    Food-specific sublingual immunotherapy is well tolerated and safe in healthy dogs : a blind, randomized, placebo-controlled study

    Background: Food allergies are increasing in prevalence but no treatment strategies are currently available to cure dogs with food allergy. Over the past decade, experimental food allergen-specific sublingual immunotherapy (FA-SLIT) has emerged as a potential treatment for food allergies in human medicine. However, FA-SLIT has not been investigated in dogs. Therefore, the objective of this study was to prospectively evaluate the safety, tolerability and dispenser sterility of FA-SLIT in healthy dogs before testing it in food allergic dogs. Eight experimental healthy beagle dogs, never orally exposed to peanut, were randomized in two groups to receive SLIT with peanut or placebo for 4 months. Subjects were monitored daily for local and systemic adverse effects. Blood samples for complete blood count and serum biochemistry, and urine for urinalysis were collected and the dogs' body weight was recorded at day 0, 35 and 119 of the SLIT treatment. Sera for the determination of peanut-specific IgG and IgE were collected at day 0, 35, 49, 70, 91, 105 and 119. Intradermal tests were performed before (day 0) and after (day 119) the experiment. The content of each dispenser used to administer treatment or placebo was tested for sterility after usage. In order to assess the presence or absence of sensitization, dogs were challenged 6 months after the end of the study with 2000 mu g of peanut extract daily for 7 to 14 days. Results: All dogs completed the study. The treatment did not provoke either local or systemic side-effects. Peanut-specific IgG significantly increased in treatment group. Even though a significant increase in peanut-specific IgE was also seen, intradermal tests were negative in all dogs before and after the experiment, and the challenge test did not trigger any adverse reactions in the treated dogs, which shows the protocol did not cause sensitization to peanut, but nevertheless primed the immune system as indicated by the humoral immune response. All dispenser solutions were sterile. Conclusions: Our results demonstrate that the used peanut-SLIT protocol is well tolerated and safe in healthy dogs. Further studies should evaluate tolerability, safety and efficacy in dogs with food allergy

    Agriculture and Climate Change in Kenya: Climate Chaos, Policy Dilemmas

    This paper analyses emerging policy discussions on climate change and agriculture in Kenya. Kenya has been ahead of many other countries in developing a national climate change strategy, and agriculture is one of the key critical sectors of interest. However, there are concerns about whether policy goals may be achieved amidst the actors’ many and diverging interests. This paper sets out to map how these debates are starting to take place in practice, and poses the following questions: what are the arguments, who is promoting them, and what are the implications for Kenya’s agricultural sector? A better understanding of the key actors, their interests and through what narratives actor-interests are mobilised is important because they will all have implications for the kinds of support farmers at the local level do or do not receive, and the extent to which their own interests are fore grounded or marginalised within the policy process. Ultimately, the policy response to climate change in the agricultural sector is one important factor which mediates local-level vulnerability

    Isolation and propagation of Trypanosoma brucei gambiense from sleeping sickness patients in south Sudan

    This study aimed at isolating Trypanosoma brucei gambiense from human African trypanosomiasis (HAT) patients from south Sudan. Fifty HAT patients identified during active screening surveys were recruited, most of whom (49/50) were in second-stage disease. Blood and cerebrospinal fluid samples collected from the patients were cryopreserved using Triladyl® as the cryomedium. The samples were stored at −150 °C in liquid nitrogen vapour in a dry shipper. Eighteen patient stabilates could be propagated in immunosuppressed Mastomys natalensis and/or SCID mice. Parasitaemia was highest in SCID mice. Further subpassages in M. natalensis increased the virulence of the trypanosomes and all 18 isolates recovered from M. natalensis or SCID mice became infective to other immunosuppressed mouse breeds. A comparison of immunosuppressed M. natalensis and Swiss White, C57/BL and BALB/c mice demonstrated that all rodent breeds were susceptible after the second subpassage and developed a parasitaemia >106/ml by Day 5 post infection. The highest parasitaemias were achieved in C57/BL and BALB/c mice. These results indicate that propagation of T. b. gambiense isolates after initial isolation in immunosuppressed M. natalensis or SCID mice can be done in a range of immunosuppressed rodent

