Synchronous bilateral Warthin's tumours of the parotid glands: a case report
Warthin's tumour is the most frequent monomorphic adenoma of the major salivary glands, representing about 2-15% of all parotid tumours. Most of the multifocal Warthin's tumours are unilateral, whereas bilateral Warthin's tumours are far less common; bilateral Warthin's tumours are metachronous with few synchronous cases having been described in the literature. Conventional Drills Versus Piezoelectric Surgery Preparation for Placement of Four Immediately Loaded Zygomatic Oncology Implants in Edentulous Maxillae: 3 Year Results of a Within Person Randomised Controlled Trial
PURPOSE. A within-person randomised controlled trial to compare the outcome of site preparation for two zygomatic oncology implants per zygoma using conventional preparation with rotary drills or piezoelectric surgery with dedicated inserts. MATERIALS AND METHODS. Twenty edentulous patients with severely atrophic maxillae and insufficient bone volumes for placing dental implants with less than 4 mm of bone height subantrally had their hemi-maxillae randomised according to a within-patient study design into implant site preparation with either conventional rotational drills or piezoelectric surgery. Two zygomatic oncology implants (unthreaded coronal portion) were placed in each hemi-maxilla. Implants that achieved an insertion torque of greater than 40 Ncm were immediately loaded with provisional screw-retained metal-reinforced acrylic prostheses. Outcome measures were: prosthesis and implant failures, any com-plications, time to place the implants, presence of post-operative haematoma, and patient preference, as assessed by blinded assessors when possible. All patients were followed up for 3 years after loading. RESULTS. In two patients, drills had to be used in the piezoelectric surgery arm in order to prepare implant sites properly. One implant from the conventional drill group did not achieve an insertion torque of greater than 40 Ncm since the zygoma fractured. Three patients dropped out. Two distal oncology implants failed in the same patient (one per group), who was not prosthetically rehabilitated. Six patients experienced at least one complication at drilled sites and five at piezoelectric surgery sites (three patients had bilateral complications), the difference not being statistically significant (odds ratio = 1.5; P [McNemar’s test] = 1.0; 95% CI of odds ratio: 0.25 to 8.98). Implant placement with con-vention drills took on average 14.35±1.76 minutes vs. 23.50±2.26 minutes with piezoelectric surgery, the implant placement time being significantly shorter with conventional drilling (difference = 9.15±1.69 minutes; 95%CI: 8.36 to 9.94 minutes; P = 0.000). Post-operative haematomas were larger at drilled sites in 11 patients and similar at both sides in nine patients (chi-square = 20.4; df = 3; P = 0.0001), and 16 patients found both techniques equally acceptable while four preferred piezoelectric surgery (chi-square = 34.4; df = 3; P<0.0001). CONCLUSIONS. Although these results may be system-dependent, and therefore cannot be generalised to other zygomatic systems with confidence, both drilling techniques achieved similar clinical outcomes. However, conventional drilling required 9 minutes less and could be used in all instances, though it was more aggressive
Outcome of Facial Nerve Integrity After Parotid Gland Surgery With and Without Intraoperative Monitoring: A Ten-Year Retrospective Study
Background: Facial nerve injury is one of the most concerning complications of parotid gland surgery, with temporary and permanent dysfunction rates varying widely in the literature. This study aimed to identify factors associated with facial nerve injury during surgery for benign parotid tumors and assess the protective efficacy of intraoperative nerve monitoring (NIM) in preventing nerve injury. Methods: This retrospective study analyzed 329 patients who underwent parotid gland surgery between 2010 and 2023. Data collected included patient demographics, tumor characteristics (size, location, histology), surgical parameters (operation time, NIM usage), and postoperative nerve function evaluated using a modified House–Brackmann scale. Descriptive and inferential statistical analyses, including Chi-square tests and logistic regression, were employed to identify predictors of facial nerve outcomes. Results: Facial nerve injury occurred in 5.2% of patients, comprising 4.6% temporary and 0.6% permanent damage. Tumors located in the deep and inferior lobes significantly increased the risk of facial paralysis/palsy (p = 0.035), while tumor size and histology showed no significant associations. Using NIM significantly reduced the risk of facial nerve injury (p < 0.05; OR: 0.35, 95% CI: 0.25–0.50). Age was also identified as a significant predictor of nerve dysfunction (p < 0.05). Conclusions: The findings emphasize the importance of NIM in mitigating facial nerve injury, especially in anatomically complex tumor locations. Tailored surgical approaches based on tumor location and patient-specific factors, combined with the routine use of NIM, are recommended to optimize nerve preservation and improve postoperative outcomes
New test for systematic skills enhancement and improvement in maxillofacial surgery training: multicentre pilot study
We know of few studies in the international scientific literature that specifically address the evaluation of surgical and clinical progress among physicians undergoing specialist training in maxillofacial surgery. Identifying a reliable tool to accurately assess both theoretical knowledge and surgical skills of trainees is essential. The primary aim of this study therefore was to design a comprehensive assessment tool that is capable of evaluating both the theoretical and practical skills of physicians undergoing specialist training in maxillofacial surgery. The methodology employed aims to ensure fairness and effectiveness in skills development, thereby optimising training activities. To meet this need, an evaluation and self-assessment test was developed for maxillofacial surgery trainees at the Ospedali Riuniti of Ancona. Data collection involved digitally administered evaluations and self-assessment tests focused on maxillofacial traumatology, based on AO trauma surgery references. Data were processed into graphs which revealed a progressive learning trend following an initial adjustment phase, leading to optimal outcomes in both clinical and surgical domains. The evaluation and self-assessment test proved to be a valuable learning tool with which to gauge advancements in clinical and surgical skills among maxillofacial surgery residents

