Management of Biesboch reservoirs for quality control with special reference to eutrophication
The three Biesbosch Reservoirs are pumped storage reservoirs, fed with rather polluted and highly eutrophic water from the River Meuse. Air injection at the bottom of the reservoirs prevents thermal stratification, which would otherwise result in serious water quality deterioration. Reservoir mixing also serves as an economic algal control measure; mixing over sufficient depth causes light to play the role of limiting factor and this, combined with zooplankton grazing, keeps the biomass of phytoplankton at acceptable levels. Special problems are caused by benthic, geosmin-producing Oscillatoria species growing on the inner embankment. Rooting up the bottom with a harrow is used as the method of control, based on underwater observations by biological staff trained as SCUBA-divers. With regard to pollutant behaviour the three reservoirs act as a series of fully mixed reactors. This enables the application of kinetic models to describe their behaviour and allows the use of a selective intake policy, e.g. for suspended solids with associated contaminants, ammonia and polynuclear aromatic hydrocarbons. A combination of selective intake and self- purification processes - enhanced by the compartmentalisation of the storage volume in three reservoirs - leads to a striking improvement for many water-quality parameters
Effectiveness of single dose rifampicin in preventing leprosy in close contacts of patients with newly diagnosed leprosy
Objective To determine the effectiveness of chemoprophylaxis using a single dose of rifampicin to prevent leprosy in close contacts.
Design Single centre, double blind, cluster randomised, placebo controlled trial.
SettingLeprosy control programme in two districts of northwest Bangladesh with a population of more than four million.
Participants28 092 close contacts of 1037 patients with newly diagnosed leprosy. 21 711 contacts fulfilled the study requirements.
Interventions A single dose of rifampicin or placebo given to close contacts in the second month of starting the index patient’s treatment, with follow-up for four years.
Main outcome measure Development of clinical leprosy.
Results 18 869 of the 21 711 contacts (86.9%) were followed-up at four years. Ninety one of 9452 contacts in the placebo group and 59 of 9417 in the rifampicin group had developed leprosy. The overall reduction in incidence of leprosy using a single dose of rifampicin in the first two years was 57% (95% confidence interval 33% to 72%). The groups did not differ between two and four years. The overall number needed to treat (NNT) to prevent a single case of leprosy among contacts was 297 (95% confidence interval 176 to 537). Differences were found between subgroups at two years, both in reduction of incidence and in NNT.
ConclusionA single dose of rifampicin given to contacts of patients with newly diagnosed leprosy is effective at preventing the development of clinical leprosy at two years. The effect was maintained, but no difference was seen between the placebo and rifampicin groups beyond two years.

De wereldvraag naar zuivelproducten uit de EG en de betekenis voor het afzetbeleid van de Gemeenschap
De publikatie geeft de belangrijkste resultaten weer van een onderzoek naar de vraag door niet-EG-landen naar zuivelprodukten uit de Gemeenschap, waarvan een volledig onderzoeksverslag eerder verscheen onder de naam "Analyse van de wereldvraag naar zuivelprodukten uit de EG". De publikatie is vooral bedoeld om beleidsmakers te informeren over de uitkomsten van het onderzoek en de daaruit te trekken konklusies voor het te voeren afzetbeleid. In het onderzoek is nagegeaan hoe het exportvolume van de EG reageert op een verandering van de exportprijzen. Veel aandacht is er voor de dominante positie van de EG op de wereldzuivelmarkt en de daaruit voortvloeiende konsekwenties voor het uitvoerbeleid. Voorts wordt aan de orde gesteld het onderscheid tussen een afzetbeleid met zo weinig mogelijk lasten voor het EG-budget en een beleid dat zo veel mogelijk bijdraagt aan de welvaart in de Gemeenscha
Analyse van de wereldvraag naar zuivelprodukten uit de EG
Het verslag bevat het resultaat van een onderzoek naar ontwikkelingen in de uitvoer van zuivelprodukten uit de EG en Nederland naar niet-EG-landen in de periode 1974-1980. Met name is bestudeerd de relatie tussen de uitgevoerde hoeveelheden produkt en de prijs waartegen de EG zuivelprodukten op de wereldmarkten aanbiedt. Van deze relatie zijn met behulp van regressie-analyse schattingen gemaakt. Het empirisch onderzoek wordt voorafgegaan door een theoretische analyse van de werking van de wereldzuivelmarkt en van optimaal restitutiebeleid onder verschillende omstandighede
Environmental chemicals impact dog semen quality in vitro and may be associated with a temporal decline in sperm motility and increased cryptorchidism
Adverse temporal trends in human semen quality and cryptorchidism in infants have been associated with exposure to environmental chemicals (ECs) during development. Here we report that a population of breeding dogs exhibit a 26 year (1988–2014) decline in sperm quality and a concurrent increased incidence of cryptorchidism in male offspring (1995–2014). A decline in the number of males born relative to the number of females was also observed. ECs, including diethylhexyl phthalate (DEHP) and polychlorinated biphenyl 153 (PCB153), were detected in adult dog testes and commercial dog foods at concentrations reported to perturb reproductive function in other species. Testicular concentrations of DEHP and PCB153 perturbed sperm viability, motility and DNA integrity in vitro but did not affect LH stimulated testosterone secretion from adult testis explants. The direct effects of chemicals on sperm may therefore contribute to the decline in canine semen quality that parallels that reported in the human
Management of patients with persistent medically unexplained symptoms: a descriptive study
Background: In 2013 the Dutch guideline for management of medically unexplained symptoms (MUS) was published. The aim of this study is to assess medical care for patients with persistent MUS as recorded in their electronic medical records, to investigate if this is in line with the national guideline for persistent MUS and whether there are changes in care over time.
