    Programmable bio-nanochip-based cytologic testing of oral potentially malignant disorders in Fanconi anemia

    Fanconi anemia (FA) is caused by mutations of DNA repair genes. The risk of oral squamous cell carcinoma (OSCC) among FA patients is 800-folds higher than in the general population. Early detection of OSCC, preferably at it precursor stage, is critical in FA patients to improve their survival. In an ongoing clinical trial, we are evaluating the effectiveness of the programmable bio-nanochip (p-BNC)-based oral cytology test in diagnosing oral potentially malignant disorders (OPMD) in non-FA patients. We used this test to compare cytomorphometric and molecular biomarkers in OSCC cell lines derived from FA and non-FA patients to brush biopsy samples of a FA patient with OPMD and normal mucosa of healthy volunteers. Our data showed that expression patterns of molecular biomarkers were not notably different between sporadic and FA-OSCC cell lines. The p-BNC assay revealed significant differences in cytometric parameters and biomarker MCM2 expression between cytobrush samples of the FA patient and cytobrush samples of normal oral mucosa obtained from healthy volunteers. Microscopic examination of the FA patient's OPMD confirmed the presence of dysplasia. Our pilot data suggests that the p-BNC brush biopsy test recognized dysplastic oral epithelial cells in a brush biopsy sample of a FA patient

    For some, the transnationalization of political action and communicative space in the European Union heralds an emergent cosmopolitan order. Need that be so? There are supranational institutions in the EU as well as transnational political and cultural spaces and cross-border communicative flows. However, the Union's member states remain key controllers of citizenship rights and purveyors of collective identities. And for many purposes they still maintain strongly bounded national public spheres. Because the EU's overall character as a polity remains unresolved, this has consequences for the organization of communicative spaces. The EU is a field of tensions and contradictions that is inescapably rooted in institutional realities. Wishful thinking about cosmopolitanism can get in the way of clear analysis

    Proteolytic cleavage events in the maturation of HIV-1 reverse transcriptase

    Each of the HIV-1 pol-encoded enzymes, protease (PR), reverse transcriptase (RT) and integrase (IN) are released during virion maturation and are active only as dimers. Of the three, only RT comprises subunits of different mass. RT in mature infectious virions is a heterodimer of 66 kDa and 51 kDa subunits, even though its gene encodes a 66 kDa protein. The RT p51 subunit is formed by HIV-1 PR-catalyzed cleavage of RT p66, resulting in the removal of a ribonuclease H (RNH) domain. Given the existence of completely active recombinant p66/66 RT homodimers and alternative RT oligomers in other retroviruses, the apparent need for p66/51 RT heterodimers in the HIV-1 virion is unclear. To determine why the generation of active viral RT requires three processing events, we introduced mutations in the p51-RNH and RT-IN protease cleavage sites of an infectious HIV-1 molecular clone. Mutation of the RT-IN cleavage site had no effect on the activity or proteolytic stability of the p98/51 RT product, although infectivity was severely attenuated. This result was similar to findings previously reported for the PR-RT cleavage site. Surprisingly, mutation of the internal p51-RNH cleavage site did not increase RT p66 content, but resulted in attenuated virus containing greatly decreased levels of RT that was primarily RT p51. We further identified a compensatory second-site mutation T477A, found to restore RT activity and processing to p66/51 RT when introduced in the background of p51-RNH cleavage site mutations. These studies demonstrate that cleavage of the internal p51-RNH junction, not the flanking N-terminal or C-terminal junctions is essential for proteolytic stability of functional RT during virion maturation. These findings further emphasize the importance of the RNH domain in regulating proteolytic generation of p66/51 RT. The overall need for the RT heterodimer is attributable to the generation of its subunits. Formation of the 51 kDa subunit or cleavage of the p51-RNH junction is essential for RT stability in the virion, whereas formation of the 66 kDa subunit is important for efficient viral replication

    Spheromak Merging And Field Reversed Configuration Formation At The Swarthmore Spheromak Experiment

    The initial results of coaxial co- and counter-helicity spheromak merging studies at the Swarthmore Spheromak Experiment (SSX) [M. R. Brown, Phys. Plasmas 6, 1717 (1999)] are reported. In its new configuration, SSX is optimized to study field reversed configuration (FRC) formation and stability by counter-helicity spheromak merging. A pair of midplane coils magnetically restricts the merging process to determine how the stability of the resulting magnetic configuration depends upon the quantity of toroidal flux remaining from the initial spheromaks. The diagnostic set at SSX, featuring the capability of measuring up to 600 magnetic field components at 800 ns time resolution, permits detailed studies of the dynamic three-dimensional magnetic structures produced during these experiments. A compact array of magnetic probes examines the local reconnection process, while a distributed array of probes examines global magnetic structure. Counter-helicity merging produces a FRC that persists for several Alfvén times, although the oppositely directed toroidal field of the initial spheromaks does not completely annihilate. The m=1 (toroidal) mode dominates late in the evolution, and is consistent with the tilt instability. Co-helicity merging produces a single elongated spheromak that rapidly tilts

