Typical and atypical pulmonary carcinoids: our institutional experience
Pulmonary carcinoids are rare malignant neoplasms, accounting for 2-5% of all lung tumors, with an approximate annual incidence of 2.3-2.8 cases per million of the population. We relate our experience of 54 patients (21 male, 33 female, mean age 53+/-15 years) treated between July 1986 and April 2006. All the patients underwent preoperative fibrobronchoscopy: preoperative diagnosis was made in 28 patients (52%). Surgical treatment consisted of: 31 standard lobectomies, 6 pneumonectomies, 5 bilobectomies, 2 sleeve lobectomies, 2 anatomic segmentectomies, 6 wedge resections; two patients were managed with sleeve bronchial procedure of the left main bronchus without lung resection. Fifty-four patients were followed with a mean time of observation of 67 months: 6 (11%) deaths occurred, at a mean period of 49 months after surgery; there were no postoperative deaths. Overall, 5-year survival was 91%, 10 years 83%: 5-year survival was 91% for typical carcoinoids (TC) vs. 88% for atypical (AC), 10 years 91% for TC and 44% for AC (significant value, P=0.0487). Carcinoid tumors are a distinct group of neuroendocrine tumors with a good prognosis in most cases. Surgery currently represents the best treatment with good results at mid- and long-term survival, according to an acceptable risk

Prevalence of H63D, S65C and C282Y hereditary hemochromatosis gene mutations in Slovenian population by an improved high-throughput genotyping assay
<p>Abstract</p> <p>Background</p> <p>Hereditary hemochromatosis (HH) is a common genetic disease characterized by excessive iron overload that leads to multi-organ failure. Although the most prevalent genotype in HH is homozygosity for C282Y mutation of the <it>HFE </it>gene, two additional mutations, H63D and S65C, appear to be associated with a milder form of HH. The aim of this study was to develop a high-throughput assay for <it>HFE </it>mutations screening based on TaqMan technology and to determine the frequencies of <it>HFE </it>mutations in the Slovenian population.</p> <p>Methods</p> <p>Altogether, 1282 randomly selected blood donors from different Slovenian regions and 21 HH patients were analyzed for the presence of <it>HFE </it>mutations by an in-house developed real-time PCR assay based on TaqMan technology using shorter non-interfering fluorescent single nucleotide polymorphism (SNP)-specific MGB probes. The assay was validated by RFLP analysis and DNA sequencing.</p> <p>Results</p> <p>The genotyping assay of the H63D, S65C and C282Y mutations in the <it>HFE </it>gene, based on TaqMan technology proved to be fast, reliable, with a high-throughput capability and 100% concordant with genotypes obtained by RFLP and DNA sequencing. The observed frequency of C282Y homozygotes in the group of HH patients was only 48%, others were of the heterogeneous <it>HFE </it>genotype. Among 1282 blood donors tested, the observed H63D, S65C and C282Y allele frequency were 12.8% (95% confidence interval (CI) 11.5 – 14.2%), 1.8% (95% CI 1.4 – 2.5%) and 3.6% (95% CI 3.0 – 4.5%), respectively. Approximately 33% of the tested subjects had at least one of the three HH mutations, and 1% of them were C282Y homozygotes or compound heterozygotes C282Y/H63D or C282Y/S65C, presenting an increased risk for iron overload disease. A significant variation in H63D allele frequency was observed for one of the Slovenian regions.</p> <p>Conclusion</p> <p>The improved real-time PCR assay for H63D, S65C and C282Y mutations detection is accurate, fast, cost-efficient and ready for routine screening and diagnostic procedures. The genotype frequencies in the Slovenian population agree with those reported for the Central European populations although some deviations where observed in comparison with other populations of Slavic origin. Regional distribution of the mutations should be considered when planning population screening.</p
LPS levels in root canals after the use of ozone gas and high frequency electrical pulses