Methods: We conducted an observational study of adult primary care patients with MUS. Routinely recorded health care data were extracted from electronic medical records of patients participating in an ongoing randomised controlled trial in 30 general practices in the Netherlands. Data on general practitioners’ (GPs’) management strategies during MUS consultations were collected in a 5-year period for each patient prior. Management strategies were categorised according to the options offered in the Dutch guideline. Changes in management over time were analysed.
Results: Data were collected from 1035 MUS consultations (77 patients). Beside history-taking, the most frequently used diagnostic strategies were physical examination (24.5%) and additional investigations by the GP (11.1%). Frequently used therapeutic strategies were prescribing medication (24.6%) and providing explanations (11.2%). As MUS symptoms persisted, GPs adjusted medication, discussed progress and scheduled follow-up appointments more frequently. The least frequently used strategies were exploration of all complaint dimensions (i.e. somatic, cognitive, emotional, behavioural and social) (3.5%) and referral to a psychologist (0.5%) or psychiatrist (0.1%).
Conclusions: Management of Dutch GPs is partly in line with the Dutch guideline. Medication was possibly prescribed more frequently than recommended, whereas exploration of all complaint dimensions, shared problem definition and referral to mental health care were used less
Broadband luminescence in defect-engineered electrochemically produced porous Si/ZnO nanostructures
The fabrication, by an all electrochemical process, of porous Si/ZnO nanostructures with engineered structural defects, leading to strong and broadband deep level emission from ZnO, is presented. Such nanostructures are fabricated by a combination of metal-assisted chemical etching of Si and direct current electrodeposition of ZnO. It makes the whole fabrication process low-cost, compatible with Complementary Metal-Oxide Semiconductor technology, scalable and easily industrialised. The photoluminescence spectra of the porous Si/ZnO nanostructures reveal a correlation between the lineshape, as well as the strength of the emission, with the morphology of the underlying porous Si, that control the induced defects in the ZnO. Appropriate fabrication conditions of the porous Si lead to exceptionally bright Gaussian-type emission that covers almost the entire visible spectrum, indicating that porous Si/ZnO nanostructures could be a cornerstone material towards white-light-emitting devices
The genus Borrelia reloaded
The genus Borrelia, originally described by Swellengrebel in 1907, contains tick- or louse-transmitted spirochetes belonging to the relapsing fever (RF) group of spirochetes, the Lyme borreliosis (LB) group of spirochetes and spirochetes that form intermittent clades. In 2014 it was proposed that the genus Borrelia should be separated into two genera; Borrelia Swellengrebel 1907 emend. Adeolu and Gupta 2014 containing RF spirochetes and Borreliella Adeolu and Gupta 2014 containing LB group of spirochetes. In this study we conducted an analysis based on a method that is suitable for bacterial genus demarcation, the percentage of conserved proteins (POCP). We included RF group species, LB group species and two species belonging to intermittent clades, Borrelia turcica Güner et al. 2004 and Candidatus Borrelia tachyglossi Loh et al. 2017. These analyses convincingly showed that all groups of spirochetes belong into one genus and we propose to emend, and re-unite all groups in, the genus Borrelia
