International Differences in Longevity and Health and Their Economic Consequences
In 1975, 50 year-old Americans could expect to live slightly longer than their European counterparts. By 2005, American life expectancy at that age has diverged substantially compared to Europe. We find that this growing longevity gap is primarily the symptom of real declines in the health of near-elderly Americans, relative to their European peers. In particular, we use a microsimulation approach to project what US longevity would look like, if US health trends approximated those in Europe. We find that differences in health can explain most of the growing gap in remaining life expectancy. In addition, we quantify the public finance consequences of this deterioration in health. The model predicts that gradually moving American cohorts to the health status enjoyed by Europeans could save up to $1.1 trillion in discounted total health expenditures from 2004 to 2050.disability, mortality, international comparisons, microsimulation
Genomic Study of RNA Polymerase II and III SNAP(c)-Bound Promoters Reveals a Gene Transcribed by Both Enzymes and a Broad Use of Common Activators.
SNAP(c) is one of a few basal transcription factors used by both RNA polymerase (pol) II and pol III. To define the set of active SNAP(c)-dependent promoters in human cells, we have localized genome-wide four SNAP(c) subunits, GTF2B (TFIIB), BRF2, pol II, and pol III. Among some seventy loci occupied by SNAP(c) and other factors, including pol II snRNA genes, pol III genes with type 3 promoters, and a few un-annotated loci, most are primarily occupied by either pol II and GTF2B, or pol III and BRF2. A notable exception is the RPPH1 gene, which is occupied by significant amounts of both polymerases. We show that the large majority of SNAP(c)-dependent promoters recruit POU2F1 and/or ZNF143 on their enhancer region, and a subset also recruits GABP, a factor newly implicated in SNAP(c)-dependent transcription. These activators associate with pol II and III promoters in G1 slightly before the polymerase, and ZNF143 is required for efficient transcription initiation complex assembly. The results characterize a set of genes with unique properties and establish that polymerase specificity is not absolute in vivo
Understanding the Economic Consequences of Shifting Trends in Population Health
The public economic burden of shifting trends in population health remains uncertain. Sustained increases in obesity, diabetes, and other diseases could reduce life expectancy − with a concomitant decrease in the public-sector's annuity burden − but these savings may be offset by worsening functional status, which increases health care spending, reduces labor supply, and increases public assistance. Using a microsimulation approach, we quantify the competing public-finance consequences of shifting trends in population health for medical care costs, labor supply, earnings, wealth, tax revenues, and government expenditures (including Social Security and income assistance). Together, the reduction in smoking and the rise in obesity have increased net public-sector liabilities by $430bn, or approximately 4% of the current debt burden. Sudden cardiac death among general population and sport related population in forensic experience.
PURPOSE: The goal of the study was to assess the causes and analyze the cases of sudden cardiac death (SCD) victims referred to the department of forensic medicine in Lausanne, with a particular focus on sports-related fatalities including also leisure sporting activities. To date, no such published assessment has been done nor for Switzerland nor for the central Europe.
METHODS: This is a retrospective study based on autopsy records of SCD victims, from 10 to 50 years of age, performed at the University Centre of Legal Medicine in Lausanne from 1995 to 2010. The study population was divided into two groups: sport-related (SR) and not sport-related (NSR) SCDs.
RESULTS: During the study period, 188 cases of SCD were recorded: 166 (88%) were NSR and 22 (12%) SR. The mean age of the 188 victims was 37.3 ± 10.1 years, with the majority of the cases being male (79%). A cause of death was established in 84%, and the pathology responsible for death varied according to the age of the victims. In the NSR group, the mean age was 38.2 ± 9.2 years and there was 82% of male. Coronary artery disease (CAD) was the main diagnosis in the victims aged 30-50 years. The majority of morphologically normal hearts were observed in the 15-29 year age range. There was no case in the 10-14 year age range. In the SR group, 91% of victims died during leisure sporting activities. In this group the mean age was 30.5 ± 13.5 years, with the majority being male (82%). The main cause of death was CAD, with 6 cases (27%) and a mean age of 40.8 ± 5.5 years. The youngest victim with CAD was 33 years old. A morphologically normal heart was observed in 5 cases (23%), with a mean age of 24.4 ± 14.9 years. The most frequently implicated sporting activities were hiking (26%) and swimming (17%).
CONCLUSION: In this study, CAD was the most common cause of death in both groups. Although this pathology most often affects adults over 35 years of age, there were also some victims under 35 years of age in both groups. SCDs during sport are mostly related to leisure sporting activities, for which preventive measures are not yet usually established. This study highlights also the need to inform both athletes and non athletes of the cardiovascular risks during sport activities and the role of a forensic autopsy and registries involving forensic pathologists for SR SCD
Frequency and effects of meeting health behaviour guidelines among adolescents
Background: To assess the relationship between overweight status and the concomitant adherence to physical activity, daily screen time and nutritional guidelines. Methods: Data were derived from the Swiss Health Behaviour in School-aged Children Survey 2006. Participants (n = 8130, 48.7% girls) were divided into two groups: normal weight (n = 7215, 44.8% girls) and overweight (n = 915, 34.8% girls), using self-reported height and weight. Groups were compared on adherence to physical activity, screen time and nutritional guidelines. Bivariate analyses were carried out followed by multivariate analyses using normal-weight individuals as the reference category. Results: Regardless of gender, overweight individuals reported more screen time, less physical activity and less concomitant adherence to guidelines. For boys, the multivariate analysis showed that any amount exceeding screen time recommendations was associated with increased odds of being overweight [>2-4 h: adjusted odds ratio (AOR) = 1.40; >4-6 h: AOR = 1.48; >6 h: AOR = 1.83]. A similar relation was found for any amount below physical activity recommendations (4-6 times a week: AOR = 1.67; 2-3 times a week: AOR = 1.87; once a week or less: AOR = 2.1). For girls, not meeting nutritional guidelines was less likely among overweight individuals (0-2 recommendations: AOR = 0.54). Regardless of weight status, more than half of the adolescents did not comply with any guideline and <2% met all three at the same time. Conclusions: Meeting current nutritional, physical activity and screen time guidelines should be encouraged with respect to overweight. However, as extremely low rates of concomitant adherence were found regardless of weight status, their achievability is questionable (especially for nutrition), which warrants further research to better adapt them to adolescent
Self-consistent theory of turbulent transport in the solar tachocline. III. Gravity waves
To understand the fundamental physical processes important for the evolution
of solar rotation and distribution of chemical species, we provide theoretical
predictions for particle mixing and momentum transport in the stably stratified
tachocline. By envisioning that turbulence is driven externally in the
tachocline (e.g. by plume penetration), we compute the amplitude of turbulent
flow, turbulent particle diffusivities, and eddy viscosity, by incorporating
the effect of a strong radial differential rotation and stable stratification.
We identify the different roles that the shear flow and stable stratification
play in turbulence regulation and transport. Particle transport is found to be
severely quenched due to stable stratification as well as radial differential
rotation, especially in the radial direction with an effectively more efficient
horizontal transport. The eddy viscosity is shown to become negative for
parameter values typical of the tachocline, suggesting that turbulence in the
stably stratified tachocline leads to a non-uniform radial differential

