    A Multimodality Hybrid Gamma-Optical Camera for Intraoperative Imaging

    The development of low profile gamma-ray detectors has encouraged the production of small field of view (SFOV) hand-held imaging devices for use at the patient bedside and in operating theatres. Early development of these SFOV cameras was focussed on a single modality-gamma ray imaging. Recently, a hybrid system-gamma plus optical imaging-has been developed. This combination of optical and gamma cameras enables high spatial resolution multi-modal imaging, giving a superimposed scintigraphic and optical image. Hybrid imaging offers new possibilities for assisting clinicians and surgeons in localising the site of uptake in procedures such as sentinel node detection. The hybrid camera concept can be extended to a multimodal detector design which can offer stereoscopic images, depth estimation of gamma-emitting sources, and simultaneous gamma and fluorescence imaging. Recent improvements to the hybrid camera have been used to produce dual-modality images in both laboratory simulations and in the clinic. Hybrid imaging of a patient who underwent thyroid scintigraphy is reported. In addition, we present data which shows that the hybrid camera concept can be extended to estimate the position and depth of radionuclide distribution within an object and also report the first combined gamma and Near-Infrared (NIR) fluorescence images.Peer-reviewedPublisher Versio

    Acute Appendicitis among Saudi and Non-Saudi Patients: A Cross-Sectional Based Study

    Objective: We conduct this study to discuss the differences between Saudi and non- Saudi patients with acute appendicitis.Background: Acute appendicitis is one of emergency surgeries in developing and developed countries. Its symptoms are vomiting, lower abdominal pain and decreased appetite. Appendicitis needs urgent surgical prouder to avoid its perforation and associated complications which may lead to death. Method: We conduct cross-sectional based study in one of khamis Mushayt, Saudi Araba. 136 patients diagnosed with acute appendicitis were included and their medical records were reviewed after getting their informed consent.Results: We included 136 patients, 90 were non-Saudi and 46 were Saudi. There were no statistically differences in their diagnosis but the distribution of the diagnosis was different.Conclusion: Acute appendicitis was more prevalent among non-Saudi patients, the diagnosis between both was with no significant differences.     Roles of telomeres and telomerase in cancer, and advances in telomerase-targeted therapies

    Telomeres maintain genomic integrity in normal cells, and their progressive shortening during successive cell divisions induces chromosomal instability. In the large majority of cancer cells, telomere length is maintained by telomerase. Thus, telomere length and telomerase activity are crucial for cancer initiation and the survival of tumors. Several pathways that regulate telomere length have been identified, and genome-scale studies have helped in mapping genes that are involved in telomere length control. Additionally, genomic screening for recurrent human telomerase gene hTERT promoter mutations and mutations in genes involved in the alternative lengthening of telomeres pathway, such as ATRX and DAXX, has elucidated how these genomic changes contribute to the activation of telomere maintenance mechanisms in cancer cells. Attempts have also been made to develop telomere length- and telomerase-based diagnostic tools and anticancer therapeutics. Recent efforts have revealed key aspects of telomerase assembly, intracellular trafficking and recruitment to telomeres for completing DNA synthesis, which may provide novel targets for the development of anticancer agents. Here, we summarize telomere organization and function and its role in oncogenesis. We also highlight genomic mutations that lead to reactivation of telomerase, and mechanisms of telomerase reconstitution and trafficking that shed light on its function in cancer initiation and tumor development. Additionally, recent advances in the clinical development of telomerase inhibitors, as well as potential novel targets, will be summarized

    Anxiety and fear of COVID-19 as potential mechanisms to explain vaccine hesitancy among adults

