Risk factors for infection with Giardia duodenalis in pre-school children in the city of Salvador, Brazil.
A cross-sectional study of 694 children aged 2 to 45 months selected from 30 clusters throughout the city of Salvador, Bahia (pop. 2.3 million) was carried out as part of a longitudinal study of diarrhoea in order to identify risk factors for infection with Giardia duodenalis. Variables studied included three social and demographic factors (such as mother's education and marital status), five relating to the peri-domestic environment (rubbish disposal, open sewers, paving of the street), seven relating to the home itself (house construction, susceptibility to flooding, water supply and sanitation) as well as a score for hygiene behaviour based on structured observation. After multivariate analysis using a hierarchical model, only four significant risk factors were found: (a) number of children in the household under five years (b) rubbish not collected from the house (c) presence of visible sewage nearby, and (d) absence of a toilet. All four were significant at the 1% level

la realización de sus actividades de la vida diaria y en la forma como interactúan
con su propio entorno para la realización de las mismas. Se pasa de una
visión eminentemente médica a una visión social. Por ejemplo, la Organización
Panamericana de la Salud en el 2001, señala que la participación restringida de
las personas con discapacidad en las actividades educativas, laborales y sociales

Três rearranjos diferentes, três fenótipos diferentes :Estudo Familiar Cromossoma 14
Introdução – Cromossomas derivativos são o resultado de rearranjos estruturais que
tanto podem ocorrer num só, como entre dois ou mais cromossomas. Estes rearranjos
dão origem a cromossomas estruturalmente anormais, podendo resultar um fenótipo
normal ou mais ou menos grave, dependendo do tipo de anomalia encontrada.
Materiais e métodos – Caso índex: homem de 55 anos, referenciado para estudos de
citogenética clássica (cariótipo com bandas GTG de alta resolução) e molecular
(MLPA – kits P036 e P070 e FISH com sonda subtelomérica especifica para o
cromossoma 14) por apresentar um quadro clínico de atraso mental. Posteriormente
realizaram-se estudos citogenéticos a uma irmã com atraso cognitivo e baixa estatura,
e a mais quatro familiares com fenótipos normais.
Resultados – O cariótipo do caso índex revelou a existência de uma anomalia
cromossómica estrutural desequilibrada num dos cromossomas 14, sugerindo uma
deleção da banda 14q32, e uma duplicação do braço curto localizada na parte terminal
do braço longo. Nos estudos de citogenética molecular, a técnica de MLPA identificou
uma deleção da região subtelomérica no braço longo do cromossoma 14, em ambos
os kits e, posteriormente, a técnica de FISH comprovou essa deleção.
Após estudos familiares, concluiu-se que dois dos irmãos apresentavam anomalias
cromossómicas distintas do caso índex, envolvendo igualmente o cromossoma 14.
Apesar de não ser possível efetuar o cariótipo à mãe (falecida), presume-se que estas
alterações tenham tido origem numa anomalia cromossómica materna, uma vez que o
pai deste indivíduo apresentava um cariótipo normal.
Conclusões – Os autores apresentam os resultados citogenéticos dos vários
indivíduos estudados, e realçam a raridade da existência de três rearranjos diferentes
(um deles aparentemente equilibrado e dois desequilibrados), envolvendo o
cromossoma 14, encontrados numa mesma família
Identification of a cytokine network sustaining neutrophil and Th17 activation in untreated early rheumatoid arthritis

synovitis. Recently, several studies have proposed neutrophils and Th17 cells as key players in the onset and
perpetuation of this disease. The main goal of this work was to determine whether cytokines driving neutrophil
and Th17 activation are dysregulated in very early rheumatoid arthritis patients with less than 6 weeks of disease
duration and before treatment (VERA).
Methods: Cytokines related to neutrophil and Th17 activation were quantified in the serum of VERA and
established RA patients and compared with other very early arthritis (VEA) and healthy controls. Synovial fluid (SF)
from RA and osteoarthritis (OA) patients was also analyzed.
Results: VERA patients had increased serum levels of cytokines promoting Th17 polarization (IL-1b and IL-6), as
well as IL-8 and Th17-derived cytokines (IL-17A and IL-22) known to induce neutrophil-mediated inflammation. In
established RA this pattern is more evident within the SF. Early treatment with methotrexate or corticosteroids led
to clinical improvement but without an impact on the cytokine pattern.