Coping with citizen demands: a field study of suckling processes in dairy herds
In dairy farms calves and cows are usually separated shortly after birth but this practice raises the question of animal welfare. This social concern may lead to bring back dairy calves to their mother. Even though suckling is quite seldom in dairy production, some farmers are using such a practice for many years. The present study aims to better know farmer’s motivations, practical implementation and farmer’s perception of the impact of suckling on performance and animal behaviour. In January 2018 a semi-quantitative survey was performed on 44 farms where calves suckle at least 24 hours their mother or a nurse cow. The farming systems differed on the size (20 to 140 cows), the type (19 conventional, 25 organic farms), the suckling (28 by the mothers, 16 by nurses), and the breed. Results indicate that main farmers’ motivations are calves’ health (52%), better working conditions (41%) and saving time (34%), with little consideration on animal welfare (7%). Numerous practices were found in terms of allowance of daily cow-calf contact and suckling duration with a difference between male and female calves. According to farmers, these practices were efficient to improved calves’ health (70%), save time (75%) and improved working conditions (52%). Weaning was considered as a stressful situation, especially after long suckling periods. Cow mooing usually stopped 2.5 d after weaning. At least 34 farmers are fully satisfied with this practice. The main challenge appears to be the management of the wildness of future heifers, by investing time to manipulate calves before weaning. The results of this survey could help to propose solutions to farmers who want to cope with the societal demand of keeping calves with dams in dairy herds

driven by a fractional Brownian motion with Hurst index 1/2<H<1, which is
analogous to the linear self-attracting diffusion. For 1-dimensional process we
study its convergence and the corresponding weighted local time. For
2-dimensional process, as a related problem, we show that the renormalized

Precise Modeling of the Exoplanet Host Star and CoRoT Main Target HD 52265
This paper presents a detailed and precise study of the characteristics of
the Exoplanet Host Star and CoRoT main target HD 52265, as derived from
asteroseismic studies. The results are compared with previous estimates, with a
comprehensive summary and discussion. The basic method is similar to that
previously used by the Toulouse group for solar-type stars. Models are computed
with various initial chemical compositions and the computed p-mode frequencies
are compared with the observed ones. All models include atomic diffusion and
the importance of radiative accelerations is discussed. Several tests are used,
including the usual frequency combinations and the fits of the \'echelle
diagrams. The possible surface effects are introduced and discussed. Automatic
codes are also used to find the best model for this star (SEEK, AMP) and their
results are compared with that obtained with the detailed method. We find
precise results for the mass, radius and age of this star, as well as its
effective temperature and luminosity. We also give an estimate of the initial
helium abundance. These results are important for the characterization of the
