    Modelling applications of photonic bandgap fibres

    Photonic crystal fibres (PCFs)[1] are one of the most exciting developments in the field of photonics that has emerged in recent years. Not only have they already led to cheap all-fibre high brightness white light sources and have sparked a renaissance in the field of nonlinear optics but they also have the potential to dramatically change the next generation of telecommunication systems. PCFs can be split into two categories, the first have a solid core and guide light by modified total internal reflection, while the second photonic bandgap fibres (PBF) guide light by photonic bandgap effects and typically have a low index core compared to the cladding. Also of interest are "arrow" fibres which have a solid core and guide light due to the arrangement of high index defects in the cladding. In this paper we will be concentrating on designing and manipulating the properties of PBFs. etc..

    HST ultraviolet spectral energy distributions for three ultraluminous infrared galaxies

    We present HST Faint Object Camera ultraviolet (230 nm and 140 nm) images of three ultraluminous infrared galaxies (ULIG: L_ir > 10^12 L_sun) selected from the IRAS Revised Bright Galaxy Sample. The purpose is to estimate spectral energy distributions (SEDs) to facilitate the identification of similar objects at high redshift in deep optical, infrared, and submm surveys. All three galaxies (VII Zw031 = IRAS F12112+0305, and IRAS F22491-1808) were well detected at 230 nm. Two of the three were marginally detected at 140 nm. The fluxes, together with ground-based optical and infrared photometry, are used to compute SEDs over a wide wavelength range. The measured SEDs drop from the optical to the ultraviolet, but the magnitude of the drop ranges from a factor of ~3 in IRAS F22491-1808 to a factor of ~100 in VIIZw031. This is most likely due to different internal extinctions. Such an interpretation is also suggested by extrapolating to ultraviolet wavelengths the optical internal extinction measured in VIIZw031. K-corrections are calculated to determine the colors of the sample galaxies as seen at high redshifts. Galaxies like VIIZw031 have very low observed rest-frame UV fluxes which means that such galaxies at high redshift will be extremely red or even missing in optical surveys. On the other hand, galaxies like IRAS F12112+0305 and IRAS F22491-1808, if seen at high redshift, would be sufficiently blue that they would not easily be distinguished from normal field galaxies, and therefore, identified as ULIGs.     Construction of the descriptive system for the assessment of quality of life AQoL-6D utility instrument

    BackgroundMulti attribute utility (MAU) instruments are used to include the health related quality of life (HRQoL) in economic evaluations of health programs. Comparative studies suggest different MAU instruments measure related but different constructs. The objective of this paper is to describe the methods employed to achieve content validity in the descriptive system of the Assessment of Quality of Life (AQoL)-6D, MAU instrument.MethodsThe AQoL program introduced the use of psychometric methods in the construction of health related MAU instruments. To develop the AQoL-6D we selected 112 items from previous research, focus groups and expert judgment and administered them to 316 members of the public and 302 hospital patients. The search for content validity across a broad spectrum of health states required both formative and reflective modelling. We employed Exploratory Factor Analysis and Structural Equation Modelling (SEM) to meet these dual requirements.Results and DiscussionThe resulting instrument employs 20 items in a multi-tier descriptive system. Latent dimension variables achieve sensitive descriptions of 6 dimensions which, in turn, combine to form a single latent QoL variable. Diagnostic statistics from the SEM analysis are exceptionally good and confirm the hypothesised structure of the model.ConclusionsThe AQoL-6D descriptive system has good psychometric properties. They imply that the instrument has achieved construct validity and provides a sensitive description of HRQoL. This means that it may be used with confidence for measuring health related quality of life and that it is a suitable basis for modelling utilities for inclusion in the economic evaluation of health programs.<br /

    Covalent attachment of fibronectin onto emulsion‐templated porous polymer scaffolds enhances human endometrial stromal cell adhesion, infiltration, and function

    A novel strategy for the surface functionalization of emulsion‐templated highly porous (polyHIPE) materials as well as its application to in vitro 3D cell culture is presented. A heterobifunctional linker that consists of an amine‐reactive N‐hydroxysuccinimide ester and a photoactivatable nitrophenyl azide, N‐sulfosuccinimidyl‐6‐(4′‐azido‐2′‐nitrophenylamino)hexanoate (sulfo‐SANPAH), is utilized to functionalize polyHIPE surfaces. The ability to conjugate a range of compounds (6‐aminofluorescein, heptafluorobutylamine, poly(ethylene glycol) bis‐amine, and fibronectin) to the polyHIPE surface is demonstrated using fluorescence imaging, FTIR spectroscopy, and X‐ray photoelectron spectroscopy. Compared to other existing surface functionalization methods for polyHIPE materials, this approach is facile, efficient, versatile, and benign. It can also be used to attach biomolecules to polyHIPE surfaces including cell adhesion‐promoting extracellular matrix proteins. Cell culture experiments demonstrated that the fibronectin‐conjugated polyHIPE scaffolds improve the adhesion and function of primary human endometrial stromal cells. It is believed that this approach can be employed to produce the next generation of polyHIPE scaffolds with tailored surface functionality, enhancing their application in 3D cell culture and tissue engineering whilst broadening the scope of applications to a wider range of cell types

