    The use of MDS and HCA enabled pharmacists to reveal their roles which reflect on country of practice and cultural differences when improving patients adherence to asthma medication

    Pharmacists are health care professionals who have a lot more to offer to the public rather than just selling pills. Pharmacists live a dichotomy role: health care consultants versus shop keepers. The aim of this research was to assess how pharmacists intervene improving patient adherence to asthma medication and to see how pharmacists perceived themselves. The research was conducted by means of a multiple choice questionnaire that was completed by 551 pharmacists. The replies were analysed using Ordinal Multidimensional Scaling and Cluster Analysis, with the routine PROXSCAL. Pharmacists revealed their roles in terms of four dimensions: a community health orientation, theoretical-applied drug orientation, the degree of involvement with the patient and the role of the pharmacist as a consultant. These dimensions have different salience in each one of the countries

    The aim of this paper is to establish the H1H^1 global well-posedness for Kirchhoff systems. The new approach to the construction of solutions is based on the asymptotic integrations for strictly hyperbolic systems with time-dependent coefficients. These integrations play an important role to setting the subsequent fixed point argument. The existence of solutions for less regular data is discussed, and several examples and applications are presented.Comment: 24 page

    Pharmacists’ and patients’ views and feedback on Italian Medicines Use Review (I-MUR)

    Rationale, aims and objectives: Although medicine review services are offered by community pharmacists in many countries, they are non-existent in Italy. A novel intervention I-MUR, was developed for patients with asthma aiming to improve medicines use. The aim of this study was to obtain pharmacists’ and patients’ views on the acceptability of the I-MUR service provided by community pharmacists to asthma patients in four regions of Italy. Methods: Pharmacists’ expectations, experiences and attitudes to delivering the I-MUR were obtained through questionnaires distributed before and after delivering the I-MUR, plus focus groups. Patients’ views were obtained via questionnaires, distributed by the pharmacists and returned anonymously. Results: Seventy-four pharmacists provided the I-MUR service to 895 asthma patients; 49 pharmacists completed both questionnaires, 53 participated in focus groups and 246 patients returned questionnaires. Barriers anticipated most frequently by pharmacists before the I-MUR were lack of time (53%) and lack of co-ordination with other health professionals (61%), while lack of financial compensation was identified by 37%. Lack of co-ordination proved the most common actual barrier (88%), with lack of financial compensation being cited less frequently after providing the intervention (8%). Ninety-six percent of pharmacists anticipated providing both education on inhaler technique and medication counselling, but in practice slightly fewer had provided these (90% and 86% respectively). Focus groups highlighted a lack of relevant undergraduate education to support medication review and structural barriers within some pharmacies, but described positive patient feedback and desire to extend the I-MUR. Patients’ respondents were positive; 62% indicated the reason for having an I-MUR as making sure that they were using medication correctly, 75% considered they benefited from it and 86% would recommend it to others. Conclusions: The I-MUR service was perceived positively by both pharmacists and patients, supporting the extension of medicine review services to community pharmacists in Italy

Confiram no link: https://revistas.unila.edu.br/anaismaloca/index :)Este trabalho procura problematizar a temática de fronteira em países da América Latina e Ibéricos, sobre o ponto de vista do cotidiano desses países, na questão social e econômica, como na tríplice fronteira entre o Brasil, Paraguai e Argentina; Brasil, Peru e Bolívia; o norte do México, na América Latina e Portugal e Espanha na península Ibérica. Nesses espaços, ocorrem migrações de pessoas, como a diária, temporária ou permanente, conforme os interesses de cada uma, e o comércio de mercadorias, de forma legal e lícita, conforme a lei do país em questão, e também a ilegal, a clandestina e a ilícita. Diante disso, observa-se que em meio à variedade de compras de mercadorias, algumas acontecem através do contrabando, situações de comércio que se insere no contexto da globalização. No entanto, esse cenário propicia a identidade de cultura de fronteiras, que consequentemente resultam no desenvolvimento do local, com as vendas de produtos, como também à renda do trabalho, e assim sendo, à ampliação urbana em tais espaços geográficos. Porém, convém lembrar que nesse trabalho são leituras das fronteiras e não sobre as fronteiras em si, pois cada uma tem sua política em questão.Este trabajo busca problematizar la temática de frontera en países latinoamericanos e ibéricos bajo el punto de vista del cotidiano de dichos países considerándose la cuestión socioeconómica como en la triple frontera entre Brasil, Paraguay y Argentina, Brasil, Peru y Bolívia, el norte de México, en Latinoamérica, Portugal y España en la península Ibérica. En estos espacios ocurren las migraciones de personas, como diaria, temporaria o permanente, de acuerdo con los intereses de cada y el comércio de mercancías, de forma legal, lícita conforme la ley e ilegal, la acción clandestina, ilícita. Por ello, se observa que las compras de esas mercancías se pasan en estos sitios o fuera de ellos, por el contrabando, inseridos en el contexto de la globalización. Sin embargo, el escenario propicia la identidad de cultura de frontera, que consecuentemente resulta en desarrollo local, con las ventas de produtos, así como los ingresos del trabajo, ampliándose los espacios geográficos.     [Abstract] How to secure better outcomes for everybody in asthma management: the international-medicines use review health technology assessment (international-MUR HTA)

