and tetracycline (P<0.05). All coagulase negative Staphylococcus species were susceptible to vancomycin and there was no statistically significant correlation between the mecA-positive isolates and resistance to trimethoprim-sulfamethoxazole or to rifampin. CONCLUSIONS: In the present study, we found that the E-test and the oxacillin salt agar screening test S (0.75 µg oxacillin per ml), when compared with polymerase chain reaction, were the most accurate currently available methods to phenotypically detect oxacillin resistance of coagulase negative Staphylococcus species. This study demonstrated that a good option for screening of ocular isolates for oxacillin resistance in the microbiology laboratory is the cefoxitin disk diffusion test and the automated Vitek system. We believe it is important to have available methods that accurately detect methicillin resistance of the less commonly encountered species, chiefly because of their increasing importance as opportunistic pathogens.OBJETIVOS: Avaliar os diferentes métodos de suscetibilidade à oxacillina, em isolados oculares, considerando a reação em cadeia da polimerase (PCR) como padrão-ouro e comparar a suscetibilidade in vitro para outros antimicrobianos de uso oftalmológico. MÉTODOS: O sistema automatizado Vitek foi utilizado para identificar as diferentes espécies de Staphylococcus coagulase negativo (SCoN). A presença do gene mecA foi determinado pela reação em cadeia da polimerase com a combinação de 2 primer sets (mecA e 16S rRNA) em uma única região. Estes resultados foram analisados e comparados com outros métodos de suscetibilidade à oxacilina: detecção da proteína PBP2a pelo teste de aglutinação em látex (SLA); E-test oxacilina; o sistema automatizado Vitek (GPS-105); o teste de triagem em ágar (OSAS) com oxacilina nas concentrações de 6,0, 1,0 e 0,75 µg oxacilina por ml e o teste de disco difusão com cefoxitina (CDD). A suscetibilidade automatizada foi obtida para os seguintes agentes antimicrobianos: fluorquinolonas, penicilina G, amoxicilina-ampicilina, cefazolina, ampicilina-sulbactam, eritromicina, clindamicina, gentamicina, tetraciclina, sulfametoxazol-trimetoprima, vancomicina e rifampicina. RESULTADOS: Dos 69 Staphylococcus coagulase negativo testados, 71% foram mecA-positivos e 29%, mecA-negativos. Todos os métodos testados apresentaram concordância estatisticamente significante com a reação em cadeia da polimerase. Houve tendência à predominância da positividade da reação em cadeia da polimerase entre os S. epidermidis comparado aos não-epidermidis, embora sem significância estatistica (78,3% vs. 56,5%; chi2= 2,54; p=0,11). O teste de triagem em ágar (0,75 µg oxacilina/ml) apresentou a melhor performance com resultados de: 100% de sensibilidade e valor preditivo negativo, 95% de especificidade e 98% de valor preditivo positivo. Os isolados mecA-positivos foram estatisticamente significativavos mais resistentes para ciprofloxacina, ofloxacina, gatifloxacina e moxifloxacina, no E-test (p=0,002; p=0,008; p=0,002 e p=0,003). Houve maior proporção estatisticamente significativa de resistência entre os Staphylococcus coagulase negativo mecA-positivos para: penicilina G, amoxicilina-ampicilina, cefazolina, ampicilina-sulbactam, eritromicina, clindamicina, gentamicina e tetraciclina. (p<=0,05) Todos os Staphylococcus coagulase negativos foram suscetíveis à vancomicina e não houve correlação estatisticamente significativa entre as amostras mecA-positivas e a resistência para sulfametoxazol-trimetoprima e rifampicina. CONCLUSÕES: No presente estudo, foi observado que o E-test e o OSAS (0,75 µg oxacilina por ml), comparado à reação em cadeia da polimerase, foram os métodos fenotípicos mais acurados em detectar a resistência à oxacilina nos Staphylococcus coagulase negativos. Foi demonstrado que os testes de disco difusão com cefoxitina e o método automatizado (Vitek) são boas opções para a triagem da resistência à oxacilina em laboratórios de microbiologia ocular.     PCR em tempo real em uveites infecciosas como um diagnóstico alternativo

    PURPOSE: Uveitis is a major visual impairment disease affecting parts or the entire uveal tract and occasionally the sclera, the cornea or the optic nerve. The disease is a major cause of ocular morbidity and blindness in immunocompetent and immunocompromised patients. In this work we analyzed the sensitivity and specificity of real-time PCR to detect the etiological agent from blood, plasma, vitreous and aqueous humor and compared with the diagnostic hypothesis. METHODS: Twenty-seven patients (13 male) were studied and Real-time PCR method was used for the detection of herpes simplex virus 1 (HSV-1), herpes simplex virus 2 (HSV-2), varicella zoster virus (VZV), cytomegalovirus (CMV), Mycobacterium tuberculosis (TB) and Toxoplasma gondii (Toxo) in the aqueous humor as well as in the vitreous, blood and plasma. RESULTS: Our results showed the presence of Toxo, CMV, VZV or HSV-2 in 19.2% of aqueous humor samples, and in 30% of vitreous humor samples. In plasma and blood samples, only CMV was detected (11.1% and 3.7%, respectively). CONCLUSION: Real-time PCR was able to detect and to confirm diagnostic hypothesis in uveitis. Our data also confirms that vitreous humor is the best source for molecular diagnosis of infectious uveitis but indicates aqueous humor samples that are easier to obtain may also be appropriate to be tested by Real-time PCR.OBJETIVO: Uveíte é a maior causa de doença ocular que afeta o trato uveal, e ocasionalmente a esclera, cornea e o nervo óptico. Esta doença é a maior causa de morbidade ocular e cegueira em pacientes imunocompetentes e imunossuprimidos. Neste trabalho nós analisamos a sensiblidade e especificidade do PCR em tempo real para detectar agentes etiológicos no sangue, plasma, humor vítreo e aquoso, e comparamos com a hipótese diagnóstica. MÉTODOS: Vinte e sete pacientes (13 homens) foram estudados e o método de PCR em tempo real foi usado para detectar o vírus da herpes simples 1 (HSV-1), vírus da herpes simples 2 (HSV-2), vírus varicella zoster (VZV), citomegalovírus (CMV), Mycobacterium tuberculosis (TB) e Toxoplama gondii (Toxo) no humor aquoso e vítreo, além do sangue e plasma. RESULTADOS: Nossos resultados mostraram a presença de Toxo, CMV, VZV ou HSV-2 em 19,2% das amostras de humor aquoso, e em 30% das amostras de humor vítreo. Nas amostras de plasma e sangue somente CMV foi detectado (11,1% e 3,7%, respectivamente). CONCLUSÃO: PCR em tempo real foi capaz de detectar e confirmar a hipótese diagnóstica em uveíte.     Evaluation of Glaucomatous Damage via Functional Magnetic Resonance Imaging, and Correlations Thereof with Anatomical and Psychophysical Ocular Findings