    A Vast Thin Plane of Co-rotating Dwarf Galaxies Orbiting the Andromeda Galaxy

    Dwarf satellite galaxies are thought to be the remnants of the population of primordial structures that coalesced to form giant galaxies like the Milky Way. An early analysis noted that dwarf galaxies may not be isotropically distributed around our Galaxy, as several are correlated with streams of HI emission, and possibly form co-planar groups. These suspicions are supported by recent analyses, and it has been claimed that the apparently planar distribution of satellites is not predicted within standard cosmology, and cannot simply represent a memory of past coherent accretion. However, other studies dispute this conclusion. Here we report the existence (99.998% significance) of a planar sub-group of satellites in the Andromeda galaxy, comprising approximately 50% of the population. The structure is vast: at least 400 kpc in diameter, but also extremely thin, with a perpendicular scatter <14.1 kpc (99% confidence). Radial velocity measurements reveal that the satellites in this structure have the same sense of rotation about their host. This finding shows conclusively that substantial numbers of dwarf satellite galaxies share the same dynamical orbital properties and direction of angular momentum, a new insight for our understanding of the origin of these most dark matter dominated of galaxies. Intriguingly, the plane we identify is approximately aligned with the pole of the Milky Way's disk and is co-planar with the Milky Way to Andromeda position vector.     A cluster randomized controlled trial of the effectiveness and cost-effectiveness of Intermediate Care Clinics for Diabetes (ICCD) : study protocol for a randomized controlled trial

    Background World-wide healthcare systems are faced with an epidemic of type 2 diabetes. In the United Kingdom, clinical care is primarily provided by general practitioners (GPs) rather than hospital specialists. Intermediate care clinics for diabetes (ICCD) potentially provide a model for supporting GPs in their care of people with poorly controlled type 2 diabetes and in their management of cardiovascular risk factors. This study aims to (1) compare patients with type 2 diabetes registered with practices that have access to an ICCD service with those that have access only to usual hospital care; (2) assess the cost-effectiveness of the intervention; and (3) explore the views and experiences of patients, health professionals and other stakeholders. Methods/Design This two-arm cluster randomized controlled trial (with integral economic evaluation and qualitative study) is set in general practices in three UK Primary Care Trusts. Practices are randomized to one of two groups with patients referred to either an ICCD (intervention) or to hospital care (control). Intervention group: GP practices in the intervention arm have the opportunity to refer patients to an ICCD - a multidisciplinary team led by a specialist nurse and a diabetologist. Patients are reviewed and managed in the ICCD for a short period with a goal of improving diabetes and cardiovascular risk factor control and are then referred back to practice. or Control group: Standard GP care, with referral to secondary care as required, but no access to ICCD. Participants are adults aged 18 years or older who have type 2 diabetes that is difficult for their GPs to control. The primary outcome is the proportion of participants reaching three risk factor targets: HbA1c (≤7.0%); blood pressure (<140/80); and cholesterol (<4 mmol/l), at the end of the 18-month intervention period. The main secondary outcomes are the proportion of participants reaching individual risk factor targets and the overall 10-year risks for coronary heart disease(CHD) and stroke assessed by the United Kingdom Prospective Diabetes Study (UKPDS) risk engine. Other secondary outcomes include body mass index and waist circumference, use of medication, reported smoking, emotional adjustment, patient satisfaction and views on continuity, costs and health related quality of life.     Chronic non-specific low back pain - sub-groups or a single mechanism?

Clinical trials evaluating the efficacy of a variety of interventions for chronic non-specific low back pain indicate limited effectiveness for most commonly applied interventions and approaches. Discussion: Many clinicians challenge the results of clinical trials as they feel that this lack of effectiveness is at odds with their clinical experience of managing patients with back pain. A common explanation for this discrepancy is the perceived heterogeneity of patients with chronic non-specific low back pain. It is felt that the effects of treatment may be diluted by the application of a single intervention to a complex, heterogeneous group with diverse treatment needs. This argument presupposes that current treatment is effective when applied to the correct patient. An alternative perspective is that the clinical trials are correct and current treatments have limited efficacy. Preoccupation with sub-grouping may stifle engagement with this view and it is important that the sub-grouping paradigm is closely examined. This paper argues that there are numerous problems with the sub-grouping approach and that it may not be an important reason for the disappointing results of clinical trials. We propose instead that current treatment may be ineffective because it has been misdirected. Recent evidence that demonstrates changes within the brain in chronic low back pain sufferers raises the possibility that persistent back pain may be a problem of cortical reorganisation and degeneration. This perspective offers interesting insights into the chronic low back pain experience and suggests alternative models of intervention. Summary: The disappointing results of clinical research are commonly explained by the failure of researchers to adequately attend to sub-grouping of the chronic non-specific low back pain population. Alternatively, current approaches may be ineffective and clinicians and researchers may need to radically rethink the nature of the problem and how it should best be managed

