Lurasidone in the Treatment of Bipolar Depression: : Systematic Review of Systematic Reviews

A burgeoning number of systematic reviews considering lurasidone in the treatment of bipolar depression have occurred since its Food and Drug Administration extended approval in 2013. While a paucity of available quantitative evidence still precludes preliminary meta-analysis on the matter, the present quality assessment of systematic review of systematic reviews, nonetheless, aims at highlighting current essential information on the topic. Methods. Both published and unpublished systematic reviews about lurasidone mono- or adjunctive therapy in the treatment of bipolar depression were searched by two independent authors inquiring PubMed/Cochrane/Embase/Scopus from inception until October 2016. Results. Twelve included systematic reviews were of moderate-to-high quality and consistent in covering the handful of RCTs available to date, suggesting the promising efficacy, safety, and tolerability profile of lurasidone. Concordance on the drug profile seems to be corroborated by a steadily increasing number of convergent qualitative reports on the matter. Limitations. Publication, sponsorship, language, citation, and measurement biases. Conclusions. Despite being preliminary in nature, this overview stipulates the effectiveness of lurasidone in the acute treatment of Type I bipolar depression overall. As outlined by most of the reviewed evidence, recommendations for future research should include further controlled trials of extended duration.Peer reviewe
The role of melatonin in mood disorders
Melatonin (N-acetyl-5-methoxytryptamine) has been discovered as a hormone secreted by the pineal gland, even though it is also synthetized in various other organs, tissues, and cells. The circadian rhythm of melatonin is often used as an indicator phase position since it is a well-defined, high-amplitude rhythm controlled by the hypothalamic suprachiasmatic nuclei. Melatonin production is controlled by this endogenous circadian timing system. It peaks during the night and is suppressed by daylight. Mood spectrum disorders, including bipolar disorder (BD), major depressive disorder (MDD), and seasonal affective disorder (SAD), have been observed to be accompanied by circadian dysregulation as well as dysregulation in melatonin secretion. Simultaneously, it has also been documented that disruptions in circadian rhythms, including the sleep/wake cycle, though environmental means can produce mood-related problems in vulnerable individuals. These findings suggested that altered circadian rhythms might be biological markers of these disorders. As melatonin is considered a chronobiotic factor, ie, able to entrain the circadian rhythms of several biological functions (eg, activity/rest, sleep/wake, body temperature, endocrine rhythms, etc), its use may provide a new therapeutic approach for the treatment of affective disorders. Fast and Accurate ISAR Focusing Based on a Doppler Parameter Estimation Algorithm
This letter deals with inverse synthetic aperture radar (ISAR) autofocusing of noncooperative moving targets. The relative motion between the target and the sensor, which provides the angular diversity necessary for ISAR imagery, is also responsible for unwanted range migration and phase changes generating defocusing. In the case of noncooperative targets, the relative motion is unknown: the ISAR needs, hence, to implement an autofocus step [motion compensation (MoCo)] to achieve high resolution imaging. This task is typically carried out via the optimization of functionals based on general image quality parameters. In this letter, we propose the use of a fast and accurate MoCo algorithm based on the estimation of the Doppler parameters, thus fully coping with the nature of the imaging system. SECCA procedure for anal incontinence and antibiotic treatment: a case report of anal abscess
Background: Fecal Incontinence (FI) can seriously affect quality of life. The treatment of fecal incontinence starts
conservatively but in case of failure, different surgical approaches may be proposed to the patient. Recently several
not invasive approaches have been developed. One of these is the radiofrequency (RF) energy application to the
internal anal sphincter.
Case presentation: We report a rare case of an anal abscess related to a SECCA procedure in a 66-year-old woman
affected by gas and FI for twenty years.
Conclusions: The complications post-SECCA procedure reported in literature are generally not serious and often
self-limited, such as bleeding or anal pain. This is a case of an anal abscess.
We suggest that this finding could consolidate the importance of administering antibiotic therapy to patients and
to run a full course of at least 6 days rather than a short-term (24 h) therapy, with the aim to minimize the
incidence of this complication