    Aims: This project is looking at the development and application of the novel tool (International–Medicines-Use-Review-Health-Technology-Assessment; International-MUR HTA) in community practice that for the first time is able to meet better asthma control and secure added value service in asthma management. More specifically it allows to: evaluate the quality of care delivered in terms of economic impact (for patient-provider-society), health outcomes and patient benefits; collect real-world evidence and evaluate longterm effect of care; provide different stakeholders with evidence-based information that would help formulate health policies in community practice that are safe, effective, patient-focused and cost-effective, balancing access to innovation and cost containment. Crucially, the tool can also support the delivery of a cost-effective and cost-saving intervention for asthma patients based on the success of the Italian-Medicines-Use-Review (I-MUR) trial [1]. Methods: Evidence from the Italian-Medicine-Use-Review (I-MUR) trial showed that the I-MUR intervention provided by the community pharmacists in asthma is effective, cost-saving and cost-effective.1 The trial allowed to model a novel framework (International-MUR-HTA) that would enable to routinely deliver the intervention, but also collect and analyse patient relevant data on its clinical-effectiveness, quality-of-life and cost-effectiveness. I-MUR-HTA was discussed within three expert-panel discussions including policy-makers, commissioners, academics, healthcare-professionals and patientrepresentatives in Italy,2 UK3 and Brussels/Europe.4 The current plan includes testing the use of the tool in RW environment across European regions. Results: Evidence collected from the expert discussions confirmed that International-MUR-HTA information is relevant to meet current NICE target for cost-effective service delivery and this is what is needed to support the evaluation of innovative effective and cost-effective health policies and promote their implementation across nations. Its implementation is underway and real-world pilots are planned to take place in different European regions. Conclusion: the International-MUR-HTA appears to be an innovative tool to promote active patient involvement into policy-decision-making and community service implementation

    Molecular characterization of SSS139, a new satellite DNA family in sibling species of the Drosophila buzzatii cluster

    We characterized sequences of a novel SSS139 RsaI satellite DNA family in Drosophila gouveai and Drosophila seriema, two members of the Drosophila buzzatii cluster (D. repleta group). The sequences were AT-rich (69%) with a monomer unit length of about 139 bp and contained two direct subrepeats of 14 bp and 16 bp, suggesting that it might have originated by the duplication of smaller sequences. Southern and dot-blot hybridization analyses also detected SSS139 in other Drosophila buzzatii cluster species (D. koepferae, D. antonietae, D. borborema and D. serido) but not in D. buzzatii.     Las redes de la supervivencia oncológica: con la vida de nuevo

    Este trabalho tem origem na tese de doutorado que objetivou compreender a experiência da radioterapia oncológica para os pacientes sob referencial teórico-metodológico da antropologia interpretativa e do método etnográfico. Os informantes foram 10 pacientes de ambos os sexos, na faixa etária de 34 a 80 anos. Os dados foram coletados de março a agosto de 2007 por entrevistas semiestruturadas e observações participantes. Pela análise interpretativa proposta por Hammersley e Atkinson, construímos os núcleos de significados - Alterações na vida do sobrevivente ao câncer e à radioterapia e O sobrevivente controlando a sua vida e tecendo as teias do futuro. Entendemos que a enfermagem ultrapassa a dimensão biológica do cuidar quando compreende as teias tecidas pelos sobreviventes oncológicos para o ajuste à nova vida, pois eles percebem os limites existenciais, incorporam a doença e o tratamento, inserindo-os num processo liminar que persiste para o resto da vida.This study originates from a doctoral dissertation that aimed to better understand patients' oncological radiotherapy experience, given the theoretical and methodological reference of interpretative anthropology and the ethnographic method. The informants were 10 patients of both sexes, aged 34 to 80. The data was collected from March to August of 2007 through semi-structured interviews and participant observations. Through the interpretative analysis proposed by Hammersley and Atkinson, we constructed the clusters of meanings - Life alterations for the cancer and radiation survivor, and The survivor controlling their lives and weaving webs of the future. We understand nursing to go beyond the biological dimension of care when it includes the webs woven by cancer survivors to adjust to a new life, as they realize their existential limits, incorporate the disease and treatment, and insert them in a preliminary process which persists for the rest of their lives.Este trabajo proviene de la tesis que objetivó comprender la experiencia de radioterapia para pacientes con cáncer, a partir del referencial teórico y metodológico de la antropología interpretativa y del método etnográfico. Los informantes fueron 10 pacientes de ambos sexos, entre 34 a 80 años. Los datos fueron recolectados de marzo a agosto de 2007, por entrevistas semiestructuradas y observaciones participantes. A través del análisis interpretativo propuesto por Hammersley y Atkinson, construimos los siguientes grupos de significados - Cambios en la vida del sobreviviente del cáncer y la radiación, y, El sobreviviente controlando su vida y tejiendo las redes del futuro. Creemos que la enfermería ultrapasa la dimensión biológica de la atención cuando incluye las redes tejidas por los sobrevivientes de cáncer para adaptarse a la nueva vida, porque ellos perciben los límites existenciales, la incorporación de la enfermedad y el tratamiento, colocándolos en un proceso limítrofe, persistente para el resto de la vida
