Mammography-based screening program: preliminary results from a first 2-year round in a Brazilian region using mobile and fixed units

contributions to the conception and design of the study.Background: Breast cancer is the most frequently diagnosed cancer and the leading cause of cancer deaths
among women worldwide. The use of mobile mammography units to offer screening to women living in remote
areas is a rational strategy to increase the number of women examined. This study aimed to evaluate results from
the first 2 years of a government-organized mammography screening program implemented with a mobile unit
(MU) and a fixed unit (FU) in a rural county in Brazil. The program offered breast cancer screening to women living
in Barretos and the surrounding area.
Methods: Based on epidemiologic data, 54 238 women, aged 40 to 69 years, were eligible for breast cancer
screening. The study included women examined from April 1, 2003 to March 31, 2005. The chi-square test and
Bonferroni correction analyses were used to evaluate the frequencies of tumors and the importance of clinical
parameters and tumor characteristics. Significance was set at p < 0.05.
Results: Overall, 17 964 women underwent mammography. This represented 33.1% of eligible women in the area.
A mean of 18.6 and 26.3 women per day were examined in the FU and MU, respectively. Seventy six patients were
diagnosed with breast cancer (41 (54%) in the MU). This represented 4.2 cases of breast cancer per 1000
examinations. The number of cancers detected was significantly higher in women aged 60 to 69 years than in
those aged 50 to 59 years (p < 0.001) or 40 to 49 years (p < 0.001). No difference was observed between women
aged 40 to 49 years and those aged 50 to 59 years (p = 0.164). The proportion of tumors in the early (EC 0 and EC
I) and advanced (CS III and CS IV) stages of development were 43.4% and 15.8%, respectively.
Conclusions: Preliminary results indicate that this mammography screening program is feasible for implementation
in a rural Brazilian territory and favor program continuation
Consumo de fibras alimentares em população adulta
OBJETIVO: As fibras alimentares estão entre os principais fatores da alimentação na prevenção de doenças crônicas. Por isso, objetivou-se estimar o consumo médio diário de fibras alimentares totais, insolúveis e solúveis, nas refeições de uma população de área metropolitana. MÉTODOS: Foi estudada uma amostra da população do Município de Cotia, SP, composta por 559 indivíduos com mais de 20 anos. O consumo alimentar foi obtido pelo método de história alimentar - dieta habitual. Foram identificadas as fontes de fibras nas refeições: desjejum, almoço e jantar. Com base na porção média, os alimentos foram classificados quanto ao conteúdo de fibras como: muito alto (7 g ou mais); alto (4,5 g a 6,9 g); moderado (2,4 g a 4,4 g) e baixo (< 2,4 g). RESULTADOS: O consumo médio diário da população foi de 24 g de fibras totais, sendo as quantidades médias de fibras insolúveis 17 g e, de solúveis, 7 g. O consumo de fibras alimentares entre mulheres e homens foi, respectivamente, 20 g e 29 g (p<0,01). A maioria dos alimentos presentes na dieta continha baixo teor de fibras. O feijão foi o único alimento com alto teor de fibras na dieta habitual e, a principal fonte de fibra na alimentação. O almoço e o jantar foram as refeições que forneceram maior quantidade de fibras. CONCLUSÕES: Constatou-se baixo consumo de fibras alimentares, com diferenças estatisticamente significante entre os sexos. As práticas alimentares revelaram que a dieta é constituída por alimentos pobres em fibras alimentares
Reductions in the Prevalence and Incidence of Geohelminth Infections following a City-wide Sanitation Program in a Brazilian Urban Centre
In the city of Salvador, a large urban centre in Northeast Brazil, a city-wide sanitation intervention started in 1997, aiming to improve the sewerage coverage of households from 26% to 80%. Our aim was to study the impact of the intervention on the prevalence and incidence of geohelminths in the school-aged population. The longitudinal study comprised two cohorts: from the beginning of 1997 to 1998, where data was collected before the intervention, and at the end of 2003 to 2004, after the intervention. Copro-parasitological examinations were carried out on every individual from both cohorts, at the start and nine months later. Demographic, socio-economic, and environmental data were collected using semi-structured questionnaires. The variables utilized to demonstrate the effects of intervention, when utilized together in a multivariate model, accounted for a 100% observed reduction in the prevalence ratio (PR) and incidence ratio (IR). As well as proving that the variables associated with the effect of the program intervention were mediators in this reduction, the reduction in the PR and IR between these periods demonstrated that modifications to the urban environment, particularly those associated with sanitary sewage systems, affected the health of the population, significantly reducing the prevalence of geohelminths
Genetic Ancestry, Race, and Severity of Acutely Decompensated Cirrhosis in Latin America
Background & Aims: Genetic ancestry or racial differences in health outcomes exist in diseases associated with systemic inflammation (eg, COVID-19). This study aimed to investigate the association of genetic ancestry and race with acute-on-chronic liver failure (ACLF), which is characterized by acute systemic inflammation, multi-organ failure, and high risk of short-term death. /
Methods: This prospective cohort study analyzed a comprehensive set of data, including genetic ancestry and race among several others, in 1274 patients with acutely decompensated cirrhosis who were nonelectively admitted to 44 hospitals from 7 Latin American countries. /
Results: Three hundred ninety-five patients (31.0%) had ACLF of any grade at enrollment. Patients with ACLF had a higher median percentage of Native American genetic ancestry and lower median percentage of European ancestry than patients without ACLF (22.6% vs 12.9% and 53.4% vs 59.6%, respectively). The median percentage of African genetic ancestry was low among patients with ACLF and among those without ACLF. In terms of race, a higher percentage of patients with ACLF than patients without ACLF were Native American and a lower percentage of patients with ACLF than patients without ACLF were European American or African American. In multivariable analyses that adjusted for differences in sociodemographic and clinical characteristics, the odds ratio for ACLF at enrollment was 1.08 (95% CI, 1.03–1.13) with Native American genetic ancestry and 2.57 (95% CI, 1.84–3.58) for Native American race vs European American race. /
Conclusions: In a large cohort of Latin American patients with acutely decompensated cirrhosis, increasing percentages of Native American ancestry and Native American race were factors independently associated with ACLF at enrollment
Psychometric properties of the Brazilian version of the Vulnerable Elders Survey-13 (VES-13)
This study aims to assess the psychometric properties of the Brazilian version of the Vulnerable Elders Survey-13 (VES-13). Interviews were carried out with individuals aged 60 years and over receiving care at an ambulatory cancer centre. Test-retest reliability was assessed using the Spearman correlation coefficient, intraclass correlation coefficients and Cohen's kappa coefficient. Construct validity was assessed by testing convergent and discriminant validity using principal component analysis and Varimax rotation. The Spearman correlation coefficient value of the comparison between test and retest scores was 0.98 (p < 0.001). All intraclass correlation coefficient values were higher than 0.60 and kappa coefficients varied between 0.33 and 0.94. Three identified factors explained 72.6% of overall sample variance. VES-13 presented good convergent validity and reasonable discriminant validity. The psychometric properties of the adapted version of the VES-13 are consistent and adequate for use with the Brazilian population
