Personal protective equipment solution for UK military medical personnel working in an Ebola virus disease treatment unit in Sierra Leone.
The combination of personal protective equipment (PPE) together with donning and doffing protocols was designed to protect British and Canadian military medical personnel in the Kerry Town Ebola Treatment Unit (ETU) in Sierra Leone. The PPE solution was selected to protect medical staff from infectious risks, notably Ebola virus, and chemical (hypochlorite) exposure. PPE maximized dexterity, enabled personnel to work in hot temperatures for periods of up to 2h, protected mucosal membranes when doffing outer layers, and minimized potential contamination of the doffing area with infectious material by reducing the requirement to spray PPE with hypochlorite. The ETU was equipped to allow medical personnel to provide a higher level of care than witnessed in many existing ETUs. This assured personnel working as part of the international response that they would receive as close to Western treatment standards as possible if they were to contract Ebola virus disease (EVD). PPE also enabled clinical interventions that are not seen routinely in West African EVD treatment regimens, whilst providing a robust protective barrier. Competency in using PPE was developed during a nine-day pre-deployment training programme. This allowed over 60 clinical personnel per deployment to practice skills in PPE in a simulated ETU and in classrooms. Overall, the training provided: (i) an evidence base underpinning the PPE solution chosen; (ii) skills in donning and doffing of PPE; (iii) personnel confidence in the selected PPE; and (iv) quantifiable testing of each individual's capability to don PPE, perform tasks and doff PPE safely
Balancing training and outcomes in total knee replacement:A ten-year review
Introduction: 10-year study examining differences in total knee arthroplasty (TKA) functional outcomes and survivorship in patients operated on by consultant and trainee orthopaedic surgeons.Method: Data was prospectively collected from all elective TKAs performed at our three linked institutions. Patient demographics, surgeon grade, and length of hospital stay were recorded. Outcomes pre-operatively and at 1, 3, 5, 7 and 10 years included mortality, need for revision surgery and function as documented by the patients' Knee Society Score.Results: 686 patients were included in the study. 450 (65.5%) patients were operated by consultant surgeons and 236 (34.4%) by trainees. On multivariate analysis no significant differences were observed between groups in length of hospital stay (p = 0.695), implant survival (p = 0.422), and function (p = 0.507) at 10 years. On Cox regression analysis no significant difference was observed in mortality (p = 0.209) at 10 years. 4 patients over this time period were lost to formal follow up.Conclusion: No significant difference was observed in the TKA outcomes between consultants and trainees 10 years post-operatively.</p
Dalitz Plot Analysis of the Decay D^+ --> K^- pi^+ pi^+ and Indication of a Low-Mass Scalar K pi Resonance
We study the Dalitz plot of the decay D^+ --> K^- pi^+ pi^+ with a sample of
15090 events from Fermilab experiment E791. Modeling the decay amplitude as the
coherent sum of known K pi resonances and a uniform nonresonant term, we do not
obtain an acceptable fit. If we allow the mass and width of the K^*_0(1430) to
float, we obtain values consistent with those from PDG but the chi^2 per degree
of freedom of the fit is still unsatisfactory. A good fit is found when we
allow for the presence of an additional scalar resonance, with mass 797 +/- 19
+/- 43 MeV/c^2 and width 410 +/- 43 +/- 87 MeV/c^2. The mass and width of the
K^*_0(1430) become 1459 +/- 7 +/- 5 MeV/c^2 and 175 +/- 12 +/- 12 MeV/c^2,
respectively. Our results provide new information on the scalar sector in

Experimental evidence for a light and broad scalar resonance in decay

Asymmetries between the production of D+ and D- mesons from 500 GeV/c pi- nucleon interactions as a function of xF and pt**2
We present asymmetries between the production of D+ and D- mesons in Fermilab
experiment E791 as a function of xF and pt**2. The data used here consist of
74,000 fully-reconstructed charmed mesons produced by a 500 GeV/c pi- beam on C
and Pt foils. The measurements are compared to results of models which predict
differences between the production of heavy-quark mesons that have a light
quark in common with the beam (leading particles) and those that do not
(non-leading particles). While the default models do not agree with our data,
we can reach agreement with one of them, PYTHIA, by making a limited number of
changes to parameters used