    BackgroundVaccine hesitancy is a significant global problem resulting from the interaction of multiple factors, including mental health factors. However, the association of COVID-19 vaccine hesitancy with mental health has not been well-examined, especially in Arab culture. This study aims to identify the correlation between anxiety/fear of COVID-19 and vaccine hesitancy among Saudi adults.MethodsAn online-based survey was administered to 558 participants from all regions of Saudi Arabia using the snowball technique. However, this sample may not be representative of the Saudi adult population. Participants responded to the Questionnaire of Vaccine Hesitancy, the COVID-19-Anxiety Questionnaire (C-19-A), and the Fear of COVID-19 Scale (FCV-19S). Data were analyzed on vaccine uptake, vaccine hesitancy, coronavirus infection, and demographic variables. The predictive factors of vaccine hesitancy were examined in one model using multiple regression analysis by the Enter method (P= 0.05).ResultsCOVID-19 anxiety and fear have significant correlations with vaccine hesitancy (Phi=0.33, P=0.017; Phi=0.29, P=0.013, respectively). Anxiety and fear were higher among unhesitating participants (t =2.469, P=0.014; t=2.025, P=0.043, respectively). Participants who had previously been infected with coronavirus were more likely to be hesitant (X2 = 23.126, P=0.000). Participants who scored high in anxiety were more likely to be vaccinated (F=3.979, P=0.019) and have a secondary school or college education (F=4.903 P=0.002). COVID-19 anxiety, gender, and coronavirus infection significantly predicted vaccine hesitancy.ConclusionAnxiety and fear of COVID-19 are among the most important factors correlated with vaccine hesitancy; unhesitant people are more likely to have anxiety and fear. COVID-19 anxiety significantly predicted vaccine hesitancy. We recommend integrating psychological care into vaccination plans to help increase the uptake rate during potential subsequent pandemics. Relevant intervention programs can be designed to help increase vaccine acceptance, deal with vaccine hesitancy, and relieve psychological symptoms during major pandemics. Psychologists can provide awareness messages, counselling seminars, online mentoring, or telemental health outreach

    Rare presentation of drug-induced oral erythema multiforme and its management: a case report

    Aim. This case report discusses a rare variant of drug-induced erythema multiforme, focusing on its clinical features, diagnosis, and management. Background. Drug-induced erythema multiforme (DI-EM) is a hypersensitivity reaction occurring due to exposure to certain medications. DI-EM typically presents as erythematous and painful lesions on the oral mucosa, including the lips, tongue, and buccal mucosa. Case description. A 42-year-old male was referred to the Oral Medicine clinic with a chief complaint of painful lip ulceration and eruptions on the body after consuming medications. A detailed history was taken, and after a thorough examination, a perilesional biopsy for histopathological and immunopathological tests was performed. Following the exclusion of all other conditions with a similar presentation, a diagnosis of drug-induced erythema multiforme (DI-EM) was established. Conclusion. DI-EM can pose a diagnostic challenge due to its presentation. However, a diagnosis can be established by excluding other conditions associated with chronic inflammation or the formation of vesicles or bullae. Clinical Significance. DI-EM is a rare variant of EM with a very low prevalence in the general population, making it a challenging condition to diagnose due to its similarity in presentation with other mucocutaneous disorders. A thorough medical history, along with histopathology and immunology tests, coupled with ruling out other diseases and noting a positive history of recent drug consumption, aids in recognizing this condition, given the absence of any confirmatory diagnostic test. Early diagnosis and cessation of causative drugs, along with supportive medications, contribute to the complete resolution of the condition

    Social and Economic Factors that Influence Health Outcomes in Family Medicine

    Over the last twenty years, the social determinants of health (SDOH) have gained more and more attention in the public health community. SDOH are non-medical elements that may be significantly impacted by social policies and have an impact on health. The increasing variety that exists within our societies makes it absolutely necessary for us to incorporate into undergraduate medical education social determinants of health such as racial factors, financial instability, partner violence, insufficient accessibility to transportation, and inadequate social supports, as well as the crucial role of health education.  Nonetheless, a growing body of research indicates that a variety of health outcomes are fundamentally caused by socioeconomic variables including wealth, income, and education together. In this review we highlight the influence of socio-economic factors on health outcomes, approaches to incorporate social determinants of health in family medicine practice, and we also talk about challenges and solutions in addressing social determinants in family medicine. The objective of this research was to assess the impact of social determinants of health on family medicine practice in which socioeconomic determinants of health might have an effect on health outcomes and healthcare delivery within the context of a family medicine clinical environment