Conclusions: VERA patients already display increased levels of cytokines related with Th17 polarization and
neutrophil recruitment and activation, a dysregulation also found in SF of established RA. 0 Thus, our data suggest

Condições socioeconômicas e padrões alimentares de crianças de 4 a 11 anos: estudo SCAALA - Salvador/ Bahia
OBJETIVOS: identificar os padrões alimentares de crianças e sua associação com o nível socioeconômico das famílias. MÉTODOS: estudo transversal com 1260 crianças de 4 a 11 anos, residentes em Salvador-Bahia que incluiu aplicação de um Questionário de Frequência Alimentar semi-quantitativo. Os padrões alimentares foram identificados, empregando-se análise fatorial por componentes principais. O nível socioeconômico foi avaliado por meio de um indicador socioeconômico composto. Regressão logística multivariada foi empregada. RESULTADOS: identificaram-se quatro padrões que explicaram 45,9% da variabilidade dos dados de frequência alimentar. Crianças que pertencem ao nível socioeconômico mais alto têm 1,60 vezes mais chance (p<0,001) de apresentarem maior frequência de consumo de alimentos do padrão 1 (frutas, verduras, leguminosas, cereais e pescados) e 3,09 vezes mais chance (p<0,001) de apresentarem maior frequência de consumo dos alimentos do padrão 2 (leite/ derivados, catchup/ maionese/ mostarda e frango), quando se compara com aquele de crianças de nível socioeconômico mais baixo. Resultado inverso foi observado no padrão 4 (embutidos, ovos e carnes vermelhas); isto é, quanto maior o nível socioeconômico menor a chance da adoção desse padrão. Tendência similar foi notada para o padrão 3 (frituras, doces, salgadinhos, refrigerante/ suco artificial). CONCLUSÕES: padrões alimentares de crianças são dependentes das condições socioeconômicas das famílias e a adoção de itens alimentares mais saudáveis associa-se aos grupos de mais altos níveis socioeconômicos
Reactive oxygen-related diseases: therapeutic targets and emerging clinical indications
SIGNIFICANCE
Enhanced levels of reactive oxygen species (ROS) have been associated with different disease states. Most attempts to validate and exploit these associations by chronic antioxidant therapies have provided disappointing results. Hence, the clinical relevance of ROS is still largely unclear.
RECENT ADVANCES
We are now beginning to understand the reasons for these failures, which reside in the many important physiological roles of ROS in cell signaling. To exploit ROS therapeutically, it would be essential to define and treat the disease-relevant ROS at the right moment and leave physiological ROS formation intact. This breakthrough seems now within reach.
CRITICAL ISSUES
Rather than antioxidants, a new generation of protein targets for classical pharmacological agents includes ROS-forming or toxifying enzymes or proteins that are oxidatively damaged and can be functionally repaired.
FUTURE DIRECTIONS
Linking these target proteins in future to specific disease states and providing in each case proof of principle will be essential for translating the oxidative stress concept into the clinic. Antioxid. Redox Signal. 23, 1171-1185
Nitrogen uptake and internal recycling in Zostera marina exposed to oyster farming: eelgrass potential as a natural biofilter
Oyster farming in estuaries and coastal lagoons frequently overlaps with the distribution of seagrass meadows, yet there are few studies on how this aquaculture practice affects seagrass physiology. We compared in situ nitrogen uptake and the productivity of Zostera marina shoots growing near off-bottom longlines and at a site not affected by oyster farming in San Quintin Bay, a coastal lagoon in Baja California, Mexico. We used benthic chambers to measure leaf NH4 (+) uptake capacities by pulse labeling with (NH4)-N-15 (+) and plant photosynthesis and respiration. The internal N-15 resorption/recycling was measured in shoots 2 weeks after incubations. The natural isotopic composition of eelgrass tissues and vegetative descriptors were also examined. Plants growing at the oyster farming site showed a higher leaf NH4 (+) uptake rate (33.1 mmol NH4 (+) m(-2) day(-1)) relative to those not exposed to oyster cultures (25.6 mmol NH4 (+) m(-2) day(-1)). We calculated that an eelgrass meadow of 15-16 ha (which represents only about 3-4 % of the subtidal eelgrass meadow cover in the western arm of the lagoon) can potentially incorporate the total amount of NH4 (+) excreted by oysters (similar to 5.2 x 10(6) mmol NH4 (+) day(-1)). This highlights the potential of eelgrass to act as a natural biofilter for the NH4 (+) produced by oyster farming. Shoots exposed to oysters were more efficient in re-utilizing the internal N-15 into the growth of new leaf tissues or to translocate it to belowground tissues. Photosynthetic rates were greater in shoots exposed to oysters, which is consistent with higher NH4 (+) uptake and less negative delta C-13 values. Vegetative production (shoot size, leaf growth) was also higher in these shoots. Aboveground/belowground biomass ratio was lower in eelgrass beds not directly influenced by oyster farms, likely related to the higher investment in belowground biomass to incorporate sedimentary nutrients
‘Trial and error…’, ‘…happy patients’ and ‘…an old toy in the cupboard’: a qualitative investigation of factors that influence practitioners in their prescription of foot orthoses
Background: Foot orthoses are used to manage of a plethora of lower limb conditions. However, whilst the theoretical
foundations might be relatively consistent, actual practices and therefore the experience of patients is likely to be less so.