Abstract The present study aims to verify the effect of ozone gas (OZY® System) and high frequency electric pulse (Endox® System) systems on human root canals previously contaminated with Escherichia colilipopolysaccharide (LPS). Fifty single-rooted teeth had their dental crowns removed and root lengths standardized to 16 mm. The root canals were prepared up to #60 hand K-files and sterilized using gamma radiation with cobalt 60. The specimens were divided into the following five groups (n = 10) based on the disinfection protocol used: OZY® System, one 120-second-pulse (OZY 1p); OZY® System, four 24-second-pulses (OZY 4p); and Endox® System (ENDOX). Contaminated and non-contaminated canals were exposed only to apyrogenic water and used as positive (C+) and negative (C-) controls, respectively. LPS (O55:B55) was administered in all root canals except those belonging to group C-. After performing disinfection, LPS samples were collected from the canals using apyrogenic paper tips. Limulus Amoebocyte Lysate (LAL) was used to quantify the LPS levels, and the data obtained was analyzed using one-way ANOVA. The disinfection protocols used were unable to reduce the LPS levels significantly (p = 0.019). The use of ozone gas and high frequency electric pulses was not effective in eliminating LPS from the root canals
A fast radio burst localized at detection to an edge-on galaxy using very-long-baseline interferometry
Fast radio bursts (FRBs) are millisecond-duration radio transients whose origins remain unknown. As the vast majority of bursts are one-off events, it is necessary to pinpoint FRBs precisely within their host galaxies at the time of detection. Here we use two purpose-built outrigger telescopes to localize FRB 20210603A at the time of its detection by the Canadian Hydrogen Intensity Mapping Experiment (CHIME). Our very-long-baseline interferometry stations localized the burst to a 0.2" × 2" final ellipse in the disk of its host galaxy SDSS J004105.82+211331.9. A spatially resolved spectroscopic follow-up revealed recent star formation (Hα emission) on kiloparsec scales near the burst position. The excess dispersion measure is consistent with expectations from the nearly edge-on disk of the host galaxy, demonstrating the utility of FRBs as probes of the interstellar medium in distant galaxies. The excess dispersion measure, rotation measure and scattering are consistent with expectations for a pulse travelling from deep within its host galactic plane, strengthening the link between the local environment of FRB 20210603A and the disk of its host galaxy. Finally, this technique demonstrates a way to overcome the trade-off between angular resolution and field of view in FRB instrumentation, paving the way towards plentiful and precise FRB localizations
CHIME/FRB Detection of Eight New Repeating Fast Radio Burst Sources
We report on the discovery of eight repeating fast radio burst (FRB) sources
found using the Canadian Hydrogen Intensity Mapping Experiment (CHIME)
telescope. These sources span a dispersion measure (DM) range of 103.5 to 1281
pc cm. They display varying degrees of activity: six sources were
detected twice, another three times, and one ten times. These eight repeating
FRBs likely represent the bright and/or high-rate end of a distribution of
infrequently repeating sources. For all sources, we determine sky coordinates
with uncertainties of 10. FRB 180916.J0158+65 has a
burst-averaged DM = pc cm and a low DM excess over the
modelled Galactic maximum (as low as 20 pc cm); this source also
has a Faraday rotation measure (RM) of rad m, much
lower than the RM measured for FRB 121102. FRB 181030.J1054+73 has the lowest
DM for a repeater, pc cm, with a DM excess of 70
pc cm. Both sources are interesting targets for multi-wavelength
follow-up due to their apparent proximity. The DM distribution of our repeater
sample is statistically indistinguishable from that of the first 12 CHIME/FRB
sources that have not repeated. We find, with 4 significance, that
repeater bursts are generally wider than those of CHIME/FRB bursts that have
not repeated, suggesting different emission mechanisms. Our repeater events
show complex morphologies that are reminiscent of the first two discovered
repeating FRBs. The repetitive behavior of these sources will enable