    PurposeTo evaluate the functional magnetic resonance imaging (fMRI) response to binocular visual stimulation and the association thereof with structural ocular findings and psychophysical test results in patients with glaucoma, and controls.MethodsCross-sectional study. Participants underwent a complete ophthalmic examination, including Humphrey 24-2 visual field (VF) testing and optical coherence tomography. Binocular VF in each quadrant was determined using an integrated method. Patients with glaucoma were assigned to three subgroups: initial, asymmetrical and severe glaucoma. Regions of interest (ROIs) were determined anatomically. fMRI (3 T) was performed using a bilaterally presented polar angle stimulus, and the accompanying changes in blood oxygen level-dependent (BOLD) signals were obtained from the occipital poles and calcarine ROIs. We used generalized estimation equation models to compare anatomical and functional data between the groups.ResultsA total of 25 subjects were enrolled, of whom 17 had glaucoma and 8 were controls. Significant associations between quadrant binocular VF sensitivities and fMRI responses were found in the occipital pole ROIs (p = 0.033) and the calcarine ROIs (p = 0.045). in glaucoma severity subgroup analysis, retinal nerve fiber layer (RNFL) thickness was associated with the BOLD response of the calcarine and occipital pole ROIs (p = 0.002 and 0.026, respectively). the initial and asymmetrical glaucoma subgroups had similar binocular VF sensitivities and RNFL thicknesses, but distinct BOLD responses.ConclusionsThe response of the visual cortex to binocular stimulation was associated with binocular VF sensitivity.     Outcomes in patients with gunshot wounds to the brain.

    Introduction:Gunshot wounds to the brain (GSWB) confer high lethality and uncertain recovery. It is unclear which patients benefit from aggressive resuscitation, and furthermore whether patients with GSWB undergoing cardiopulmonary resuscitation (CPR) have potential for survival or organ donation. Therefore, we sought to determine the rates of survival and organ donation, as well as identify factors associated with both outcomes in patients with GSWB undergoing CPR. Methods:We performed a retrospective, multicenter study at 25 US trauma centers including dates between June 1, 2011 and December 31, 2017. Patients were included if they suffered isolated GSWB and required CPR at a referring hospital, in the field, or in the trauma resuscitation room. Patients were excluded for significant torso or extremity injuries, or if pregnant. Binomial regression models were used to determine predictors of survival/organ donation. Results:825 patients met study criteria; the majority were male (87.6%) with a mean age of 36.5 years. Most (67%) underwent CPR in the field and 2.1% (n=17) survived to discharge. Of the non-survivors, 17.5% (n=141) were considered eligible donors, with a donation rate of 58.9% (n=83) in this group. Regression models found several predictors of survival. Hormone replacement was predictive of both survival and organ donation. Conclusion:We found that GSWB requiring CPR during trauma resuscitation was associated with a 2.1% survival rate and overall organ donation rate of 10.3%. Several factors appear to be favorably associated with survival, although predictions are uncertain due to the low number of survivors in this patient population. Hormone replacement was predictive of both survival and organ donation. These results are a starting point for determining appropriate treatment algorithms for this devastating clinical condition.     Toronto Alexithymia Scale: Adaptação da Versão Brasileira para Adultos de Baixa Escolaridade

    For the purpose of studying Alexithymia in low-educated adults, we intend to adapt the Brazilian version of the Toronto Alexithymia Scale (TAS-26) and to verify its internal consistency. With that aim, we translated the original TAS-26 (English) to Portuguese, adopting a colloquial language, without content distortion. An exploratory qualitative study interviewed 50 women (38-65 years, educationCon objeto de estudiar la Alexitimia en adultos con bajo nivel de educación, buscamos adaptar la versión brasileña de la Escala de Alexitimia de Toronto (TAS-26) y comprobar su consistencia interna. Para esto, traducimos la TAS-26 original (inglés) al portugués, con la adopción de un lenguaje sencillo, sin cambiar el contenido. Una encuesta cualitativa entrevistó a 50 mujeres (38-65 años, escolaridadCom objetivo de estudar a alexitimia em indivíduos adultos de baixa escolaridade, decidimos adaptar a versão brasileira da Toronto Alexithymia Scale (TAS-26) e verificar sua consistência interna. Para isso, traduzimos a TAS-26 original (inglês) para o português, com linguagem coloquial, sem alteração de conteúdo.
