THE IMPACT OF INCENTIVE-BASED EDUCATION ON MOTHERS’ UNDERSTANDING AND PREPAREDNESS FOR THEIR PRENATAL AND POSTPARTUM EXPERIENCES: A REFLECTIVE STUDY
Incentive-based education has a clear purpose and vision for the populations served. It is important for these organizations to demonstrate that their impact is positive, and that resources from the community are utilized efficiently. The purpose of this study is to identify the impacts of incentive-based education on the understanding and preparedness of a mother’s prenatal and postpartum experiences. Baby Steps, a non-profit incentive-based education program, was used as the research site. Clients of Baby Steps who have been involved in the program since a time during their pregnancy and who have had their baby between 3-12 months ago participated in this study.
The mothers who participated in this survey reported needing financial, emotional, and/or educational support during and following their pregnancy. Survey participants’ ages ranged from 18 years to 34 years old, and came from a variety of ethnic backgrounds and countries of origin. The educational background of participants ranged from elementary school being the highest grade finished, to the completion a college degree. The findings of this study were able support the research hypothesis as these mothers were able to find at least some of the support they needed by participating in the Baby Steps program, thus yielding positive prenatal and postpartum outcomes
Der Ileus - ein Findling in der modernen Medizin?
Zusammenfassung: Kaum einem Krankheitsbild liegt ein so breites differenzialdiagnostisches Spektrum zugrunde wie dem Ileus. Insbesondere die hochgradige Obstruktion und der komplette Ileus können über eine bakterielle Translokation und Mikroperfusionsstörungen des dilatierten Darmes zu intestinalen Perforationen und einer Kreislaufinstabilität führen. Klare Algorithmen zur schnellen Kategorisierung des Ileus sind deshalb unabdingbar. Dabei sind insbesondere die Lokalisation des Kalibersprungs beim mechanischen Ileus als auch der Grad der Obstruktion wichtige Kriterien. Die Anamnese, klinische Untersuchung und das konventionelle Röntgenbild des Abdomens sind elementare Bestandteile der Basisdiagnostik. Als Goldstandard der erweiterten bildgebenden Diagnostik hat sich die Computertomographie etabliert. Während für die meisten hochgradigen mechanischen Obstruktionen des Dünndarms, die in drei Vierteln der Fälle durch Adhäsionen und Briden bedingt sind, eine explorative Laparotomie indiziert ist, stehen beim obstruierenden Dickdarmileus auch endoskopische Optionen zur Verfügung. Combined whole-body vibration, resistance exercise, and sustained vascular occlusion increases PGC-1α and VEGF mRNA abundances
We previously reported that high load resistance exercise with superimposed whole-body vibration and sustained vascular occlusion (vibroX) markedly improves cycling endurance capacity, increases capillary-to-fibre ratio and skeletal muscle oxidative enzyme activity in untrained young women. These findings are intriguing, since increases in oxidative muscle phenotype and endurance capacity are typically induced by endurance but not heavy resistance exercise. Here, we tested the hypothesis that vibroX activates genes associated with mitochondrial biogenesis and angiogenesis. Eight healthy, recreationally resistance-trained young men performed either vibroX or resistance exercise (RES) in a randomised, cross-over design. Needle biopsies (M. vastus lateralis) were obtained at rest and 3h post-exercise. Changes in relative gene expression levels were assessed by real-time quantitative PCR. After vibroX, vascular endothelial growth factor and peroxisome proliferator-activated receptor-γ coactivator 1α mRNA abundances increased to 2- and 4.4-fold, respectively, but did not significantly change above resting values after RES. Other genes involved in mitochondrial biogenesis were not affected by either exercise modality. While vibroX increased the expression of hexokinase II, xanthine dehydrogenase, and manganese superoxide dismutase mRNA, there were no changes in these transcripts after RES. This study demonstrates that high load resistance exercise with superimposed whole-body vibration and sustained vascular occlusion activates metabolic and angiogenic gene programs, which are usually activated after endurance but not resistance exercise. Thus, targeted modification of high load resistance exercise by vibration and vascular occlusion might represent a novel strategy to induce endurance-type muscle adaptation
Prospective randomized controlled trial of simulator-based versus traditional in-surgery laparoscopic camera navigation training
Background: Surgical residents often use a laparoscopic camera in minimally invasive surgery for the first time in the operating room (OR) with no previous education or experience. Computer-based simulator training is increasingly used in residency programs. However, no randomized controlled study has compared the effect of simulator-based versus the traditional OR-based training of camera navigation skills. Methods: This prospective randomized controlled study included 24 pregraduation medical students without any experience in camera navigation or simulators. After a baseline camera navigation test in the OR, participants were randomized to six structured simulator-based training sessions in the skills lab (SL group) or to the traditional training in the OR navigating the camera during six laparoscopic interventions (OR group). After training, the camera test was repeated. Videos of all tests (including of 14 experts) were rated by five blinded, independent experts according to a structured protocol. Results: The groups were well randomized and comparable. Both training groups significantly improved their camera navigational skills in regard to time to completion of the camera test (SL P=0.049; OR P=0.02) and correct organ visualization (P=0.04; P=0.03). Horizon alignment improved without reaching statistical significance (P=0.20; P=0.09). Although both groups spent an equal amount of actual time on camera navigation training (217 vs. 272min, P=0.20), the SL group spent significantly less overall time in the skill lab than the OR group spent in the operating room (302 vs. 1002min, P<0.01). Conclusion: This is the first prospective randomized controlled study indicating that simulator-based training of camera navigation can be transferred to the OR using the traditional hands-on training as controls. Marine Protected Areas in Areas Beyond National Jurisdiction: Defining Success for Conservation and Management