    Industry reports that benefits of PLM are difficult to assess because the same benefit can be expressed as a function of time, cost, quality, or any combination. Based on a review of the PLM literature in an earlier study, a PLM Process Model and an initial list of PLM related metrics was generated and later confirmed through interviews with experienced PLM users. In the current study, the original PLM Process Model was refined and the list of metrics was subjected to an exploratory factor analysis in which specific metrics were found to be related to one of four factors: Inputs, Processes, Outputs, and Outcomes. Based on the results of this study, a Product Lifecycle Implementation Maturity Model was developed that serves as a program-level guide in helping to quantify PLM performance in support of meeting organizational strategic goals

    Probing ISM Structure in Trumpler 14 & Carina I Using The Stratospheric Terahertz Observatory 2

    We present observations of the Trumpler 14/Carina I region carried out using the Stratospheric Terahertz Observatory 2 (STO2). The Trumpler 14/Carina I region is in the west part of the Carina Nebula Complex, which is one of the most extreme star-forming regions in the Milky Way. We observed Trumpler 14/Carina I in the 158 μ\mum transition of [C\,{\sc ii}] with a spatial resolution of 48'' and a velocity resolution of 0.17 km s1^{-1}. The observations cover a 0.25^\circ by 0.28^\circ area with central position {\it l} = 297.34^\circ, {\it b} = -0.60^\circ. The kinematics show that bright [C\,{\sc ii}] structures are spatially and spectrally correlated with the surfaces of CO clouds, tracing the photodissociation region and ionization front of each molecular cloud. Along 7 lines of sight that traverse Tr 14 into the dark ridge to the southwest, we find that the [C\,{\sc ii}] luminosity from the HII region is 3.7 times that from the PDR. In same los we find in the PDRs an average ratio of 1:4.1:5.6 for the mass in atomic gas:dark-CO gas: molecular gas traced by CO. Comparing multiple gas tracers including HI 21cm, [C\,{\sc ii}], CO, and radio recombination lines, we find that the HII regions of the Carina Nebula Complex are well-described as HII regions with one-side freely expanding towards us, consistent with the champagne model of ionized gas evolution. The dispersal of the GMC in this region is dominated by EUV photoevaporation; the dispersal timescale is 20-30 Myr.Comment: ApJ accepte

    Switching to Tenofovir Alafenamide, Coformulated With Elvitegravir, Cobicistat, and Emtricitabine, in HIV-Infected Patients With Renal Impairment: 48-Week Results From a Single-Arm, Multicenter, Open-Label Phase 3 Study

    BACKGROUND: Tenofovir alafenamide (TAF) is a novel tenofovir prodrug with improved renal and bone safety compared with TDF-containing regimens. We report the 48 week safety and efficacy of a once-daily single tablet regimen of elvitegravir 150 mg (E), cobicistat 150 mg (C), emtricitabine 200 mg (F), and TAF 10 mg (E/C/F/TAF) in HIV-1-infected patients with mild to moderate renal impairment. METHODS: We enrolled virologically suppressed HIV-1-infected subjects with estimated creatinine clearance (CrCl) 30-69 mL/min in a single-arm, open-label study to switch regimens to E/C/F/TAF. The primary endpoint was the change from baseline in glomerular filtration rate estimated using various formulae. This study is registered with ClinicalTrials.gov, number NCT01818596. FINDINGS: We enrolled and treated 242 patients with mean age 58 years, 18% Black, 39% hypertension, 14% diabetes. Through week 48, no significant change in estimated CrCl was observed. Two patients (0.8%) discontinued study drug for decreased creatinine clearance, neither had evidence of renal tubulopathy and both had uncontrolled hypertension. Subjects had significant improvements in proteinuria, albuminuria, and tubular proteinuria (P < 0.001 for all). Hip and spine bone mineral density significantly increased from baseline to week 48 (mean percent change +1.47 and +2.29, respectively, P < 0.05). Ninety-two percent (222 patients) maintained HIV-1 RNA <50 copies per milliliter at week 48. INTERPRETATION: Switch to E/C/F/TAF was associated with minimal change in GFR. Proteinuria, albuminuria and bone mineral density significantly improved. These data support the efficacy and safety of once daily E/C/F/TAF in HIV+ patients with mild or moderate renal impairment without dose adjustment