The factors that affect the prescription decisions that practitioners make about individual patients is unknown and hence
the way in which clinical experience interacts with knowledge from training is not understood. Further, other influences
on orthotic practice may include the adoption (or not) of technology. Hence the aim of this study was to explore, for
the first time, the influences on orthotic practice.
Methods: A qualitative approach was adopted utilising two focus groups (16 consenting participants in total; 15
podiatrists and 1 orthotist) in order to collect the data. An opening question “What factors influence your orthotic
practice?” was followed with trigger questions, which were used to maintain focus. The dialogue was recorded
digitally, transcribed verbatim and a thematic framework was used to analyse the data.
Results: There were five themes: (i) influences on current practice, (ii) components of current practice, (iii) barriers
to technology being used in clinical practice, (iv) how technology could enhance foot orthoses prescription and
measurement of outcomes, and (v) how technology could provide information for practitioners and patients. A final
global theme was agreed by the researchers and the participants: ‘Current orthotic practice is variable and does not
embrace technology as it is perceived as being not fit for purpose in the clinical environment. However, practitioners
do have a desire for technology that is usable and enhances patient focussed assessment, the interventions, the clinical
outcomes and the patient’s engagement throughout these processes’.
Conclusions: In relation to prescribing foot orthoses, practice varies considerably due to multiple influences.
Measurement of outcomes from orthotic practice is a priority but there are no current norms for achieving this.
There have been attempts by practitioners to integrate technology into their practice, but with largely negative
experiences. The process of technology development needs to improve and have a more practice, rather than
technology focus
Primary resistance of HIV to antiretrovirals among individuals recently diagnosed at voluntary counselling and testing centres in the metropolitan region of Recife, Pernambuco
Determining the prevalence and type of antiretroviral (ARV) resistance among ARV-naïve individuals is important to assess the potential responses of these individuals to first-line regimens. The prevalence of primary resistance and the occurrence of recent infections among individuals with human immunodeficiency virus (HIV)/acquired immune deficiency syndrome (AIDS) were identified among recently diagnosed patients at five sexually transmitted disease/AIDS testing and counselling centres in the metropolitan region of Recife (RMR), Pernambuco, Brazil, between 2007-2009. One-hundred and eight samples were analysed using the Calypte® BED assay. Males predominated (56%), as did patients aged 31-50 years. Twenty-three percent presented evidence of a recent HIV infection. The median CD4+ T lymphocyte count was 408 cells/mm³ and the median viral load was 3.683 copies/mL. The prevalence of primary resistance was 4.6% (confidence interval 95% = 1-8.2%) based on criteria that excluded common polymorphisms in accordance with the surveillance drug resistance mutation criteria. The prevalence of resistance to non-nucleoside reverse transcriptase, nucleoside/nucleotide reverse transcriptase and protease inhibitors were 3.8%, 1.5% and 0.8%, respectively. Fifty-seven percent of strains were from clade B, 37.7% were clade F and 3.1% were clade C; there were no statistically significant differences with respect to resistance between clades. Recent infection tended to be more common in men (p = 0.06) and in municipalities in the south of the RMR (Jaboatão dos Guararapes and Cabo de Santo Agostinho) (p = 0.046).