With growing attention to and use of marine protected areas [MPAs], there are an increasing number of policy goals ascribed to these area-based management tools [ABMT]. One expectation is that an MPA can increase system “resilience”, yet oftentimes resilience – including whether we are considering social, economic or ecological resilience – stays unspecified. In recent years, there has also been a specific focus on MPAs as tools to promote climate change resilient ocean systems. Through a meta-analysis of the scientific literature and an analysis of over one thousand three hundred voluntary commitments made at the United Nation Ocean Conference, this work presents a typology of how the concept of resilience is beyond deployed in MPA science and policy-making. Further analysis, supplemented by semi- structure interviews and surveys highlights the diversity of ways in which practitioners define MPA success. These analyses reveal that – in contemporary international ocean governance – different stakeholders are connecting MPAs to different forms of resilience. This work also highlights a disconnect between expressed goals of MPAs, such as cultural effectiveness, and what is deemed important in practice (ecological factors)
Plant sulfate transporters in the low phytic acid network : some educated guesses
A few new papers report that mutations in some genes belonging to the group 3 of plant sulfate transporter family result in low phytic acid phenotypes, drawing novel strategies and approaches for engineering the low-phytate trait in cereal grains. Here, we shortly review the current knowledge on phosphorus/sulfur interplay and sulfate transport regulation in plants, to critically discuss some hypotheses that could help in unveiling the physiological links between sulfate transport and phosphorus accumulation in seeds
Patterns of HER-2/neu Amplification and Overexpression in Primary and Metastatic Breast Cancer
Background: Only 25% of patients with HER-2/neu-positive metastatic breast tumors respond favorably to trastuzamab (Herceptin) treatment. We hypothesized that a high failure rate of patients on trastuzamab could result if some of the metastases were HER-2 negative and these metastases ultimately determine the course of the disease. Methods: We used tissue microarrays (TMAs) containing four samples each from 196 lymph node-negative primary tumors, 196 lymph node-positive primary tumors, and three different lymph node metastases from each lymph node-positive tumor to estimate HER-2 gene amplification by fluorescence in situ hybridization (FISH) and Her-2 protein overexpression by immunohistochemistry (IHC). Results: FISH and IHC analyses gave the same result with respect to HER-2 status for 93.7% of the tissues contained in the TMAs. Tissue samples were, therefore, considered to be HER-2 positive if they were positive for either HER-2 DNA amplification or Her-2 protein expression and HER-2 negative if both FISH and IHC gave a negative result. The HER-2 status of lymph node-positive primary tumors was maintained in the majority of their metastases. For HER-2-positive primary tumors, 77% (95% confidence interval [CI] = 59% to 90%) had entirely HER-2-positive metastases, 6.5% (95% CI = 8% to 21%) had entirely HER-2-negative metastases, and 16.3% (95% CI = 5% to 34%) had a mixture of HER-2-positive and HER-2-negative metastases. For HER-2-negative primary tumors, 95% (95% CI = 88% to 98%) had metastases that were entirely negative for HER-2. Early natural killer cell counts in blood predict mortality in severe sepsis
Producción CientíficaIn an attempt to evaluate the quantitative changes in the status of immunocompetence in severe sepsis over time and its potential influence on clinical outcome, we monitored the evolution of immunoglobulins (Igs) (IgG, IgA and IgM), complement factors (C3 and C4) and lymphocyte subsets (CD4+ T cells, CD8+ T cells, B cells (CD19+) and natural killer (NK) cells (CD3-CD16+CD56+)) in the blood of 50 patients with severe sepsis or septic shock at day 1, day 3 and day 10 following admission to the ICU.
Twenty-one patients died, ten of whom died within the 72 hours following admission to the ICU. The most frequent cause of death (n = 12) was multiorgan dysfunction syndrome. At day 1, survivors showed significantly higher levels of IgG and C4 than those who ultimately died. On the contrary, NK cell levels were significantly higher in the patients who died. Survivors exhibited a progressive increase from day 1 to day 10 on most of the immunological parameters evaluated (IgG, IgA, IgM, C3, CD4+, CD8+ T cells and NK cells). Multivariate Cox regression analysis, including age, sex, APACHE II score, severe sepsis or septic shock status and each one of the immunological parameters showed that NK cell counts at day 1 were independently associated with increased risk of death at 28 days (hazard ratio = 3.34, 95% CI = 1.29 to 8.64; P = 0.013). Analysis of survival curves provided evidence that levels of NK cells at day 1 (> 83 cells/mm3) were associated with early mortality.
Our results demonstrate the prognostic role of NK cells in severe sepsis and provide evidence for a direct association of early counts of these cells in blood with mortality