    Surgical Outcomes Following Patella Fracture Repair: A Single-Center Retrospective Cohort Study

    Introduction Patella fractures are an uncommon yet complex injury, accounting for approximately 1% of presentations to orthopedic departments. Surgical management is indicated for unstable, displaced, or comminuted fracture patterns, with the aim of restoring the extensor mechanism and preserving knee function. Various surgical techniques are available, each with differing complication profiles. This study evaluated surgical outcomes following patella fractures over a 15-year period within a single-center orthopedic department. Methods A retrospective cohort study was conducted. A single-center trauma and orthopedic database was reviewed over a 15-year period from 2008 to 2023. Inclusion criteria comprised patients presenting with patella fractures. Exclusion criteria included patients transferred to another healthcare provider for management, those lost to follow-up, or patients who did not engage with planned follow-up. Data analyzed included fracture morphology, time to surgery, surgical technique, complications, reoperation rates, and recovery duration. All patients followed a standardized physiotherapy protocol. Results A total of 100 patella fractures were included, of which 65/100 (65%) underwent surgical fixation. The mean age of patients requiring operative management was 60 years (range: 17-95), and 64.6% were female. The most prevalent fracture morphologies were transverse (44.6%) and comminuted (38.4%). The most commonly used surgical technique was tension band wiring (TBW, 64.6%), followed by cerclage wiring, partial excision with tendon repair, and cannulated screws. Across all surgical patients, the average time to recovery was 7.4 months. TBW had a mean recovery duration of 6.8 months. The shortest recovery times were observed for open reduction and internal fixation with screws (3.25 months), and the longest with excision of distal bone (24 months). Patients managed conservatively had the shortest recovery time at 2.7 months. Postoperative complications occurred in 44.6% of surgical patients. The leading complication was metalwork irritation (24.6%), which led to reoperation in 13/65 patients. Other complications included malunion, stiffness, wound infection, and non-union. There was no significant difference in recovery time between patients operated on within one day versus later. Conclusions Surgical management of patella fractures is associated with good functional outcomes; however, this study found a high rate of postoperative complications and subsequent reoperations. The most predominant reason was hardware irritation associated with TBW. These findings highlight the need for careful planning and individualization based on fracture morphology and patient factors. Recovery time is influenced by the fixation method, and current evidence on optimal postoperative recovery expectations is limited. These results underscore the need for further research to guide surgical decision-making and rehabilitation protocols

    The Long-Baseline Neutrino Experiment: Exploring Fundamental Symmetries of the Universe

    The preponderance of matter over antimatter in the early Universe, the dynamics of the supernova bursts that produced the heavy elements necessary for life and whether protons eventually decay --- these mysteries at the forefront of particle physics and astrophysics are key to understanding the early evolution of our Universe, its current state and its eventual fate. The Long-Baseline Neutrino Experiment (LBNE) represents an extensively developed plan for a world-class experiment dedicated to addressing these questions. LBNE is conceived around three central components: (1) a new, high-intensity neutrino source generated from a megawatt-class proton accelerator at Fermi National Accelerator Laboratory, (2) a near neutrino detector just downstream of the source, and (3) a massive liquid argon time-projection chamber deployed as a far detector deep underground at the Sanford Underground Research Facility. This facility, located at the site of the former Homestake Mine in Lead, South Dakota, is approximately 1,300 km from the neutrino source at Fermilab -- a distance (baseline) that delivers optimal sensitivity to neutrino charge-parity symmetry violation and mass ordering effects. This ambitious yet cost-effective design incorporates scalability and flexibility and can accommodate a variety of upgrades and contributions. With its exceptional combination of experimental configuration, technical capabilities, and potential for transformative discoveries, LBNE promises to be a vital facility for the field of particle physics worldwide, providing physicists from around the globe with opportunities to collaborate in a twenty to thirty year program of exciting science. In this document we provide a comprehensive overview of LBNE's scientific objectives, its place in the landscape of neutrino physics worldwide, the technologies it will incorporate and the capabilities it will possess.Comment: Major update of previous version.