    Early intervention for obsessive compulsive disorder : An expert consensus statement

    © 2019 Elsevier B.V.and ECNP. All rights reserved.Obsessive-compulsive disorder (OCD) is common, emerges early in life and tends to run a chronic, impairing course. Despite the availability of effective treatments, the duration of untreated illness (DUI) is high (up to around 10 years in adults) and is associated with considerable suffering for the individual and their families. This consensus statement represents the views of an international group of expert clinicians, including child and adult psychiatrists, psychologists and neuroscientists, working both in high and low and middle income countries, as well as those with the experience of living with OCD. The statement draws together evidence from epidemiological, clinical, health economic and brain imaging studies documenting the negative impact associated with treatment delay on clinical outcomes, and supporting the importance of early clinical intervention. It draws parallels between OCD and other disorders for which early intervention is recognized as beneficial, such as psychotic disorders and impulsive-compulsive disorders associated with problematic usage of the Internet, for which early intervention may prevent the development of later addictive disorders. It also generates new heuristics for exploring the brain-based mechanisms moderating the ‘toxic’ effect of an extended DUI in OCD. The statement concludes that there is a global unmet need for early intervention services for OC related disorders to reduce the unnecessary suffering and costly disability associated with under-treatment. New clinical staging models for OCD that may be used to facilitate primary, secondary and tertiary prevention within this context are proposed.Peer reviewe

    The Dipole Formalism for Next-to-Leading Order QCD Calculations with Massive Partons

    The dipole subtraction method for calculating next-to-leading order corrections in QCD was originally only formulated for massless partons. In this paper we extend its definition to include massive partons, namely quarks, squarks and gluinos. We pay particular attention to the quasi-collinear region, which gives rise to terms that are enhanced by logarithms of the parton masses, M. By ensuring that our subtraction cross section matches the exact real cross section in all quasi-collinear regions we achieve uniform convergence both for hard scales Q ~= M and Q >> M. Moreover, taking the masses to zero, we exactly reproduce the previously-calculated massless results. We give all the analytical formulae necessary to construct a numerical program to evaluate the next-to-leading order QCD corrections to arbitrary observables in an arbitrary process.Comment: 71 pages, LaTeX using axodra

    Pulse oximetry values of neonates admitted for care and receiving routine oxygen therapy at a resource-limited hospital in Kenya.

    Aim There are 2.7 million neonatal deaths annually, 75% of which occur in sub-Saharan Africa and South Asia. Effective treatment of hypoxaemia through tailored oxygen therapy could reduce neonatal mortality and prevent oxygen toxicity. Methods We undertook a two-part prospective study of neonates admitted to a neonatal unit in Nairobi, Kenya, between January and December 2015. We determined the prevalence of hypoxaemia and explored associations of clinical risk factors and signs of respiratory distress with hypoxaemia and mortality. After staff training on oxygen saturation (SpO2) target ranges, we enrolled a consecutive sample of neonates admitted for oxygen and measured SpO2 at 0, 6, 12, 18 and 24 h post-admission. We estimated the proportion of neonates outside the target range (≥34 weeks: ≥92%; <34 weeks: 89–93%) with 95% confidence intervals (CIs). Results A total of 477 neonates were enrolled. Prevalence of hypoxaemia was 29.2%. Retractions (odds ratio (OR) 2.83, 95% CI 1.47–5.47), nasal flaring (OR 2.68, 95% CI 1.51–4.75), and grunting (OR 2.47, 95% CI 1.27–4.80) were significantly associated with hypoxaemia. Nasal flaring (OR 2.85, 95% CI 1.25–6.54), and hypoxaemia (OR 3.06, 95% CI 1.54–6.07) were significantly associated with mortality; 64% of neonates receiving oxygen were out of range at ≥2 time points and 43% at ≥3 time points. Conclusion There is a high prevalence of hypoxaemia at admission and a strong association between hypoxaemia and mortality in this Kenyan neonatal unit. Many neonates had out of range SpO2 values while receiving oxygen. Further research is needed to test strategies aimed at improving the accuracy of oxygen provision in low-resource settings