    Knowledge and Awareness of Diabetes Mellitus Disease among High School Students in King Abdulaziz Military City, Tabuk, Saudi Arabia

    BACKGROUND: Saudi Arabia is considered to be one of the highest countries in the Middle East for the incidence of diabetes mellitus (DM). Data are lacking regarding knowledge and awareness about DM among school students in Saudi Arabia. AIM: The study aimed to assess the level of knowledge and awareness of DM among high school students within the military city, Tabuk, Saudi Arabia. METHODS: A descriptive type of cross-sectional study was conducted among 278 high school students applying a convenience sampling technique. The sample size was calculated using OpenEpi, Version 3. Self-administered questionnaires were distributed to the high school students (male and female) after official communication with the school’s dean. The level of knowledge and awareness was categorized into “adequate” and “inadequate” as per each topic/question, and also as per each response/answer. Data entry and analysis were carried out using the Statistical Package for the Social Sciences. Pearson’s Chi-square tests were performed to explore if there is any significant association between the knowledge and awareness level of the high school students and their (i) gender, (ii) age, and (iii) level of education. RESULTS: More than half of the high school students had adequate level of knowledge and awareness about DM in terms of symptoms (67.3%), complications (56.5%), monitoring method (62.6%), lifestyle modifications (63.7%), frequency of routine eye check-up (63.3%), important factors for blood sugar control (79.1%), treatment (56.5%), and management of hypoglycemia symptoms (57.6%). On the contrary, a large number of the students showed inadequate level of knowledge and awareness about the disease in terms of definition (80.6%), major causes (57.9%), effect of high blood pressure (51.8%), frequency of routine blood pressure check-up (55%), rationale of a regular urine test (58.3%), medication for DM (66.9%), and duration of medication (69.8%). However, no significant associations were found between the knowledge and awareness level of the high school students about the definition or major causes of DM and the (i) gender, (ii) age group, and (iii) level of education of the students. CONCLUSION: The level of knowledge and awareness of a considerable number of high school students regarding DM was inadequate, and some of them possessed various misconceptions about this particular chronic disease. Health authorities and school authorities in the region should offer special efforts to improve the level of knowledge and awareness of the students through regular health education campaigns

    A machine learning heuristic to identify biologically relevant and minimal biomarker panels from omics data

    Background: Investigations into novel biomarkers using omics techniques generate large amounts of data. Due to their size and numbers of attributes, these data are suitable for analysis with machine learning methods. A key component of typical machine learning pipelines for omics data is feature selection, which is used to reduce the raw high-dimensional data into a tractable number of features. Feature selection needs to balance the objective of using as few features as possible, while maintaining high predictive power. This balance is crucial when the goal of data analysis is the identification of highly accurate but small panels of biomarkers with potential clinical utility. In this paper we propose a heuristic for the selection of very small feature subsets, via an iterative feature elimination process that is guided by rule-based machine learning, called RGIFE (Rule-guided Iterative Feature Elimination). We use this heuristic to identify putative biomarkers of osteoarthritis (OA), articular cartilage degradation and synovial inflammation, using both proteomic and transcriptomic datasets.Results and discussion: Our RGIFE heuristic increased the classification accuracies achieved for all datasets when no feature selection is used, and performed well in a comparison with other feature selection methods. Using this method the datasets were reduced to a smaller number of genes or proteins, including those known to be relevant to OA, cartilage degradation and joint inflammation. The results have shown the RGIFE feature reduction method to be suitable for analysing both proteomic and transcriptomics data. Methods that generate large ‘omics’ datasets are increasingly being used in the area of rheumatology.Conclusions: Feature reduction methods are advantageous for the analysis of omics data in the field of rheumatology, as the applications of such techniques are likely to result in improvements in diagnosis, treatment and drug discovery

To meet global challenges, high impact research inevitably requires interdisciplinary collaboration. This is recognised by all major research funders. Therefore engaging with the work of others and forging collaborations across subject areas is an essential skill for the next generation of researchers
