An assessment of pulse transit time for detecting heavy blood loss during surgical operation

work is properly cited.The main contribution of this paper is the use of non-invasive measurements such as electrocardiogram (ECG) and photoplethysmographic (PPG) pulse oximetry waveforms to develop a new physiological signal analysis technique for detecting blood loss during surgical operation. Urological surgery cases were considered as the control group due to its generality, and cardiac surgery as experimental group since it involves blood loss and water supply. Results show that the control group has the tendency of a reduction of the pulse transient time (PTT), and this indicates an increment in the blood flow velocity changes from slow to fast. While for the experimental group, the PTT indicates high values during blood loss, and low values during water supply. Statistical analysis shows considerable differences (i.e., P <0.05) between both groups leading to the conclusion that PTT could be a good indicator for monitoring patients' blood loss during a surgical operation.The National Science Council (NSC) of Taiwan and the Centre for Dynamical Biomarkers and Translational Medicine, National Central University, Taiwan
Perception of Nuclear Energy and Coal in France and the Netherlands
This study focuses on the perception of large scale application of nuclear energy and coal in the Netherlands and France. The application of these energy-sources and the risks and benefits are judged differently by various group in society. In Europe, France has the highest density of nuclear power plants and the Netherlands has one of the lowest. In both countries scientists and social scientists completed a questionnaire assessing the perception of the large scale application of both energy sources. Furthermore, a number of variables relating to the socio cultural and political circumstances were measured. The results indicate that the French had a higher risk perception and a more negative attitude toward nuclear power than the Dutch. But they also assess the benefits of the use of nuclear power to be higher. Explanations for these differences are discussed
Social influence in networks of practice: An analysis of organizational communication content
Networks of Practice (NoPs) facilitate knowledge sharing among geographically dispersed organization members. This research tests whether social influence in NoPs is reinforced by actors' embeddedness in practice (knowledge about informal content), organizational embeddedness (knowledge about formal organizational content), structural embeddedness (knowledge about who knows what), and relational embeddedness (knowledge about informal relationships). A full-fledged automated content analysis on all postings on four NoPs maintained by a multinational chemical company revealed four dimensions in communication content that largely coincide with the proposed embeddedness types. We measured social influence by assessing to what extent actors' use of uncommon language traits was adopted in the responses to the postings. Hypothesis testing revealed that network members who communicate about informal practice, and know who knows what, exert more social influence than others. The results suggest that network members' social influence is rooted in their utilitarian value for others, and not in their organizational or relational embeddedness. © The Author(s) 2011
Impaired perceptual learning in a mouse model of Fragile X syndrome is mediated by parvalbumin neuron dysfunction and is reversible.
To uncover the circuit-level alterations that underlie atypical sensory processing associated with autism, we adopted a symptom-to-circuit approach in the Fmr1-knockout (Fmr1-/-) mouse model of Fragile X syndrome. Using a go/no-go task and in vivo two-photon calcium imaging, we find that impaired visual discrimination in Fmr1-/- mice correlates with marked deficits in orientation tuning of principal neurons and with a decrease in the activity of parvalbumin interneurons in primary visual cortex. Restoring visually evoked activity in parvalbumin cells in Fmr1-/- mice with a chemogenetic strategy using designer receptors exclusively activated by designer drugs was sufficient to rescue their behavioral performance. Strikingly, human subjects with Fragile X syndrome exhibit impairments in visual discrimination similar to those in Fmr1-/- mice. These results suggest that manipulating inhibition may help sensory processing in Fragile X syndrome
Минералогические исследования в пещерной системе Снежная-Меженного-Иллюзия (Западный Кавказ, Бзыбский хребет): предварительные результаты и направления дальнейших работ
В статье приводятся сведения о минеральном составе водных хемогенных и водных механических отложений в пещерной системе Снежная-Меженного-Иллюзия. В состав водных хемогенных отложений входят Mg- и Sr-содержащий кальцит, арагонит, гипс, гидромагнезит, целестин, стронцианит, доломит, гетит, рутил и циркон. Водные механические отложения сложены преимущественно доломитом, кварцем и кальцитом. В схожих по морфологии и микроклимату частях пещерной системы наблюдаются одинаковые вторичные минералы.У статті наводяться відомості про мінеральний склад водних хемогенних і водних механічних відкладень в печерній системі Сніжна-Меженого-Ілюзія. До складу водних хемогенних відкладень входять кальцит, який містить Mg і Sr, арагоніт, гіпс, гідромагнезіт, целестин, стронціаніт, доломіт, гетит, рутил і циркон. Водні механічні відкладення складені переважно доломітом, кварцом і кальцитом. У схожих за морфологєю та мікрокліматом частинах печерної системи спостерігаються однакові вторинні мінерали.The article presents the preliminary characteristic of the mineral composition of chemogenic formations and clastic deposits of Snezhnaya-Mezhennogo-Illusia cave system. Chemogenic formations are composed by Mg- and Sr-calcite, aragonite, gypsum and hydromagnesite, celestite, strontianite, dolomite, goethite, rutile and zircon. Clastic sediments are composed mainly by dolomite, quartz and calcite. Same secondary minerals are observed in those parts of the cave system that have similar morphology and microclimate

Abstract:
Objective: The objective of the present study was to examine knowledge on vitamin D and calcium in a cohort of older adults and to test the association between health knowledge, vitamin D status and dietary calcium intake.