Combined supra-transorbital keyhole approach for treatment of delayed intraorbital encephalocele: A minimally invasive approach for an unusual complication of decompressive craniectomy
Background: Intraorbital encephalocele is a rare entity characterized by the herniation of cerebral tissue inside the orbital cavity through a defect of the orbital roof. In patients who have experienced head trauma, intraorbital encephalocele is usually secondary to orbital roof fracture. Case description: We describe here a case of a patient who presented an intraorbital encephalocele 2 years after severe traumatic brain injury, treated by decompressive craniectomy and subsequent autologous cranioplasty, without any evidence of orbital roof fracture. The encephalocele removal and the subsequent orbital roof reconstruction were performed by using a modification of the supraorbital keyhole approach, in which we combine an orbital osteotomy with a supraorbital minicraniotomy to facilitate view and access to both the anterior cranial fossa and orbital compartment and to preserve the already osseointegrated autologous cranioplasty. Conclusions: The peculiarities of this case are the orbital encephalocele without an orbital roof traumatic fracture, and the combined minimally invasive approach used to fix both the encephalocele and the orbital roof defect. Delayed intraorbital encephalocele is probably a complication related to an unintentional opening of the orbit during decompressive craniectomy through which the brain herniated following the restoration of physiological intracranial pressure gradients after the bone flap repositioning. The reconstruction of the orbital roof was performed by using a combined supra-transorbital minimally invasive approach aiming at achieving adequate surgical exposure while preserving the autologous cranioplasty, already osteointegrated. To the best of our knowledge, this approach has not been previously used to address intraorbital encephalocele
First baseline data of the Klinefelter ItaliaN Group (KING) cohort: clinical features of adult with Klinefelter syndrome in Italy
Background Klinefelter syndrome (KS) is frustratingly under-diagnosed. KS have a broad spectrum of clinical features, making it difficult to identify. Objective We describe KS clinical presentation in a large Italian cohort. Design This is the first observational cohort study within a national network, the Klinefelter ItaliaN Group (KING). Primary outcomes were to describe the basic clinical features and the actual phenotype of KS in Italy. Secondary outcomes were to determine age at diagnosis and geographical distribution. Methods We performed a basic phenotyping and evaluation of the hormonal values of 609 adult KS patients. Results Mean age at diagnosis was 37.4 +/- 13.4 years. The overall mean testicular size was 3 ml, and 2.5 ml in both testes in untreated KS group. BMI was 26.6 +/- 5.8 kg/m(2), and 25.5% of KS had metabolic syndrome (MetS). LH and FSH were increased, and mean total testosterone were 350 +/- 9.1 ng/dl. A descriptive analysis showed that 329 KS patients were evaluated in Northern Italy, 76 in Central and 204 in Southern Italy. Analysis of variance demonstrated significant statistical differences (p < 0001) between the age at diagnosis of the three geographical groups. Compared with the expected number among male patients matched for age in Italy, only 16% of KS patients received a diagnosis. Conclusions These data are the results of the only national database available that collects the clinical and hormonal data of the KS patients, currently referred at the KING centers. In Italy the typical KS patient is overweight, with small testes, and elevated LH and FSH. Only 25.5% of them are diagnosed with MetS. Early detection and timely treatment are mandatory
Phenotypic variation within European carriers of the Y-chromosomal gr/gr deletion is independent of Y-chromosomal background.
BackgroundPrevious studies have compared sperm phenotypes between men with partial [1] deletions within the AZFc region of the Y chromosome with non-carriers, with variable results. Here, we have investigated a separate question, the basis of the variation in sperm phenotype within gr/gr deletion carriers, which ranges from normozoospermia to azoospermia. Differences in the genes removed by independent gr/gr deletions, the occurrence of subsequent duplications or the presence of linked modifying variants elsewhere on the chromosome have been suggested as possible causal factors. We set out to test these possibilities in a large sample of gr/gr deletion carriers with known phenotypes spanning the complete range.ResultsWe assembled a collection of 169 men diagnosed with gr/gr deletions from six centres in Europe and one in Australia, and characterized the DAZ and CDY1 copies retained, the presence or absence of duplications and the Y-chromosomal haplogroup. Although our study had good power to detect factors that accounted for 655.5% of the variation in sperm concentration, no such factor was detected. A negative effect of gr/gr deletions followed by b2/b4 duplication was observed within the normospermic group, which remains to be further explored in a larger study population. Finally, we observed significant geographical differences in the frequency of different subtypes of gr/gr deletions which may have relevance for the interpretation of case control studies dealing with admixed populations.ConclusionsWe conclude that the phenotypic variation of gr/gr carriers in men of European origin is largely independent of the Y-chromosomal background