    Impact of Optimally Minimizing Delay Times on Safety at Signalized Intersections in Urban Areas, Case Study: The City of Virginia Beach

    Optimally minimizing delay times at signalized intersections can significantly improve both traffic flow and safety. However, most traffic flow optimizing tools do not measure the effect on safety. This study uses nonlinear programming (NLP) algorithms to optimally minimize delay times and employs both Safety performance functions (SPFs) and empirical Bayes (EB) before-after methodology to measure the impact on safety presented as a Crash Modification Factor (CMF). A crash modification factor (CMF) is a multiplicative factor used by transportation practitioners to compute the expected number of crashes at specific study site(s) after a countermeasure has been proposed or is implemented. Using 2013 traffic data from seventeen signalized intersections located in Virginia Beach, the results show that optimally minimizing intersection delay times can result in a safety improvement of approximately 26.46% that is a CMF of 0.735. This result is not conclusive, but the significance of the findings shows the need for further investigations and potential inclusion in the future editions of the Highway Safety Manual (HSM)

    Operational risk modeling for general insurance companies in Kenya

    A Research project submitted in partial fulfillment of the requirements for the degree of Bachelor of Business Science in Actuarial Science at Strathmore UniversityThis study looked at the quantification of operational risk based capital for general insurance companies in Kenya. It is important to note that the regulator requires all insurance companies to compute risk based capital annually. The study pointed out the various operational risk categories and analyzed the operational risk modeling approaches that have been developed in the insurance sector globally. In Kenya, the model used by the regulator to quantify operational risk capital is that recommended by the actuarial profession in the United Kingdom (Solvency II). The main shortcomings of the model used by the regulator were cited as lack of prudence in the estimation of capital requirements and the failure to truly indicate how insurance company operations interact leading to operational losses. The study then illustrated how a proxy-a hybrid modeling approach, could be used to quantify operational risk. The hybrid model was shown to be more prudent than the standardized approach used by the regulator. The methodology involved modeling a general insurance company and creating a hybrid simulations model for operational risk losses. Further, operational risk capital estimates were computed using the model by the regulator and the hybrid simulations model. The operational risk capital estimates were compared and tested for adequacy. The results led to the conclusion that the hybrid model yielded a more prudent operational risk capital estimate than the model used by the regulator. Based on the overall conclusion that the standardized method may not be fully adequate in computing operational risk capital, it is hoped that this study will encourage best practice in computing operational risk capital.     Biosorption of Chromium and Nickel from Industrial Oil Mill Wastewater Using Groundnut Pod Waste Activated Carbon

    Groundnut shell activated carbon was developed and characterized by chemical activation using phosphoric acid (H3PO4) for the uptake of Cr and Ni in a batch biosorption process. The purpose of this study was to reduce the spread of heavy metals in industrial oil mill wastewater. In this study characterization of activated carbon using, surface chemistry (FTI-IR), surface area (BET), surface morphology, and elemental identification (SEM/EDX) were all carried out, and the BET surface area was 689.41 m2/g for groundnut shell activated carbon. This study was also executed to determine the optimum biosorption efficiency parameters for Cr and Ni removal using Response Surface Methodology (RSM) to obtain maximum biosorption efficiency. The factors considered were temperature (25-55oC), adsorbent dosage (0.2-3 g) and contact time (1-2 hrs). Biosorption efficiency was the response. ANOVA analysis was carried out to analyse the most effective factor in experimental design response. The optimum conditions for removal of Cr and Ni were adsorbent dosage 0.40 g, contact time 1.1 hr and temperature 42.02 oC, which shows the maximum biosorption efficiency of 97.1% for Cr removal and 94.8% for Ni removal. Isotherm models analyses showed that the biosorption process was best fitted to Langmuir model and was physical. Results of the kinetic studies and thermodynamic parameters revealed that the biosorption process followed a pseudo-second-order, endothermic, and spontaneous in nature