Methods: The participants of this cross-sectional survey consisted of 426 individuals (≥65 years), living in residential homes. Participants were tested for their knowledge on vitamin D and calcium using a standardized questionnaire. Serum 25-hydroxyvitamin D3 (25(OH)D3) levels and dietary calcium intake were measured.
Results: The mean serum 25(OH)D3 level was 39.1 (±21.4) nmol/l and the mean daily dietary calcium intake was 826 (±242) mg/day. Of the participants, only 3
Clinical practice guidelines for the foot and ankle in rheumatoid arthritis: a critical appraisal
Background: Clinical practice guidelines are recommendations systematically developed to assist clinical decision-making and inform healthcare. In current rheumatoid arthritis (RA) guidelines, management of the foot and ankle is under-represented and the quality of recommendation is uncertain. This study aimed to identify and critically appraise clinical practice guidelines for foot and ankle management in RA. Methods: Guidelines were identified electronically and through hand searching. Search terms 'rheumatoid arthritis', 'clinical practice guidelines' and related synonyms were used. Critical appraisal and quality rating were conducted using the Appraisal of Guidelines for Research and Evaluation (AGREE) II instrument. Results: Twenty-four guidelines were included. Five guidelines were high quality and recommended for use. Five high quality and seven low quality guidelines were recommended for use with modifications. Seven guidelines were low quality and not recommended for use. Five early and twelve established RA guidelines were recommended for use. Only two guidelines were foot and ankle specific. Five recommendation domains were identified in both early and established RA guidelines. These were multidisciplinary team care, foot healthcare access, foot health assessment/review, orthoses/insoles/splints, and therapeutic footwear. Established RA guidelines also had an 'other foot care treatments' domain. Conclusions: Foot and ankle management for RA features in many clinical practice guidelines recommended for use. Unfortunately, supporting evidence in the guidelines is low quality. Agreement levels are predominantly 'expert opinion' or 'good clinical practice'. More research investigating foot and ankle management for RA is needed prior to inclusion in clinical practice guidelines
A hand hygiene intervention to decrease infections among children attending day care centers: Design of a cluster randomized controlled trial
Background: Day care center attendance has been recognized as a risk factor for acquiring gastrointestinal and respiratory infections, which can be prevented with adequate hand hygiene (HH). Based on previous studies on environmental and sociocognitive determinants of caregivers' compliance with HH guidelines in day care centers (DCCs), an intervention has been developed aiming to improve caregivers' and children's HH compliance and decrease infections among children attending DCCs. The aim of this paper is to describe the design of a cluster randomized controlled trial to evaluate the effectiveness of this intervention.Methods/design: The intervention will be evaluated in a two-arm cluster randomized controlled trial among 71 DCCs in the Netherlands. In total, 36 DCCs will receive the intervention consisting of four components: 1) HH products (dispensers and refills for paper towels, soap, alcohol-based hand sanitizer, and hand cream); 2) training to educate about the Dutch national HH guidelines; 3) two team training sessions aimed at goal setting and formulating specific HH improvement activities; and 4) reminders and cues to action (posters/stickers). Intervention DCCs will be compared to 35 control DCCs continuing usual practice. The primary outcome measure will be observed HH compliance of caregivers and children, measured at baseline and one, three, and six months after start of the intervention. End of the spectacular decrease in fall-related mortality rate: Men are catching up
Objectives: We determined time trends in numbers and rates of fall-related mortality in an aging population, for men and women. Methods. We performed secular trend analysis of fall-related deaths in the older Dutch population (persons aged 65 years or older) from 1969 to 2008, using the national Official-Cause-of-Death-Statistics. Results. Between 1969 and 2008, the age-adjusted fall-related mortality rate decreased from 202.1 to 66.7 per 100 000 older persons (decrease of 67%). However, the annual percentage change (change per year) in mortality rates was not constant, and could be divided into 3 phases: (1) a rapid decrease until the mid-1980s (men -4.1%; 95% confidence interval [CI] = -4.9, -3.2; women -6.5%; 95% CI, -7.1, -5.9), (2) flattening of the decrease until the mid-1990s (men -1.4%; 95% CI = -2.4, -0.4; women -2.0%; 95% CI = -3.4, -0.6), and (3) stable mortality rates for women (0.0%; 95% CI = -1.2, 1.3) and rising rates for men (1.9%; 95% CI = 0.6, 3.2) over the last decade.
